Fatality from Road Traffic Accident in Guinea: A Retrospective Descriptive Analysis  [PDF]

Abstract: Objectives: Causes and risk factors that result in fatal road traffic accident have not been described at the national level in Guinea yet. The goal of this study is to explore the causes and risk factors related to fatal road traffic accident, identified most vulnerable road users, and inform the road traffic prevention policy in Guinea. Methods: We made a retrospective descriptive analysis based on national fatal road traffic accident data from the Department of Health Information at the Guinean Ministry of Health for year 2011. Results: In 2011, road traffic accident was responsible for an aggregate number of 1655 deaths with an overall death rate of 15.3 per 100,000 population. Male experienced more than twice the risk of death from road traffic accidents (21.9 deaths per 100,000 population) compared with female (9.0 deaths per 100,000 population). While taking the population as a whole, the highest death rate was found among the middle aged in 35 - 49 age group accounting for (29.7 deaths per 100,000 population), followed successively by young adults age group 25 - 34 years (24.6 deaths per 100,000 population), and the middle aged in 50 - 64 age group (22.9 deaths per 100,000 population). Principally, occupants, motorcyclists and pedestrians sustained considerable burden of deaths respectively (9.2; 2.9; 2.2 per 100,000 population). In re-gional setting, the highest death rate was found in Upper Guinea (19.5 per 100,000 population), followed by Forest Guinea (18.7 per 100,000 population) and Middle Guinea (16.8 per 100,000 population). A large proportion of male was killed as motorcyclist than female while high per-centage of female died as occupant than male for all age group. The regional distribution showed that when a remarkable number of occupant death were observed in Upper and Forest Guinea, more people died as pedestrian and pedal cyclist in Conakry. Conclusions: This study demonstrated that most of the deaths were among occupants, motorcyclists and pedestrians, and the productive workforce aged 25 - 49 years. It was found that majority of the deaths happened in Upper Guinea followed by Forest Guinea. Improvement of roads design, strict enforcement of road safety laws and raising the awareness of general public about the causes and risks factors of road traffic accident through various channels are highly required which will promote economic growth in the local communities and then help people escape the poverty trap.
Reported Estimates of Adverse Pregnancy Outcomes among Women with and without Syphilis: A Systematic Review and Meta-Analysis

Abstract: Background To estimate probability of adverse pregnancy outcomes (APOs) among women with and without syphilis through a systematic review of published literatures. Methodology/Principal Findings Chinese and English literatures were searched for studies assessing pregnancy outcomes in the presence of maternal syphilis through August 2013. The prevalence estimates were summarized and analyzed by meta-analysis. Fifty-four literatures involving 11398 syphilitic women and 43342 non-syphilitic women were included from 4187 records initially found. Among untreated mothers with syphilis, pooled estimates were 76.8% for all APOs, 36.0% for congenital syphilis, 23.2% for preterm, 23.4% for low birth weight, 26.4% for stillbirth or fetal loss, 14.9% for miscarriage and 16.2% for neonatal deaths. Among syphilitic mother receiving treatment only in the late trimester (>28 weeks), pooled estimates were 64.4% for APOs, 40.6% for congenital syphilis, 17.6% for preterm, 12.4% for low birth weight, and 21.3% for stillbirth or fetal loss. Among syphilitic mothers with high titers (≥1:8), pooled estimates were 42.8% for all APOs, 25.8% for congenital syphilis, 15.1% for preterm, 9.4% for low birth weight, 14.6% for stillbirth or fetal loss and 16.0% for neonatal deaths. Among non-syphilitic mothers, the pooled estimates were 13.7% for all APOs, 7.2% for preterm birth, 4.5% for low birth weight, 3.7% for stillbirth or fetal loss, 2.3% for miscarriage and 2.0% for neonatal death. Begg's rank correlation test indicated little evidence of publication bias (P>0.10). Substantial heterogeneity was found across studies in the estimates of all adverse outcomes for both women with syphilis (I2 = 93.9%; P<0.0001) and women without syphilis (I2 = 94.8%; P<0.0001). Conclusions/Significance Syphilis continues to be an important cause of substantial perinatal morbidity and mortality, which reminds that policy-makers charged with resource allocation that the elimination of mother-to-child transmission of syphilis is a public health priority.

Abstract: 目的:分析湖南省2013年5岁以下儿童主要死亡原因及其死亡的基本特征,为减少儿童死亡、保护儿童健康,制定儿童生存、发展、保护战略措施提供依据。方法:对湖南省各市州2013年5 747例5岁以下儿童死亡资料进行回顾性分析,采用描述性统计的方法分析湖南省5岁以下儿童的死因构成及死亡的特征,应用卡方检验分析湖南省各市州2013年5岁以下儿童死亡情况的差异。结果:1)2013年湖南省5岁以下儿童死亡率(under-5 mortality rate,U5MR)为7.00‰;2)线性趋势卡方检验表明2002至2013年近10年内湖南省U5MR均呈下降趋势(均P<0.05);3)2013年湖南省5岁以下儿童死亡原因中溺水所占比重最大,为13.35%,其次是先天性心脏病、早产或低出生体质量、肺炎及意外窒息,所占比重依次为12.65%,11.66%,10.82%和8.13%;4)卡方检验表明:2013年湖南省各市州U5MR存在差异(χ2=163.185,P<0.05)。结论:2013年湖南省U5MR较之前明显下降;2013年各市州U5MR存在差异;2013年湖南省城市与农村的U5MR也存在差异;2013年湖南省5岁以下儿童死亡原因中溺水所占比重最大,肺炎所占比重是第4位

Abstract: The nodD3 gene ofRhizobium meliloti is transcribed via promoter P1 or P2. Gel retardation assay showed binding of SyrM to the P1 upstream region of nodD3. DNaseI footprint analysis demonstrated that the binding site of SyrM in nodD3 P1 region consists of two inverted repeat sequences arranged in tandem. SyrM seems to bind to DNA in the form of dimer or tetramer and requires the two inverted repeat sequences for binding.

Abstract: Nitrogen fixation (nif) genes of diazotrophic enteric bacteria,Enterobacter cloacae orKlebsiella pneumoniae, are regulated bynif LA operon, in which thenif A product, NifA positively regulatesnif gene transcription, whereas the nifL product NifL represses it under oxygen or in excess of fixed nitrogen. Two-hybrid system was used to detect the possible interaction between NifA and NifL. The preliminary results illustrate that NifL does interact with NifA. The interaction between NifL and NtrC has also been shown.

Mutational analysis of the structure basis for the multimerization function of NifA central domain

Abstract: InKlebsiella pneumoniae (Kp) NifA central domain, when the conservative amino acid residue Thr-290 in C3 region was replaced by Val, the function of NifA for activating the transcription ofnif genes was lost. Thus the conservative Thr-290 residue seems critical for the activation function of NifA central domain. This point mutant of NifA central domain is used to examine the putative multimerization function of NifA central domain by merodiploid experiment. The results showed that the NifA central domain bore the multimerization determinants of NifA protein. A series of truncated mutants of NifA were constructed to determine the structural elements at the central domain critical for multimerization. It demonstrates that amino acid residues 252–453 are involved in the multimerization function of NifA central domain.

Abstract: Our previous work showed that an enoyl-ACP reductase gene fabI1 of Sinorhizobium meliloti was down-regulated in the nifA mutant nodule bacteria. To gain a better understanding of fabI1 gene, a single site insertion mutant was constructed in this study. The fabI1 mutant was retarded in cell growth, and its ability to grow on media with high concentration of NaCl was reduced. In addition, the mutant was completely defective in swarming phenotype. During symbiosis, the fabI1 mutant had delayed nodule formation on host plants. Despite the fact that FabI1 protein showed 66% identity with another enoyl-ACP reductase FabI2 in S. meliloti, defects in fabI1 were not rescued by the plasmidborne version of fabI2. This indicated the different functions of the two FabI proteins in S. meliloti.

Abstract: ThegdhA genes of IRC-3 GDH strain and IRC-8 GDH+ strain were cloned, and they both successfully complemented the nutritional lesion of anE. coli glutamate auxotroph, Q100 GDH . However, thegdhA gene from the mutant IRC-8 GDH+ strain failed to complement the glutamate deficiency of the wild type strain IRC-3. ThegdhA genes of the wild type and mutant origin were sequenced separately. No nucleotide difference was detected between them. Further investigations indicated that thegdhA genes were actively expressed in both the wild type and the mutant. Additionally, no GDH inhibitor was found in the wild type strain IRC-3. It is thus proposed that the inactivity of GDH in wild type is the result of the deficiency at the post-translational level of thegdhA expression. Examination of the deduced amino acid sequence ofBacillus licheniformis GDH revealed the presence of the motifs characteristic of the family I-type hexameric protein, while the GDH ofBacillus subtilis belongs to family II.
