A Prospective Study of Headache after Carotid Endarterectomy: Incidence and Clinical Characteristics  [PDF]

Abstract: Headache associated with Carotid Endarterectomy (CEA) is a known condition; however, its incidence and etiology are not well defined. In this study, we determined the incidence and evaluated the characteristics of headache following CEA. This prospective study was conducted between January and July 2009, in 32 patients submitted to CEA. 68.7% of patients were male; mean age was 67.3 years-old. Headache incidence was 37.5%, ipsilateral to the CEA in 91% of patients; pressure was the most common pain quality (81.2%) and affected the frontal region alone in 37.5% of headache episodes. All headache episodes were of mild intensity. No correlation (p > 0.05) was found between sexes and no significant value (p > 0.05) was determined between the presence of headache and the mean degree of stenosis in the ipsilateral and contralateral carotid operated. Headache following CEA is a common condition; in most cases it is ipsilateral to the procedure, pressure type, mild and self-limiting.

Abstract: ganglioglioma are tumors presenting neoplastic glial cells and nerve cells, very rarely found in the pineal region. only eight cases have been previously published in the literature. we present the case of a 14 years-old male patient with a ganglioglioma of the pineal region. the patient was treated surgically by a suboccipital transtentorial approach with complete removal. histopathologic specimens with immunostainning revealed a ganglioglioma grade i. we conclude that these tumors are rare and should be treated surgically aiming total remove. if it is not possible or in case of recorrence the patient should be followed and radiotheraphy could be considered.

Abstract: Os gangliogliomas s o neoplasias mistas, compostas de elementos gliais e neuronais, extremamente raros na regi o da glandula pineal. Na presente revis o da literatura foram encontrados oito casos publicados. Apresentamos o caso de paciente de 14 anos, masculino, com ganglioglioma da regi o da pineal, tratado cirurgicamente, com exérese total da les o por via suboccipital transtentorial. O estudo histológico mostrou tratar-se de ganglioglioma grau I, confirmado por imuno-histoquímica. Conclui-se que tais tumores s o raros e que se deve optar pela cirurgia, objetivando a exérese total. Quando isso n o for possível, ou no caso de recorrência, o paciente deve ser acompanhado clínica e radiograficamente, considerando-se a radioterapia como tratamento complementar.

Abstract: lipoma of the cerebellopontine angle is a very rare tumor. we report the case of a 14-years-old female, with left side deafness during three years, associated with headache. ct scan showed an hypodense mass, without enhancement at the cerebellopontine angle. the patient was treated surgically by left retrosigmoid approach. the lesion involved the eighth and seventh cranial nerves and only a partial removal was performed. the postoperative course was uneventful. she had no more headache; the deafness of the left side remained unchanged. asymptomatic lipoma of the cerebellopontine angle can be treated conservatively, although those with progressive symptoms should be treated surgically, with total or partial remove based on their neurovascular involvement.

Abstract: the better understanding of the natural history of the cavernous malformations and the improvement of diagnostic methods and of microsurgical techniques have made the management of cavernous malformations possible through the conservative treatment, radiosurgery, and microsurgical resection. we present 33 cases operated at our service at santa casa hospital, belo horizonte, from 1992 to 2001. cortical and subcortical cavernomas manifested by epilepsy (57.5%) or mainly by hemorrhage (15.1%) were surgically approached. the deep lesions (basal ganglia, talamo and brain steam) represented 27.7% of our cases. they should only be operated when located near the pial or ependimary surface. the resection of spinal cord lesions (5.5%) and of deep brain lesions is also recommended when they present progressive focal deficit (13.8%) or recurrent episodes of hemorrhage (13.8%). small and deep seated cavernomas that do not present bleeding must be conservatively treated. there has been no evident favourable result related to radiosurgery so far.

Abstract: Os lipomas do angulo pontocerebelar s o tumores muito raros. Em geral s o assintomáticos, achados fortuitos em autópsia, tomografia computadorizada (TC) de cranio ou ressonancia nuclear magnética do encéfalo. Os autores apresentam o caso de paciente, feminina, branca, 14 anos, há três anos com diminui o da audi o à esquerda e há um ano com cefaléia, tipo hemicrania à esquerda. A TC de cranio evidenciou les o hipodensa, sem capta o de contraste, no angulo pontocerebelar esquerdo. A paciente foi submetida a cirurgia por via retrossigmóidea para abordagem da les o, que se apresentou de colora o amarelada, característica de tecido adiposo. Tendo em vista o envolvimento de estruturas nervosas (VII e VIII nervos cranianos), optou-se pela exérese parcial da les o. A paciente evoluiu com melhora da cefaléia, sem piora do déficit auditivo. Após três anos de seguimento, mantém o quadro estável. Conclui-se que os lipomas do angulo pontocerebelar, quando assintomáticos, podem ser tratados conservadoramente. Entretanto aqueles associados a sintomatologia persistente e progressiva devem ser operados. A exérese total ou parcial vai depender do envolvimento ou n o das estruturas neurovasculares adjacentes.
Cavernoma do ventrículo lateral: relato de caso

Abstract: As malforma es cavernosas podem ocorrer em qualquer localiza o, apesar disso, os cavernomas intraventriculares s o extremamente raros. Na revis o aqui realizada, foram encontrados 28 casos de cavernoma do ventrículo lateral na literatura que, juntamente com o presente relato, somam 29 casos. CASO: Trata-se de paciente de 15 anos, feminina, com cefaléia há 8 meses. Há um mês apresentou cefaléia intensa, súbita, seguida de torpor. Recuperou a consciência após 48 horas. Foi encaminhada à Santa Casa de Belo Horizonte onde apresentou novo episódio de cefaléia intensa, súbita, seguida de torpor. A tomografia de cranio mostrou les o volumosa, arredondada, com área de hiperdensidade, sugestiva de sangramento recente, no corno frontal do ventrículo lateral. A paciente foi submetida a cirurgia para exérese da les o, cujo estudo histológico mostrou tratar-se de hemangioma cavernoso. A paciente evoluiu bem, sem complica es. CONCLUS O: Apesar de raros, os cavernomas devem fazer parte do diagnóstico diferencial das les es intraventriculares, pois o diagnóstico pré-operatório equivocado pode levar a tratamento inapropriado destas les es benignas, curáveis pela cirurgia.

Abstract: the authors report the case of a 50-year-old female patient with right orbital headache secondary to a giant intracavernous aneurysm, diagnosed as raeder paratrigeminal neuralgia. the patient was surgically treated with internal carotid artery trapping together with brain revascularization (superficial temporal artery to a deep branch of the middle cerebral artery). the patient had an excellent outcome. the identity of the syndrome and the differential diagnosis are discussed based on literature review.

Abstract: carotid body tumors are rather uncommon. even though there is a great amount of literature on the subject, the controversy regarding its biological behavior and therapeutics and mainly, the surgical management still remains. we present eight patients with carotid body tumors (total of 9 tumors, one bilateral) surgically treated at the neurosurgery department of santa casa in belo horizonte, from 1989 to 1999. the age ranged from 11 to 66-years-old (35,6±17.7). four patients were women and four were men. we had satisfactory postoperative results with low morbidity and no deaths. based on our experience and on the review of the literature, some aspects of this disease are discussed. we conclude that carotid body tumors are uncommon and should be treat with carefully surgery techniques to obtain low morbimortality rates.

Abstract: cavernous malformations are uncommon lesions that are usually present in the cerebral hemispheres. they occur rarely in the ventricular system, and even more rarely in the lateral ventricle. only 28 cases have been previously reported in the literature. case: we present one case of lateral ventricle cavernoma in a 15-years-old female patient, who suffered of mild chronic headache for 8 months, followed by two episodes of sudden intensive headache and stupor with complete recovery after 48 hours. ct scan was performed and revealed a voluminous size, hiperdense mass in the frontal horn of the lateral ventricle. the surgical access to site was through transcallosal interhemisphere approach. the patient had a good recovery without complications. conclusion: although lateral ventricle cavernomas are rare they should be considered in the differential diagnosis of intraventricular tumors. a wrong preoperative diagnosis has sometimes induced a wrong therapy, such as radiotherapy, for these surgically curable benign lesions.
