Usefulness of Liver and Spleen Acoustic Radiation Force Impulse (ARFI) for the Evaluation of Cirrhotic Patients  [PDF]

Abstract: Objective: To evaluate the correlation between ARFI and Child-Pugh classification. Secondary Aims: 1) To compare ARFI values (hepatic, splenic and spleno-hepatic index) from cirrhotic to normal population; 2) To correlate biochemical parameters of liver function and ARFI. Materials and Methods: 58 cirrhotic patients (referenced to US for surveillance or to clarify any hepatic decompensation) were included in this prospective study, as well as 38 healthy subjects who underwent ultrasonography for other reasons than hepatic evaluation. All had ARFI liver and spleen evaluation on ACUSON S2000 ARFI equipment. The best cut-off liver and spleen values for the diagnosis of cirrhosis in comparison to the normal subjects were determined using SPSS v20. Results: Mean liver ARFI values in controls and cirrhotic patients were respectively 1.18 ± 0.22 m/s and 2.93 ± 0.50 m/s. The ROC curve demonstrated an AUC 0.998 and the best cut-off was 1.89. Mean spleen ARFI values in controls and cirrhotic patients were respectively 2.60 ± 0.42 m/s and 3.03 ± 0.71. The ROC curve demonstrated an AUC 0.766 and the best cut-off was 2.73 m/s. The splenohepatic index showed a worse AUC than ARFI liver. A weak correlation was found between the ARFI liver and Child-Pugh. We found no statistically significant differences for spleen ARFI values and Child-Pugh. We found a statistically significant correlation between liver ARFI and bilirubin, ALKP, GGT, AST and AST/ALT ratio; and with spleen ARFI and ALKP and AST/ALT ratio. Conclusion: We showed that there is a tendency of higher levels of liver ARFI values for higher Child-Pugh classification suggesting a definite trend for higher values with more severe disease.

Abstract: Pancreas transplantation is an effective treatment for type 1 diabetes mellitus and is being increasingly performed worldwide. Early recognition of graft-related complications is fundamental for graft survival; thus, radiologists must be aware of the transplantation technique, pancreas-graft imaging and postoperative complications. We present normal pancreas-graft imaging appearances and the imaging features of postoperative complications.

Abstract: The modular vector field of a Poisson-Nijenhuis Lie algebroid $A$ is defined and we prove that, in case of non-degeneracy, this vector field defines a hierarchy of bi-Hamiltonian $A$-vector fields. This hierarchy covers an integrable hierarchy on the base manifold, which may not have a Poisson-Nijenhuis structure.

Abstract: asthma is a very prevalent chronic disease in the general population, characterized by reversible obstruction of respiratory airways. multiple studies demonstrate an improper control of the disease, and one of the main reasons for that is reduced adherence to treatment. non-adherence can be classified as non intentional or intentional, and the causes can be related to the motivations, beliefs and knowledge of the patient or due to the therapeutic regimen. the reasons for non-adherence are non-comprehension of the treatment goals; non-comprehension of disease severity; fears related to drugs; psychological conditions such as depression; complexity of treatment regimen; difficulties with administration route, mainly inhaler. after recognizing these issues as cause of non-adherence, different approaches should be considered: set up a good physician -patient communication; promote asthma education; schedule regular appointments; simplify therapeutic regimen. it is important to promote adherence to achieve a good asthma control, and consequently a better quality of life and reduction of asthma associated costs.

Abstract: In medical practice, the treatment and prognosis in the end of life has associated to ethical dilemmas, which are established in conformity with individual or collective beliefs and values. Dysthanasia from Greek, means to make death difficult, and it is currently an ethical problem with significant consequences. Considering that death itself has two moments, the process of death and the moment of death, dysthanasia is the undue prolongation of the process of death with the help of technological devices that allows the life sustaining procedures. Although it is through the technological advances that the moment of death can be delayed, it is the beliefs and values that are deep rooted in the sub conscience of the physicians that are responsible for the demeanor of end of life ethical dilemmas. Beliefs and values when encompassed in areas like phenomenology of knowledge, dialectic of technology, conflict of values and existentialism and metaphysics, can somehow explain this issue that is current, emerging and compelling.

Abstract: the concept of "mental health" is comprehensive, and it isn't always easy to define or to identify its determinants. however, in the same way that "health" isn't merely the absence of disease, mental health is also more than just the absence of mental disorder. thus, it has increasingly been understood as the product of multiple and complex interactions that include biological, psychological and social factors. this article reviews some of the social and economic determinants of mental health, including factors like working conditions and unemployment, education, poverty, housing conditions, urbanization, sexual discrimination and gender based violence, early experiences and family interactions, social exclusion and stigma, culture, and stressful life events. finally, the importance of the knowledge of social and economic determinants of mental health in a public health perspective is also highlighted, as well as its relevance to reduce the global burden of disease and to improve the mental health of populations.

Abstract: the purpose of this research is to study depression in institutionalized older people in the district of bragan?a. the aims are to determine the prevalence of depression and assess the existence of some known risk factors for its appearance and maintenance. the participants were 186 elderly residents in 14 residential homes in the district of bragan?a. instruments used were the mini mental state examination, the geriatric depression scale and the barthel index. to estimate the adjustment to institutional status and importance given to participation in leisure activities, the index of adaptation (a = 0.61) and the index of activity and recreation (a = 0.63) were created. the feeling of loneliness was self-reported by participants. a high prevalence rate of depression (47%) was observed. depression was more prevalent among women (51%) than men (40%). depression was also related to lower cognitive levels, lower adjustments to institutional life, reduced participation in leisure activities, higher rates of loneliness and greater dependency in activities of daily living. in summary, almost half of the older people studied in the district of bragan?a were depressed. depression in the institutional context was predicted by loneliness, less importance given to leisure activities and regarding the male or female condition.

Abstract: Esta investiga o pretende estudar a depress o em idosos institucionalizados do distrito de Bragan a. Tem como objetivos determinar a prevalência da depress o e apurar a existência de alguns dos fatores de risco conhecidos para o seu aparecimento e manuten o. Participaram neste estudo 186 idosos residentes em 14 lares do distrito de Bragan a. Foram utilizadas a Escala de Avalia o do Estado Mental, a Escala de Depress o Geriátrica e o índice de Barthel. Para estimar a adapta o à situa o institucional e a importancia dada à participa o em atividades de lazer, construíram-se os índices de Adapta o (a = 0.61) e de Atividade de Lazer (a = 0.63). O sentimento de solid o foi autoavaliado pelos sujeitos. Observou-se uma elevada taxa de prevalência de depress o (47%), mais prevalente entre as mulheres (51%) do que entre os homens (40%). A depress o relacionou-se com o menor nível cognitivo, menor adapta o à vida institucional, menor importancia dada às atividades de lazer, maior índice de solid o e maior dependência nas atividades de vida diárias. Em suma, quase metade da popula o de idosos inquiridos apresenta depress o que foi prevista pela solid o, pela menor importancia dada às atividades de lazer e em fun o de pertencer ao sexo masculino ou feminino. Esta investigación pretende estudiar la depresión en adultos mayores institucionalizados en el distrito de Bragan a. Tiene por objetivos determinar la prevalencia de la depresión y determinar la existencia de algunos de los factores de riesgo conocidos para su aparecimiento y mantenimiento. Participaron en este estudio 186 adultos mayores residentes en 14 residencias de ancianos del distrito de Bragan a. Fueron utilizadas las escalas de Estado Mental y de Depresión Geriátrica, así como el índice de Barthel. Para estimar la adaptación a la situación institucional y la importancia dada a la participación en actividades del tiempo libre se construyeron índices de Adaptación (a = 0.61) y de Actividad de Ocio (a = 0.63). El sentimiento de soledad fue autoevaluado por los sujetos. Se observó una elevada tasa de prevalencia de depresión (47%), más prevalente entre las mujeres (51%) que entre los hombres (40%). La depresión se relacionó con el menor nivel cognitivo, menor adaptación a la vida institucional, menor importancia dada a las actividades del tiempo libre, mayor índice de soledad y mayor dependencia en las actividades de la vida diaria.
