Prevalência e correlatos de doen?a vascular no exame de ultrassom em pacientes em hemodiálise

Abstract: background: patients on hemodialysis present an increased risk of cardiovascular death. intimal media thickness (imt) and presence of arterial calcifications are well-known risk factors for cardiovascular death in hemodialysis patients. objective: to assess the prevalence of imt and arterial calcifications in hd patients and to correlate image findings with clinical and laboratory data. methods: cross-sectional study involving 75 patients on dialysis for >12 months. patients underwent b-mode ultrasound scan (us) for determination of imt of the distal third of the common carotid arteries. arterial calcifications were assessed by us of carotids, femoral and tibial arteries, and labeled positive if calcification was found in any arterial site. results: patients were 52±13 years old, 57% were males and 16% were diabetics. imt > 0.9 mm was found in 57% of cases and arterial calcifications at us in 48%. aging (decades) and smoking were associated with both increased imt (adjusted odds ratio [aor] = 3.4, p < 0.001; aor = 4.4, p = 0.045, respectively) and presence of vascular calcifications (aor = 3.0, p < 0.001; aor = 6.8, p = 0.011, respectively). high intact parathyroid hormone levels (per each 100 pg/ml) were significantly associated with increased imt (aor = 1.7, p = 0.021), but not with vascular calcification. in contrast, diabetes and time on dialysis (years) were significant determinants for calcifications at us (aor = 15.0, p = 0.009; aor = 1.39, p = 0.020), but not for increased imt. conclusion: increased imt and calcifications at us are common findings in hemodialysis patients. aging and smoking are consistent determinants for both image alterations. parathyroid hormone elevation is associated with increased imt. diabetes and time on dialysis substantially increase the risk for arterial calcification.

Abstract: Henoch-Schönlein purpura is a vasculitis of small vessels, characterized by tissue deposition of IgA, affecting predominantly the skin, intestine, joints and kidneys. It mainly affects the pediatric age group, especially from 2 to 11 years old. Most often, it is preceded by episode of upper airway infection from days to weeks. The most common clinical manifestations are purpuric cutaneous lesions located in the lower limbs and buttocks, abdominal pain in colic, nausea, vomiting, diarrhea, in addition to arthralgia/arthritis, usually with spontaneous resolution. Renal impairment usually manifests with hematuria and proteinuria, and renal function may also be impaired. The case described refers to a female patient, 86 years old, with a previous history of infection of the upper airways, followed by purpura in the lower limbs and renal impairment: serum creatinine 3.1 mg/dL (N 0.5 - 1.2 mg/dL); glomerular filtration rate 15 ml/min (N > 60 ml/min); urea 118 mg/dL (N < 40 g/dL); proteinuria 4.497 mg/24h. The present report aims to draw attention to the occurrence of Henoch-Schönlein purpura in an atypical age group, with significant renal impairment, improvement only with conservative treatment, thus avoiding the use of immunosuppressants and the risks of immunosuppression and toxicity related to such drugs.
An action research model for the management of change in continuing professional distance education

Abstract: This paper proposes an action research model as basis for the management of change in continuing professional distance education (CPDE). The model proposed emerged from the need to manage a complex change process from traditional paper-based distance education to e-learning. In order to illustrate and support the model proposed, this paper describes and discusses such a change process in a CPDE Masters programme.The Educational Management Action Research (EMAR) model conjugates pedagogical thinking, curriculum design and organisational context. This model facilitates dialogue of all parties engaged in the design and delivery of CPDE. This enables educational managers to effectively lead change in their courses.

Abstract: In this paper we reflect on experiences of delivering a database design module at Sheffield University and discuss some of the issues and problems relating to teaching, learning and assessment in this area. In particular, the paper discusses the tensions that are experienced when delivering a module to a mixed group presenting a diversity of backgrounds, ranging from the technologically sophisticated to the ICT-na ve. This situation makes life difficult for those who facilitate learning, since they must walk the tightrope between superficiality and technological impenetrability.

Abstract: For a long time, informal communication has played an important role in the process of creating and sharing scholarly knowledge. However, only a handful of initiatives have seriously attempted to exploit the advantages of informal communication processes as a complement to the classic mechanisms of scholarly communication. This article describes the developments that resulted from the DSpace-dev@University of Minho project, particularly, the ones that focus the Commenting add-on: an extension developed for the DSpace platform which aims at providing Institutional Repositories with informal communication capacities. The article also alludes to some ideas that can be taken as the basis for future developments about this subject, namely: to transform the current add-on into a cross-repository and cross-platform service; to make each comment have its own descriptive metadata in order to facilitate its discovery by search engines; and to improve significantly its graphical user interface.
Hypermedia as an experiential learning tool: a theoretical model

Abstract: The process of methodical design and development is of extreme importance in the production of educational software. However, this process will only be effective, if it is based on a theoretical model that explicitly defines what educational approach is being used and how specific features of the technology can best support it. This paper proposes a theoretical model of how hypermedia can be used as an experiential learning tool. The development of the model was based on a experiential learning approach and simultaneously aims at minimising the inherent problems of hypermedia as the underlying support technology.
Predictors of Cognitive Decline in the Early Stages of Parkinson's Disease: A Brief Cognitive Assessment Longitudinal Study

Abstract: Our objectives were to perform a longitudinal assessment of mental status in early stage Parkinson’s disease (PD) patients, with brief neuropsychological tests, in order to find predictive factors for cognitive decline. Sixty-one, early stage, and nondemented patients were assessed twice, over a 2-year interval, with a global cognitive test (mini-mental state examination (MMSE)) and a frontal function test (frontal assessment battery (FAB)) and motor function scales. Dementia and hallucinations were diagnosed according to the DSM-IV criteria. Cognitive function scores did not decrease significantly, except for FAB lexical fluency score. Four patients presented with dementia at followup. The MMSE score below cut-off, worse gait dysfunction, the nontremor motor subtype, and hallucinations were significantly related to dementia. Rigidity and speech dysfunction were related to dementia and a decrease in FAB scores. We can conclude that decline in the MMSE and FAB scores is small and heterogeneous in the early stages of PD. Scores below cut-off in the MMSE could be helpful to predict dementia. Nontremor motor deficits could be predictive factors for frontal cognitive decline and dementia. 1. Introduction Parkinson’s disease is a movement disorder, defined by a combination of tremor, rigidity, bradykinesia, and gait disturbances [1]. Lately, a constellation of nonmotor symptoms has also been described [2]. Cognitive dysfunction, which can ultimately lead to dementia in a great number of cases [3], is a cause of great incapacity in PD. As therapeutic alternatives develop [4], the need for an early detection of cognitive deficits and for accurate prediction of cognitive outcome increases. Brief cognitive tests could be useful for a rapid screening of patients at higher risk for cognitive decline. They also could be of use for following cognitive decline and predicting cognitive outcome. Noncognitive symptoms at the baseline, like motor dysfunction severity, or specific motor symptoms could also be useful as cognitive outcome predictors. Several studies have defined significant clinical heterogeneity at disease onset [5, 6] which could determine prognosis. In previous work, we found that early stage, nondemented PD patients presented with significantly lower scores in the frontal assessment battery (FAB) and the mini-mental state examination (MMSE), when compared to non-PD aged controls, and that MMSE scores were related to nontremor motor scores [7].
