Endotension: rupture of abdominal aortic aneurysm

Abstract: aortic endovascular exclusion technique called 'chimney' consists of placing stents through abdominal aortic visceral branches and a prosthesis that excludes the thoraco-abdominal aneurysm. stents and an aortic endoprosthesis are placed in the renal arteries. this method is primarily used when open surgery is too risky. the mechanism that provides aneurysm sac increase without the visible presence of endoleaks has not been fully elucidated. the expansion of the aneurysm sac, due to endotension, is difficult to diagnose, even with the use of advanced imaging tests. its diagnosis is made by exclusion. we present a case of a late complication in a high-risk patient after a 'chimney' endovascular procedure. following the surgery, the patient presented a ruptured aneurysm sac without a visible endoleak. a second intervention was not feasible due to the high risk of occluding all of the branches, and complicated by previous 'chimney'. endotension is a possible cause of aneurysm rupture and death.
The Mathematics in the Social Studies Textbook: A Critical Content Analysis and Implications for Students’ Reasoning  [PDF]

Abstract: We report on a case study of the?mathematical?content of a 10th?grade?social studies?textbook. We develop our case in three analytical steps. First, we identify, describe, and categorize the full range of mathematics in the book. Put simply, we ask: What mathematical forms (e.g., Cartesian graphs and problems) do we find and what kinds of mathematical work do they require? Second, we characterize and critically evaluate the mathematics content in the textbook, focusing in particular on the kinds of mathematics literacy and student reasoning that the book fosters. Third and finally, we operationalize a measure of the “density” of mathematics in the textbook—that is, an estimate of the presence and pervasiveness of mathematical objects and practices relative to other disciplinary contentsand track how such “density” has changed over the past three editions of the same volume. Doing so helps us further contextualize and elaborate the prior analyses, but also surfaces shifts in the patterns of mathematics presence in that textbook series, including the growing encroachment of mathematics exercising and visual/representational presence.
Diuretics: again the first step in the treatment of most patients with hypertension

Abstract: The stepped care approach for the management of hypertension was introduced in the first United States National Guidelines issued by the National High Blood Pressure Education Program [1]. Diuretics were indicated as the first step, based on their efficacy in the first trials of prevention of cardiovascular disease and because of their better tolerability compared to the other antihypertensive drugs that were available at that time, such as the ganglion blocking agents, guanethidine, hydralazine, and reserpine. These other antihypertensive drugs were recommended as second and third steps for patients who were not controlled by diuretics alone.In the report by the Joint National Committee on Detection, Evaluation, and Treatment of High Blood Pressure (JNC) in 1984 (JNC-III) [2], the first recommended step was extended to include β-blockers. Four years later, in the JNC-IV report, the recommendations were extended further to include four classes of blood-pressure-lowering drugs [3]. In the subsequent guidelines, the Committee returned to diuretics or β-blockers as the first antihypertensive option, but, because of the "compelling indications", the recommendations were extended to include a wide range of the other classes of blood-pressure-lowering drugs [4].The introduction of newer classes of antihypertensive drugs was based on their presumed better tolerance and on the expectation that they would improve the prevention of coronary heart disease, which was lower than expected in the first trials, mainly attributed to adverse metabolic effects of diuretics. Physicians around the world became convinced of these advantages and the use of the newer classes of drugs surpassed diuretics and β-blockers for antihypertensive treatment.The results of recent trials and reanalysis of the previous ones show that we should return to older practices. Abstract: bakhtin analyzed dostoevsky's poetics not by a partial synthesis in terms of the speech of the writer or one of his characters, but through the dialogue that would leave its contingency to assume a structural role. however, bakhtin could not demonstrate how dostoevsky's work would constitute an integral polyphonic totality. with the contribution from the critical theory, we will try to reapproach bakhtin's polyphony to the contradictory notion of totality through the dialectical method.

Abstract: the article focuses on the conditions in which research in higher education is carried out in brazil, at the level of post graduate studies in our universities. the main concern is the assessment process of the post graduate courses supervised by capes. after a brief presentation of the emergence and recent evolution of the country's post graduate studies in education, principles, procedures and effects of the evaluative process are related to a culture of performativity, as conceived by the british researcher stephen ball. it is argued that such category is helpful to the analysis of capes evaluation. in the final comments, the environment of post graduate courses, characterized by a culture of performativity, is compared to the way teachers have been reacting to and experiencing recent curricular reforms, according to ivor goodon's recent research. possible alternatives are then sought of.

Abstract: the article presents serial data on african atlantic demographics in rio de janeiro in the seventeenth, eighteenth, and early decades of the nineteenth century, highlighting parish death, marriage, and baptismal registers. these represent partial, local findings from a broader demographic study underway on various urban and rural regions of slaveholding brazil, based on a variety of historical registers, which are used in an analysis of sociodemographic patterns (age, occupation, kinship, disease, gender, price) and variations in african ethnonyms from 1650 through 1870.

Abstract: the paper starts with the idea that urban space is not a product of nature but of men′s labour. it proceeds presenting a new way of focusing and analyzing urban segregation, using the city of s?o paulo as a case study. through several social indicators, presented in 8 illustrations, it shows urban segregation in s?o paulo. it shows however that this new way also applies to any brazilian metropolitan area. important aspects shown by this new way are: on the one hand, the possibility it offers of analyzing the relationship between urban segregation of residences, working places and the overall urban spatial structure; on the other hand, the possibility of analyzing the relationship between urban segregation and the social inequality which prevails in brazilian society as well as with social domination. it finally shows the relationship between human production of urban space and human production of time and the importance of the former.

Abstract: à partir d’une enquête avec des pères homosexuels fran?ais membres de l’association des parents et futurs parents gays et lesbiens (apgl), cet article analyse les contextes sociaux d’émergence du désir d’enfant et certains enjeux liés à l’élaboration du projet parental. l’univers associatif est ici considéré non seulement comme un lieu de visibilité, mais aussi comme un espace de création de nouvelles formes familiales. ainsi, le désir d’enfant est analysé comme un fait social dont l’expression et la genèse peuvent être situées dans des contextes socioculturels particuliers.

Abstract: the ubiquitousness of normal yawning and the existence of abnormal yawning warrant an understanding of this reflex. its mechanisms and functional role are not entirely known. a review of the literature reveals that yawning is a brain stem arousal reflex with both peripheral and central loops subserving reversal of brain hypoxia or hypoxemia. behaviorally, yawning is a semi-involuntary act that occurs also because of loss of interest in the surroundings and it is not necessarily associated with fatigue. socio-environmental factors can influence the emergence of yawning. dopaminergic, acetylcholinergic, acthergic and oxytocinergic systems are involved in the generation and modulation of yawning in animal experimentation.
