Mesenteric Adenitis as a Differential Appendicitis Diagnosis: Case Report  [PDF]

Abstract: Mesenteric adenitis consists of an important differential diagnosis of potentially serious diseases such as acute appendicitis, since they manifest themselves with a similar clinical picture. Case report: A 17-year-old male patient presented with periumbilical abdominal pain for 2 days, of abrupt onset and moderate intensity, who gave away with a common analgesic, which progressed to the right iliac fossa (RIF) and increased of intensity. He also reported liquid diarrhea, with several episodes a day, without mucus and/or blood, 38°C fever, anorexia and vomiting. He was hospitalized for investigating a possible diagnostic of Acute Appendicitis. In the exams, it was considered mesenteric adenitis, and antibiotic therapy was prescribed. The patient had complete improvement at the end of treatment. Conclusions: Given its importance as a differential diagnosis of numerous pathologies, as well as controversial before the therapeutic and diagnostic conduct, it is essential to report these cases in order to elucidate the specificities of this disease.

Abstract: Introduction: Splenic cysts are uncommon and usually do not generate signs and symptoms, being diagnosed by incidental findings. Case report: A 35-year-old, female patient attended the Luiz Gioseffi Jannuzzi School Hospital (HELGJ) presenting episodes of vomiting and abdominal pain in the left hypochondrium. She was hospitalized for diagnostic investigation. Computed tomography showed the splenic cyst. The procedure was the surgical approach by laparotomy with splenectomy. Histopathological confirmed the diagnosis. Discussion: Compared to the scarce reports in literature, the case described presented typical epidemiology and clinical features. Conclusions: Given the rarity of this pathology cases, it is essential to report these in order to elucidate the specificities and also to discuss diagnostic and therapeutic methods.

Abstract: Introduction: Gastrointestinal stromal tumor (GIST) occurrence is uncommon and usually manifested by nonspecific signs and symptoms, which hinders its reasoning as a diagnostic hypothesis. The objective is to report a case of female GIST with curative treatment through video-laparoscopy with partial gastrectomy. Case report: A 61-year-old female patient attended the Luiz Gioseffi Jannuzzi School Hospital (HELGJ) emergency room, with a sudden onset of upper gastrointestinal bleeding with hemodynamic instability. After stabilization, she was admitted for diagnostic investigation, showing sub-epithelial lesion of antrum in upper digestive endoscopy (UDE) and solid-cystic mass in the gastric antrum to computed tomography (CT). The conduct was the video laparoscopy surgical approach with partial gastrectomy. The histopathological and immunohistochemical aspects of the surgical specimen showed a gastrointestinal stromal tumor (GIST) in a gastric antrum of low grade of malignancy, positive e-Kit and mycotic index (50 cga) < 5. Discussion: Compared to the scarce reports in literature, the case described presented typical epidemiology, but the clinical picture was not the most incident. Conclusions: Given the rarity of this pathology cases, it is essential to report these in order to elucidate the specificities and also to discuss diagnostic and therapeutic methods.

Abstract: putrescine, spermidine and spermine contents and peroxidase activity (ec 1.11.1.7) were determined in plants of phaseolus vulgaris l. cv carioquinha submitted to saline stress (50 and 100 mm nacl). changes were observed in polyamine contents, mainly in putrescine. increase in peroxidase activity also occurred with nacl concentration, for both concentrations used. the results showed alterations in the polyamines contents and peroxidase activity in plants of phaseolus vulgaris grown under salinity.

Abstract: O teor de poliaminas (putrescina, espermidina e espermina) e a atividade enzimática da peroxidase (EC 1.11.1.7) foram determinados em plantas de Phaseolus vulgaris L. cv Carioquinha, após terem sido submetidas a estresse salino (50 e 100 mM de NaCl). Foram observadas altera es nos teores das poliaminas, principalmente putrescina, que aumentou com o tempo e a concentra o de NaCl. Também ocorreu aumento na atividade da peroxidase em ambas concentra es de NaCl utilizadas. Os resultados mostraram altera es no metabolismo de poliaminas e peroxidases nas plantas de feij o cultivadas em meio salino.
Recognition of hand shape drawings on vertical and horizontal display

Abstract: the visual recognition of body parts activates somato-motor representations in the brain. in the present study, we investigate the influence of the plane in which hand drawings are displayed (vertical or horizontal) on mental rotations evoked by a handedness recognition task. sixteen right-handed volunteers participated in an experiment where the task was to evaluate the handedness of drawings of the human hand presented in different perspectives and orientations while the manual reaction time (mrt) was measured. for eight volunteers, the hand drawings were displayed on a vertical screen monitor, while for the remainder a mirror was employed and the same drawings appeared on the horizontal plan. our main finding was that there are no differences in mrts among the drawings displayed vertically or horizontally, with some exceptions. however, the mrts were longer when the hands in the drawings assumed configurations that were more awkward to perform using real movements. these results show that the implicit movements involved with handedness recognition are mainly dependent on biomechanical constraints for distal (hand), but not proximal (shoulder) movements.

Abstract: objective: the aim of this study was to present a minimally invasive anterolateral access route and to ascertain whether this enables total hip replacement without compromising the quality of the implant positioning, while maintaining the integrity of the gluteus muscles. method: a retrospective study was conducted on 260 patients (186 females and 74 males) of average age 62 years. there were 18 bilateral cases, thus totaling 278 hips. all the patients had osteoarthritis and had undergone non-cemented total hip arthroplasty (metal-metal or metal-polyethylene) between october 2004 and december 2007. a minimally invasive anterolateral access route was used, measuring 7 to 10 cm in length, according to body weight and the size of the femoral head. the patients were assessed clinically regarding age, sex and presence of the trendelenburg sign, and radiologically regarding acetabular and femoral positioning. results: the acetabular inclination was between 30° and 40° in 78 patients, between 41° and 50° in 189 patients, and 51° or over in 11 patients. on anteroposterior radiographs to study femoral positioning, the positioning was central in 209 cases, 41 presented valgus deviation and 28 presented varus deviation. on lateral views, 173 were central, 67 anterior and 38 posterior. the mean duration of the procedure was 63 minutes. regarding complications, there were five cases of infection, three of deep vein thrombosis, two of hip dislocation, 80 of lengthening of the lower limbs and five of shortening of the operated limb. the trendelenburg sign was present in four cases, of which one showed superior gluteal nerve injury. conclusion: the minimally invasive anterolateral access route made it possible to perform total hip arthroplasty without compromising the positioning of the implants, thereby maintaining the integrity of the gluteus muscles.

Abstract: Periodontal disease is one of the possible risk factors for cardiovascular diseases. Among cardiovascular diseases, atherosclerosis stands in bold relief. The endotoxin increase at periodontal tissue and/or its introduction at systemic circulation can produce atherosclerosis and start a thrombus formation. One of the possible mechanisms of this interaction can be represented by pathogenic events similar between both diseases. Periodontal prevention and treatment seems to minimize its role as risk factor for cardiovascular diseases. More studies are necessary to determine the association between periodontal disease and cardiovascular disease.

Abstract: Introduction: Syphilis consists in a systemic infect contagious pathology with a chronic character. The etiological agent consists of an anaerobic spirochete bacterium, Treponema pallidum. Laboratory diagnosis can be made through a direct investigation of the etiologic agent or non-treponemal (VDRL) and treponemal (FTA-Abs) serological tests. False-negative results are rare. Objectives: The present report presents a rare case of a man with a secondary syphilis syndrome who was initially undiagnosed due to the prozone effect, since he presented VDRL in low titers, and was later confirmed with a new diluted serum sample and VDRL and FTA-Abs, which were positive. Conclusion: A close look should be given to patients who, although the clinical condition is compatible, complementary exams may not be, and clinical cases should be carefully evaluated so that the patient is not treated late.
Análisis retrospectivo de la continencia urinaria en pacientes sometidos a braquiterapia post-resección transuretral prostática

Abstract: introduction and objective: some candidates for brachytherapy present with a previous history of prostate transurethral resection (turp). nonetheless, turp generates a cavity that may lead to technical difficulties in performing brachytherapy. such condition make a history of previous turp a relative contra-indication for brachytherapy. the aim of this study is to evaluate the role of brachytherapy in urinary continence, in a group of patients with previous history of turp. materials and methods: in our study, we analysed a group of 16 patients submitted to turp pre-brachytherapy. the mean quality of life score from icsmalesf questionnaire ranged from 0 to 1 before and after brachytheraphy (average: 0,18 -before; average: 0,20 - after). there was no estatistically significant difference between the former values (p< 0,001). after brachytherapy patients were evaluated for lower urinary tract symptoms. the time interval between turp and brachytherapy ranged from 30 to 90 days (average: 60 days). post-brachytherapy follow-up varied between 3 and 60 months (average: 30 months). results: none of the 16 patients evaluated presented lower urinary tract symptoms nor urinary incontinence. medical complications observed in patients were: dysuria, erectile dysfunction, scrotal ecchymosis and urinary retention . the cases of dysuria, scrotal ecchymosis and urinary retention observed were of short-duration, the last one with spontaneous resolution after 7 days of vesical catheterism. the patient presenting erectile dysfunction had a good response to oral type 5 fosfodiesterase inhibitor three times a week, after a six month period. conclusion: in accordance with other studies, we believe that with some strategy turp should not be considered a relative contra-indication for brachytherapy.
