Altera??o de mobilidade de prega vocal unilateral: avalia??o subjetiva e objetiva da voz nos momentos pré e pós-fonoterapia

Abstract: purpose: this study aims to analyze the objective and subjective evaluation of voice quality in a unilateral vocal fold mobility alteration on prior and post speech therapy. methods: this is a retrospective study through chart review, which analyzed the recorded voices of 12 individuals with otorhinolaringology diagnosis of unilateral vocal fold paralysis. the collected material was the sustained emission of the vowel /a/, and then connected speech. the voices before and after therapy were analyzed by using grbasi scale. the parameters for spectrographic analysis were: shape of the spectrographic tracing, degree of browning of harmonics, stability of the tracing of harmonics, presence of noise, presence of harmonics, presence of sub-harmonics and harmonics definition. the measurement of the maximum phonation time for the vowel /a/ represented the highest value among three emissions. the obtained data were submitted to statistical analysis through wilcoxon test ranking. results: in the perceptual analysis, the parameter that has changed in the post-treatment was breathiness (b) (p=0.003), followed by dysphonia grade (g) (p=0.004) and asthenia (a) (p=0,01), and those results were statistically significant. as for the spectrogram, we found an improvement of the tracing in 91% of patients, and the changed parameters were: increase in number of harmonics (32%) and reduction in noise (24%). measurement of the maximum phonation time of the vowel /a/ was higher in the post-speech therapy (p = 0.003). conclusion: speech therapy treatment of unilateral vocal fold paralysis improved perceptual-hearing and spectrographic data and the maximum phonation time of voice.

Abstract: OBJETIVO: avaliar de forma subjetiva e objetiva a voz de pacientes com paralisia unilateral de prega vocal nos momentos pré-tratamento e pós-tratamento. MéTODOS: trata-se de um estudo retrospectivo por meio de revis o de prontuário, que analisou as grava es de vozes de 12 indivíduos com diagnóstico otorrinolaringológico de paralisia unilateral de prega vocal. O material de voz colhido foi a emiss o sustentada da vogal /a/, seguida de fala encadeada. As vozes pré e pós-terapia foram analisadas por meio da escala GRBASI, análise espectrográfica e medida do tempo máximo de fona o (TMF). Os parametros para análise espectrográfica foram: forma do tra ado, grau de escurecimento dos harm nicos, continuidade do tra ado, presen a de ruídos, presen a de sub-harm nicos e harm nicos definidos. A medida do TMF da vogal /a/ representou a maior de três emiss es. Os dados obtidos foram submetidos a análise descritiva de tendência central e dispers o, e ao Teste Wilcoxon. RESULTADOS: na análise perceptivo-auditiva, o parametro que mais se modificou no momento pós- tratamento foi o de soprosidade (B) (p=0,003), seguido do grau da disfonia (G) (p=0,004) e astenia (A) (p=0,01), sendo que estes resultados foram estatisticamente significantes. Com rela o ao espectrograma, houve melhora do tra ado em 91% dos pacientes, e os parametros que mais se modificaram foram: aumento do número de harm nicos (32%) e diminui o do ruído (24%). A medida do TMF da vogal /a/ apresentou-se significantemente maior no momento pós-fonoterapia (p=0,003%). CONCLUS O: pacientes com paralisia de prega vocal que foram submetidos ao tratamento fonoaudiológico apresentaram melhora dos dados perceptivo-auditivos, espectrográfico e do TMF. Abstract: This article analyzes the main challenges involved in the ethical review of social research projects that use qualitative techniques for gathering data. The Brazilian ethical review system was constructed having biomedical sciences as the main reference. This article goes over some of the main points of ethical tension in social research by discussing five classic cases in human and social sciences. It supports the possibility of social research being included in the current ethical review system, as long as there is a sensitization of the ethics committees regarding the methodological particularities imposed by qualitative techniques. Finally, the need for specific ethical guidelines for social research is also considered.

Abstract: purpose: to compare the electroglottographical evaluation results in women with otorhinolaryngological reports of vocal nodules and presence of glottal gap and in women with no larynx alteration. method: 20 women took part in this study, 10 with otorhinolaryngological exam of vocal nodule, associated with glottal gap, and 10 with voices evaluated in the perceptive-audio analysis as neutral and electroglottographic evaluation without larynx alteration. the age group varied from 27 to 55 year old, with average of 37.60 for the study group and 36.00 for the control group. the voice material was the habitual emissions of the sustained vowel /a/. we extracted the measures of quotient of contact, fundamental frequency, jitter, as well as the sorting and interpretation of the kinds of waves and presence of electroglottographic knee. the statistical analysis has been accomplished by the fisher exact test and t of student. results: all traces were sorted as type ii (100%). the difference in the presence or absence of knees between groups was not observed. the average of quotient of contact was 42.52 for the study group, and 45.56 for the control group. in spite of the minor value for the studied group, there has been no significant difference between groups for the fundamental frequency and jitter. the average of the fundamental frequency was 193.18 and 211.69 and, the average value of jitter was1.21 and 2.9 for the study and control group, respectively. conclusion: women with mass lesion in the vocal folds show higher jitter values and minor values as for the electroglottographic fundamental frequency.

Abstract: purpose: to compare the measurement of nasal air escape, carried out using the glatzel mirror, in children with and without upper respiratory tract obstruction, according to sex and age. methods: the sample comprised 43 subjects, with mean age of 6.9 years, divided into four groups: children with surgical indication to clearing of the upper respiratory tract (surgical group), allergic children with upper respiratory tract obstruction (allergic group), allergic children with obstruction and surgical indication (surgical and allergic group), and children without obstruction (control group). nasal air escape measurement was carried out using the altmann's millimetered nasal mirror, and data was measured in cm2 by the autocad 2002 software. data was analyzed using descriptive statistics and hypotheses testing, with a significance level of 5%. results: the control group presented the highest mean (6.53 cm2) of nasal air escape, while surgical and allergic group presented the lowest mean (3.07 cm2). the means of the allergic group and the surgical group were 4.74 cm2 and 5.91 cm2, respectively. only the surgical and allergic group presented statistically significant difference compared to the control group. conclusion: the glatzel mirror demonstrated not to be a trustworthy instrument for the evaluation of nasal permeability in the analyzed sample, except in cases of severe nasal obstruction, that is, allergic patients with surgical indication. no correlations were found with the variables sex and age.

Abstract: the most common mutational voice disorder is the mutational falsetto. the voices of four young male patients with mutational falsetto were acoustically and auditory-perceptively analyzed in pre- and post-therapy situations to verify the result of speech-language intervention. for data collection, the patients were asked to say the days of the week. four speech-language pathologists performed auditory assessment of these emissions using auditory-perceptual parameters: overall grade of dysphonia, roughness, breathiness, strain, instability, and pitch. the emissions of each patient corresponding to pre- and post-therapy were randomly presented to the evaluators, who did not have prior knowledge of the condition being analyzed. the assessment was performed by comparison, and the evaluators should inform whether the second emission was better, worse or unchanged when compared to the first. when change was observed, they should select three auditory-perceptual parameters regarding the expressive modification. the evaluators were informed only about subjects' age and gender. mean, maximum and minimum fundamental frequencies were extracted from each emission of the patients using the praat program (version 5132). semitone values were also extracted and analyzed. data was statistically analyzed. in the post-therapy condition, the evaluators found an improvement in the vocal quality of all patients, as well as a decrease in the mean, minimum and maximum fundamental frequencies and in the semitones, resulting in a deeper and more stable vocal pattern.

Abstract: OBJETIVO: investigar a ades o a orienta es fonoaudiológicas de professores da rede municipal de ensino que foram atendidos no Ambulatório de Voz do Hospital das Clínicas da Universidade Federal de Minas Gerais e que receberam alta da fonoterapia. MéTODO: foi realizada entrevista por meio de telefone a 39 pacientes atendidos no período de agosto de 2007 a dezembro de 2008 no Ambulatório de Voz do Hospital das Clínicas da Universidade Federal de Minas Gerais. A entrevista visou analisar a ades o a orienta es após alta fonoterápica. RESULTADOS: os resultados desta pesquisa apontam que 34 pacientes (87,1%) referem seguir as orienta es fonoaudiológicas, quatro (10,2%) seguem parcialmente e um (2,5%) refere n o seguir. CONCLUS O: os professores, em sua maioria, aderem às orienta es fonoaudiológicas após alta fonoterápica. Os resultados sugerem que a terapia fonoaudiológica favorece a manuten o de um comportamento vocal saudável. PURPOSE: to investigate the adherence to voice therapy guidelines of teachers in municipal schools that were treated at the Voice Clinic of Hospital das Clínicas, Federal University of Minas Gerais, and that were discharged from voice therapy. METHOD: 39 patients treated at the Voice Clinic of Hospital das Clínicas, Federal University of Minas Gerais were interviewed by phone from August 2007 to December 2008. The interviews aimed to analyze the adherence to the guidelines after voice therapy discharge. RESULTS: this study outcome indicates that 34 patients (87.1%) report following the guidelines, four (10.2%) do it partially and one (2.5%) referred not following the guidelines. CONCLUSION: the majority of teachers adhere to the guidelines after voice therapy discharge. The outcome suggests that the voice therapy helps to maintain the standard of voice and a healthy vocal behavior.

Abstract: OBJETIVO: comparar os resultados da avalia o eletroglotográfica entre mulheres adultas com laudo otorrinolaringológico de nódulos vocais e presen a de fenda glótica com mulheres sem altera es laríngeas. MéTODO: trata-se de 20 mulheres, sendo 10 com exame otorrinolaringológico de nódulo vocal, associado à fenda glótica, e 10 com vozes avaliadas na análise perceptivo-auditiva como neutras e avalia o otorrinolaringológica sem altera o laríngea. A faixa etária variou de 27 a 55 anos, com média de 37,60 e 36,00 para grupo estudo e controle, respectivamente. O material de voz foi a emiss o habitual da vogal sustentada /a/. Foram extraídas as medidas de quociente de contato, frequência fundamental, jitter, bem como a classifica o e interpreta o dos tipos de ondas e presen a do joelho eletroglotográfico. A análise estatística foi realizada pelo teste Exato de Fisher e T de Student. RESULTADOS: todos os tra ados foram classificados como tipo II (100%). N o foi observada diferen a na presen a ou ausência de joelho entre grupos. A média do quociente de contato foi de 42,52 e 45,56 para grupo estudo e controle, respectivamente. Apesar do menor valor para o grupo estudado, n o houve diferen a significante entre grupos. Houve rela o estatisticamente significante entre grupos no que se refere à frequência fundamental e jitter. A média da frequência fundamental foi 193,18 e 211,69 e o valor médio de jitter foi 1,21 e 2,9 para o grupo estudo e controle, respectivamente. CONCLUS O: mulheres com les o de massa nas pregas vocais apresentam maiores valores de jitter e menores valores para frequência fundamental eletroglotográfica. Abstract: OBJETIVO: avaliar de forma objetiva e subjetiva a voz de pacientes com disfonia espasmódica nos momentos pré e pós aplica o de toxina botulínica A. MéTODO: as emiss es vocais de onze pacientes do sexo feminino foram registradas antes e após (15 dias) o tratamento. As amostras vocais foram analisadas por duas fonoaudiólogas com experiência em voz por meio da análise perceptivo-auditiva (escala GRBASI) e da análise espectrográfica. RESULTADOS: na análise perceptivo-auditiva com vogal sustentada os parametros que alteraram após o tratamento foram o grau de severidade, tens o e instabilidade, enquanto na fala encadeada foram o grau de severidade e a tens o. Na análise espectrográfica ocorreu melhora do tra ado após o tratamento sem significancia estatística entre os parametros. CONCLUS O: ocorreu melhora significante dos aspectos perceptivo-auditivos após o tratamento e, portanto, as inje es de toxina botulínica A mostraram-se eficazes no tratamento da disfonia espasmódica no grupo estudado. PURPOSE: to analyze in an objective and subjective manner the voice of patients with spasmodic dysphonia in the moments before and after botulinum toxin A. METHOD: the vocal emissions of eleven women patients were recorded before and after (15 days) treatment. The vocal samples were analyzed by two experienced speech therapists through the perceptual analysis (GRBASI scale) and spectrographic analysis. RESULTS: in the perceptual analysis with subtended vowel, the altered parameters were degree of severity, strain and instability, while in connected speech only degree of severity and strain changed after treatment. In the perceptual analysis with sustained vowel, the parameters that have changed, were the degree of severity, strain and instability, while in connected speech only degree of severity and strain changed after treatment. The spectrographic analysis was improved after treatment with no statistical significance found among the parameters. CONCLUSION: there was significant improvement of the vocal analysis after treatment and, therefore, injections of botulinum toxin A were effective in the treatment of spasmodic dysphonia in this group.
