Desempenho funcional de pacientes com fibrose cística e indivíduos saudáveis no teste de caminhada de seis minutos

Abstract: objective: to compare patients with cystic fibrosis and healthy individuals in terms of their functional performance on the six-minute walk test (6mwt). methods: a prospective, cross-sectional study involving healthy individuals and patients with cystic fibrosis treated at a referral university hospital in the city of campinas, brazil. the 6mwt was administered in accordance with the american thoracic society guidelines, and it was repeated after a 30-min rest period. for all of the participants, rr, hr, spo2, and borg scale scores were obtained. for the cystic fibrosis patients, nutritional status and spirometric values were determined. patients with pulmonary exacerbation were excluded. spearman's correlation coefficient and repeated measures anova were used. results: the cystic fibrosis group comprised 55 patients, and the control group comprised 185 healthy individuals. the mean ages were 12.2 ± 4.3 and 11.3 ± 4.3 years, respectively. the six-minute walk distance (6mwd) was significantly shorter in the cystic fibrosis group than in the control group for both tests (547.2 ± 80.6 m vs. 610.3 ± 53.4 m for the first and 552.2 ± 82.1 m vs. 616.2 ± 58.0 m for the second; p < 0.0001 for both). the 6mwd correlated with age, weight, and height only in the cystic fibrosis group. during the tests, spo2 remained stable, whereas hr and rr increased. conclusions: in our sample, functional performance on the 6mwt was poorer among the cystic fibrosis patients than among the healthy controls in the same age bracket, and we found immediate repetition of the test to be unadvisable.
Pneumonia complicada por pneumatocele gigante em crian?a com síndrome da imunodeficência adquirida: importancia da fisioterapia respiratória

Abstract: objective: to describe the effects of chest physiotherapy in preventing respiratory complications in a child with acquired immunodeficiency syndrome and a giant pneumatocele. case description: an eight-year-old female child was sent to the physiotherapy service of a university hospital due to pulmonary hypersecretion, repeated pneumonias and presence of a giant pneumatocele. the physiotherapy team selected active airway clearence techniques and expiratory flow techniques to promote pulmonary hygiene and to reduce risks for respiratory complications. the follow-up around was accomplished by a multiprofissional team with the objective of avoiding recurrent pulmonary infections, pneumatocele's rupture and need for long-term hospitalization. the child's pulmonary condition remained stable and no changes in the size of the pneumatocele were seen in repeated x-rays. comments: the role of chest physiotherapy in patients with giant pneumatoceles is not well defined in the literature. there is no evidence of beneficial effects of this therapy and there are no guidelines that address the most efficient and safest techniques. currently, much is discussed about the risks and complications arising from chest physiotherapy. in this case, regular physiotherapy intervention was important to maintain child's pulmonary integrity, but studies are needed to evaluate techiques and clinical relevant effects.

Abstract: from the study of globo repórter television program this article discusses the media articulations between nature and gender issues. we selected two editions, shown in consecutive weeks, which undertake great effort to explain the differences of gender and for maternal love, since essentialists notions and the privilege of the biologic body, in accordance with the efforts these medical sciences have been making in search of the "truth" about the issue, despite the social and cultural aspects of this construction.

Abstract: on main purpose of scientific production analysis, published in the period between 1998-2007, on the subject of psychotherapy, take into consideration: distribution of publications year by year; regular advisers; quantity and gender of authors; types of research; focus on the theory of the productions; institutional affiliation of the authors and strategies for psychological evaluation. the body is constituted by articles published in scientific periods, indexed to the pepsic databases (n=117) was obtained by the time the word "psychotherapy" was used. the analysis presented is quantitative. authors of the productions are, mainly, the feminine sex. there were a higher number of theoretical works. five from the regular advisers own 50.5% of the entirely searched production. psychoanalytic approach had more theoretical dominance. most authors are affiliated to brazilian institutions, which are concentrated in the southeast region. there were a higher prevalence for the use of interviews and scales as assessments strategies.
Tratamento cirúrgico do pneumotórax espontaneo primário no primeiro episódio

Abstract: purpose: to assess the results of the surgical treatment of primary spontaneous pneumothorax (psp) by means of axillary thoracotomy, blebs resection and abrasive pleurodesis, by comparing the results with the different options of treatment, and to discuss the surgical approach as the best therapeutic option for patient with the first occurrence of psp. material and methods: 35 axillary thoracotomy with abrasive pleurodesis for the treatment of psp were carried out at the department of thoracic surgery of the university of caxias do sul between january 1996 and march 1999. results: 23 patients with their first episode of psp and 12 patients in their second episode were treated. the pleural drains were kept for in an average period of 2.7 ± 1.6 days. the average time in hospital was about 4.9 ± 2.4 days. morbidity was 8.5% and mortality rate was zero. there was no relapse in an average period of 22 months of follow-up. conclusion: the indication of axillary thoracotomy and abrasive pleurodesis to the patients with primary spontaneous pneumothorax (psp) at the first or recurrent episode showed low rates of morbidity and no relapse, besides short periods of hospitalization and pleural drain permanence. the possibility of indication as the first therapeutic method for the psp should be considered.

Abstract: this paper explores babies' communicative intentionality and the acquisition of this ability in the first year of life. it presents a theoretical discussion of this theme, looking at research that points to the social-communicative dimension of language and its relevance to the child's social cognition, as well as the adult's role in developing this ability. it also emphasizes the narrow relationship between triadic contexts and communicative intention, the different points of view on the evolutional moments in which this ability emerges and the nuances of the investigation into the infant's acts of communication, mainly when such acts incorporate the intentional element.

Abstract: the present study investigated the use of directives which can function to direct, control and keep the child's attention during the interactions. the directives utterances may show different functions in the interactions in which they are used depending on characteristics such as gender. in this study the possible variations in the use of maternal directives were analyzed by considering the child's gender. sixteen mother-child dyads participated in this study. children's age varied between 24-30 months. the dyads were video-recorded in natural environment for 20 minutes. the transcriptions of the sessions were carried out following the norms of the computational system child language data exchange system (childes). mann-whitney u test indicated a significant difference in the maternal speech directed to boys and girls. these results showed that the maternal directives were directed more to boys than to girls. these results were discussed considering the level of the children linguistic development and the context in which the utterances were presented.

Abstract: this study identifies the primary socio-communicative abilities evidenced by infants in three points in their first year of life, and the different configurations in each age. emphasis is given to the idea that understanding child socio-cognitive and communicative dimensions is essential to acquire a better understanding of the genesis of human development and language. six mother-infant dyads participated in the study. infants were observed in their homes for 40 minutes of free play at the ages of six, nine and 12 months of age. the results demonstrated that the socio-communicative abilities of the infants were reconfigured in each subsequent age, permitting us to identify, especially at the age of 12 months, a higher frequency of intentional communicative abilities. the empirical data obtained in this study contribute to a better understanding of infant social cognition in the first year of life, with a focus on the bidirectional nature of mother-child interaction.

Abstract: this article aims to discuss the relevance of joint attention skills and its contribution to social cognition and language acquisition. also, it defends mainly the idea that in the first year of life babies develops, thought joint attention and cultural insertion, a kind of intentional communication based on the first intersubjective interactions with adults. the discussion about this thematic includes divergent points of view about when the child became able to establish joint attention periods, and the moment in which that skills is consolidated. the importance of studies that investigated joint attention in the first year of life is emphasized in order to have a better understanding of that ability considering its implications to the development of communication and language.

Abstract: the purpose of this study was to analyse the maternal perceptions about babies’ sociocommunicative skills in their first year of life. the study was based on the premise that the parents` perception about the baby’s intentional communication can influence the dynamics of the initial interactions and the parental strategies to respond to the babies’ behaviours. this research involved the participation of 45 mothers of babies aged six, nine and twelve months old. in order to know the maternal perceptions about the babies’ intentional communicative skills the pragmatic profile of everyday communication skills in children, a structured interview focused on sociocommunicative aspects of language, was used. the results indicated variation on the maternal perception related to the baby’s age, indicated by the mothers’ descriptions about different sociocommunicative abilities of their babies which, at twelve months, included the use of gestures with intentional vocalization. it showed, also, that some of the mothers seemed to ignore the presence of sociocommunicative abilities in their babies, what can influence the way they interact with their children. it is considered that the data of this study are important to the parents and professionals comprehension about the socioecommunicative dimension of language, to the detection of alterations on sociocommunicative abilities of joint attention and intentional communication, as well as for planning interventions to promote child development.
