Abstract: this work presents the results of a hybrid neural network (hnn) technique as applied to modeling scfe curves obtained from two brazilian vegetable matrices. a series hybrid neural network was employed to estimate the parameters of the phenomenological model. a small set of scfe data of each vegetable was used to generate an extended data set, sufficient to train the network. afterwards, other sets of experimental data, not used in the network training, were used to validate the present approach. the series hnn correlates well the experimental data and it is shown that the predictions accomplished with this technique may be promising for scfe purposes.

Effectiveness and safety of endotracheal tube cuffs filled with air versus filled with alkalinized lidocaine: a randomized clinical trial

Abstract: context and objective: high intracuff pressure in endotracheal tubes (ets) may cause tracheal lesions. the aim of this study was to evaluate the effectiveness and safety of endotracheal tube cuffs filled with air or with alkalinized lidocaine. design and setting: this was a prospective clinical study at the department of anesthesiology, faculdade de medicina de botucatu, universidade estadual paulista. methods: among 50 patients, et cuff pressures were recorded before, 30, 60, 90 and 120 minutes after starting and upon ending nitrous oxide anesthesia. the patients were randomly allocated to two groups: air, with et cuff inflated with air to attain a cuff pressure of 20 cmh2o; and lido, with et cuff filled with 2% lidocaine plus 8.4% sodium bicarbonate to attain the same pressure. et discomfort before tracheal extubation, and sore throat, hoarseness and coughing incidence were studied at the time of discharge from the post-anesthesia care unit, and sore throat and hoarseness were studied 24 hours after anesthesia. results: pressures in lido cuffs were significantly lower than in air cuffs (p < 0.05). tracheal complaints were similar for the two groups, except for lower et discomfort and sore throat incidence after 24 hours and lower systolic arterial pressure at the time of extubation in the lido group (p < 0.05). conclusion: et cuffs filled with alkalinized lidocaine prevented the occurrence of high cuff pressures during n2o anesthesia and reduced et discomfort and postoperative sore throat incidence. thus, alkalinized lidocaine-filled et cuffs seem to be safer than conventional air-filled et cuffs.

Abstract: Os chamados movimentos anti-sistêmicos vêm, segundo Wallerstein, intensificando-se cada vez mais nos tempos atuais. As contradi es, conflitos e tens es, frutos da hegemonia do capital, permitiram uma ressignifica o dos movimentos sociais, visando articular novas lutas numa perspectiva mundial, ou seja, a globaliza o do capital possibilitou a globaliza o dos movimentos anti-sistêmicos. No momento em que os movimentos anti-sistêmicos s o vistos como principal referência para a incessante luta contra o capital, questiona-se aqui, afinal, o que vem a ser um movimento anti-sistêmico. Apesar das evidências de seu surgimento, em que sentido é possível afirmar sua legitimidade? Nesta luta quem é o verdadeiro inimigo? Desta forma, tem-se como objetivo discutir, sob uma abordagem crítica e reflexiva, uma breve evolu o histórica dos movimentos anti-sistêmicos, as linhas conceituais, os rumos e impasses político-ideológicos dos chamados movimentos anti-sistêmicos. Palavras-chave: Movimentos anti-sistêmicos. Movimentos sociais. Globaliza o. The anti-systemic activities are, according to Wallerstein, getting enhanced more and more, nowadays. The contradictions, conflicts and tensions, consequences of capitalism, have opened the way for new significances inside social activities intending to articulate new fights in a world perspective, in other words, the capital globalization has allowed the globalization of anti-systemic activities. At the time the anti-systemic activities are seen as main reference to the never-ending fight against capital, we question, after all, what really is an anti-systemic activity. Despite the evidences of its beginning, how can we affirm its legitimacy? Who is the real enemy in this fight? This way, we have, as a goal, a critical and reflexive discussion, a brief historical evolution of the anti-systemic activities: the conceptual lines, the courses and the political and ideological impasses of the anti-systemic activities. Abstract: Este trabalho tem por objetivo analisar as formas como os temas vida e morte s o abordados na educa o. Inicialmente, procedeu-se a esta análise nos Parametros Curriculares Nacionais e na Proposta Curricular de Santa Catarina e posteriormente, a pesquisa foi realizada nos livros didáticos adotados pela maior institui o da rede pública estadual da regi o do médio vale do Itajaí - SC, levando-se em conta os níveis fundamental e médio. Pode-se afirmar que tanto nos documentos oficiais, quanto nos livros didáticos os temas vida e morte est o presentes, mas se comparados entre si, prevalecem as ocorrências para o fen meno da vida. Evita-se mencionar a morte de forma explícita e, em alguns casos, ela é ignorada ou escondida. Acredita-se que a educa o como propulsora na aquisi o do conhecimento possa promover a desmistifica o da morte preparando os sujeitos, desde cedo, tanto para a vida quanto para a morte. Reprodutibilidade da determina??o do limiar anaeróbico em pacientes com insuficiência cardíaca

Abstract: background: the anaerobic threshold (at) provides information on functional capacity in heart failure (hf). however, the visual determination of the at by ventilatory methods is subjective, being susceptible to differences between examiners. objective: to evaluate the inter- and intra-examiner reproducibility in the determination of the at in patients with mild to moderate hf, using visual-graphic and v-slope methods. to compare and correlate the results. methods: after performing a cardiopulmonary exercise test on a treadmill, visual-graphic and v-slope methods were used for the analysis of the at. to assess the reproducibility, three examiners determined the at twice by each method, on different days. for statistical analysis, we used intra-class correlation coefficient (icc) with p <0.05. results: we evaluated a total of 16 subjects, with a mean age of 45.9 ± 9.7 years, left ventricle ejection fraction of 20.5 ± 8.1%, and peak exercise oxygen consumption of 20.6 ± 7.8 ml/kg?min-1. the intra-examiner reproducibility was high in both methods for the three examiners, with icc values between 0.87 and 0.99. the inter-examiner reproducibility was moderate in both visual-graphic method (icc = 0.69) and v-slope method (icc = 0.64). when comparing methods, the icc found was 0.91. conclusion: at determination by visual-graphic and v-slope methods showed high and moderate inter- and intra-examiner reproducibility, respectively. moreover, both methods showed good agreement when compared with each other. these results suggest that both methods can be used in a reproducible way in at assessment of patients with mild to moderate hf.

Abstract: ehrlichia sp. and anaplasma platys are gram-negative micro-organisms, obligate intracellular parasites, residing in cytoplasmic vacuoles of leukocytes and platelets, found in peripheral blood or tissue. few reports have been made about ehrlichiosis and anaplasmosis in cats in brazil, which are based on the presence of morulae in leukocytes and platelets, or by detecting antibodies. the objective of this study was to investigate the natural infection with ehrlichia sp. and a. platys in cats in campos dos goytacazes-rj, by hematoscopia and dna detection of these agents. samples of whole blood and serum from 91 cats, regardless of race, gender and age. blood count, serum biochemistry and pcr using primers for ehrlichia sp. and a. platys were perfomed. data from hematoscopia showed 9.89% of morulae only in platelets. the dna of a. platys was detected in 13.18% of the 91 samples and 44.44% of the positive at hematoscopia. the dna of ehrlichia sp. was not detected in any sample. all animals studied did not show clinical signs neither positive laboratory results. the data suggest that domestic cats can serve as potential reservoirs for a. platys as asymptomatic form of related diseases

Abstract: background and objectives: spinal opioids may cause some undesirable effects, the most frequent of which is itching. in spite of its low morbidity rate, itching may cause severe discomfort to patients in addition to prolonging hospital stay. this study aimed at evaluating different therapeutic options to prevent itching after spinal sufentanil. methods: participated in these study 100 patients scheduled for non-obstetric procedures, who were randomly distributed in five groups: control (no treatment - c); 2.5 mg droperidol (d); 10 mg nalbuphine (n); association of previous drugs (dn); and 8 mg ondansetron (o). pruritus was quantitatively evaluated at 30 minutes, 1, 2 and 3 hours after spinal sufentanil. results: groups c and o had significantly higher incidence of itching as compared to groups d, n and dn. however, there has been no significant difference in the need for specific treatment with naloxone among groups. conclusions: itching prevention in our study, regardless of the drug used, has decreased its severity and has limited the need for specific treatment with naloxone.
Menopausal Status and Metabolic Syndrome in Women in Climacteric Period Treated at a Clinic in Southern Brazil  [PDF]

Abstract:

Background: This study aims to understand the relationship between menopausal status and the presence of Metabolic Syndrome in women from 40 to 65 years, as well as to describe the distribution of each component of Metabolic Syn- drome according to sample characteristics. Methods: A cross-sectional study was conducted with 551 women treated at a clinic in southern Brazil. MetS and its components were defined according to NCEP-ATP III and menopausal status as pre, peri, and post-menopause. Prevalences of menopausal status and of MetS and its components were calculated. Estimates of prevalence ratios crude and adjusted with confidence intervals of 95% were calculated by Poisson Regression with robust variance. Demographic, socioeconomic, behavioral, and reproductive characteristics were considered as potential confounding factors in multivariable models based on a conceptual framework of MetS determination. Results: The prevalence of Metabolic Syndrome in the sample was 56.1% (CI95%: 51.9 to 60.2), being more common among older women (56 to 65 years), with low education, menarche < 11 years old, with three or more pregnancies and in the post-menopausal period. In multivariate analysis, there was an increase of prevalence ratios when comparing perimenopause and post-menopause with pre-menopause; however, the confidence intervals include the unit. Regarding the analysis of isolated components in the sample, the most prevalent altered components were: hypertension (84.8%; CI95%: 81.7 to 87.8), waist circumference (66.4%; CI95%: 62.5 to 70.4) and HDL cholesterol (51.7%; CI95%: 47.5 to 55.9). There was a linear increase on mean blood glucose through menopausal status. Conclusions: Our study indicates variation on the distribution of MetS and each component according to menopausal status and other women characteristics. Future studies on MetS should also have foresight to use this type of approach to improve understanding and targeting of actions and programs focusing on women in this period of life.

Escala de Coma de Glasgow: subestima??o em pacientes com respostas verbais impedidas

Abstract: frequent question in the use of glasgow coma scale (gcs), on the acute phase, on inpatients due to encephalic cranial trauma (tbi) is the subestima because of the impeditive situations such as tracheostomy/endotracheal intubation, sedation and palpebral edema. the aim of this study was identify and determining the subestima on the total score of gcs when the score 1 is used on the impediment situations for your assessment. the sample studied had 76 inpatients for tbi at the hospital das clínicas da faculdade de medicina da universidade de s?o paulo. within 42 (55,3%) inpatients, there are not impediments and were carried out 136 assessments. in 34 (44,7%), there are impediments characterized by intubation or tracheostomy, being or not related to palpebral edema and sedation and the total evaluation was 310. within them, the total score varied of 3 to 11 with the more frequent scores in 3 and 6. through estimated values by linear regression since the scores obtained in eo and vrm, the following subestimas were obtained: mean=1.03±1.36, median=0.54 (intubation or tracheostomy), mean=0,40±0.79, median=0,00 (intubation or tracheostomy + sedation), mean=0.57±0.96, median=0.27 (intubation or tracheostomy + sedation + palpebral edema). it was concluded that, in the serious ect, the total score of gcs maintained a vrm in 1, although underestimated, it is near the real one.
