Abstract: this work reports the experience of the intensive care program directed to psychotic patients in salvador. during four years and a half, 40 patients were followed by interns and furthermore 111 professionals were trained in mental health care based on the great experience gained with the program. the program included home care, therapeutic assistance (ta), living spaces, family support and support to individual needs, all this conducted by two supervisors, who assisted the interns using multiple theoretical references in the interpretation and intervention with individuals who have, in general, a precarious possibility of dealing with the situations due to serious mental, social and familiar disorganization. the results of the project can be measured as a pedagogical experiment that has an impact on the plans of formation and assistance in mental health. the program prompted the theoretical production of the trainees what led, in the university, to the creation of the laboratory of studies of interpersonal relationships and mental health. the professional growth of the interns reflected their great social recognition and different qualification when working with mental health as professionals. in the care plan, a large improvement in the quality of life of patients with significant exemption from the hospital as a means of treatment was observed. this study points to the need of building spaces of practice that bet and encourage independence, creativity and ethical commitment of the students.

Abstract: Este trabalho relata a realiza o de uma experiência de estágio em uma Unidade de Saúde da Família em Santa Maria-RS. O objetivo foi analisar a prática da Psicologia comunitária e as suas estratégias de facilita o em comunidades inseridas em uma unidade de saúde pública. Participaram dessa experiência usuários atendidos pela equipe multidisciplinar que haviam sido encaminhados para o servi o de Psicologia. No contexto estudado, as estratégias utilizadas foram entrevistas, visitas domiciliares, conversas informais e intera o nos espa os comunitários. Essas estratégias se mostraram importantes para o mapeamento da realidade comunitária, das redes de servi os e para verificar como estas funcionam. No que se refere às quest es éticas, observou-se a importancia de romper os padr es de atendimento psicológico tradicional, quando se constrói uma ética que abrange os princípios da interdisciplinaridade. Observou-se que estratégias de atua o em comunidade na perspectiva da Psicologia comunitária podem favorecer a es mais integradas às necessidades da popula o atendida pela ESF. Tal experiência apontou a importancia do questionamento sobre os modelos de atua o prestados pela Psicologia nos servi os de saúde pública, se for discutida uma proposta de atua o que esteja comprometida com o fortalecimento e o engajamento comunitário para o exercício e a garantia da cidadania dos usuários. Este trabajo relata la realización de una experiencia de pasantía en una Unidad de Salud de la Familia en Santa María - RS. El objetivo ha sido el de analizar la práctica de la Psicología comunitaria y sus estrategias de facilitación en comunidades insertadas en una unidad de salud pública. Han participado de esa experiencia usuarios atendidos por el equipo multidisciplinario que habían sido dirigidos al servicio de Psicología. En el contexto estudiado, las estrategias utilizadas han sido entrevistas, visitas domiciliares, conversaciones informales e interacción en los espacios comunitarios. Esas estrategias se han mostrado importantes para el mapeo de la realidad comunitaria, de las cadenas de servicios y para verificar cómo éstas funcionan. En lo que se refiere a las cuestiones éticas, se ha observado la importancia de romper los estándares de atendimiento psicológico tradicional, cuando se construye una ética que abarca los principios de la interdisciplinaridad. Abstract: the main objective of this case study was to understand the construction of care in the homecare program for bedridden patients (pada) of a basic health unit (ubs) in porto alegre, rs. data were obtained from 13 caretaker groups in the ubs, and participating observation recorded in field diary in the patient's homes. an analysis of the discourse practices was performed inspired in foucault's discourse concepts speeches and in studies on ethics and self-care. in the groups, the caretaker occupied a space that we metaphorically called the delphic oracle, a place for acceptance, listening and support. hearing the dialogues that took place between the team and the caretakers made us face the contradiction present in the institutional discourse, which both stimulates self-care, and imposes rules, duties, tasks. care as a citizenship right opposite to care as submission and subjugation created tension inside the group several times. foucault's notion of care comprises a synthesis of the exercise of a person over his- or herself, making him or her better as a human being and, at the same time, capacitating him or her to become a better citizen

Abstract: this study discusses the creation of a new children palliative care program based on the family health program, considering the level of care at home and yielding to family requests. the study focused on eighteen members of nine families of technology dependent children (tdc) who were hospital patients at instituto fernandes figueira (iff): four who are being assisted by its palliative care program programa de assistência domiciliar interdisciplinar (padi); three who were inpatients waiting for inclusion in the program, and finally two inpatients already included in padi. padi was chosen because it is the only child palliative care program in brazil. the results are positive in regards to the connection established between the families and the health care team, the reception of the children, the explanation to the family concerning the disease, and the functional dynamics between the padi and iff. as negative points, difficulties arose as a result of the implementation of the program, from its continuity to the worsening or illness of the entire family. in conclusion, although the padi is the iff's way of discharging patients, the domiciliary cares taken by the family health program, well articulated with the healthcare system, would be ideal for being the adequate assistance for such.
Preventing child maltreatment: An evidence-based update

Abstract: Child maltreatment is a significant public health problem associated with a broad range of negative outcomes in children and adolescents that can extend into adulthood. This review summarizes information about programs aimed at the prevention of child maltreatment evaluated by controlled trials, with a focus on home visitation programs. It does not include programs aimed at prevention of child sexual abuse, the subject of a separate review in this series. We discuss those programs that include one or more measures of child maltreatment and related outcomes (reports of abuse and neglect, injuries, hospitalizations and emergency room visits). Most programs targeting at-risk families have not shown evidence of effectiveness in preventing abuse or neglect. An important exception is the Nurse Family Partnership (NFP), a program provided by nurses to first-time socially disadvantaged mothers beginning prenatally that has undergone rigorous evaluation in three randomized controlled trials. It has shown consistent effects in reducing reports of maltreatment and associated outcomes as well as additional benefits in maternal and child health in high-risk families. A second exception is the promising Early Start program provided by nurses and social workers to at-risk families beginning postnatally. One randomized controlled trial of the program has shown reduced rates of parental reports of severe abuse and hospital attendance for injuries and poisonings, based on records. The characteristics of the NFP and Early Start programs are discussed with special emphasis on ways in which they differ from other home visitation programs.
Viewpoints of the attendings and medical students about the Students' evaluation of the attendings, Mazandaran

Abstract: Background and purpose: Evaluation is one of the most important bases of promoting the quality of education. It is very important to know the attendings’ and students’ viewpoints as evaluated and evaluator groups. So in this study the viewpoint of the attendings and students of School of Medicine of Mazandaran University of Medical Sciences (MUMS) about the students evaluation of the attendings, were studied.Materials and Methods: In this descriptive-cross sectional study, all faculty members (FM) and students of the school of Medicine were included in the study by census sampling.Data collection was done using a questionnaire including demographic questions and educational variables, the FM and students viewpoint about manner of evaluation (time, the way of distributing the forms and the explanation), the student's features in evaluation (awareness, truthfulness and puposes) and the instrument of evaluation (questionnaires and items). Results: 345 individuals (95 FM and 250 students) took part in this study. The manner of evaluation (time, distribution and explanations) to most of FMs' and students’ opinion was appropriate. About student's awareness and truthfulness in filling in the questionnaires; a considerable percentage of FMs (20-40%) had a negative opinion and over half of them had a negative attitude about the whole process of evaluation. About the items of theoretical, practical and clinical questionnaires, while the students laid stress on aspect such as: being a teacher and concepts transferring, on attendings opinion these aspects were less important. Conclusion: To most FMs and students opinion, the manner of evaluation is appropriate but a considerable negative attitude exists among attendings about students’ awareness and truthfulness in filling in the questionnaire. The priorities of evaluation are different in these two groups’ point if view.
La gestión de úlceras por presión en residencias de mayores: Obstáculos y facilitadores

Abstract: introduction and aims: the pu is a problem that adversely affects the quality of life among the elderly, contributing to morbidity and mortality in the institutionalized population. the heterogeneous reality of care and epidemiological variability of these injuries make it difficult to approach in socio-sanitary institutions. knowing the management of pressure ulcers in nursing homes and obstacles to its proper management, raising strategies to optimize care. method: literature review on the basis of national and international data. results: the adoption and implementation of best practice guidelines for prevention and treatment of pu offers a high variability. obstacles are detected at the managerial and institutions, economic and material costs, training, staff attitudes and motivations, implementation and care. conclusions: the use of programs tailored to the centers, the continued commitment of the institution, comprehensive plans and multidimensional, interdisciplinary training and continuous assessment would be some recommendations for improving care for prevention and treatment of elderly patients with pu.
Home infusion program for Fabry disease: experience with agalsidase alfa in Argentina

Abstract: fabry disease is an x-linked lysosomal storage disorder caused by inherited deficiency of the enzyme α-galactosidase a. enzyme replacement treatment using agalsidase alfa significantly reduces pain, improves cardiac function and quality of life, and slows renal deterioration. nevertheless, it is a life-long treatment which requires regular intravenous infusions and entails a great burden for patients. our objective was to evaluate retrospectively the safety and tolerability of the home infusion of agalsidase alfa in patients with fabry disease in argentina. we evaluated all the patients with fabry disease who received home infusion with agalsidase alfa 0.2 mg/kg between january 2005 and june 2011. the program included 87 patients; 51 males (mean age: 30 years) and 36 females (mean age: 34 years). a total of 5229 infusions (mean: 59 per patient; range: 1-150) were administered. a total of 5 adverse reactions were seen in 5 patients (5.7% of patients and 0.9% of the total number of infusions). all were mild in severity and resolved by reducing the rate of infusion and by using antihistaminics. all these 5 patients were positive for igg antibodies, but none of them presented ige antibodies and none suffered an anaphylactic shock. in our group 18 patients were switched from agalsidase beta to agalsidase alfa without complications. home infusion with agalsidase alfa is safe, well tolerated and is associated to high compliance.

Abstract: home care is an assistance strategy that emphasizes patient autonomy, as well as se/(-care in the domiciliary setting. this strategy involves planning, coordination, and performance of several services. this study is of a descriptive type, contemplating a qualitative approach by means of a bibliographical review. data were analyzed through the content analysis technique, with two categories being identified: home care programs and systematization of care. home care reduces lhe number of clinical complicatíons, additíonal hospffalizatíons, and hospital costs. upon outlining a home care program, objectíves must be established wffh the purpose of effecting the desired changes. when carrying out lhe systematízatíon, nurses must build ties and a parlnership with the patient and his/her family.
Asistencia ventilatoria no invasiva domiciliaria en ni?os: impacto inicial de un programa nacional en Chile

Abstract: introduction: prolonged mechanical ventilation may be provided to children with chronic ventilatory failure as non-invasive ventilatory assistance at home (niva). objective: to describe clinical characteristics, evolution outcomes and health related quality of life (hrqol) of pediatric patients admitted into the chilean niva program. patients and methods: medical files of patients included in the program, were reviewed during a period of 24 months (2006-2008), using follow-up protocols. results: there were a total of 177 children, with an average age of 9.7 ± 4.7 years. fourteen patients died of non-related causes. diagnoses were: neuromuscular disease (nmd) 64%, myelomeningocele 6%, obstructive sleep apnea syndrome 6%, lung disease 20% and miscellaneous 4%. compared to the previous year, hospitalization decreased from 60 to 21% (p < 0.00001) and from 1.3 hospitalizations/patient/year to 0.3 (p < 0.00001). hrqol improved significantly, auquei 17.2 ± 5.6 (p < 0.05), irs 26.4 ± 8.2 points (p < 0.05) in 82 patients. in 21 patients with nmd an average increase of 27 cmh2o on muscular inspiratory pressure (p < 0.05) after respiratory training was observed. conclusions: the niva program reduced hospitalizations and improved hrqol. in a group of patients the muscle strength increased with stable respiratory function.
