Metastasis to Paranasal Sinuses as the First Presenting Sign of Renal Cell Carcinoma  [PDF]

Abstract: The clear cell carcinoma is a relatively rare tumor, represented approximately 85% by renal cell carcinoma (RCC), but has a characteristic of early metastatic, which sometimes spread to unusual places. Metastatic tumors in the paranasal sinuses are very rare. This case report a 65 years-old patient who presented sinusitis and epitaxis as a first sign of renal cell carcinoma with metastasis to the right maxillary sinus, which was opereted at Lagoa Hospital—RJ and strengthen the inclusion of this disease in the differential diagnosis of sinus disease. The renal cell carcinoma is a relatively rare tumor, representing approximately 3% of malignant tumors. About a third of patients with renal cell carcinoma have metastasis at diagnosis. The sinuses are a rare location of metastasis. The maxillary sinuses and the paranasal sinuses are most commonly affected by metastatic tumors to the sinuses region. The most frequent primary tumor to metastasis to paranasal sinuses is the renal cell carcinoma. This report highlights the rarity of the case and the importance in the differential diagnosis of sinus disease.
Partial Nephrectomy as Treatment of an Atypical Metastasis from Prostate Cancer—A Case Report and Review of Literature  [PDF]

Abstract: Background: Prostate cancer is the second most common type of cancer in man and the second in cancer-specific deaths in this population in the world. Most of the causes of death related to prostate cancer are due to its distant metastases, with the most common sites being: skeleton, distant lymph nodes, liver and lung. Renal metastasis is rare, and studies suggest infiltration due to arterial microembolization of the tumor. A key point in this scenario is the clinical suspicion of differential diagnoses, to offer the patient an effective therapy in such a specific case. Aim: To report a case of a patient with prostate cancer undergoing partial nephrectomy whose histopathological report revealed a metastatic lesion of that primary site. Case Presentation: 74 years old man, referred in May 2015 due to high PSA level and lumbago. PSA 323.11 ng/dl, rectal examination cT3a; biopsy was performed and histopathological study reported bilateral prostate adenocarcinoma, Gleason’s score 8 (4 + 4). Patient’s staging showed multiple secondary implants on skeletal scintigraphy. Tomography revealed solid exophytic lesion in the lower pole of the right kidney (4.7 × 3.6 cm); prostate without cleavage planes with seminal vesicles and pelvic node enlargement. Hormone therapy was initiated, PSA levels dropped to 9.51 ng/dl and total testosterone < 50 ng/dl. Partial nephrectomy was planned, initially by laparoscopy, but converted to laparotomy in December 2015. Procedure lasting 3 hours, minimal blood loss, no perioperative complications, discharged on the 3rd postoperative day. Histopathological report described undifferentiated malignant neoplasm, requiring immunohistochemistry that confirmed prostate adenocarcinoma. Patient remains hormone therapy, with no progression of the disease so far. Conclusion: Given the rarity of these cases, it is not possible to presume that nephrectomy enhances the survival rates. However, we observed that partial nephrectomy was a good choice for our patient, being the first case described in the literature. More reports should be available and studies with higher levels of evidence should be conducted to assist us in patient orientation and decision making.

Abstract: Avaliaram-se as características de carca a de bovinos machos n o-castrados de três grupos genéticos submetidos a dois planos de alimenta o, em confinamento. Utilizaram-se 56 bovinos com idade média de 18 meses. Doze animais foram abatidos no início do experimento e os demais (16 Nelore selecionados, 12 Nelore n o-selecionados e 16 Caracu selecionados, com peso vivo médio inicial de 404, 345 e 434 kg, respectivamente), distribuídos em delineamento inteiramente casualizado, em esquema fatorial 2 x 3, composto por dois níveis de alimenta o (restri o alimentar: fornecimento de 65 g de MS/kgPV0,75 por dia e ad libitum: oferta do alimento duas vezes ao dia) e três grupos genéticos. A dieta foi formulada com silagem de milho como volumoso e apresentou rela o volumoso:concentrado 50:50. O abate foi realizado quando os animais atingiram 4 mm de gordura subcutanea, medida por ultra-som. Observou-se efeito da intera o nível de alimenta o x grupo genético somente sobre a porcentagem de tecido ósseo. O peso e a profundidade da carca a e os pesos dos cortes dianteiro e traseiro n o diferiram entre os grupos selecionados e foram superiores aos dos animais Nelore n o-selecionados. O plano ad libitum proporcionou maior rendimento do traseiro. O rendimento de carca a n o sofreu influência da alimenta o e, juntamente com a porcentagem do traseiro, foram maiores para os zebuínos. A espessura de gordura subcutanea real e a porcentagem de tecido adiposo foram maiores para os Nelore n o-selecionados, enquanto a área de olho-de-lombo e a porcentagem de tecido muscular foram maiores nos bovinos Caracu.

Abstract: The estimated Harmonic Mean for Genetic Values (MHVG), Relative Performance of Genetic Values (PRVG) and the Harmonic Mean of Relative Performance of Genetic Values (MHPRVG) were used as measures to provide and facilitate the interpretation of genotypic stability and adaptability of Eucalyptus grandis W. Hill ex Maiden. The different criteria were applied as alternatives to the simulation of selection for productivity, stability, adaptability, as well as for the joint value of these attributes together. The estimates were generated by using the mixed model, assuming genetic effects to be random. The software SELEGEN-REML/BLUP was used to evaluate progeny trials and to estimate statistical parameters. The best trees to be used in seedling and clonal orchards were identified. Fifty three half-sib families from three Australian provenances were tested in the municipalities of Mogi Gua u, Boa Esperan a do Sul and Ca apava, in the State of S o Paulo. Compact family blocks designs were used, involving variable numbers of replicates, with six-trees linear plots, in a 3 x 2 m spacing. Two types of data transformation were tried in order to evaluate their efficiency in the reduction of the genetic, environmental and phenotypic variances. MHVG, PRVG and MHPRVG statistics proved to be useful in simulations for the selection of individuals with the attributes considered, providing options for choosing strategies and criteria by considering different values for the estimated genetic gains. The joint analysis of experiments in different locations confirmed that substantial gains can be obtained in DBH through individual selection based on their genetic values. This can be achieved by either thinning the trial, which leads to the formation of a seedling seed orchard or vegetative propagation of the best individuals, in order to establish a clonal seed orchard. The genetic correlation between locations indicated that a single selected population and a single clonal seed orchard will suffice to represent all test locations. This mean a substantial saving at this stage of the tree improvement program. Genetic variability was achieved with the moderates values obtained from individual heritability, in the narrow sense, for growth in DBH in the three locations studied. The adoption of selection strategies and criteria proposed here will result in a selected population (seedling seed orchard) with two hundred individuals of the highest genetic value, while maintaining an adequate effective family number and producing gains in DBH in the order of 12.89 % to 24.33 % over the o

Abstract: O objetivo do trabalho foi comparar duas formas de corre o ou padroniza o de dados de crescimento de plantas e avaliar a sua eficiência na redu o da heterogeneidade das variancias genética, ambiental e fenotípica presentes em testes de progênies de Eucalyptus grandis W. ex Maiden. Cinqüenta e três progênies de três procedências australianas foram testadas nos municípios paulistas de Mogi Gua u, Boa Esperan a do Sul e Ca apava, sob delineamento de blocos de famílias compactadas, com número de repeti es variáveis por local e parcelas lineares de seis plantas cada, no espa amento de 3 m x 2 m. O programa computacional SELEGEN-REML/BLUP foi utilizado para as análises genéticas e identifica o dos melhores indivíduos e progênies para compor uma popula o de melhoramento e seu respectivo uso em pomares de sementes por mudas ou clonais. A corre o de dados efetuada pela raz o (hi/him) entre a raiz quadrada da herdabilidade no local i e a média das raízes quadradas das herdabilidades, considerando a análise conjunta de locais, apresentou maior eficiência quando comparada à n o corre o ou corre o de dados feita pelo desvio padr o fenotípico ( f s ), usualmente empregado. Adicionalmente, os resultados das análises conjuntas de locais confirmaram que ganhos de alta magnitude podem ser obtidos para o crescimento em diametro, com a sele o de indivíduos baseada nos seus valores genéticos, tanto pelo desbaste dos testes, transformando-os opcionalmente em pomares de sementes por mudas, como por meio da propaga o vegetativa dos melhores indivíduos para um pomar clonal de sementes. Em fun o dos valores obtidos para a correla o genética entre os locais, uma única Popula o Selecionada e um único Pomar Clonal de Sementes podem ser estabelecidos ao atendimento de regi es representativas dos três locais experimentais testados, implicando em economia de tempo e recursos. A variabilidade genética presente foi representada pelos valores moderados obtidos de herdabilidade individual, no sentido restrito, para o crescimento em diametro à altura do peito (DAP), nos três locais estudados. Abstract: a research project involving eighty first-year medical students who observed the experiences of first-time patients admitted to a university hospital in south-eastern brazil was undertaken during the first semester of 1989. the students conducted 260 interviews and followed the patients through from the time of admittance, observing the patients' expectations, values and attitudes towards minor complaints. the objective of the experience was to open a forum for discussion about excessive specialization and inadequacies in the curriculum in preparing the future medical professional to meet the needs of the population. both the criticisms in the students' testimonies and the data collected at the interviews, as well as the concrete facts and pratical proposals regarding the modification of the curriculum, concur with the edinburgh declaration of 1988. they point to the need for the adoption of a mode by which teaching and assistance activities, might be integrated from the very beginning of the physician's training.

Abstract: Foi realizada pesquisa com 80 alunos, no primeiro período do ciclo básico de medicina, que acompanharam o percurso de pacientes de primeira vez no ambulatório de um hospital universitário da regi o do Sudeste do Brasil. Foram feitas 260 entrevistas, proporcionando aos "alunos observadores" uma experiência controlada da trajetória dos pacientes, permitindo um confronto com outros valores, atitudes e expectativas, assim como a observa o das queixas e sugest es dos usuários. O objetivo da pesquisa era o de abrir discuss o, entre os alunos, desde o primeiro ano da gradua o médica, sobre o modelo hegem nico de especializa o precoce, questionando a identidade do futuro médico moldada por um currículo inadequado a responder às demandas concretas de cuidados da popula o. Os resultados, tanto pelos depoimentos dos alunos, como pela interpreta o dos dados, estimulam a implementa o de estratégias integradoras das atividades docentes-assistenciais, desde o início da forma o profissional, com o objetivo de modificar o panorama do ensino atual.
Desempenho de cultivares de alface tipo americana em cultivo de outono no sul de Minas Gerais

Abstract: with the objective of evaluating iceberg lettuce (lactuca sativa l.) cultivars grown under autumn condition, an experience in santana da vargem, mg, brazil, was carried out from march to june of 2002. the experimental design was in randomized complete blocks with thirteen treatments (cultivars adal, 4553 r7, ael 10, iglo, rocco rz, rubette, rx 66, rpt 3030, rpt 2020, paloma, sandy, legacy e raider) and three replicates. the cultivars 45-53r7, rpt 2020, paloma, raider and rpt 3030 showed statistically higher total fresh matter than the other cultivars, with the yield ranging 1,010.9; 1,029.4; 1,044.9; 1,075.2 and 1105.2 g plant-1, respectively. the cultivars 45-53r7, rpt 2020, paloma, raider, rpt 3030, ael 10 and rocco, with yield of commercial fresh matter ranging from 599.1 to 739.1 g plant-1 were statistically more productive than the other cultivars. the cultivar 45-53r7 with commercial head circumference of 49.2 cm stood out in relation to the other cultivars. in relation to the stem length, they did not show statistically significant difference. for commercial head symmetry, "rpt 2020", "raider" and "rpt 3030", with notes of 4.6; 5.0 and 5.0; respectively, were more symmetric to the other cultivars.

Abstract: the melon produced in the state of rio grande do norte is cultivated under irrigation and fertigation. with the use of fertigation and knowledge of the nutrients uptake curves it is possible to divide the total dose of nutrients in several applications during the crop cycle. thus, the objectives of this study were to obtain curves of dry matter accumulation and the uptake of nitrogen, phophorus, potassium, calcium and magnesium for the "pele-de-sapo" melon plants and to determine the total uptake of these nutrients at the end of the crop cycle. the experimental design was an entirely randomized blocks with three replications. plants were sampled at 22, 33, 43, 54 and 69 days after the sowing and were determined the dry matter and plant contents of n, p, k, ca and mg. it was observed that: 1) more than 50% of the extracted nutrients were accumulated in the vegetative part of the plant; 2) potassium, calcium and nitrogen are the nutrients more demanded by the melon plant; 3) the period of larger demand of nutrients happened between 43 and 54 days after sowing; 4) leaves and fruits are the principal drains of nutrients in the whole cycle of the crop.

Abstract: crisphead lettuce cultivars and inbred lines adapted to winter cultivation were evaluated in a trial of 42 cultivars, in santana da vargem, minas gerais state, brazil. the experimental design was of randomized complete blocks, with 3 replications. total fresh weight varied from 772 to 1268 g plant-1. the best performance was obtained from the inbred lines p1823-0014206-1 (1268 g plant-1) and p1823-0014325-3 (1232 g plant-1), which did not differ one from the other. the commercial fresh weight varied from 378 to 729 g plant-1 and a variation of 561 to 729 g plant-1 was obtained among statistically superior genotypes. the commercial head circumference varied from 35.6 to 49.2 cm. the inbred line with the biggest head circumference (p1823-0014325-3 with 49.2 cm) was statistically different from the other genotypes. stem length varied from 1.9 to 4.9 cm, these results being considered acceptable for industrial processing. a variation including all degrees of scale notes was obtained for bremia lactucae resistance, being considered resistant: svr2210 (4.2); gx635 (4.2); p1843-1000148-1 (4.5); p1831-0015585-1 (4.5); p1831-0015338-1 (4.5); p1790-011945-1 (4.7); p1814-0013283-1 (4.7); p1843-1000697-1 (4.7); p1843-1000171-1 (4.7); p1843-1000179-2 (5.0); p1843-1000572-1 (5.0); p1692-0001843-1 (5.0); p1831-0015110-1 (5.0); p1831-0015060-1 (5.0); p1831-0015583-2 (5.0); p1831-0015309-1 (5.0); p1831-0015058-2 (5.0); p1823-0014206-1 (5.0); p1831-0015036-1 (5.0) and p1823-0014325-3 (5.0). the control cv. raider presented high susceptibility to the disease.
