Implications of Discordance in World Health Organization 1997 and 2009 Dengue Classifications in Adult Dengue

Abstract: Background Revised dengue guidelines were published by the World Health Organization (WHO) in 2009 addressing severe dengue cases not classified by dengue hemorrhagic fever (DHF) and shock syndrome (DSS). Methods and Principal Findings We conducted a retrospective cohort study to compare WHO 2009 and 1997 classifications using 1278 adult dengue cases confirmed by polymerase chain reaction assay from Singapore epidemics in 2004 and 2007 (predominantly serotype 1 and 2 respectively).DHF occurred in 14.3%, DSS 2.7% and severe dengue 16.0%. The two WHO dengue classifications were discordant in defining severe disease (p<0.001). Five DSS patients (15%) were classified as non-severe dengue without warning signs. Of severe dengue patients, 107 did not fulfil DHF criteria. Of these, 14.9% had self-resolving isolated elevated aminotransferases, 18.7% gastrointestinal bleeding without hemodynamic compromise and 56.1% plasma leakage with isolated tachycardia. We compared both guidelines against requirement for intensive care including the single death in this series: all six had severe dengue; only four had DHF as two lacked bleeding manifestations but had plasma leakage. Increasing length of hospitalization was noted among severe cases with both classifications but the trend was only statistically significant for WHO 2009. Length of hospitalization was significantly longer for severe plasma leakage compared with severe bleeding or organ impairment. Requirement for hospitalization increased using WHO 2009 from 17.0% to 51.3%. Conclusions While the WHO 2009 dengue classification is clinically useful, we propose retaining criteria for plasma leakage and hemodynamic compromise from WHO 1997, and refining definitions of severe bleeding and organ impairment to improve clinical relevance having found that differences in these accounted for the discordance between classifications. Findings from our retrospective study may be limited by the study site - a tertiary referral center in a hyperendemic country - and should be evaluated in a wider range of geographic settings.
IL-1β, IL-6, and RANTES as Biomarkers of Chikungunya Severity

Abstract: Background Little is known about the immunopathogenesis of Chikungunya virus. Circulating levels of immune mediators and growth factors were analyzed from patients infected during the first Singaporean Chikungunya fever outbreak in early 2008 to establish biomarkers associated with infection and/or disease severity. Methods and Findings Adult patients with laboratory-confirmed Chikungunya fever infection, who were referred to the Communicable Disease Centre/Tan Tock Seng Hospital during the period from January to February 2008, were included in this retrospective study. Plasma fractions were analyzed using a multiplex-microbead immunoassay. Among the patients, the most common clinical features were fever (100%), arthralgia (90%), rash (50%) and conjunctivitis (40%). Profiles of 30 cytokines, chemokines, and growth factors were able to discriminate the clinical forms of Chikungunya from healthy controls, with patients classified as non-severe and severe disease. Levels of 8 plasma cytokines and 4 growth factors were significantly elevated. Statistical analysis showed that an increase in IL-1β, IL-6 and a decrease in RANTES were associated with disease severity. Conclusions This is the first comprehensive report on the production of cytokines, chemokines, and growth factors during acute Chikungunya virus infection. Using these biomarkers, we were able to distinguish between mild disease and more severe forms of Chikungunya fever, thus enabling the identification of patients with poor prognosis and monitoring of the disease.

Abstract: the aim of this paper is to study the changes in market concentration in the brazilian mining and manufacturing industries that took place from 1996 to 2003. the paper decomposes the herfindahl-hirschman index into variations in the size distribution and the number of firms. the results show that the average concentration did not change greatly in the period analyzed; however, a small group of industries showed significant differences in concentration. in these cases, changes in size distribution appear to have more importance than changes in the number of firms.

Abstract: in the current context of high uncertainty, many parallels have been traced between the economic crisis of 2008 and the great depression, that stirred the world between 1929 e 1933. however, despite the financial losses that recently occurred, the disposition for state intervention in our days is much higher and thus an important element in the formulation of economic policies. this is a difference that suggests a less catastrophic outcome to the current crisis.

Abstract: lucien goldmann's literary studies fall into two distinct phases. in the first, relying mainly on contributions of the young lukács (soul and form and the theory of the novel), goldmann sought to establish a homology between the novelistic form and social life. moreover, falling back on history and class-consciousness, he sought to give a historicist and dialectical treatment to the youthful texts of the hungarian thinker by interpreting literature as an articulate expression of the "views of the world" held by various classes and social groups. in the second phase, marked by pessimism and the influence of structuralist ideas, then in vogue, goldmann's essays ceased to seek mediations between the written work and social life. taken by surprise by the maelstrom of 1968, goldmann engaged himself in a self-critical movement, interrupted by his premature death in 1970.

Abstract: this manuscript discusses the implications of the productive paradigm changes in health and environment of rural area of brazil. it analyzes production, economy and demography data, facing social and structural determinants with health and environmental reflexes, revealing a serious panorama that includes: a) environmental impacts associated to large productive chains, such as corn, soy-bean, cotton and sugar cane; b) chemical contamination of diverse environment compartments due to pesticides and fertilizers used in agriculture; c) the use of natural land in the expansion of agriculture and cattle farming activities; and d) the reflexes of these actions on individuals, workers or not, that are more and more forced to deal with extreme vulnerability situations because of the risk of rural work processes. thus, the present article points that macro-determinants (or driven-forces) tends to configure the health status of rural brazil inhabitants which, day after day, face a series of challenges in order to guarantee dignity in their work and quality of life.

Abstract: using an indicator of labor productivity, this paper aims to assess the contribution of structural changes in the brazilian manufacturing and mining industries to productivity growth, during the 1970-2001 period. the conclusion is that: (i) there is no evidence to sustain that the brazilian industry has definitely reversed the productivity growth rates slowdown trend; (ii) there is no clear evidence of a positive influence of the structural bonus; and (iii) job creation seems to be concentrated in low productivity growth sectors.

Abstract: The aim of this study was to bring out the applicability ofhomeopathy in the treatment of injuries in soccer. To ensure the achievement of the objective of this study it was decided to conduct an exploratory study,operated by means of literature. It is understood this strategy as being very appropriate in order to review, analyze, interpret and criticize theoretical considerations, even create new paradigms and propositions of explanation and understanding of phenomena from all branches of knowledge. From this study we can conclude that homeopathy, before his medical essence, seeks to assess the human being in its totality, always seeking the balance and strengthening its vital energy and may be used in the treatment of several pathologies. Regarding the treatment of sports injuries in soccer, it is concluded that homeopathy may be another therapeutic value, both to aid the healing process as well as in patients who opt for non-use of anti-inflammatory drug is the incompatibility,either the production of side effects they cause.
