Abstract: objective: to determine the prognostic value of pre b immunophenotype and its variants on early treatment response among of acute pediatric lymphoblast leukemia. patients and methods: a case control study nested in a cohort was carried out with male and female patients 15 years and younger with recently diagnosed pre b lymphoblast leukemia. a panel of b, t, monoclonal antibodies of the myelo monocytic and megakaryocytic cell type was used. response was assessed by bone marrow aspiration 14 days post treatment. results: 54 patients were included. the median age was 7 years (2 months - 14 years) median leukocyte count was 13,450/mm3 (1200-986,000/mm3). we identified 29 cases with late pre b immune phenotype, 19 cases with common pre b and 6 cases with early pre b immunophenotype. eleven patients also displayed myeloid antigens. a significant association (p=0.034) was found between early treatment response and the presence of myeloid antigens. no association was found between the pre b immunophenotype, age and leukocyte count with early treatment response (p=0.264). conclusions: we need to pay special emphasis on early treatment response in children with lymphoblast leukemia as our study did not corroborate the common finding that clinical factors and immune phenotype can be predictive factors.

Abstract: Commercial websites do not only have to catch the attention of the web surfer, who in the end can also become a potential customer, they are also engaged in the crucial task of guiding the reader through the complex maze of semiotic systems which constitutes a web page. Successful sites are those giving the reader assistance to be able to find the relevant information he or she is seeking. Although we assume that this navigation aid is not exclusively verbal, the aim of thispaper is to analyse some of the linguistic means by which this navigation is assisted and, more particularly, to explore the presence of textual metadiscourse. After giving a definition of the concept of metadiscourse, we explore the insights that can be gained from the field of usability, within the area of Computer-Human Interaction, which will help us to detect more clearly metadiscursive devices present in commercial websites. Then we categorize the different typesof textual metadiscourse and try to find some distinctive features of their presence in digital metadiscourse. Finally, we extend the classification to a group of devices which is restricted to digital discourse, hypertextual links or nodes. In so far as they help readers to navigate through digital documents, links play an outstanding role in commercial websites and can be classified according to the metadiscoursive function they carry out.

Abstract: recently, a rapid bedside assay for quantitative determination of cti and cpk-mb has been developed that provides a positive or negative result in 10 to 15 minutes allowing for a better therapeutic approach. the objective of our study was to validate the diagnostic usefulness of cardiac troponin i in patients with chest pain. we determined sensitivity, specificity, positive and negative predictive values in 40 patients that arrived to the hospital with chest pain. these patients were assigned to four different groups: group a: patients with acute myocardial infarction. group b: patients with unstable angina and normal ecg. group c: patients with atypical chest pain and normal ecg. group d: control. eighteen (45%) patients were woman and 22 (55%) were men; age 54.1 ± 26, range 32 to 85 years. in group a, sensitivity, specificity, positive and negative predictive values for cti were 95%, for cpk-mb, they were 40, 50, 90, 7.1%. for group b, cti: 64, 90, 90, 64%; cpk-mb: 50, 90, 87, 56%. group c, cti and cpk mb 25, 95, 50, 86%. group d, cti and cpk-mb: 50, 95, 50, 95%. this study suggests that the rapid bedside qualitative test through cardiac troponin i assessment is a test with higher predictive value for early diagnosis of acute myocardial infarction than cpk-mb.
Modeling of the Water Table Level Response Due to Extraordinary Precipitation Events: The Case of the Guadalupe Valley Aquifer  [PDF]

Abstract:

A two-dimensional algorithm for underground water flow simulation was modified and adapted to the geohydrologic conditions of the Guadalupe valley located in the state of Baja California in northern México. In order to solve the numerical model using the balance equation, the central finite differences with spatial and temporal constant increments method were used. Such model considers a heterogeneous and transient unconfined aquifer. Modeling and calibration processes are presented using the data of water table levels provided by the water level data loggers installed in a monitoring network and precipitation data from climatic stations of both seasons: 2009-2010 and 2010-2011. It was possible to locate, correlate and model specific rainfall-recharge events with the aim of obtaining an estimation of how these events are directly reflected on the water table level of the aquifer and how it reacts against simulated extraordinary events.

Abstract: introduction: anxiety disorders are highly prevalent in primary and specialty care. to reduce anxiety the use of relaxation therapy is recommended as an alternative therapy to psychopharmacology. the aim of the study is determine whether relaxation therapies benefit patients with anxiety. materials and methods: we used quantitative methodology: a quasi-experimental study before and after a, longitudinal, prospective study. subjects studied: patients diagnosed with anxiety who agreed to participate in relaxation therapy at the mental health center infante (murcia), from december 2009 to march 2010. (n = 39). independent variable was relaxation therapy and the dependent variable was anxiety. other variables were sex, age, educational level and treatment with psychotropic drugs. validated scale used was stai, scores comparing pre-post-intervention (t student applies, xi square confidence interval (p <0.05). results: the test was performed by pre-post stai 39 patients, 69% female and 31% male. the mean age was 39'84 years. 77% were taking medication and 23% were not. there was a clearly dominance of secondary education (eso). mean state anxiety before was 68 and after 56. stai state (before / after) with a significance level of p <0'027, so the null hypothesis was accepted. it was observed that in patients not taking psychotropic drugs, the decrease in anxiety was significant at p <0'036. conclusions: workshops in relaxation techniques are helpful in reducing anxiety in specialized care, and are more effective in patients with anxiety who do not take medication.

Abstract: the recurrent image of the broken or divided sword is studied in the amadís de gaula. since the two-part breakage or division must be taken as a stylistic rule or a formulistic cliché within the amadís, we consider the only two cases of a three-part broken sword as a stylistic deviation of important consequences at the semantic level. later on, we study these two cases in the light of traditional and medieval number symbolism and in their relationships with narrative contexts of any episode in which they occur, and so we bring forward new elements in order to support the thesis of a happy end for the up to now primitive lost ur-amadís.
La intervención quirúrgica: Rito de paso en ambiente hospitalario

Abstract: the ritual is to analyze belongs to the field of health / disease. this is a surgical intervention, an act everyday without any apparent significance beyond the purely technique. the history of surgery is as old as man. in civilizations like egyptian or pre-columbian find evidence of interventions including skull. for lain entralgo "cranial trepanation executed from the palaeolithic in the most diverse places on the planet was undertaken with two different purposes, sometimes combined with each other: the surgical and magic." this ritual takes place today in a hospital setting, which is in line with the model defined total institution goffman. the process takes place at the hospital del oriente de asturias, whose job as a surgical nurse block for six years, which provides a participant observation of the same. the main contribution of this paper is its ability to uncover the meanings of surgical procedure from an anthropologic perspective. this may serve as the basis for applying critical thinking and asking about our professional actions because things mean more than they seem.

Abstract: in the viii conference of the north american nursing diagnosis association (nanda) in 1988 is presented to the world health organization (who), a specific version of a diagnostic taxonomy nurse in order to be included in the revision of the international classification of diseases (icd-10) as an appropriate system for classifying nursing practice. based on the recommendations of the who itself are produced various documents relating to the integration of nursing diagnoses and interventions in professional practice. these actions allow a part of a spread of a common language and the profession itself, and other systematic work. this will incorporate the different nursing diagnoses used in the nursing specialty of labour, in each case identifying the interventions that could make a career commensurate with their skills and enabling the development of a profession based on scientific evidence.
