Contribui??o para o diagnóstico e o tratamento das fístulas arteriovenosas pulmonares após a opera??o de Glenn bidirecional

Abstract: objective: to determine the incidence of pulmonary arteriovenous fistulae (pavfs) after the bidirectional glenn operation and the possible independent variables that could influence their appearance; to confirm the use of microbubble contrast echocardiography for the detection of pavfs; and to test the sensitivity and specificity of pulmonary angiography. methods: from march 1990 to december 1995, 59 patients were operated upon. their ages ranged from 2 to 132 months (mean, 32.7± 33.6). all underwent clinical and laboratory examinations, microbubble contrast echocardiography, and cardiac catheterization. results: of the 54 survivors, 20 (37.0%) had pavfs. the ages ranged from 2 to 132 months (mean, 29.6±29.7). in 13 (65%) patients, the bidirectional glenn operation was performed on the right-hand side; in 2 (10.0%) patients, on the left-hand side; and in 5 (25%) patients, it was bicaval. the follow-up of the patients with pavfs ranged from 4 to 84 months (mean, 32.4±21.65), and that of the patients without fistulae ranged from 1 to 77 months (mean, 23.4±18.8), with statistical significance (p=0.04). the pavfs were diagnosed by use of microbubble contrast echocardiography in 20 cases; the examination was considered positive when return of microbubbles through the pulmonary veins was detected. pavfs were observed in the right lung in 9 (45%) patients, in the left lung in 3 (15%) patients, and in both lungs in 8 (40%) patients. the pulmonary angiography showed alterations compatible with pavfs in 16 patients, with a sensitivity of 80%. conclusion: the incidence of pavfs after the bidirectional glenn operation was high (37%). the time interval elapsed after the bidirectional glenn operation was the only independent variable that significantly correlated with the appearance of pavfs (p=0.04). microbubble contrast echocardiography was the standard diagnostic method. the pulmonary angiography showed a sensitivity of 80.0%.

Abstract: background: indications for the implantation of sirolimusand paclitaxel-eluting stents are expanding and include complex lesions and subsets of patients with clinical and demographic characteristics very different from those of early pivotal randomized trials. the desire registry was planned to monitor the safety of both cypher? and taxus? stent available in brazil since 2002. methods and results: from may 2002 through march 2007, 2043 patients treated with one or more than one stent (either cypher? or taxus?) at hospital do cora??o da associa??o do sanatório sírio were included in this registry. mean age was 63.8 (11.3) years; 76.6% were male and 28.4% had diabetes. a total of 2,415 lesions were treated and 2,983 stents were implanted: 2,608 cypher? and 375 taxus?. acute and subacute (<30 days); late thrombosis (31 days - 360 days) and very late thrombosis (>360 days) occurred in 0.34%, 0.73% and 0.34% of the patients, adding up to a 1.42% overall rate. likewise, the major adverse cardiac event rate was low and added up to 8.6% (154 patients), including: 45 (2.51%) cardiac deaths; 50 (2.8%) myocardial infarctions and 59 (3.3%) of additional revascularizations. conclusions: this long-term analysis of the desire registry shows the high efficacy and safety profiles of drug-eluting stents, with a low thrombosis rate, which is similar to those of randomized trials.
Comparative study of the use of diltiazem as an antispasmodic drug in coronary angiography via the transradial approach

Abstract: objective: to evaluate the impact of the use, prior to the procedure, of injectable diltiazem to prevent complications. methods: between september 2000 and july 2001, 50 patients underwent transradial coronary angiography and were randomized to receive placebo (gi) or diltiazem (gii) through a catheter inserted into the radial artery. all patients received isosorbide mononitrate. ultrasound analyses of the radial artery were performed before examination, 30 minutes afterwards, and 7 days afterwards to evaluate the flow, the diameter, and the artery output. results: the radial artery diameter of gi was 2.4± 0.5 mm before the procedure and 2.3±0.5 mm after 30 minutes (ns), whereas in gii the diameter was 2.2±0.3 mm before the examination and 2.5±0.4 mm 30 minutes after it (p<0.001). radial artery output in group 1 was 7.3±5.l2 ml/min before the examination and 6.1±3.5 ml/min 30 minutes after the examination (ns), and gii had an increase of 5.9±2.5 ml/min before examination to 9.05± 7.78 ml/min after the examination (p=0.04). complications (spasm, occlusion, and partial obstruction) occurred in 4 patients (17.4%) in gi and did not occur in gii (p=0.04). conclusion: the study suggests a decrease in vascular complications through the transradial access for coronary angiography with the use of diltiazem as an antispasmodic drug, resulting in the significant increase in the diameter of the radial artery and radial artery output.

Abstract: the incidence of diabetes mellitus has continuously risen over recent years. patients with this chronic condition have a higher risk of developing coronary artery disease. unfortunately, due to the frequent diffuse involvement of multiple vessels, the long term outcomes after percutaneous intervention, whether by balloon catheter or bare-metal stent, has always been poor and frequently inferior to cardiac surgery for this subset of patients. recently, the introduction of drug-eluting stents (des) has brought new hope to treat patients with more complex diseases percutaneously. in this paper we report a successful percutaneous approach of a 63-year-old insulin dependent diabetic patient treated with nine des (taxustm, boston scientific).

Abstract: objective: to compare dual-head coincidence gamma camera (dcd-ac) with dobutamine stress echocardiography (dse) in viability assessment, using functional recovery as the gold standard. methods: twenty-one patients were prospectively studied, with coronary artery disease and severe left ventricular dysfunction undergoing dse and dcd-ac at baseline and dse three months after revascularization. results: of the 290 segments analyzed, 83% were akinetic, 15% hypokinetic and 2% dyskinetic at rest. dse identified 68% of these segments as non-viable. dcd-ac identified 56% of these segments as normal (dysfunctional segments with preserved metabolism and perfusion), 30% as viable (preserved metabolism and reduced perfusion) and 14% as non-viable (reduced perfusion and metabolism). of the dse non-viable segments, dcd-ac identified 80% as normal or viable and 19.9% as non-viable (p<0.001). in hypokinetic segments viability and normal segments were detected in a higher proportion by both methods (p<0.001). dse sensibility and specificity were 48.3% and 78.1% respectively. dcd-ac sensibility and specificity was 92.2% and 20.0%. dcd-ac identifies a higher incidence of function improvement in normal segments than in viable and non-viable. conclusion: dcd-ac classified as normal or viable most of the non-viable dse segments. in assessment of functional recovery segments after three months, dcd-ac showed a high sensibility but low specificity.

Abstract: purpose: to investigate the presence of vertigo complaint in middle-aged hypertension patients. methods: a transversal study. composed by 154 patients of both genders, aged from 45 to 64 years, included in the research after sample estimation. hypertension was verified through blood pressure readings and by a systematized questionnaire about hypertension and the use of medication for blood pressure. vertigo was assessed through audiological anamneses. results: there is a significant association between blood hypertension and vertigo. conclusion: the results in this research, through evidence of association between blood hypertension and vertigo complaint, can be a base for health professionals concerned with alterations caused by blood hypertension.
Diversity of eating patterns and obesity in older adults—A new challenge  [PDF]

Abstract:

The increase in the variety of food choices influences the eating patterns of older adults, which is in turn increases the occurrence of obesity. This study aimed at identifying eating patterns and their association with obesity in a representative sample of older adults living in an urban area and registered in the basic health unit in the city of Botucatu, S?o Paulo, Brazil. This is a cross-sectional study and data collection took place from March to June of 2011 through the application of a validated food frequency questionnaire for older adults, a socio-demographic survey and an anthropometric evaluation. Eating patterns were identified through principal component analysis. Scores of individual consumption were divided in tertiles, characterizing as low, moderate or high adherence of the individuals to each pattern. Logistic regression models were fitted for the outcomes “general obesity” and “abdominal obesity” and the tertiles of consumption adjusting by sociodemographic variables. Six eating patterns were identified: Healthy foods, Snacks and weekend meals, Fruits, Light and whole foods, Mild diet and Traditional diet. It was found that the adherence to healthy foods is protective against obesity as well as adherence of snacks and weekend meals are risk of obesity. Eating patterns and their recognized influence on obesity comprise an issue that deserves continuous attention in order to evaluate collectively the eating profile, and develop specific nutritional guidelines for older adults.

Abstract: purpose: to evaluate the overall experience of our institution with percutaneous occlusion of the patent ductus arteriosus. methods: since december/92, 150 patients underwent 178 procedures (85 - rashkind technique; 87 - gianturco coils; 6 - duct occlud). median age was 6.5 years (1 to 57). mean minimum ductal diameter was 3.05±1.24mm (1 to 8). clinical and echocardiographic evaluations were performed periodically. results: adequate implantation was achieved in 143 (95.3%) patients. prevalence of immediate residual shunting was 52.1%, falling to 15.9% during follow-up. this figure decreased to 5.2% after new additional procedures. umbrella and coil embolization occurred in 1 and 12 procedures, respectively. hemolysis and mild stenosis of left pulmonary artery were observed in one patient each. there was no mortality. conclusion: this therapeutic modality is safe and efficacious with good follow-up results.

Abstract: this work presents the results of morphological and physical-chemical characteristics of a sugar cane bagasse ash material sample produced under controlled burning conditions. the investigation was carried out by analyzing chemical composition, x-ray diffraction, 29si nuclear magnetic resonance, morphology, thermal analysis, particle size, specific surface, and density. moreover, the pozzolanic activity of the ash was evaluated by pozzolanic activity index and chapelle's method. the results suggest that the sugar cane bagasse ash has adequate properties to be used as pozzolan in construction materials.

Abstract: aim: to identify likely association between blood hypertension and hearing loss. design: a non-paired case-control study. setting: institutional work carried out at universidade norte do paraná, in south brazil. material and method: 154 cases and 154 controls, both genders, aged 45 to 64, included in the research after sample estimation. methodology: hypertension was verified through blood pressure readings and by a systematized questionnaire about hypertension and the use of medication for blood pressure. hearing was assessed through tonal threshold audiometrics and audiologic anamneses. non-conditional logistic regression was used in order to control likely confusion or modification of effect of other variables on interest associations. results: there is a significant association between blood hypertension and hearing loss. hearing loss in the population under study suggests that hypertension is an accelerating factor of degeneration of the hearing apparatus due to aging. conclusions: the results in this research, through evidence of association between blood hypertension and hearing loss, can allow for an integrated work of cardiologists, nephrologists, otorhinolaryngologists, audiologists and other health professionals concerned with alterations caused by blood hypertension.
