Roflumilast attenuates allergen-induced inflammation in mild asthmatic subjects

Abstract: 25 subjects with mild allergic asthma were randomized to oral roflumilast 500 mcg or placebo, once daily for 14 days in a double-blind, placebo-controlled, crossover study. Allergen challenge was performed on Day 14, and FEV1 was measured until 7 h post challenge. Methacholine challenge was performed on Days 1 (pre-dose), 13 (24 h pre-allergen), and 15 (24 h post-allergen), and sputum induction was performed on Days 1, 13, 14 (7 h post-allergen), and 15.Roflumilast inhibited the allergen-induced late phase response compared to placebo; maximum % fall in FEV1 (p = 0.02) and the area under the curve (p = 0.01). Roflumilast had a more impressive effect inhibiting allergen-induced sputum eosinophils, neutrophils, and eosinophil cationic protein (ECP) at 7 h post-allergen (all p = 0.02), and sputum neutrophils (p = 0.04), ECP (p = 0.02), neutrophil elastase (p = 0.0001) and AHR (p = 0.004) at 24 h post-allergen.This study demonstrates a protective effect of roflumilast on allergen-induced airway inflammation. The observed attenuation of sputum eosinophils and neutrophils demonstrates the anti-inflammatory properties of PDE4 inhibition and supports the roles of both cell types in the development of late phase bronchoconstriction and AHR.ClinicalTrials.gov: NCT01365533Asthma is characterized by the presence of cough, wheeze, dyspnea, reversible airway obstruction and airway hyperresponsiveness. Abstract: this article presents and analyses the dramatic staging by the mother-of-saint of a terreiro in rio de janeiro, which recounts the departure, the voyage, and the arrival of african slaves in brazil. the play is put on as an educational activity by the children and members of the family-of-saint of the candomblé terreiro. the narrative conveys a central idea: that the slaves came to brazil bearing with them a material and intangible culture, and a memory of traditions transferred to brazil by those who were able to resist the laws of slavery and the conditions of servility. the candomblé, a religion with strong political implications in this instance, is a structural element of the narrative, as its deities become characters in the play, those of slaves and ancestors who become not only deities but also liberators. the figure of the slave as victim is thus reversed and transformed into hero, while a new narrative of the brazilian nation gives slaves and africa a leading and civilizing role.

Abstract: Dans ce papier, nous nous référons à la théorisation ancrée pour approfondir l’un des premiers segments de la trajectoire de la maladie cancéreuse, celui de l’annonce du diagnostic. Notre point de départ est la théorie des contextes de conscience , formulée en 1965 par Strauss et Glaser. Identifiées , durant la phase empirique, comme catégories principales ,par une analyse inductive, les différents contextes de l’annonce du diagnostic se sont construits à partir des différentes stratégies d’ annonce répertoriées au moment de l’ observation ,des intentions des professionnels, des attentes livrées par les patients en matière d’ information souhaitée, des conditions spécifiques entourant l’action .Les informateurs principaux(n= 21 ) sont des patients atteints d’ un cancer et hospitalisés pour une cure de chimiothérapie. D’autres informateurs contextuels ( cliniciens oncologues, bénévoles, parents et proches , cadres hospitaliers…) contribuent également au recueil des données. L’objectif poursuivi est de prendre en compte les motivations des praticiens et le sens qu’ils accordent à l’étape de l’annonce et de les recontextualiser en référence à l’encadrement prévu par l’instance normative . On y discute quelques conditions spécifiques, liées au contexte organisationnel qui freinent le degré d’ ouverture et on s’ interroge de l’ impact de ce manque d’ ouverture sur l’entrée dans la maladie ainsi que sur les prochaines étapes de chaque itinéraire thérapeutique
The physical environment and major plant communities of the Tankwa- Karoo National Park

Abstract: Apart from Acocks (1988) there are no published descriptions of the vegetation of the greater Tanqua and Doring River drainage basin (Bayer et al. 1993). A botanical and physical description of the Tankwa- Karoo National Park (TKRNP) which occurs in Veldtype 31b (Acocks 1988) is provided. The three dominant geological formations, older glacigenic deposits of the Dwyka Group, followed by the succession of siliciclas- tic sediments of the Permian Ecca Group, with flat dolerite sills and dykes, underlie eight distinct plant communities. The plant communities can be divided into large open plains dominated by Galenia africana and Tripteris sp. in the erosion rills, Malephora luteola and Augea capensis common in the low lying areas and Zygophyllum microcarpum, Brownanthus ciliatus and Galenia crystallina common on the more shaly concave plains and low shale hills. Slightly elevated rocky areas are dominated by Ruschia cf. robusta, Ruschia spinosa communities, while crusts of stemless mesembs such as Rhinephyllum macra denium, Hereroa fimbriata and Cheiridopsis acuminata are found on the desert paved areas. Annual Asteraceae covers all the denuded and sparsely vegetated areas after good winter rains while annual mesembs colonise on the more sodic sites. A total of 259 plant species were collected sporadically over a period of eight years, this includes 65 succulents and seven species endemic to the Tanqua Karoo and immediate adjacent area of the Roggeveld Mountain Range and Sutherland. Four Tanqua Karoo endemic species were found in the park.

Can Online Decision Aids Support Non-Cognitive Web Shopping Approaches?

Abstract: This article investigates two research questions concerning web shopping tools. The first asks how online decision aids can support a consumer’s non-cognitive decision processes. The second asks how these tools support non-cognitive online shopping for products of different categories. To answer these questions, the author conducted a thorough literature review in the fields of management information systems, e-commerce and consumer behaviour. The results show that e-shoppers may adopt several non-cognitive decision-making approaches. Not one tool is sufficient to support all of these, but web stores should offer a selection of decision aids to satisfy their customers’ needs. These tools need to be adapted as well to the categories of products offered in each web store.

Quelle analyse pour les pronoms clitiques du cadien ?

Abstract: Depuis les travaux de Perlmutter (1971), Kayne (1975) et Zwicky (1977), les pronoms clitiques des langues romanes en général et du fran ais en particulier ont fait l’objet d’une recherche intense. Ces travaux, en ce qui concerne le fran ais, se sont d’abord concentrés sur le fran ais standard ou de référence (FR) avant de se tourner dans les années 1990 vers des variétés de fran ais parlés informels (FPI) tels que celui de l’Hexagone (Zribi-Hertz, 1994), du Québec (Roberge,1990 et Auger,1995) ou encore du fran ais pied-noir (Roberge, 1990), l’accent étant mis le plus souvent sur les pronoms sujets. Deux approches principales se dégagent de ces recherches sur le fran ais, l’une traitant les pronoms clitiques comme des arguments et l’autre, commes des affixes flexionnels du verbe. Nous présenterons ici les pronoms clitiques sujets et objets du fran ais cadien, une variété de fran ais parlée dans le sud-ouest de la Louisiane, sur un territoire dénommé Acadiana, un triangle ayant pour base la c te du Golfe du Mexique et pour sommet, la paroisse d’Avoyelles. Les pronoms clitiques de cette variété de fran ais essentiellement orale, dérivée de dialectes du Grand- Ouest, Poitou principalement (Bodin, 1987), arborent un certain nombre de particularismes qui en font un terrain d’étude intéressant de la variation dans ce domaine en fran ais. Nous verrons que l’examen des données cadiennes révèle que les deux approches principales mentionnées ci-dessus pour le fran ais s’avèrent inadéquates pour rendre compte des pronoms clitiques de cette variété dont le comportement se distingue à la fois de ceux du FR et des FPI même s’il partage plus de traits communs avec ces derniers. Nous montrerons pour conclure que, si les pronoms clitiques cadiens semblent bien impliqués dans un processus de grammaticalisation en direction du statut d’affixes marqueurs d’accord, ils présentent plusieurs traits dont une telle analyse ne saurait rendre compte. Psychoeducational Interventions for Family Caregivers of Seniors across Their Life Trajectory: An Evidence-Based Research Program to Inform Clinical Practice

Abstract: Family caregivers of the elderly are growing in number and the care they are called upon to deliver in industrialized countries is becoming increasingly demanding and complex. Empirical research shows that the caregiving situation can have a significant impact on the health of these caregivers often on account of stress, physical and psychological exhaustion, and a sense of being overwhelmed. In this context, the quality of life of these caregivers depends in large part on professional educational and support interventions. The purpose of this paper is to present three innovative psychoeducational intervention programs developed and empirically tested by the research team of the Université de Montréal’s (Québec, Canada) Chair in Nursing Care for Seniors and Their Families over the past fifteen years. These interventions have been developed together with family caregivers experiencing different stressful situations across their care trajectory. The results of evaluative studies of these programs provide evidence to inform professional clinical practice. Future directions for caregiving research are discussed. 1. Introduction Family caregivers of the elderly are growing in number in North America, as in all industrialized countries, and the care they are called upon to deliver is becoming more complex. Research shows that caregiving can have a significant impact on their health. Indeed, caregiver health is often undermined by stress, physical and psychological exhaustion, and a sense of being overwhelmed—all ailments generated by the responsibilities of caring for their aging relatives [1, 2]. As a result, family caregivers are increasingly considered an at-risk clientele within the healthcare system and their quality of life (QoL) has come to depend on professional support in the form of educational interventions, counselling, and respite services. In response to the situation, numerous initiatives have been developed in the past few decades to help family caregivers. However, the results of evaluative research, systematic literature reviews, and meta-analyses regarding these initiatives have, in general, proved inconclusive [3–10]. In fact, the interventions and services developed have had modest effects at best and do not seem to have a significant influence on caregiver health and QoL. Among the reasons put forth to explain these conclusions, it appears above all that the elements of the proposed interventions are not specific enough to meet the particular needs of caregivers at the different stages of their trajectory.
