Epidemiology, Clinical and Paraclinical Presentations of Pulmonary Embolism: A Cross-Sectional Study in a Sub-Saharan Africa Setting  [PDF]

Abstract: Background: Venous thromboembolic (VTE) disease burden is increasing worldwide, representing a major cause of cardiovascular death and public health problem. Pulmonary embolism (PE) is the most serious clinical presentation of VTE. Epidemiological and clinical data on PE are still lacking in Africa, particularly in Cameroon. This study aimed at determining the clinical features as well as imaging presentation and outcome of pulmonary embolism. Methods: A cross-sectional study was carried out in three hospitals in Douala. We retrospectively reviewed patient records admitted for PE from January 2009 to May 2017. We collected data on epidemiology, clinical presentation, venous Doppler/pulmonary computed tomographic angiography (CTA), and outcome. Results: We included 103 patients (56 males) with PE. Their median age was 52 years. The main risk factors were obesity (49.5%), hypertension (35.0%), long trip (24.3%) and cancer (18.4%). The most frequent clinical presentations were dyspnoea (83.4%), chest pain (78.6%), and cough (40.8%). Additionally, pleural effusion (32.8%), atelectasis (25.7%) and pulmonary hyperlucency (20.0%) were the most frequent findings on chest X-ray. The main abnormalities found in electrocardiogram were sinus tachycardia (63.4%), S1Q3T3 aspect (37.6%) and right axial deviation (28.7%). Moreover, right cavities dilatation (26.0%), pulmonary artery hypertension (35.0%) and pulmonary artery dilatation (21.0%) were the main echocardiographic findings. Low-molecular-weight heparin (LMWH) was the initial anticoagulant in all cases. There were 19 (18.4%) in-hospital deaths, and the mean hospital stay was 8.5 ± 5 days. Conclusion: PE is not rare in our setting. Obesity, long trip and cancer are the main risk factors, while dyspnoea, chest pain and respiratory distress are the main clinical presentation for PE. LMWH remains the therapeutic agent of choice. Lastly, in-hospital mortality is very high.

Abstract: Background: Venous thromboembolism (VTE) is a major cause of morbidity and mortality worldwide. It is also the most common complication in hospitalized patients. Aims:?To?study the in-hospital prevalence of VTE, describe the socio-demographic characteristics of patients, determine the frequency of risk factors, describe the clinical presentations, and determine the short term outcome of VTE in hospitalized patients in a low-income tertiary hospital setting.Methods: We carried out a cross-sectional descriptive retrospective study over a period of 6 years and 4 months (January 2008 to April 2014) in the Douala General HospitalCameroon. Patients were cases of confirmed venous thromboembolic disease (VTE).Results: A total of 78 case files were retained for this study, giving an in-hospital prevalence of 4.4 per 1000 admissions. There were 42 (53.8%) males and 36 (46.1%) females. Their ages ranged from 18 to 89 years (median: 53 years, [IQR: 40?-?61]).There were 37 (47.4%) cases of Deep Vein Thrombosis (DVT), 31 (39.7%) cases of Pulmonary Embolism (PE), and 10 (12.8%) cases of PE associated with DVT (12.8%). The main risk factors were obesity (44.9%), hypertension (37.2%), immobility (20.5%), and long-haul travel (17.9%). The most frequent clinical presentations in PE were dyspnea (80.5%) and chest pain (65.9%). There were 8 (10%) in-hospital deaths. Conclusion: About twelve cases of VTE are seen yearly at the DGH, with an in-hospital mortality of ten percent. Obesity and hypertension were the main risk factors, with dyspnea and chest pain being the main clinical manifestations in PE, and lower limb swelling the main symptom in DVT.

Safety, Handling and Electrical Performances of Bradycardia Leads in Acute Conditions: Results from the FINE Registry  [PDF]

Abstract: Background: Beflex is an active fixation atrial and ventricular lead with a retractable screw; X-Fine is a passive fixation ventricular lead. These two bradycardia lead models were evaluated in the FINE study, an observational prospective trial conducted in France and Spain. Methods: Patients enlisted for pacemaker or defibrillator implants were enrolled. The primary objective was to assess acute dislodgement rates at the 3-month follow-up visit. Safety and electrical performances of the leads were assessed in acute conditions at implant and at the follow-up visit up to three months later. A handling questionnaire was submitted to implanting investigators immediately after implant. Results: A total of 2254 patients were enrolled in 95 centers; investigators implanted 1153 active atrial leads, mainly in the right atrium; 1021 active right ventricular leads, mainly in the septum and 712 passive right ventricular leads, mainly in the apex. After a mean follow-up of 54.9 ± 37.6 days, dislodgement rates were 1.0% and 1.6% for atrial and ventricular active, and 3.2% for ventricular passive leads. No unexpected adverse reactions were observed during the course of the study and the electrical performances at implant and follow-up visits remained within normal ranges. Overall, most investigators (84%) rated leads’ handling as superior (better or best) to what observed with other bradycardia leads. Conclusion: Different bradycardia leads showed a dislodgement rate of 1.0% and 1.6% for atrial and ventricular active leads, and 3.2% for ventricular passive leads, at 3-month follow-up. Acute safety and electrical performances were within expected ranges and very good handling performances were observed.

Abstract: Postmodernist reasoning signifies a move from traditional viewpoint to an accelerated technological terrain, the implication of this diversion is noticeable in the eclectic nature of life in contemporary society; all things become a continual flickering without any recognizable or perpetual presence. As a literary concept, postmodernism fashions the problematic landscape for the appreciation of the disordered nature of the world as it focuses on the dismantling of traditional values and affirmation of a fragmented society. Postmodernism, with its subtle emphasis on capitalism, has greatly exacerbated the post-colonial tendencies in Tanure Ojaide’s God’s Medicine-Men and Other Stories as money becomes a recurrent motif and a chief signifier. The stories underpin the overarching effect of cultural materialism, individuals and the society as everyone is caught in its web; these stories could also be read as the author’s depiction of a society that is going through a cultural flux.

Abstract: The infection by enterotoxigenic Escherichia coli (ETEC) is one on the most important causes of neonatal and post-weaning diarrhea (PWD) in piglets. ETEC causes significant morbidity and mortality, resulting in a large economic loses in the porcine industry. One of the most common ETEC in swine is serotype 0149 which carries the K88 (F4) adhesin that enables the attachment of the bacteria to the intestinal epithelium. ETEC colonizes ileum [1,2], penetrate the epithelium and its pathogenesis is ascribed to the production of different combination of heat-labile (LT) and heat-stable (ST) enterotoxins [3,4].Antibiotic growth promoters (AGPs) have been long used in animal feeding to prevent neonatal and PWD in piglets. The use of AGPs increases the prevalence of bacteria resistant to antibiotics in farm animals and constitutes a potential risk of antibiotic resistance transference to human pathogenic bacteria, following to consumption of animal derived products [5]. The European ban of AGPs in animal production (EC 1831/2003) increased the need to develop new alternatives [6] to control and prevent animal colonization by pathogenic bacteria and somehow guarantee animal welfare and food safety.Probiotics and prebiotics are interesting alternatives to AGP for animal feeding. They are believed to control pathogenic bacteria colonization and to enhance the mucosal immune system, resulting in a decreased pathogenic load and improving animal welfare [7]. The yeast Saccharomyces cerevisiae var. Boulardii (Scb) is a well-known probiotic with proven effects for the treatment and prevention of gastrointestinal diseases (see [8] for review). Typically, between 30-60% of Saccharomyces yeast wall is composed by polysaccharides [9] and specifically mannose and galactose mannans represent respectively more than 50 μg/mg of yeast dry mass [10]. The post-common-envelope, binary central star of the planetary nebula Hen 2-11

Abstract: We present a detailed photometric study of the central star system of the planetary nebula Hen 2-11, selected for study because of its low-ionisation filaments and bipolar morphology - traits which have been strongly linked with central star binarity. Photometric monitoring with NTT-EFOSC2 reveals a highly irradiated, double-eclipsing, post-common-envelope system with a period of 0.609 d. Modelling of the lightcurve indicates that the nebular progenitor is extremely hot, while the secondary in the system is probably a K-type main sequence star. The chemical composition of the nebula is analysed, showing Hen 2-11 to be a medium-excitation non-Type I nebula. A simple photoionisation model is constructed determining abundance ratios of C/O and N/O which would be consistent with the common-envelope cutting short the AGB evolution of the nebular progenitor. The detection of a post-common-envelope binary system at the heart of Hen 2-11 further strengthens the link between binary progeny and the formation of axisymmetric planetary nebulae with patterns of low-ionisation filaments, clearly demonstrating their use as morphological indicators of central star binarity.
NGC 6778: Strengthening the link between extreme abundance discrepancy factors and central star binarity in planetary nebulae

Abstract: We present new optical spectra of the nearby, bright, planetary nebula NGC 6778. The nebula has been known to emit strong recombination lines for more than 40 years but this is the first detailed study of its abundances. Heavy element abundances derived from recombination lines are found to exceed those from collisionally excited lines by a factor of ~20 in an integrated spectrum of the nebula, which is among the largest known abundance discrepancy factors. Spatial analysis of the spectra shows that the abundance discrepancy factor is strongly, centrally peaked, reaching ~40 close to the central star. The central star of NGC 6778 is known to be a short period binary, further strengthening the link between high nebular abundance discrepancy factors and central star binarity.

Abstract: The near-field dynamics of an aircraft wake is studied by means of temporal large-eddy simulations, with and without considering the effect of engine jets. In the absence of jets, the simulations showed the roll-up of the initial vorticity sheet shed by the wing and the occurrence of short-wavelength instability in a pair of primary co-rotating vortices. The main consequence of the instability is the modification of the internal structure of the vortex, compared to the two-dimensional stable behavior. The presence of engine jets affects the roll-up of the vorticity sheet and causes an enlargement of the final merged vortex core compared to the case without jets.

Abstract: Using in CPT a P and T violations we show that the equation of the positron is exactly the same as the one of the electron, on the condition that both the sign of the charge and the electromagnetic potential are changed. As a consequence the velocities are both in direction to the future and the masses are both positive and, in similar experiences, the behaviours of the two particles are the same. These theoretical results are in quite agreements with the experiments of the LEP.
