Reliability of the interpretation of coronary angiography by the simple visual method

Abstract: OBJECTIVE: Evaluation of inter and intraobserver reproducibility of by the visual method interpretation of cineangiogram in a clinically based context. METHODS: Five interventional cardiologists analyzed 11 segments of 8 coronary cineangiograms at a two month apart sessions. The percent luminal reduction by the lesions were analyzed by two different classifications: in one (A) the lesions were graded in 0% = absent, 1-50% = mild, 51 - 69 = moderate, and > or = 70% = severe; the other classification (B) was a dichotomic one : <70% = nonsignificant and > or = 70%=significant lesions. The agreement were measured by the kappa (k) index. RESULTS: Interobserver agreement was moderate for classification A (1st measurement, k = 0.36 -- 0.63, k m = 0.49; 2nd measurement, k = 0.39-0.68, k m = 0.52) and good for classification B (1st measurement, k = 0.55-0.73, k m = 0.63; 2nd measurement, k = 0.37-0.82, k m = 0.61). Intraobserver levels of agreement were k = 0.57-0.95 for classification A and 0.62-1.0 for classification B. CONCLUSION: The higher level of reproducibility obtained by adopting the dichotomous criteria usually considered for ischemic limits demonstrates that in the present clinical context, the reliability of the simple visual method is adequate for the identification of patients with clinically significant lesions and candidates for myocardial revascularization procedures.

Abstract: the objective of this work was to study the feedlot performance of hereford, bulls or steers, produced by two years old cows, kept in feedtot during 193 days, from weaning to slaughter at fourteen months of age, reaching carcass weight of 205kg at slaughter. the roughage/concentrate ratio was reduced yadually, beginnig with 70/30 and ending with 40/60. the roughages used were sorghum silage, chopped sugar cane and oats hay. bulls showed a higher average daily gain (1.23 vs 1.09 kg) and better food convertion (5.8 vs 6.8) than steers. the efficiency of converting mcal of digestible energy into kilogram of weight gain, was also better for bull calves (16.0 vs 18.6 mcal/kg). consumption of dry matter and digestible energy, booth adjusted for metabolic size were 109. l vs 106.1 g and 299. l vs 291.2 kcal for steers and bulls, respectively.
Electrically Detected Magnetic Resonance Modeling and Fitting: An Equivalent Circuit Approach

Abstract: Continuous-wave electrically detected magnetic resonance (cw-EDMR or just EDMR) and its variants are powerful tools to investigate spin-dependent processes in materials and devices. The use of quadrature detection improves the quality and selectivity of EDMR analysis by allowing, for example, the separation of individual resonant spin lines. Here we propose an equivalent circuit model in order to better understand the EDMR quadrature signal in a variety of different situations. The model considers not just the electrical response of the sample but of cables and measuring circuit and its influence on the resulting spectral lines. Recent EDMR spectra from Alq3 based OLEDs, as well as from a-Si:H reported in the literature, were successfully described by the model. Moreover, the model allows the implementation of a fitting routine that can be easily used to determine accurate values of crucial parameters such as g-factor and linewidth of the resonant lines.

Abstract: in order to study certain epidemiological features of multidrug-resistant tuberculosis (mdr-tb) carriers and their influence on the control and treatment, a group of patients was evaluated over a four-year period, selected by: mycobacterium tuberculosis isolation from sputum; resistance to rifampin, isoniazid and one more drug, or, failure of reserve regimen, all cases were from a tuberculosis reference unit in the city of s?o paulo. a total of 182 patients were reviewed, with a mean age of 35.7±6.8 years and 112 (61.5%) were male. these patients was classified according to therapeutic history, as: primary mdr-tb (with initial sensitivity test) 11 (6%); post primary mdr-tb (after irregular use previous treatment) 134 (74%), and indeterminate mdr-tb (failure after regular use of initial and reserve regimens) 37 (20%). contagion was identified in 41/170 patients, acquired through domiciliary rather than institutional transmission. there were four familial outbreaks and none were institutional. the most frequent condition associated with these cases was abandonment of therapy (45%) followed by alcoholism (27%), sequential failure in the treatment regimens (23%), mdr contagion (15%), drug reaction (6%), hiv positive (4%) and diabetes (3%). there was resistance to rifampin+isoniazid in 100%, streptomycin in 83% and ethambutol in 47%. conventional x-ray revealed cavities in all, though only 35 (19%) were unilateral. these cases are discussed and some suggestions presented.
Perfil epidemiológico de 58 portadores de síndrome de Klippel-Trénaunay-Weber acompanhados no Ambulatório da Santa Casa de S?o Paulo

Abstract: background: klippel-trenaunay-weber syndrome is a rare disorder about which there are few articles available (most of them are sporadic case reports related to complications). objective: to assess the epidemiological profile of patients with klippel-trenaunay-weber syndrome. methods: medical records of 58 patients followed at the lymphatic disease and angiodysplasia outpatient clinic for the discipline of vascular surgery at the school of medical sciences of santa casa de s?o paulo were reviewed. results: the disorder affected males and females equally (30 males and 28 females). the mean age of patients under treatment was 12.8 years. in most cases, the syndrome had been diagnosed in childhood, with port wine stain as the first sign noticed by the family at birth or during the first year of life. the most frequently reported symptom was debilitating pain usually associated with symptoms of venous stasis. the ceap classification was c0 and c1 for younger patients and c4-c6 for most older patients. arteriovenous fistulas were diagnosed in 8.5% of cases. positive family history was reported by only 6.8% of patients. conclusion: klippel-trenaunay and parkes weber syndromes are different presentations of a same disorder and they can be studied as one, klippel-trenaunay-weber syndrome. childhood is the optimal moment to identify affected patients and to reduce progression of venous insufficiency and bony and soft tissue hypertrophy. compression should be indicated for all affected patients to reduce progression of peripheral venous disease.

Abstract: the aim of this study was to evaluate the metabolic profile of the use of soyabean hulls associated with two sources of non-protein nitrogen (npn) on the finishing of feedlot lambs. eighty crossbred suffolk non-castrated lambs aging 69±6 days and weighing 23.11±1.78 kg were used. lambs were randomly allocated into 4 experimental groups according to the npn source: conventional urea (1%); protected urea; 1% protected urea + 0.5% conventional urea; and a control group, which did not receive urea supplementation. blood samples were collected weekly to determine the plasma concentrations of urea, albumine, γ-glutamiltransferase (ggt), calcium and phosphorus. the control group had higher average daily gain and final weight than protected urea and protected urea + conventional urea groups; however, it did not differ from the conventional urea group. no differences between the groups were observed for dry matter intake, feed conversion or warm carcass yield and carcass finishing. regarding the serum protein profile, the o group fed conventional urea had higher serum concentration of urea than that of protected + conventional urea. urea supplementation did not improve the performance of feedlot lambs fed diets with high level of soybean hulls.
Estudo clínico randomizado comparando antimoniato de meglumina, pentamidina e anfotericina B para o tratamento da leishmaniose cutanea ocasionada por Leishmania guyanensis

Abstract: fundamentals: american tegumentary leishmaniasis (atl) treatment remains a challenge, since most available drugs are injectable and only a small number of comparative, randomized clinical trials have been performed to support their use. moreover, treatment outcome may depend on the causative species of leishmania. objectives: to evaluate and compare the efficacy and tolerability of meglumine antimoniate, pentamidine isethionate, and amphotericin b in the treatment of atl caused by leishmania (viannia) guyanensis. methods: 185 patients were selected according to the eligibility criteria and randomly allocated into three groups - two groups with 74 patients each, and one group with 37 patients, which underwent meglumine, pentamidine and amphotericin b treatment, respectively. doses, mode of administration and time periods of treatment followed the current recommendations for each drug. patients were re-examined one, two and six months after completion of treatment. results: no differences were observed among the therapeutic groups in relation to gender, age, number or site of lesions. intention-to-treat (itt) analysis showed efficacy of 58.1% for pentamidine and 55.5% for meglumine (p=0.857). the amphotericin b group was analyzed separately, since 28 patients (75.7%) in this group refused to continue participating in the study. mild or moderate adverse effects were reported by 74 (40%) patients, especially arthralgia (20.3%) in the meglumine group, and pain (35.1%) or induration (10.8%) at the site of injection in the pentamidine group. conclusion: pentamidine and meglumine show similar efficacy in the treatment of atl caused by l. guyanensis. given the low efficacy of both drugs, there is an urgent need for new therapeutical approaches.
Toxicidade hepática é rara em pacientes com artrite reumatoide usando terapia combinada de leflunomida e metotrexato

Abstract: objective: some studies have reported that adding leflunomide (lef) to the treatment of rheumatoid arthritis (ra) in patients who do not respond to methotrexate (mtx) improved efficacy but increased the risk of liver toxicity. this study aimed at assessing the incidence of liver toxicity in patients with active ra using the lef and mtx combination therapy in comparison with that of patients on mtx monotherapy. methods: between february and september 2009, 97 consecutive patients followed up at the university hospital of the universidade federal de santa catarina, brazil, were enrolled. ra patients on mtx alone or using the lef and mtx combination had their medical records systematically reviewed. the alanine/aspartate aminotransferase enzymes were retrospectively analyzed since the beginning of treatment with mtx or mtx plus lef. hepatotoxicity was defined as an increase of at least two-fold the upper limits of normal of the liver enzymes. results: 71 ra patients were included in the study: 36.6% were using 20-25 mg/week of mtx alone and 63.4% were using 20-25 mg/week of mtx plus 20 mg/day of lef. of the patients on the combination therapy, 11.1% had abnormal levels of liver enzymes versus 11.5% of the patients on monotherapy (p = 1.0). abnormal aminotransferase levels have been seen with both mtx and lef monotherapies in patients with ra. in our study, no difference was found between the percentages of aminotransferase elevations of patients being treated with mtx alone or in combination with lef. conclusion: the combination of mtx and lef in ra patients is generally safe and well tolerated.

Abstract: this analysis of the discussion surrounding the efficacy of the plague serum produced by manguinhos institute in the early twentieth century begins with an overview of oswaldo cruz's service as head of the public health directorship (diretoria de saúde pública). the controversy itself is then addressed, through an exploration of correspondence exchanged by physicians oswaldo cruz, miguel pereira, vital brazil, chapot prévost, and francisco fajardo. their letters reveal how bacteriology in brazil was then marked by uncertainty and experimentation, even while this field of knowledge publicly touted itself as safe and incontestable. the article shows how arguments of an extra-scientific nature interfere with both research development and the acceptance of medical products.
