Total Knee Arthroplasty: Patellar Replacement’s Influence in Patient Quality of Life Postoperatively  [PDF]

Abstract: Objective: To study the influence of patellar resurfacing on quality of life under a standardised score. The chosen score was the Knee Society Score (KSS), which considers range of motion, pain, contractors, alignment and stability. Methods: Retrospective review of 60 patients submitted to total knee arthroplasty (TKA) was divided into two groups: with patellar resurfacing (PR) and without patellar resurfacing (NPR). We reviewed data from these patients containing the KSS, which are routinely used in our service. Data were analysed from 2013 to 2014, with a minimal patient follow-up of 18 months. Results: There were no unsatisfactory scores in our study. Total scores (KSS) were similar between the groups (mean KSS: PR = 88.36 ± 9.64; NPR = 84.26 ± 9.44 [p = 0.1]). While stratifying into groups (poor, fair, good, excellent), we found excellent results in 84.8% of PR versus 59.3% NPR (p = 0.056). These data did not reach statistical significance and can therefore be considered a coincidental result. Conclusion: Although these results did not show statistical significance (p = 0.056), patellar resurfacing patients tend to get more “excellent” results after 18 months of follow-up in quality of life when evaluated by KSS. Perhaps a larger amount of patients would suffice to acquire statistical significance in a future study.

Abstract: fiberoptic bronchoscopy is a useful procedure in the diagnosis of histoplasmosis, when the clinical material is adequately processed. ten cases of disseminated histoplasmosis in patients with aids are presented here, with a mycological evaluation in culture, mycosel? and sabouraud's chloramphenicol agar of clinical specimens obtained through fiberoptic bronchoscopy. the yield of mycosel? culture was 60%, while in sabouraud's chloramphenicol agar it was 20%, supporting the importance of selective medium in the isolation of histoplasma capsulatum var capsulatum from potentially contaminated clinical specimens, as well as the importance of clinical information for the laboratory as the key for a correct diagnosis.

Abstract: A fibrobroncoscopia é um procedimento útil no diagnóstico da histoplasmose, quando o material clínico é processado adequadamente. S o apresentados dez casos de histoplasmose disseminada em pacientes com SIDA, com avalia o micológica em cultivo, nos meios Mycosel e ágar-Sabouraud cloranfenicol de espécimes clínicos colhidos por fibrobroncoscopia. A positividade do cultivo em Mycosel foi de 60%, enquanto no ágar-Sabouraud cloranfenicol foi de 20%, evidenciando a importancia do meio seletivo no isolamento do Histoplasma capsulatum var capsulatum de espécimes clínicos potencialmente contaminados, bem como a importancia das informa es clínicas para o laboratório, sendo esta a chave para o diagnóstico correto.

Abstract: objective: to analyze the association between paracoccidioidomycosis (pcm) and cancer in a series of 25 cases and to review the literature on this topic. methods: a retrospective review of 25 cases diagnosed with pcm and cancer, retrieved from a series of 808 consecutive adult patients diagnosed with pcm based on tests conducted in the mycology laboratory of the santa casa complexo hospitalar, in the city of porto alegre, brazil, between 1972 and 2007. the diagnosis of pcm was confirmed by means of direct microscopic examination, histopathological examination or immunodiffusion test. all cancer cases were confirmed by histopathological or cytopathological examination. results: respiratory symptoms were the principal complaints of the patients evaluated. pulmonary involvement predominated, followed by skin and lymph node involvement. the most prevalent tumor was bronchial carcinoma, in 15 patients, followed by other types of carcinoma, and 1 patient had hodgkin's lymphoma. in 16 patients (64%), the site of the pcm was the same as that of the tumor. in most cases, pcm treatment consisted of the isolated administration of sulfanilamide, sulfamethoxazole-trimethoprim, ketoconazole, itraconazole or amphotericin b. the most common treatment for cancer was surgery, followed by radiotherapy and chemotherapy. of the 25 patients, 12 were cured of pcm, and 4 died. in 9 patients, the final outcome was unknown. in the general population of the area under study, the prevalence of lung cancer was significantly higher in smokers with pcm than in smokers without pcm (p < 0.001). conclusions: a diagnosis of pcm appears to increase the risk of lung cancer.
Estrutura das exporta??es e crescimento econ?mico: uma análise empírica, 1985-2003

Abstract: the paper discusses the idea that the quality of exports matters for economic growth. the influence of the structure of exports on the rates of economic growth is discussed within the context of a growth model with external constraints. to test the relationship between exports and growth, exports are divided in two groups: a) dynamic from a schumpeterian perspective (high-tech sectors) and b) dynamic from a keynesian perspective (international demand grows at higher rates than the average). the key hypothesis is that countries whose total exports have higher shares of sectors with keynesian and schumpeterian efficiency will grow faster. the empirical results are consistent with this hypothesis. in addition, the results suggest that schumpeterian efficiency tends to have a stronger impact on growth than keynesian efficiency.

Abstract: the tomato fruit shape is a result of action of various gene loci that can act in several ways: alone, with pleiotropic effects or interacting with each other or with genes responsible for the fruit weight. according to this, the study of the inheritance of this characteristic, very important for the improvement of the species, is complex and difficult to implement. this work was carried out at the horticultural and experimentation section of josé do rosário vellano university, during the period of july 2003 through march, 2005 with the objective to determine the fruit shape of the cherry tomato plant. two wild genotype contrasting tomato plants were utilised with the fruit shape: long fruit shape (lf) and round fruit shape (rf). reciprocal crosses and backcross were realised and the fruit shape was evaluated in the generations origininated from these crosses. all plants of the f1 generation produced round shape fruit. in the f2 generation, 111 plants produced round fruit shape and 47 plants produced long fruit shape. on the backcross generation (f1 x lf) 47 plants produced round fruit shape and 42 plants produced long fruit shape. these results when analysed by chi square, confirmed mendelian segregation 3:1 and 1:1 respectively. it was concluded that the studied fruit shape has monogenic inheritance. the allele that determines fruit round shape has complete dominance over the allele that determines elongated shape fruit with possible action of the sun locus.
