Familial benign chronic pemphigus (Hailey-Hailey Disease): use of topical immunomodulators as a modern treatment option

Abstract: benign chronic familial pemphigus (hailey-hailey disease) is a rare autosomal dominant blistering skin disorder characterized by suprabasal cell separation (acantholysis) of the epidermis. the hailey brothers first described it in 1939. hailey-hailey disease usually appears in the third or fourth decade, although it can occur at any age. heat, sweating and friction often exacerbates the disease, and most patients have worse symptoms during summer. it is characterized clinically by a recurrent eruption of vesicles and bullae at the sites of friction and intertriginous areas. we report a 51-year-old male presenting with grey-brown hyperkeratosis with partial papillomatosis and lichenification in the axillary and inguinal areas and infiltrated erythematous lesions in the infraorbitary region, on the side of the face. biopsies obtained from inguinal and axillar areas revealed parakeratotic crusts overlying an acantholytic epidermis. a biopsy from one of the lesions from the infraorbital area showed a jessner-kanof lymphocytic infiltration. the patient was treated with antimicrobials and four days later, topical pimecrolimus was started, leading to an improvement of the clinical picture. the efficacy of pimecrolimus in our case suggests that cellular immunity couldplay a role in thepathogenesis of hailey-hailey disease.

Abstract: the decline of more orthodox versions of liberalism in the first decade of this century has provided an opportunity for the reevaluation of state action. in brazil, the recovery of the state's investment capacity and the updates (albeit inadequate updates) to the state's administrative structure have revitalized the fields of planning and development, giving them new, challenging, and more complex goals. but to what extent should the concept of "planning and development" be reconsidered in this new context? the paper explores aspects of the complexity of current events, which themselves raise the need to retrain and reframe meanings and practices of government planning, as well as of the very development that has been planned for the country during this period. we conclude by stating that development is a collective, cumulative, and continuous process of learning and achievements, the dimensions or descriptions of which are grouped together - theoretically and politically - both together and as equivalent levels of strategic importance, because today, finally, it is clear that we are not talking about development.

Abstract: O declínio das vers es mais ortodoxas do liberalismo na primeira década do século XXI ofereceu oportunidade para a revaloriza o da atua o estatal. No Brasil, a recupera o da capacidade de investimento do Estado e a atualiza o - ainda que insuficientes - de sua estrutura administrativa revitalizaram os campos do planejamento e desenvolvimento, outorgando-lhes novos e mais complexos e desafiadores objetivos. Como repensar, portanto, o bin mio "planejamento-desenvolvimento" nesse novo contexto? O ensaio explora aspectos da complexifica o do tempo presente, o que, por sua vez, suscita a necessidade de requalificar e ressignificar os sentidos e as práticas do planejamento governamental e do próprio desenvolvimento pretendido ao país na atual quadra histórica. Conclui se afirmando que o desenvolvimento é processo coletivo, contínuo e cumulativo de aprendizado e conquistas, cujas dimens es ou qualificativos agregam-se - teórica e politicamente - tanto em simultaneo como em patamares equivalentes de importancia estratégica, pois hoje, afinal, sabe-se que ou é assim ou n o se está falando de desenvolvimento. Abstract: understanding the resilient modulus (mr) of the sub-grade soils and materials composing the layers of road pavements is crucial for an efficient analysis of their structural behavior as a whole. due to the importance of the granular materials as layers of flexible road pavements, it has been a practice to determine their resilient response and that of their chemically stabilized mixtures in order to understand their mechanical behavior under repeated loads when used as layers of forest road pavement. this work was conducted to identify the geotechnical and resilient properties of a sandy soil in the county of vi?osa-mg, in its natural state and after stabilization with lime and tar and to propose empirical correlations between the resilient modulus of these materials and geotechnical parameters easily determined from laboratory testing data.

Abstract: This is a retrospective study on gigantomastia correction in obesity facing the technique of nipple-areolar grafts and its complexity. The main question is the technical-surgical care during its execution aiming to achieve reparative results that can return to these women the desire to live with better quality of life and health. Method: We performed a retrospective study of all obese patients with severe gigantomastia who underwent reduction mammaplasty with an operative technique using the superior pedicle, with special care for the nipple-papillary grafts. The study was carried out at a single institution between 2001 and 2013, in a total of 30 cases. This operative technique was presented at first time in 1980, with no changes up to now. We emphasized the perfect decortication of the areola and nipples until they were translucent after the maneuvers of Schwartzman and subsequent grafts operated in both breasts. Results: Results were considered satisfactory, from the reparative and aesthetic point of view, emphasizing the degree of improvement observed in women undergoing technique. Discussion: We can say that during all this time of aesthetic and therapeutic-restorative surgery, due to its realization in a single surgical time, there was big impact on the aesthetics of patients and on their families, even taking into account possible complications in the post-operative. Conclusion: The superior pedicle technique, performed as described here, is a safe and reliable procedure in patients with severe gigantomastia.

Abstract: The use of organic waste in agriculture has been used aiming at greater productivity, decrease in the cost of production and sustainable use of resources. The present study aimed at evaluating the effect of phosphate fertilization with the maintenance of organomineral fertilizers, combining poultry manure and soluble or reactive phosphate, in the production of grains in corn culture and the residual effect for subsequent crops of beans and soybeans. The experiment was conducted in Sete Lagoas, MG, in 2015, using three fertilizers with sources of different phosphorus sources (triple super phosphate—STP, organomineral with STP and organomineral with Bayovar phosphate) applied at the doses of 65, 130, 195 and 260 kg·ha¯1 of P2O5 total, and compared to the control without phosphate fertilization. We evaluated the contents of foliar P, P accumulation in the grains, yields of corn grains, beans and soy, accumulated productivity and export of accumulated P of the three cultures. The three cultures assessed presented higher productivity when compared to the average of the factorial treatments that received phosphate fertilization relative to the control. Organomineral fertilizers increased grain production, obtaining average productivity equal to or greater than those obtained with the exclusive use of STP.

Abstract: The present study had the objective of evaluating economical performance of Clambunk skidder in log drags activities. The study was developed in an eucalyptus crop property of a Minas Gerais State company that uses a mechanized full-tree system of harvest. The economical evaluation was made with the operational costs determination. The productivity varied from 18.34 to 72.08 m3.h-1 and the effective operational cost was of R$ 268.61 / hour. O presente estudo teve como objetivo avaliar economicamente a atividade de arraste em áreas montanhosas, utilizando-se um trator florestal arrastador com esteiras (Clambunk skidder). O estudo foi conduzido em povoamentos de eucalipto de uma empresa florestal do Estado de Minas Gerais, no sistema de colheita de árvores inteiras. A avalia o econ mica consistiu na determina o dos custos operacionais e da distribui o destes custos. A produtividade do Clambunk skidder variou de 18,34 a 72,08 m3.h-1, com custo operacional de R$ 268,61 por hora efetiva.

Abstract:

In Portugal, over the last 20 years, we have seen great strides in addressing intimate partner violence against women by their male partners. Measuring and understanding progress, and achieving sustainable systems change is a major challenge and to achieve this aim we need to identify events, persons and settings that contribute for the critical factors that create long term transformations in the systems and the creation of new resources [1]. Since the healthcare system is one of the main actors of intervention with survivors, the goal of this project was to interview key stakeholders from the healthcare system in order to understand the main changes they perceive have happened over the past two decades. Based on interviews with key stakeholders, our study explores the evidences of the changes that occurred in the healthcare system, the intersections with other systems and their visions for the future.

Abstract: It is a syndrome with a set of signs and symptoms distinct from depression or panic syndrome, which is not characterized by the fear of dying, but by the desire to die in the form of suicide or in another quick way. Unlike depression or panic, it occurs more frequently between 60 and 80 years of age, according to the World Health Organization (WHO) report, which puts Brazil in eighth place among the countries with the highest incidence of suicides in the age range of 70 years. The report estimates a death every 40 seconds in the world, with India ranking first among the countries surveyed, according to the United Nations agency, in the WHO 2012 registries. It can occur in any person, regardless of color, religion, social class or schooling, being more common, however, among men, in the ratio of nine men to two women. It is very common among health practitioners or the self-employed. Causes are related to the general delusion involving body and mind, focusing on the lack of care. By the description of the dictionary, soul is the union of body and spirit, which some prefer to call mind. “Empty Soul” represents the lack of the very essence of life, a total void of wills, a finding of abandonment. For these symptoms or illness, the treatment has followed the line of “antidepressants” aimed at the reuptake of serotonin, such as SSRIs, MAOIs, anxiolytics (benzodiazepines) and painless techniques that reach the brain, used in neurology and psychiatry. We suggest, as a therapeutic idea, the realization of “Healing Workshops”, with the support being given in the form of meetings with motivational orientations, through positive stimuli, daily and constant psychotherapies, which the client will attend until cure is verified. This degree of disorder has presented in alarming proportions, especially after 2015. This work is justified by to the dramaticity of the deaths, such as: knife, shot, fall of great height, hunger strikes and forced isolation in prisons or kidnappings. In addition, the Brazilian Institute of Geography and Statistics (IBGE) has estimated, for the year 2025, in Brazil, a population of 32 million elderly people. This manuscript reports signs and symptoms presented by patients who thought it was not worthwhile to continue living.
