Busulfan and melphalan as conditioning regimen for allogeneic hematopoietic stem cell transplantation in acute myeloid leukemia in first complete remission

Abstract: background: allogeneic hematopoietic stem cell transplantation with hla-identical donors has been established for the treatment of acute myeloid leukemia patients for over 30 years with a cure rate of 50% to 60%. objectives: to analyze the overall survival of patients and identify factors that influence the outcomes of this type of transplant in patients in 1st complete remission who received a busulfan and melphalan combination as conditioning regimen. methods: twenty-five consecutive patients with acute myeloid leukemia were enrolled between 2003 and 2008. the median age was 34 years old (range: 16 - 57 years). all patients received cyclosporine and methotrexate for prophylaxis against graft-versus-host disease. median neutrophil engraftment time was 16 days (range: 7 - 22 days) and 17 days (range: 7 - 46 days) for platelets. sinusoidal obstructive syndrome was observed in three patients, seven had grade ii acute graft-versus-host disease and one extensive chronic graft-versus-host disease. results: the overall survival by the kaplan-meier method was 48% after 36 months with a plateau at 36 months after transplantation. intensive consolidation with high-dose arabinoside resulted in an improved survival (p-value = 0.0001), as did grade ii acute graft-versus-host disease (p-value = 0.0377) and mild chronic graft-versus-host disease (p-value < 0.0001). thirteen patients died, five due to infection within 100 days of transplant, two due to hemorrhages, one to infection and graftversus-host disease and three relapses followed by renal failure (one) and infection (two). the cause of death could not be determined for two patients. conclusion: the busulfan and melphalan conditioning regimen is as good as other conditioning regimens providing an excellent survival rate.
Evaluation of early hospital discharge after allogeneic bone marrow transplantation for chronic myeloid leukemia

Abstract: context and objective: the increasing number of patients waiting for bone marrow transplantation in our service led to the implement of an early hospital discharge program with the intention of reducing the interval between diagnosis and transplantation. in this study we analyzed the results from early discharge, with outpatient care for patients with chronic myeloid leukemia who underwent allogeneic bone marrow transplantation. design and setting: retrospective study at the bone marrow transplantation unit of hospital das clínicas, faculdade de medicina da universidade de s?o paulo. methods: we compared clinical outcomes within 100 days post-transplantation, for 51 patients with chronic myeloid leukemia (cml) who received partially outpatient-based allogeneic hematopoietic stem cell transplantation, and the results were compared with a historical control group of 49 patients who received inpatient-based hematopoietic stem cell transplantation. results: there were significantly fewer days of hospitalization (p = 0.004), pseudomonas-positive cultures (p = 0.006) and nausea and vomiting of grade 2-3 (p < 0.001) in the outpatient group. there were no significant differences in mortality between the groups and no deaths occurred within the first 48 days post-transplantation in the outpatient group. conclusions: this partially outpatient-based hematopoietic stem cell transplantation program allowed an increased number of transplantations in our institution, in cases of cml and other diseases, since it reduced the median length of hospital stay without increasing morbidity and mortality.
Autologous hematopoietic stem cell transplantation in classical Hodgkin's lymphoma

Abstract: background: hodgkin's lymphoma has high rates of cure, but in 15% to 20% of general patients and between 35% and 40% of those in advanced stages, the disease will progress or will relapse after initial treatment. for this group, hematopoietic stem cell transplantation is considered one option of salvage therapy. objectives: to evaluate a group of 106 patients with hodgkin's lymphoma, who suffered relapse or who were refractory to treatment, submitted to autologous hematopoietic stem cell transplantation in a single transplant center. methods: a retrospective study was performed with data collected from patient charts. the analysis involved 106 classical hodgkin's lymphoma patients who were consecutively submitted to high-dose chemotherapy followed by autologous transplants in a single institution from april 1993 to december 2006. results: the overall survival rates of this population at five and ten years were 86% and 70%, respectively. the disease-free survival was approximately 60% at five years. four patients died of procedure-related causes but relapse of classical hodgkin's lymphoma after transplant was the most frequent cause of death. univariate analysis shows that sensitivity to pre-transplant treatment and hemoglobin < 10 g/dl at diagnosis had an impact on patient survival. unlike other studies, b-type symptoms did not seem to affect overall survival. lactic dehydrogenase and serum albumin concentrations analyzed at diagnosis did not influence patient survival either. conclusion: autologous hematopoietic stem cell transplantation is an effective treatment strategy for early and late relapse in classical hodgkin's lymphoma for cases that were responsive to pre-transplant chemotherapy. refractory to treatment is a sign of worse prognosis. additionally, a hemoglobin concentration below 10 g/dl at diagnosis of hodgkin's lymphoma has a negative impact on the survival of patients after transplant. as far as we know this relationship has not been previously report

Abstract: context and objective: liposomal daunorubicin has been used to treat hematological malignancies, including multiple myeloma (mm). the goal was to evaluate efficacy, side-effects and toxicity of liposomal daunorubicin and dexamethasone ("dd protocol"). design and setting: prospective study at sírio-libanês, s?o camilo, brasil and alem?o oswaldo cruz hospitals. methods: twenty consecutive patients with active mm received four cycles of liposomal daunorubicin intravenously for two hours (25-30 mg/m2/day) on three consecutive days per month, with oral dexamethasone (10 mg every six hours) on four consecutive days three times a month. results: the male/female ratio was 1:1 and median age 60. nine patients were stage iia, ten iiia and one iiib. the median from diagnosis to starting dd was 13 months. all patients received four cycles, except one. fifteen had already received chemotherapy before dd. responses of > 50% reduction in serum monoclonal paraprotein were observed in six patients after first cycle (30%), six after second (30%) and four after third (20%), while four (20%) did not obtain this. initially, 17 patients (85%) had anemia: 12 (70%) achieved correction. progressive disease was observed in three patients (15%), while one had minimal response, four (20%) partial and 12 (60%) complete. hematological toxicity was acceptable: three patients (15%) had neutrophils < 1,000/mm3; none had thrombocytopenia. gastrointestinal toxicity was mild: nausea (10%), anorexia (15%) and no vomiting. conclusions: this treatment has mild toxicity and good response rate. it may therefore be feasible before autologous bone marrow transplantation.

Abstract: myeloablative or non-myeloablative related allogeneic bone marrow transplantation is a curative treatment in some oncologic and hematologic diseases. unfortunately, only 25 to 30% of patients find a fully matched related donor. national and international donor programs are an option, however, much time is lost and patients can not wait because of their advance disease status or clinical deterioration. partially matched family donor allogeneic bone marrow transplantation may be an option. this article reviews the literature on this subject.

Abstract:

The aim of this work was to characterize and to bring into relationship of the net radiation with the latent heat flux equivalent to water (mm) in a patch of Atlantic Forest within the metropolitan area of Sao Paulo (RMSP). The estimation of the latent heat flux (LE) was made by the energy balance equation with the Bowen ratio. Measurements of net radiation and psychometrics gradients were made. Soil heat flux (G) was measured by temperature sensors in two depths. Measurements were made at Park of Science and Technology of Sao Paulo University (CIENTEC), Sao Paulo, Brazil, between 2011 and 2013. Results indicated that evapotranspiration varies seasonally and the amount of water evaporated exceeds 3.0 mm·day-1 in spring/summer and in autumn/winter the amount is approximately 1.0 mm·day-1. The evapotranspiration is increased under RMSP urban climate conditions with higher air temperature and lower moisture especially in summer season.

Abstract: this is a case report of a child with aids who has taken part into psychotherapeutic sessions for an year. considering the emotional difficulties experienced by the patient regarding the disease as well as the loss of his parents, caused by aids, an analysis based on psychodynamic psychology was conducted in relation to the unconscious fantasies, anxieties and defenses. at first, the child had used denial as a defense and expressed himself mainly through drawings. along the play psychotherapy, the child was able to use less primitive defenses and express himself in other ways, such as games and drawings. the child's suffering seemed to be a result of abandonment and guilt feelings in relation to the disease and to his parents` death, as well as to unconscious fantasies of having provoked their death.

Abstract: high levels of heterogeneity and instability characterize the brazilian labor market. in addition, the growth of the percentage of women in the labor force is especially due to the fact that more married women are working, which means that female role in families and society has been undergoing changes. this paper investigates the relationship among occupational and non-occupational characteristics of the "head of the family" and his "spouse" as a way to obtain inferences about family well-being. the data-base used is the "occupational mobility survey" linked to the "employment and unemployment survey", which were carried out in the s?o paulo metropolitan area from april to december 2001. typologies were created by applying the "grade of membership", a method that deals better with heterogeneity. a high degree of heterogeneity of marriages was found. age, education and the spouses' economic strategies, especially related to the wife's labor force participation, make up the determinant factors to distinguish the couples.

Abstract: the objective of this work was to define guidelines for the indication of hematopoietic stem cells transplantation (hsct) in the treatment of acute myeloid leukemia (aml) in brazil. the role of hsct in the treatment of aml was discussed by the authors and presented to the brazilian society of bone marrow transplantation in a meeting to formulate and ratify the brazilian guidelines on hsct. this consensus was based on a review of international publications and on the brazilian experience in hsct for the treatment of aml. the optimal treatment for aml in first complete remission (1cr) has not been defined yet. there is consensus on the indication of allogeneic hsct with myeloablative conditioning for patients who present high risk cytogenetic changes. allogeneic hsct is not indicated for low cytogenetic risk 1rc patients and, apparently, allogeneic and autologous hsct and consolidation chemotherapy are similar for intermediate risk patients.
