Papillary Carcinoma Arising in Thyroglossal Duct Cyst: A Retrospective Analysis (Cancer of the Thyroid or Primary Cancer of the Thyroglossal Cyst)  [PDF]

Abstract:
Thyroglossal duct cysts are one of the most common congenital abnormalities of the cervical region. Complications of these swellings are rare, and among these, appearance of a carcinoma has also been noted. Thyroglossal duct cyst carcinoma (TGDC) is a rare entity and its management is controversial. The incidence of thyroid papillary carcinoma in thyroglossal duct cyst is less than 1%, in most cases, the diagnosis is made postoperatively. We report the case of an adult female patient with a papillary carcinoma arising in a thyroglossal duct cyst. Our aim is defining a clinical protocol to diagnose the thyreoglossal duct carcinoma through clinical features, radiological investigations, cytological and histopathological examinations and, through this diagnostic protocol, to choose the best surgical approach. The literature contains 300 cases of this pathology; papillary histotype constitutes about 80%, then squamocellular carcinoma, mixed folliculo-papillary carcinoma and adenocarcinoma follow. The literature suggests two different hypotheses: neoplasia originating from ectopic thyroid tissue and plurifocal theory. The adopted diagnostic investigations are: ultrasonography, Fine Needle Aspiration Biopsy (FNAB), Magnetic Resonance Imaging (MRI). Our patient was treated using a modified Sistrunk’s procedure operation, in which thyroidectomy proved crucial for the correct diagnosis and continuation of appropriate treatment. Our case confirms the difficulty in distinguishing a primitive thyroglossal duct carcinoma from a synchronous metastatic papillary carcinoma of the thyroid. This dilemma often remains unresolved. The two different surgical approaches reported in the literature, one more conservative and the other more aggressive, apparently alternatives are instead complementary and adequate when strict diagnostic criteria and adequate follow-up, are observed. The first year follow-up includes the thyroglobulin level determination and a neck ecografic scan every 3 months. The patient has been following for two years without any metastasis.
Leucine Aminopeptidase, β-Glucosidase and Alkaline Phosphatase Activity Rates and Their Significance in Nutrient Cycles in Some Coastal Mediterranean Sites

Abstract: In aquatic microbial ecology, knowledge of the processes involved in the turnover of organic matter is of utmost importance to understand ecosystem functioning. Microorganisms are major players in the cycling of nutrients (nitrogen, phosphorus) and carbon, thanks to their enzymatic activities (leucine aminopeptidase, LAP, alkaline phosphatase, AP, and β-glucosidase, β-GLU) on organic polymers (proteins, organic phosphates and polysaccharides, respectively). Estimates of the decomposition rates of organic polymers are performed using fluorogenic compounds, whose hydrolysis rate allow us to obtain information on the “potential” metabolic activity of the prokaryotic community. This paper refers the enzyme patterns measured during recent oceanographic cruises performed in some coastal Mediterranean sites, not yet fully investigated in terms of microbial biogeochemical processes. Mean enzyme activity rates ranged from 5.24 to 5558.1 nM/h, from 12.68 to 244.73 nM/h and from 0.006 to 9.51 nM/h for LAP, AP and β-GLU, respectively. The highest LAP and AP activity rates were measured in the Gulf of Milazzo (Tyrrhenian Sea) and in the Straits of Messina, in association with the lowest bacterioplankton abundance; in contrast, the lowest ones were found in the northern Adriatic Sea. β-GLU was more active in the Straits of Messina. Activity rates were analysed in relation to the main environmental variables. Along the northern Adriatic coastal side affected by the Po river, significant inverse relationships linked LAP and AP with salinity, pointing out that fluvial inputs provided organic substrates for microbial metabolism. Both in the Gulf of Manfredonia and in the Straits of Messina, LAP and AP levels were inversely related with the concentration of nitrate and inorganic phosphorus, respectively. In the Gulf of Milazzo, high cell-specific AP measured in spite of phosphorus availability suggested the role of this enzyme not only in phosphorus, but also in carbon release.

Abstract: The relationship between religiosity and educational attainment is an important question in the sociology of religion literature. It is widely contested whether the natural outgrowth of the spreading rational worldview and the increase of educated people can account for the decline of religious adherence. Is there any other explanation for the different opportunities of religious and non-religious societal groups to obtaining the highest educational level? After the political transformation in Central and Eastern Europe, one of the most important challenges of restructuring the educational system was how different cultural groups would be able to infuse their own spirituality into their children's education after the domination of the totalitarian ideology. The Hungarian case is unique because of the mixed confessional landscape, the populous Hungarian minority outside the border, the alternating hard and soft periods of religious harassment. Recently, more than half of the Hungarian population can be described as religious in their own way, one sixth strongly affiliated with churches, and another sixth are atheists. However, several studies showed that basic indicators of social status were very strongly and negatively interrelated with religiosity. It turned out that preferred educational views, values, approaches and priorities regarding the norms at schools differ very sharply according to the religious views, and belonging to a religious network significantly supports educational careers. This paper is a comprehensive review of research on the educational functions of denominational schools and religious communities in contemporary Hungary.

Abstract: dog-human communication has been widely investigated recently for different theoretical reasons, in most cases through dogs' comprehension of human gestural cues. dogs have been reported to be very skilful in comprehending a variety of human pointing gestures in many independent studies. this paper provides a short overview of the possible explanations behind the dogs' exceptional communicational abilities towards humans from an evolutionary perspective, concluding that the different and seemingly contradictory hypotheses are not exclusive but they might have a synergic effect.
An activity-based system of models for student mobility simulation

Abstract: The proposed system of models adopt the activity-based travel approach combined with the conjoint use of the RP and SP methodologies, allowing a better understanding and prediction of users responses to travel demand management measures. The model structure is very realistic for student mobility simulation, helping the analyst in evaluating the effects of park pricing policies and the introduction of a new transport system in the travel users choices.

Abstract: The author proposes a reflection on the evolution and possible future developments of Architectural Technology as an academic discipline, in light of its historical roots, through a path that sees it divided into different areas of specialized knowledge, all heavily characterized by attention to the methodological aspects in the project. Faced with potential developments, the author identifies two alternatives: one aimed at consolidating specialized aspects according to a plural vision of technologies, the other aimed at reconstructing a uni- que integrated discipline. The author takes the second direction, proposing to guide Archi- tectural Technology, in research and training, towards the construction of generalized skills, albeit with controlled specificities, that support the project’s capacity to direct.

Abstract: La Nascita di Venere di Botticelli e la Bagnante di Mirò. Quasi quattrocentocinquant’anni di storia dividono questi due quadri che, anche dal punto di vista stilistico, non presentano punti di contatto. Il semplice accostarli, però, suggerisce un legame intuitivo, come se la Venere botticelliana fosse stata il riferimento di Mirò nel momento in cui ha tracciato le linee stilizzate della sua Bagnante. Guardando la tela del pittore catalano si potrebbe ipotizzare che la Venere del Rinascimento abbia ripreso il suo fluttuare sull’acqua e, dopo un lungo viaggio, sia approdata ad altre acque: quelle della Bagnante narranti la storia di un tempo distante. Complice di questo viaggio il concetto warburghiano di Nachleben per il quale può accadere che condensazioni significative di immagini del passato riprendano il “movimento”, assumendo, però, un significato che si sottopone a continue trasformazioni nel tempo e nello spazio. Nel movimento dei capelli e dei panneggi della Primavera e della Nascita di Venere, Warburg ravvisa una funzione iconografica, poetica e “patetica” in cui vede riflettersi l’atmosfera della Firenze rinascimentale, attratta dal movimento pieno di grazia e di passione di cui la Ninfa rappresenta l’emblema. The Nascita di Venere by Botticelli, the Bagnante by Mirò. Almost four-hundred and fifty years divide these two paintings that, also from a stylistic point of view, have no meeting points. But, simply by catching a glimpse of the two atmospheres painted, the observer feels the tight bound that lies between them: as if the Botticellian Venere had been the reference for Mirò when he was drawing the stylized lines of his Bagnante. When you observe the canvas of the Catalan artist it easily comes to you the idea that the figure you are looking at could be the Venere of Botticelli which, after a long travel along time and space, has arrived to a new land of water (the one of the Bagnante), located in a distant and different time, but where the Nymph can still dance with her striking elegance. An important role in this travel is played by the concept of Nachleben described by Warburg. According to it, it can happen that significant images from the past restart again their “movement” in the future, but with a transformed meaning.

Abstract: Le lettere qui esaminate, conservate nella Biblioteca Marucelliana a Firenze, sono scritte da Napoli dal famoso incisore di pietre dure Francesco Maria Gaetano Ghinghi (1689-1762) al celebre erudito e antiquario Anton Francesco Gori. Esse sono ricche di interessanti notizie relative all’attività del Real Laboratorio delle Pietre Dure di Napoli, fondato nel 1737, di cui il Ghinghi fu il primo direttore, e coordinatore dell’équipe di artisti fiorentini che si erano trasferiti con lui. Il Ghinghi fornisce dati sulla circolazione di gemme a Napoli in quel periodo, sulle antichità di Ercolano (di cui il Gori fu uno dei primi a scrivere) e sulla permanenza a Napoli di Antonio Pichler, capostipite di una illustre famiglia di incisori, che il Ghinghi conosceva bene, perché entrambi lavoravano per il noto collezionista di gemme Philipp von Stosch. Inoltre, in un quadro di scarsa disponibilità di calchi riproducenti gemme della collezione Farnese, acquista gran valore l’invio di tali impronte da parte del Ghinghi al Gori, che così aveva gemme non documentate altrimenti. The letters analysed in this article, now in the Biblioteca Marucelliana in Florence, are written from Naples by the famous gem-engraver, Francesco Maria Gaetano Ghinghi (1689-1762) to the renowned scholar and antiquarian Anton Francesco Gori. They give very interesting news about the activity of The Laboratorio delle Pietre Dure in Naples, that began its production in 1738, formed by Ghinghi, the first Director, and the Florentine craftsmen who had come with him. Ghinghi offers informations to reconstitute gems’s circulation in Naples, in that period; relates about antiquities found at Herculaneum (Gori was one of the first writer of those excavations), and about Antonio Pichler in Naples, founder of illustrious gem-engravers’s family, who Ghinghi knew very well, because both worked for the famous collector Philipp von Stosch. One might ask if the Bourbons, jealous of their gems’s collection, were acquainted with Ghinghi sent casts of their intaglios, made by himself, to Gori, interested in the possession of gems unknown.
