Gastroenteropancreatic Neuroendocrine Tumors in Multiple Endocrine Neoplasia Type 1

Abstract: We reviewed the literature about entero-pancreatic neuroendocrine tumors in Multiple Endocrine Neoplasia type 1 syndrome (MEN1) to clarify their demographic features, localization imaging, practice, and appropriate therapeutical strategies, analyzing the current approach to entero-pancreatic neuroendocrine tumors in MEN1. Despite the fact that hyperparathyroidism is usually the first manifestation of MEN1, the penetrance of these tumors is similar. They are characterized by multiplicity of lesions, variable expression of the tumors, and propensity for malignant degeneration. Both the histological type and the size of MEN1 neuroendocrine tumors correlate with malignancy. Monitoring of pancreatic peptides and use of imaging exams allow early diagnosis and prompt surgical treatment, resulting in prevention of metastatic disease and improvement of long-term survival. Surgery is often the treatment of choice for MEN1-neuroendocrine tumors. The rationale for surgical approach is to curtail malignant progression of the disease, and to cure the associated biochemical syndrome, should it be present.
Surgical approach in patients with hyperparathyroidism in multiple endocrine neoplasia type 1: total versus partial parathyroidectomy

Abstract: usually, primary hyperparathyroidism is the first endocrinopathy to be diagnosed in patients with multiple endocrine neoplasia type 1, and is also the most common one. the timing of the surgery and strategy in multiple endocrine neoplasia type 1/hyperparathyroidism are still under debate. the aims of surgery are to: 1) correct hypercalcemia, thus preventing persistent or recurrent hyperparathyroidism; 2) avoid persistent hypoparathyroidism; and 3) facilitate the surgical treatment of possible recurrences. currently, two types of surgical approach are indicated: 1) subtotal parathyroidectomy with removal of at least 3-3 k glands; and 2) total parathyroidectomy with grafting of autologous parathyroid tissue. transcervical thymectomy must be performed with both of these procedures. unsuccessful surgical treatment of hyperparathyroidism is more frequently observed in multiple endocrine neoplasia type 1 than in sporadic hyperparathyroidism. the recurrence rate is strongly influenced by: 1) the lack of a pre-operative multiple endocrine neoplasia type 1 diagnosis; 2) the surgeon's experience; 3) the timing of surgery; 4) the possibility of performing intra-operative confirmation (histologic examination, rapid parathyroid hormone assay) of the curative potential of the surgical procedure; and, 5) the surgical strategy. persistent hyperparathyroidism seems to be more frequent after subtotal parathyroidectomy than after total parathyroidectomy with autologous graft of parathyroid tissue. conversely, recurrent hyperparathyroidism has a similar frequency in the two surgical strategies. to plan further operations, it is very helpful to know all the available data about previous surgery and to undertake accurate identification of the site of recurrence.

Abstract: Background: Hepatocellular carcinoma (HCC) is one of the most common cancer in the world. Liver resection (LR) is the most used therapy in well compensated liver cirrhosis and maybe used as a first-line treatment. Aim of the study is to evaluate survival rates in patients who underwent LR for HCC and to identify risk factors able to influence the prognosis. Material/Method: A retrospective study was carried out in 115 patients whounderwent LR for HCC. We evaluated overall and disease-free survival rates at 1, 3 and 5years (y) and a series of variables included: type of resection, clamping, blood loss, transfusions, tumor size, presence of capsule, satellite nodules and vascular invasion. Results: The 1-, 3-, 5-y survival rates were 90.2%, 67% and 52.7%, and disease-free survival rates

Abstract: The aim of this paper is to present a new proposal for the classification of Academic institutions in terms of quality of teaching. Our methodological proposal borrows concepts from operational risk, such as scorecard models, employed to assess University performances, on the basis of both the perceived and the actual quality. We propose to summarize opinion data using new non parametric indexes able to exploit efficiently the ordinal nature of the analysed variables and to integrate different sources of data. In particular we show how web survey methods can improve the quality and robustness of collected data, especially when integrated with students career data. Empirical evidence is given on the basis of real data from the University of Pavia.

Abstract:

This contribution deals with a generative approach for the analysis of textual data. Instead of creating heuristic rules forthe representation of documents and word counts, we employ a distribution able to model words along texts considering different topics. In this regard, following Minka proposal (2003), we implement a Dirichlet Compound Multinomial (DCM) distribution, then we propose an extension called sbDCM that takes explicitly into account the different latent topics that compound the document. We follow two alternative approaches: on one hand the topics can be unknown, thus to be estimated on the basis of the data, on the other hand topics are determined in advance on the basis of a predefined ontological schema. The two possible approaches are assessed on the basis of real data.

Systemic Risk of Conventional and Islamic Banks: Comparison with Graphical Network Models  [PDF]

Abstract: The main aim of this paper is to compare the stability, in terms of systemic risk, of conventional and Islamic banking systems. To this aim, we propose correlation network models for stock market returns based on graphical Gaussian distributions, which allows us to capture the contagion effects that move along countries. We also consider Bayesian graphical models, to account for model uncertainty in the measurement of financial systems interconnectedness. Our proposed model is applied to the Middle East and North Africa (MENA) region banking sector, characterized by the presence of both conventional and Islamic banks, for the period from 2007 to the beginning of 2014. Our empirical findings show that there are differences in the systemic risk and stability of the two banking systems during crisis times. In addition, the differences are subject to country specific effects that are amplified during crisis period.
