Aplastic anemia: immunosuppressive therapy in 2010

Abstract: Acquired aplastic anemia (AA) is the typical bone marrow failure syndrome characterized by an empty bone marrow; an immune-mediated pathophysiology has been demonstrated by experimental works as well as by clinical observations. Immunusuppressive therapy (IST) is a key treatment strategy for aplastic anemia; since 20 years the standard IST for AA patients has been anti-thymocyte globuline (ATG) plus cyclosporine A (CyA), which results in response rates ranging between 50% and 70%, and even higher overall survival. However, primary and secondary failures after IST remain frequent, and to date all attempts aiming to overcome this problem have been unfruitful. Here we review the state of the art of IST for AA in 2010, focusing on possible strategies to improve current treatments. We also discuss very recent data which question the equality of different ATG preparations, leading to a possible reconsideration of the current standards of care for AA patients.
Repeated Physical Training and Environmental Enrichment Induce Neurogenesis and Synaptogenesis Following Neuronal Injury in an Inducible Mouse Model  [PDF]

Abstract: Neuronal loss as a consequence of brain injury, stroke and neurodegenerative disorders causes functional impairments ranging from cognitive impairments to physical disabilities. Extensive rehabilitation and trainning may lead to neuroprotection and promote functional recovery, although little is known about the molecular and cellular mechanisms driving this event. To investigate the underlying mechanisms and levels of functional recovery elicited by repeated physical training or environmental enrichment, we generated an inducible mouse model of selective CA1 hippocampal neuronal loss. Following the CA1 neuronal injury, mice underwent one of the above mentioned conditions for 3 months. Exposure to either of these stimuli promoted functional cognitive recovery, which was associated with increased neurogenesis in the subgranular zone of dentate gyrus and enhanced synaptogenesis in the CA1 subfield. Notably, a significant correlation was found between the functional recovery and increased synaptogenesis among survived CA1 neurons. Collectively, these results support the utilization of cognitive and physical stimulation as approaches to promote recovery after neuronal loss and demonstrate the potential of this novel mouse model for the development of therapeutic strategies for various neurological disorders associated with focal neuronal loss.

Abstract: objective: identify the rate of absenteeism in the nursing team in surgical-clinical units of a philanthropic hospital in the interior of the state of s?o paulo, and determine the technical safety index for each professional category. methods: data were collected from documents of the nursing and personnel departments, covering the year 2006, using the equations proposed by gaidzinski. results: the percentage of planned absences (days off, holidays and vacations) corresponded, respectively, to 17%, 3.1% and 9%. maternity leaves were identified as the main reason for unplanned absences for the nursing workers, followed by medical leaves. the technical safety index varied from 32% to 47% for nurses and from 35% to 40% for nursing auxiliaries and nursing technicians. conclusion: the identification of the safety index for covering absences provides nurses with tools for planning nursing human resources quanti-qualitatively.

Abstract: serum lipid levels are multifactorial traits determined by a high number of genetic and environmental factors. the identification of the genetic component of this traits have been intensively investigated in the last years. these studies focused mainly on polymorphisms in genes coding for structural proteins and enzymes related to lipid metabolism. more recent studies showed that the effect of these polymorphisms depend in part on the interactions of different genotypes with classic risk factors like tabagism, overweight or physical inactivity. the variability observed in these genes seems to influence drug response of those commonly used in hyperlipidemia treatment.

Abstract: the objectives of the present study were to: characterize medical-surgical clinic units of a philanthropic hospital and the nursing team performing in these units; identify the clientele's care profile, investigate the time length of care provided to the patients by the nursing team. data were collected from documents of personnel and nursing departments and the application of the classification instrument of patients. to calculate the care hours; the equation proposed by the hospital commitment quality (hcq) was used. the findings showed relationship of 1:1 nursing professional/bedside and variation from 0.10 to 0.21 nursing/bedside. the patients needed, mainly, minimum (47.1% to 79.6%) and intermediate (17.7% to 38.6%) care, and the average time spent by the nursing team ranged from 4.1 to 5.1 hours. the values showed that the care hours provided by the nursing team were not enough to assist to the patients' care complexity.
Restructuring the Nursing Staff and its Influence on Care Hours

Abstract: this descriptive study aimed to calculate and compare the nursing staff at the medical-surgical clinical units of a philanthropic hospital in current and projected situations, and to investigate how much time the nursing team delivers patient care in the current and projected situations. gaidzinski's method was used to calculate the nursing staff, and the equation proposed by the hospital quality commitment (hqc) to estimate care hours. the findings showed an increase of 33% in the staff, with a 68.4% increase in the number of nurses and 15.6% in the number of technicians / nursing auxiliaries. according to the projected situation, the care hours varied from 5.7 to 7.2. the number of nursing and the mean care time provided to the patients were inadequate according to the clientele's care needs. this could impair the quality of care.
Social representations about support for breastfeeding in a group of breastfeeding women

Abstract: this study aimed to get to know the social representations about support for breastfeeding in a group of breastfeeding women, as well as to identify the actions in their social environment these women perceive as supportive in their breastfeeding processes. data were collected through a qualitative approach, using recorded semistructured interviews, organized in accordance with the collective subject discourse and analyzed under the premises of social representations theory. results showed that the representations of women in this study about support for breastfeeding consist of actions available in the hospital, family and work contexts. in these women's perspective, support is a broad phenomenon that involves aspects of encouragement, promotion and protection to breastfeeding.

Abstract: Os níveis de lipídeos séricos s o características multifatoriais determinadas por um grande número de fatores genéticos e ambientais. A identifica o do componente genético dessas características tem sido intensamente investigada nos últimos anos. Esses estudos têm enfocado principalmente polimorfismos nos genes que codificam proteínas estruturais e enzimas relacionadas com o metabolismo de lipídeos. Estudos mais recentes mostraram que o efeito desses polimorfismos depende em parte das intera es dos diferentes genótipos com os fatores de risco clássicos tais como tabagismo, sobrepeso ou sedentarismo. A variabilidade encontrada nesses genes parece também influir na resposta a fármacos comumente utilizados no tratamento das hiperlipidemias.

Abstract: hematopoietic stem cell proliferation leads to telomere length decreases at each cellular division. some authors disagree about the telomere influence on the reduction of the proliferative potential and capacity of self renewal. here we review telomerase function in the biology of the hematopoietic system, in normal or differentiation and its influence on the ageing of hematopoietic stem cells. the constant cellular renewal required to maintain the hematopoietic system, provides embryonic stem cells, as well as malignant cells, an increased proliferative capacity. this is marked by the detection of telomerase enzyme activity and possible telomere maintenance. clinical trials will be required to clarify telomerase activity in hematopoietic stem cells, its possible use as a diagnostic marker and its use for prognostic purposes.
