Abstract: htlv-i is associated with a broad spectrum of manifestations, including tropical spastic paraparesis and adult t-cell leukemia/lymphoma. arnold chiari syndrome is a condition characterized by herniation of the cerebellar tonsils through the foramen magnum. this condition should be suspected in all patients with headache and impaired motor coordination. syringomyelia is a developmental anomaly that leads to the formation of an intramedullary cavity. its clinical presentation is classically characterized by syringomyelic dissociation of sensation, with suspended distribution in the proximal portion of the trunk and upper limbs and preservation in other regions. we report here a case of association of the three diseases, which is rare in clinical practice, illustrating the difficulty in the diagnosis and therapeutic management of these conditions.
Evaluation of laboratory markers of progression of HIV disease to death

Abstract: introduction: one of the important current problems in hiv/aids infection is the establishment of epidemiological and laboratorial prognostic parameters during patient follow-up. this study aimed at analyzing the evolution of laboratory tests: cd4 lymphocyte count, viral load, hemoglobin (hb), aspartate aminotransferase (ast), alanine aminotransferase (alt), and the epidemiological variables sex and age as prognostic factors for survival in progression to death among aids patients. methods: a retrospective study was conducted using analysis of medical records, and prospective 24-month follow-up of patients with hiv/ aids attended at the president vargas hospital outpatient clinic, a reference center in hiv/ aids attendance in the state of maranh?o, brazil. the study analyzed patients aged 10 to 60 years old, who manifested aids and who were not using antiretroviral therapy or had used it for less than 5 years. the chi-square test was used for statistical analysis. results: the sample included 100 patients - 57 were current outpatients, and 43 had died. the variables viral load (p=0.726), alt (p=0.314), sex (p=0.687), and age (p=0.742) were analyzed, and no evidence of association between them and worst prognosis was observed. conclusions: a significant relation was verified between low hb levels (p=0.000) and cd4 (p=0.000) and shorter survival.

Abstract: introduction: visceral leishmaniasis is a serious public health problem that requires global control strategies, especially with respect to factors that may intervene in reducing the incidence of endemicity. in this work, rainfall density and temperature were correlated with the incidence of human cases in an area endemic for leishmaniasis in s?o luis do maranh?o, northeastern brazil. methods: notification of human cases by the national health foundation/regional coordination of maranh?o (funasa/corema) from 2002 to 2010 was used. ecological data (mean temperature and rainfall density) were provided by the meteorological office of state. results: a significant association was verified between the number of vl cases and rainfall rate but not in the analysis concerning mean temperatures. conclusions: these data suggest that the control actions in visceral leishmaniasis should be performed during rainy season in the state of maranh?o, which is in the first half of the year.

Abstract: comparative study regarding igg and ige anti-leishmania as markers of infection and illness among residents of a visceral leishmaniasis visceral leishmaniasis endemic area, s?o luis, ma in the period from may 1999 to may 2000. all the 1,016 individuals younger than 16 years old were tested for the presence of igg by elisa. a total of 174 (17,1%) children revealed a positive igg test and 4 children showed symptoms of classical visceral leishmaniasis during the time of the survey: 85 ige anti-leishmania elisa tests with positivity of 43,5% were realized. in this sample, all 7 children with past-visceral leishmaniasis in the sample were igg positive and 4 (57,1%) were ige positive, even after a 7 year post treatment period. three children with current visceral leishmaniasis were evaluated, and all of them were positive for both tests. the detection of antileishman ige antibodies presented as a good marker for infection by leishmania chagasi in endemic areas but not as a disease marker.
Relationship between fluid-attenuated inversion-recovery (FLAIR) signal intensity and inflammatory mediator's levels in the hippocampus of patients with temporal lobe epilepsy and mesial temporal sclerosis

Abstract: we investigated a relationship between the flair signal found in mesial temporal sclerosis (mts) and inflammation. twenty nine patients were selected through clinical and mri analysis and submitted to cortico-amygdalo-hippocampectomy to seizure control. glutamate, tnfα, il1, nitric oxide (no) levels and immunostaining against il1β and cd45 was performed. control tissues (n=10) were obtained after autopsy of patients without neurological disorders. the glutamate was decreased in the temporal lobe epilepsy (tle) -mts group (p<0.001), suggesting increased release of this neurotransmitter. the il1β and tnfα were increased in the hippocampus (p<0.05) demonstrating an active inflammatory process. a positive linear correlation between flair signal and no and il1β levels and a negative linear correlation between flair signal and glutamate concentration was found. lymphocytes infiltrates were present in hippocampi of tle patients. these data showed an association between hippocampal signal alteration and increased inflammatory markers in tle-mts.

Abstract: ethanol, methanol and water extracts representing mostly native plant species found in the amazon region were prepared, respectively, by maceration, continuous liquid-solid extraction and infusion, followed by evaporation and freeze-drying. the freeze-dried extracts were tested for lethality toward aedes aegypti larvae at test concentrations of 500 mg / ml. in general, methanol extracts exhibited the greatest larvicidal activity. the following 7 methanol extracts of (the parts of) the indicated plant species were the most active, resulting in 100% mortality in a. aegypti larvae: tapura amazonica poepp. (root), piper aduncum l. (leaf and root), p. tuberculatum jacq. (leaf, fruit and branch). and simaba polyphylla (cavalcante) w.w. thomas (branch).
Marijuana Craving Questionnaire (MCQ-SF/Vers?o Brasil): valida??o semantica

Abstract: objective: the aim of this study was to translate and adapt culturally the marijuana craving questionnaire (mcq-sf)10 which evaluates the craving for marijuana in a brazilian sample. method: the marijuana craving questionnaire (mcq-sf) was translated from english to portuguese, administered to 10 subjects, submitted to a brainstorming in a group of three people for individual and verbal reproduction, item by item. back-translation was executed, a translation for the original language, based on first translation and from brainstorming. soon after, it was translated again into portuguese. a committee of specialists analyzed all translations. results: after the committee considerations and a pilot study with 30 subjects, the final version of mcq-sf/vers?o brasil was built. conclusion: the results showed a satisfactory semantic equivalence between versions. the mcq/vers?o brasil can be useful to evaluate the craving for marijuana on the dependents of this substance.

Adapta??o transcultural para o idioma português do Cocaine Craving Questionnaire - Brief

Abstract: background: the evaluation of craving is very important to treatment of cocaine dependence and the cross-cultural adaptations of scales are needed for increase effectiveness in the evaluation and treatment of them. objective: the objective of this study was to translate, to adapt culturally and verify the semantic equivalence of the cocaine craving questionnaire - brief (ccq-brief), which evaluates the craving for cocaine. method: the ccq-brief was translated from english to portuguese, administered to 10 persons, and then submitted to brainstorming in a group of 4 persons for individual and verbal reproduction, item by item. backtranslation was executed based on first translation and from brainstorming to the origin language. soon after, it was translated again into portuguese. all the process was analyzed by a committee of specialists, which emitted a decision and the pertinent comments. results: considering the decision of the specialists and one last pilot study, was constructed of the final brazilian version of the ccq-brief and the version adapted that scale for crack users. discussion: results indicated a satisfactory semantic equivalence between versions and the ccq-brief that may be important in the treatment of the cocaine dependents.

Abstract: introduction: this study presents the development and validation of the inventory of outcome expectancy in marijuana users (inventário de expectativas de resultados em usuários de maconha, ierum), which evaluates the outcome expectancy regarding marijuana use, and may be useful in the treatment and prognosis of marijuana-dependent individuals. method: the sample comprised 181 male (n = 145) and female (n = 36) marijuana users. their age ranged from 18 to 55 years old (28.34±8.57). one hundred and eighteen patients were hospitalized at a chemical dependency unit, 23 were being treated in an outpatient clinic, 25 were receiving treatment in a self-help group (drug addicts anonymous, alcoholic anonymous), and 15 were not being treated. the 17-item scale was developed based on the experience related to chemical dependency and scale validation, with the semantic validation being the first phase of the process. results: according to the factor analysis, the ierum was divided into five factors (emotional aspects, perception, craving, cognitive aspects, and sexual aspects) that accounted for 60.18% of the variance. conclusion: the ierum showed good psychometric properties and can be used to evaluate the outcome expectancy related to marijuana use.
