Tratamento da doen?a hemorroidária com ligadura elástica: estudo prospectivo com 59 pacientes

Abstract: rubber band ligation (rb) is considered a minimally invasive method for the treatment of hemorrhoidal disease (hd) and has advantages in relation to hemorrhoidectomy whereby: simplicity of execution, outpatient realization and no need of anesthesia. it is an effective method, especially in the hd grade ii. however, shows complications, and the most frequents are: anal pain, tenesmus, hematochezia and urinary retention. some studies have shown severe complications as major bleeding that needs blood transfusion. thus, this study evaluated the effectiveness and the morbidity of the treatment of hd by the method of rb. it was a prospective study with 59 patients. five (8,5%) patients had hd graded in the first degree, 33 (55,9%) in the second degree and 21 (35,6%) in the third. all patients were submitted to at least two sessions. in the 135 sessions performed, we found: hematochezia in 62 (45.9%), severe pain in 39 (28.9%), vagal symptoms in 10 (7.4%) and pseudostrangulation in 1 (0.7%) session. the cure rate of hemorrhoidal prolapse among patients with hd grade ii was 87.9% and among those with hd grade iii, 76.2%. the treatment of hd by the method of rb proved to be safe and with good cure rate.
Somos uma comunidade de prática? Are we a community of practice?

Abstract: Este artigo analisa a caracteriza o de um possível caso de CdP, em compara o com outros tipos de arranjos grupais presentes nas organiza es, de acordo com a defini o fundadora de Ettiene Wenger. Trata-se de um estudo de caso sobre uma empresa de consultoria organizacional que há mais de 10 anos realizava - e publicava em livro - uma pesquisa em que fazia a intrigante pergunta: "somos uma comunidade de prática?". Uma década depois, voltou-se a essa empresa para fazer às pessoas que com ela se relacionam a mesma pergunta (e outras mais), que pudessem contribuir para a identifica o mais precisa das CdPs. Na conclus o este artigo revela que os entrevistados, mesmo identificando em sua experiência profissional diversos elementos que s o típicos das CdPs, n o se consideram efetivamente uma CdP. Diante de tal negativa, fica a pergunta: será possível realmente a existência de comunidades de prática em organiza es da produ o e do trabalho? In spite of the growing number of discussions about communities of practice in organizational theory, there have been still few academic studies on this subject in Brazil. As empirical basis, this paper takes the example of a consulting firm that had conducted and published a research on communities of practice, 10 years earlier, and had asked itself: "are we a community of practice?" The author addressed people still working at that company, asking them the same question (and others) to improve the understanding on the meaning of a community of practice. In conclusion, the research shows that the interviewees, even identifying in their professional experience typical elements of the communities of practice, do not consider the organization, in fact, a community of practice. At last, the question remains: are there really communities of practice in production and work organizations?

Abstract: Este artigo analisa fragilidades conceituais presentes nos manuais de Teorias Administrativas, usados como referência primária nas constru es dos significados básicos da Administra o, com os alunos dos cursos de gradua o das maiores e mais importantes Institui es de Ensino Superior (IES) brasileiras. Em sentido amplo, sua discuss o situa-se no debate internacional sobre a crise do atual modelo de ensino de Administra o, surgido nos anos 1990, e que p e em dúvida o sentido e a finalidade da chamada management education. Em sentido estrito, seu interesse analítico essencial s o os problemas oriundos do uso (e abuso) de metáforas no discurso científico como apresentado em manuais didáticos introdutórios das teorias administrativas. Sua perspectiva teórica, contrapondo-se a uma concep o de significado na qual o discurso científico seria o espa o de uma literalidade plena, apoia-se em discuss es no ambito da constru o discursiva das ciências para reconhecer a presen a inevitável e n o necessariamente danosa das metáforas no discurso científico. Neste sentido, ele assume a metáfora como um fen meno linguístico-cognitivo de natureza semantico-pragmática. Ao analisar os manuais de teorias administrativas, esta pesquisa confirmou sua hipótese de que a literatura de forma o profissional em Administra o, usada na educa o dos futuros administradores por todo o País, é marcada por uma série de fragilidades conceituais básicas que, ao simplicizarem a complexidade dos fen menos organizacionais, resultam em um empobrecimento conceitual pelo uso (ou abuso) metafórico. E conclui que tais manuais, pretendendo facilitar o entendimento dos conceitos e tornar sua leitura acessível e agradável, abrem m o da complexidade e da riqueza explicativa dos conceitos originalmente elaborados em pesquisas científicas no próprio campo da Administra o, terminando por traduzirem-se no que aqui poderíamos chamar de uma Pop Science.

Abstract: Este artigo discute algumas quest es sobre formalidade-informalidade a partir da abordagem da antropóloga-lingüista Judith Irvine. O artigo analisa uma comunidade de prática (CdP) de consultores organizacionais, estudada empiricamente por meio da observa o participante. O objetivo principal é tratar das dificuldades encontradas na observa o, entendidas no texto a partir do que denominamos, com base em Irvine, de implica es metodológicas das "(in)formalidades" à pesquisa. O artigo conclui que os aspectos em que se desdobram tais "(in)formalidades" marcam profundamente o discurso em análise e devem ser notados, n o para que se possa esterilizar o discurso desses elementos (como se isso fosse possível e como se tais elementos fossem indesejáveis e/ou extrínsecos), mas para que essas dimens es de análise estendam o foco do código à situa o, e desta àquele, enriquecendo o trato social do fen meno lingüístico e a competência de observa o do pesquisador. This article discusses a few issues on formality-informality based on linguistic anthropologist Judith Irvine's approach. It uses participant observation to analyze an organizational consultants' community of practice (CoP). The main objective is to deal with the difficulties found during the observation, which, based on Irvine, are called the (in)formalities methodological implications for the research. It concludes that the different aspects of such (in)formalities have a deep effect on the discourse under analysis and must be noticed not so much as to sterilize these elements' discourse (as if this was possible and as though those elements were undesirable and/or extrinsic), but so that these analytical dimensions extend the focus of the code to the situation - and vice-versa - enriching the social approach of the linguistic phenomenon and the researcher's observational capabilities.

Abstract: anus neoplasm accounts for 2 to 4% of colorectal tumors, being more prevalent around the seventh and the eighth decades. females are mostly affected, and the ratio is 3:1. its increased prevalence amongst the population in the past years is probably related to the higher number of people that are affected by sexually transmitted diseases, mainly human papillomavirus (types 16 and 18, mostly) and/or the human immunodeficiency virus. diagnosis is based on clinical findings and anatomopathological tests. the treatment of choice is radiochemotherapy, and the rescue surgery with abdominoperineal resection is used for recurrence and persistence cases. a retrospective and prospective longitudinal observational study was performed with 11 patients diagnosed with anal neoplasm from 2004 to 2010. six (54.5%) were females and five (45.5%) were males. the incidence was higher in the sixth decade, at the mean age of 54.45 years. the most frequent histological type observed was the epidermoid carcinoma, and the most frequent cell differentiation type was the moderately differentiated. chemotharapy associated with radiotherapy was used in 81.9% of the patients, and abdominoperineal resection was necessary as a rescue surgery in 18.2% of the patients.

Abstract: the objective of this work was to evaluate the growth of green pepper cv. atlantis under different spatial arrangements. three spacing arrangements between double and simple planting rows (1.5x0.5 m, 1.6x0.4 m and 1.7x0.3 m) and four spacing distances between plants in the rows (0.2, 0.3, 0.4 and 0.5 m) combined in factorial scheme were evaluated. a randomized block design in split-plots divided in time with three replicates was used. the growth assessment was done in nine occasions, 14 days apart, and the first one was performed 14 days after transplanting (dat). leaf area (la), leaf area index (lai), dry masses of leaves (ldm), stem (sdm), fruit (frdm) and total shoot (tdm); absolute growth rate (agr), rates of net assimilation (nar) and relative growth (rgr); and leaf area (lar) and leaf mass (lwr) ratios were evaluated until 126 dat. the changes in la, rgr, lwr and lar did not depend on row spacing, but spacing influenced ldm, sdm, frdm and tdm, lai and agr. the increase in spacing reduces lai and lar, and increases la, ldm, sdm, frdm, tdm, agr and nar, but does not influence rgr and lwr.

Abstract: what can you do when a scholar makes use of one of the course evaluation instruments to criticize the methodology adopted by the professor? suddenly, i wrote on the student's text and before give it back to him, i shared critique and a reply with a fellow professor (also author of this article). coincidentally, we had already attended a pedagogic course in our university. it is a great moment for us to articulate reflections awaked by felipe's critique with readings, debates and views shared during this (and over this) course. starting from one experience of teaching-learning, we present here our view about education, about our role in this process and a theoretical basis supported by humberto maturana and francisco varela's biology of knowing (biocog) theory. this experience allowed us to restructure the methodology applied in an organization analysis's discipline of a management course - our last objective.

Abstract: in four districts of the city of rio dc janeiro focal treatment essays with granulated abate at 1 ppm and perifocal treatment essays with wettable powder of sumithion at 2,5% were performed. these were made either alone or in combination as well as associated to insecticides applied at ultra low volume. the premise indices obtained one month after the treatments indicates that the focal treatment alone is effective, no other addicional methods being necessary. the perifocal treatment is not effective and did not prevent the development of aedes aegypti larvae and other insects in tires which had their external surface painted with sumithion and malathion.

Abstract: neuropsychiatric manifestations, especially depression, anxiety and psychosis, are common in systemic lupus erythmatosus (sle). the psychological stress and the use of corticoids have been blamed for these psychopathological manifestations. a literature review was realized on the efficacy of psychological interventions in the treatment of sle, using research data bases, through the descriptors "psychotherapy" and "lupus", including randomized clinical trials and prospective studies. six articles were found, of these, four were randomized clinical trials, and two prospective studies. five articles related evidence of pronounced improvement in patients that had psychological follow-up and one article did not found such evidence. psychological intervention can be an important tool in the treatment of sle.

Abstract: this article aims to analyze the work of the community agents of health linked to two government programs: the program of community agents of health and the program health of the family. the agents integrate workers' teams and their function are to transmit information about hygiene and health in the communities where they act, to lift data on incidence of diseases in the population and to direct the patients for service in the unit of health of the family. for that they should be residents of the community, to do the link between community and the team. they are constituted in an atypical worker, be for the characteristics of the labor agreements (temporary, outsourcing, informal), be for the ownership of a social capital like demanded competence. in other words, a relational characteristic as condition to exercise the occupation. the work of the agents reflects the new forms of implementation of social policies decentralized by the state and the new forms of manpower use, out of the picture of the traditional civil servants. the research consisted from survey applied to 53 community agents of health in twelve teams in jo?o pessoa, campina grande, casserengue and cacimba de dentro; interviews with syndical leaderships, nurses, practitioners and state health managers.
