Prevalence of Spinal Metastasis in Neurosurgical Procedures: A Descriptive Study  [PDF]

Abstract: Objective: This study sought to identify the prevalence of operations for spinal epidural neoplastic metastases relative to other spine and neurosurgical operative procedures. Methods: This study was descriptive and involved a retrospective review of data collected from patients who underwent neurosurgeries between February 1997 and January 2015 at a single quaternary hospital. The examined population was distributed across five descriptive categories to perform numerical distributions among neurosurgical operative procedures. Results: A total of 12,802 neurosurgical procedures were identified. These procedures were classified as follows: Skull and brain, 11,192 (87.42%); spinal column and spinal nervous tissue, 1462 (11.42%); and peripheral nerves, 148 (1.16%). Surgical procedures for the 1462 (100%) cases of spinal column and spinal nervous tissue diseases were distributed by nosology as follows: Degenerative intervertebral disk and spondylosis, 768 (52.54%); neoplastic, 279 (19.08%); traumatic, 221 (15.11%); congenital, 163 (11.14%); infectious and inflammatory, 27 (1.85%); and vascular, 4 (0.28%). With respect to the distribution of the 279 (100%) surgical procedures for spinal column and spinal nervous tissue neoplastic diseases, 124 (44.44%) procedures were for intradural neoplasms, and 155 (55.56%) procedures were for epidural spinal column neoplasms. The 155 (100%) operations for epidural neoplastic diseases were distributed into two groups: Primary epidural neoplasms, 42 (27.10%); and secondary epidural neoplasms, 113 (72.90%). Spinal column epidural neoplastic metastases (secondary neoplasms) represented 0.88% of the 12,802 neurosurgical procedures. Conclusions: Surgical procedures for spinal metastasis are uncommon with respect to all neurosurgical operative procedures. Trend analysis reveals an unchanging trend of prevalence for these procedures.
Cranioestenose da sutura metópica: efeito teratogênico do valproato de sódio. Relato de caso

Abstract: objective: the aim of this report is to warn that sodium valproate used during pregnancy can produce craniosynostosis in the newborn, particularly trigonocephaly. method: we describe a case of trigonocephaly in a six month-old girl, daughter of a young non-smoker couple, whose mother had used phenobarbital 100 mg daily and sodium valproate 500 mg twice daily during the whole pregnancy. we also review current literature about this topic. result: bone sclerosis over the metopic suture was confirmed during surgery. bibliographical review yields previous reports on valproate teratogenicity, mainly determining metopic suture craniosynostosis. conclusion: sodium valproate used during pregnancy can produce craniosynostosis by teratogenic effect, specially trigonocephaly (premature fusion of metopic suture).

Abstract: OBJETIVO: alertar que o uso de valproato de sódio durante a gravidez pode determinar cranioestenose no recém-nascido, em especial a trigonocefalia. MéTODO: relato de um caso de trigonocefalia em menina de 6 meses, filha de pais jovens, n o fumantes e cuja m e fez uso de fenobarbital 100 mg/dia e valproato de sódio 500 mg duas vezes/dia durante toda a gravidez. Foi realizada revis o bibliográfica sobre o assunto. RESULTADO: no ato cirúrgico p de-se confirmar a presen a de esclerose óssea sobre a sutura metópica. A revis o bibliográfica permitiu o encontro de relatos prévios sobre a teratogenicidade do valproato de sódio, que determina principalmente cranioestenose da sutura metópica. CONCLUS O: o uso de valproato de sódio durante a gravidez pode determinar como a o teratogênica a cranioestenose, especialmente a trigonocefalia (fechamento precoce da sutura metópica).

Abstract: meningiomas are neoplasms derived from arachnoid cells with their origin linked to idiopathic genetic abnormalities (delection of the long arm of chromosome 22), predisposing diseases and radiotherapy induction. we report the case of a 50 years-old man radiation-induced meningioma 20 years after the diagnosis, surgical and radiation treatment of an oligodendroglioma. the supporting diagnostic criteria of radiation-induced meningiomas are discussed and the pertinent literature of the theme is revised.

Abstract: we report on a seven years-old boy with complex partial seizures and the presence of low grade glioma in left fronto-parietal region. the magnetic resonance imaging showed focal non-hemorrhagic lesion in the splenium of the corpus callosum. the description of the transient lesion in the splenium of the corpus callosum was related in three previous studies, in patients with epilepsy. thus, the observed transient focal lesion in the splenium of the corpus callosum of this child, probably, has correlation with to prolonged focal partial seizures and not to the presence of glioma low grade.
Presentation and treatment of carotid cavernous aneurysms

Abstract: we analyzed a group of patients with the diagnosis of internal carotid aneurysms in its intracavernous segment, with emphasis in prevalence, clinical features, treatments, evolution and neurological prognosis. neurological signs and symptoms at initial presentation were registered and compared with final outcome. patients were divided into two stratified groups, one with 19 patients which underwent interventionist treatment, and another with 21 patients who were conservatively treated. the present study demonstrated that intervention is significantly correlated with a better prognosis considering evolution of pain symptoms secondary to neurovascular compression (p=0,002). regarding neurological deficits, an interventionist approach was also significantly correlated with better outcome in comparison with initial presentation (p=0,008). these results indicate that interventionist treatment determines improvement or resolution of pain symptoms in comparison with patients conservatively treated, as well as stabilization or partial improvement of neuro-ophthalmological deficits.

Abstract: INTRODU O: O acúmulo de conhecimentos tem sobrecarregado o conteúdo curricular do curso de gradua o médica. As escolas médicas devem estar preocupadas em definir um conteúdo temático mínimo, relacionado com os transtornos mais frequentes; buscamos sugerir o conteúdo temático mínimo, para o curso de Neurologia na gradua o médica. MéTODO: 1. Identificamos os locais de trabalho dos médicos jovens, fora do Hospital Escola(HE) pela análise de resposta às cartas enviadas a 6415 médicos residentes (MR) do Estado de S o Paulo e de 201 entrevistas pessoais com MR; 2. Verificamos quais os transtornos neuro-psiquiátricos (TNP) mais frequentes na popula o, através da análise dos registros diagnósticos de pacientes que procuraram o Pronto Socorro (PS) de três institui es: Municipal de Taubaté-SP, Municipal de S o José dos Campos-SP e Faculdade de Medicina da Santa Casa de Misericórdia de S o Paulo-SP. RESULTADOS: 1. Os MR s o jovens e, fora do HE, trabalham em PS. 2.Os diagnósticos mais frequentes nas três institui es foram: alcoolismo, cefaléia, coma, demência, desmaio, doen a encéfalo-vascular, epilepsia, hemiplegias ou paraplegias, meningite, neuropatia periférica, paralisia facial, tontura, transtornos psíquicos e traumatismo craniencefálico. CONCLUS O: Estes diagnósticos mais frequentes s o os temas relevantes para o conteúdo programático do curso de Neurologia na gradua o médica.
Endoscopic endonasal transsphenoidal approach for pituitary adenomas: technical aspects and report of casuistic

Abstract: objective: analyse technical aspects, effectiveness and morbidity of the endoscopic endonasal transphenoidal approach for pituitary adenomas. method: from january 2005 to september 2008, 30 consecutive patients underwent endoscopic endonasal resection of pituitary adenomas with a follow up from 3 to 36 months. their medical charts were retrospectively analysed. results: there were 18 women and 12 men, mean age 44 years (range 17-65 yr). among the 30 patients, 23 had macroadenomas and 7 microadenomas. twelve patients had non-functioning tumors, 9 had acth-secreting tumors, 8 had gh-secreting tumors and 1 prolactinoma. complete resection and hormonal control was achieved in all microadenomas. macroadenomas were completely removed in 6 patients, subtotal resection in 6 and partial resection in 11. three patients had diabetes insipidus and 5 had csf leaks treated with lumbar drainage. conclusion: the endonasal endoscopic approach for pituitary tumors is effective and has low morbidity.
Spontaneous thrombosis of internal carotid artery: a natural history of giant carotid cavernous aneurysms

Abstract: objective: to describe five cases of giant carotid cavernous aneurysms which evolved with spontaneous thrombosis of internal carotid artery (stica), with emphasis at epidemiology, clinical presentation, natural history, related factors and neurological outcome. method: there were 711 consecutives patients with 802 aneurysms with and without surgical treatment during a period of 19 years. we selected 35 patients with 40 carotid cavernous aneurysms (5%) of which 20 (50%) were giant aneurysms. among those cases, 5 patients evolved with stica (25%). symptoms and findings at presentation were recorded and compared with those at outcome. results: clinical presentation was commonly related to atherosclerotic factors such as elevated blood pressure (80%), diabetes mellitus (40%) and dislipidemy (40%). all patients presented with hemicranial headache, ophthalmparesy and retro bulbar pain, and after stica all presented improvement of symptoms. after stica, 4 patients had regression of deficit, 2 partial and 2 complete. four patients had sensorial trigeminal neuropathy in v1 and v2 territories, also showing improvement of symptoms after stica. conclusion: stica is a common outcome in giant carotid cavernous aneurysms, and is related with significant improvement of symptoms; however, it may be catastrophic for those patients without efficient collateral circulation.
Stereotactic biopsy for intracranial lesions: clinical-pathological compatibility in 60 patients

Abstract: objective: image guided stereotactic biopsy (sb) provides cerebral tissue samples for histological analysis from minimal lesions or those that are located in deep regions, being crucial in the elaboration of therapeutic strategies, as well as the prevention of unnecessary neurosurgical interventions. method: sixty patients with central nervous lesions underwent sb from november 1999 to march 2008. they were followed up to 65 months. preoperative diagnosis was based on clinical presentation and neuro-radiological features, pathologic diagnosis, clinical outcome. the compatibility of these findings with the pathologic diagnosis was analyzed. results: considering diagnosis confirmation when inflammatory hypothesis were made, our accuracy was of 76%, with 94% of those cases having clinic-pathological correspondence after an average of 65.2 months of follow up. considering diagnosis confirmation with the preoperative hypothesis of neoplasm, our accuracy was of 69% with 90% of these cases having clinic-pathological correspondence after an average of 47.3 months of follow-up. morbidity rate was of 5% and mortality was zero. the diagnosis rate was 95%. conclusion: stereotactic biopsy represents a safe and precise method for diagnosis. anatomic and histopathological analyses have high compatibility with long-term clinical outcome.
