Laparoscopic colorectal surgery: what to expect from an initial experience

Abstract: laparoscopic colorectal surgery is less traumatic when compared to traditional surgery techniques, with well-established advantages. the objective of this study was to report the experience in laparoscopic surgical treatment of colorectal diseases. method: catalog all patients submitted to laparoscopic colorectal surgery performed by one surgeon and perform a descriptive analysis of key data from these records. results: the study analyzed data from 43 patients who underwent laparoscopic colorectal surgery. most were females (n=30; 69.77%) in relation to males (n=13; 30.23%), mean age of 57.21 years old. among the indications for surgery, diverticular disease was the most frequent (n=20; 46.51%), followed by malignancy (n=13; 30.23%). most patients underwent rectosigmoidectomy (n=28; 65.12%), followed by right hemicolectomy (n=6; 13.95%), with conversions in five cases (11.63%). the study observed a tendency towards increased number of surgeries, reduced average operative time as well as decreased conversions to laparotomy along the studied period. conclusion: laparoscopic colorectal surgery is a safe procedure, and with the technical development of the team, the results have been increasingly good.

Abstract: introduction: colorectal polyps and flat injuries are important for the prevention of colorectal cancer due to the malignancy of adenomas. objective: to describe the demographic profile of patients with endoscopic diagnosis of colorectal polyps and/or flat injuries at university hospital of universidade federal de sergipe and torres medical center. methods: a total of 6,919 records were evaluated from january 2002 to december 2007 with respect to the following variables: age, gender, origin, indication; injury evaluation os number, size, morphology, histology, dysplasia degree, topography; synchronics and metachronics injuries and associated diagnoses. results: 1,031(13.51%) colorectal polyps and flat injuries were found in 935 tests, accounting for 826 patients, 46% males and 54% females. the age ranged from 3 to 96 years with average of 53.64. the most frequent topographic distribution of polypoid injuries were rectum and sigmoid (58.40%). the injuries were sessile in 52.80% of the cases, pedicle in 27.90%, and flat injuries in 19.30%. there were synchronical injuries in 23.48% of patients and metachronic in 22 patients. histologically, 43.36% were adenomas, being 85.70% tubular, 9.60% tubulovillous and 4.70% villous; hyperplastic polyps were found in 30.64% of the cases, inflammatory in 15.80% and 10.20% presented other histological types. the adenomas showed low-grade dysplasia in 83.40% and high degree in 16.60% of the patients. seven were adenoma-carcinomas, one was carcinoid and one was gastrointestinal stromal tumor. conclusions: the colonoscopy and polypectomy are important in diagnosis and prevention of colorectal cancer.

Abstract: the accomplishment of the colonoscopy can cause decurrent complications of the preparation, sedation and of the disgnostic and/or therapeutical procedures. these events, relatively common, around possess tax of morbidity of 1% e, in general, transitory and are subnotificados. objective: to evaluate the incidence of complications of the sedation and accomplishment of the colonoscopy, during and immediately after the same one; to relate them it the 0 variable: sort, age, comorbid, amount of the drugs for sedation/analgesic, hypertension, hypotension, hypoxia, tachycardia, bradycardia. method: observational, prospective and descriptive study, being accompanied by it colonoscopy of 90 patients in private clinic. one used midazolan and meperidina for the sedation. it was surveyed arterial pressure, saturation of oxygen and cardiac frequency during and immediately before and after the examination. results: of the examined patients, 53.3% (n=48) were men. the age average was 48,4 years. for sedation they had on average used 2,95mg of midazolan and 29,5mg of meperidina. of the initially hypertensive ones, 11 (40.7%) had remained hypertensive to the end of the examination. saturation of lesser oxygen was observed that 90% in 16 (17.8%) patient ones; bradycardia in 19 (21.1%) and tachycardia in 13 (14.4%). nauseas, vomits and abdominal pain had been alterations gifts in 11 (12.2%) patient ones and had had to the preparation of the large bowel for the colonoscopy. no important complication was verified.
Evaluation of the effectiveness of 4% formalin in the treatment of hemorrhagic actinic proctitis

Abstract: radiotherapy is an important discovery as to the treatment of pelvic tumors. proctitis is frequently observed nowadays, and can be divided into acute and chronic. treatment with 4% formalin solutions has been used with positive results in literature. objective: to evaluate the effectiveness and morbidity rates related to the use of 4%formalin in hemorrhagic chronic actinic proctitis. methods: we evaluated the sigmoidoscopy records and reports of 11 patients with chronic hemorrhagic actinic proctitis from february to december 2010, coming from the serbian colorectal university hospital of the state of sergipe. results: the study was comprised of 11 patients (36.36% were females and 63.63% were males). mean age was 67.7 years. mean time between the end of radiotherapy and the onset of symptoms was 6.6 months. the treatment was completely effective in 27.27% of the cases, and reduced rectal bleeding in 100% of patients. the following main complications were observed: chills (9%), tenesmus (18.18%) and mild stenosis (9%). conclusions: the 4%formalin solution has fewer side effects, and its administration is very inexpensive. the treatment is effective and reduces bleeding in almost 100% of cases.
Estudo demográfico do cancer de cólon e reto no estado de Sergipe

Abstract: in brazil, the colorretal cancer (crc) was the fifth cause of death for cancer in men and fourth, in women in the period of 1995-1999. however, there are few registers on the epidemiologicals aspects of this pathology in sergipe, being this the focus of this study. objective: to catalogue all crc cases diagnosed in sergipe in the period of 03 years (2003-2005), tracing a demographic study. method: the data had been harvested in findings of all pathological anatomy laboratories on the state. results: the crc was more frequent in women (n=201; 56,62%) than in men (n=154; 43,38%). the average age was 60,27 years old, and 39 (10,99%) had up to 40 years old. adenocarcinoma was the histology type more found, present in 295 (83,10%) patients. in sergipe, the incidence of crc in 2003, 2004 and 2005 was 4,57; 4,72 and 5,29 per 100.000 men and, 6,07; 5,57 and 6,98 per 100.000 women respectively. in the capital, in this respectives years, these numbers had increased to 10,25; 10,57 and 10,29 per 100.000 men and 12,92; 13,16 and 15,07 per 100.000 women. conclusions: the achieved data are similar to the ones of the literature, showing an increase in the annual incidences of the neoplasm.
Eficácia e seguran?a dos antipsicóticos atípicos nas demências: uma revis?o sistemática

Abstract: objective: concerns have been raised about efficacy and adverse events of atypical antipsychotics in the treatment of behavioural and psychological symptoms of dementia (bpsd). this paper is an update on current evidence of this theme. methods: review of published meta-analysis and randomized placebo-controlled trials (rcts) in the last ten years. results: three meta-analysis and nine rcts were evaluated. evidence suggests that risperidone (1mg/day), olanzapine (5 to 10mg/day), and aripiprazole (2 to 15mg/day) are effective in treating aggression and/or bpsd overall; risperidone (1mg/day) reduces psychosis. adverse events were mainly somnolence, extrapyramidal symptoms, urinary tract infection or incontinence, and abnormal gait with drug treatment. atypical antipsychotics were associated with increased risk for cerebrovascular adverse events and mortality in elderly patients with dementia. conclusion: low doses of risperidone, olanzapine, and aripiprazole are effective in treating aggression and/or bpsd overall, and risperidone reduces psychosis associated with dementia. in view of the increased risk of cerebrovascular adverse events and mortality, the use of atypical antipsychotics in individuals with dementia should be reserved for patients with moderate/severe behavioural symptoms.

Abstract: indirect enzyme-linked immunosorbent assays (elisas) based on recombinant msp1a and msp2 from a brazilian isolate of anaplasma marginale were developed to detect antibodies against this rickettsia in cattle. the high sensitivities (99% for both tests) and specificities (100% for both tests) were confirmed with sera from cattle positive or negative for a. marginale antibodies, respectively, by immunofluorescent antibody test. by the analysis of 583 sera from cattle of three regions of the state of pernambuco, brazil, the agreement between both tests was high, with a kappa index of 0.89. the similar performances of the elisas suggest that both tests can be used in epidemiological surveys for detection of antibodies to a. marginale in cattle.
On the Statistical Properties of the 3D Incompressible Navier-Stokes-Voigt Model

Abstract: The Navier-Stokes-Voigt (NSV) model of viscoelastic incompressible fluid has been recently proposed as a regularization of the 3D Navier-Stokes equations for the purpose of direct numerical simulations. In this work we investigate its statistical properties by employing phenomenological heuristic arguments, in combination with Sabra shell model simulations of the analogue of the NSV model. For large values of the regularizing parameter, compared to the Kolmogorov length scale, simulations exhibit multiscaling inertial range, and the dissipation range displaying low intermittency. These facts provide evidence that the NSV regularization may reduce the stiffness of direct numerical simulations of turbulent flows, with a small impact on the energy containing scales.

Abstract: some equipments are used in precision farming to improve operational conditions. a light bar is an equipment used to guide a vehicle in adjacent bands, reducing the overlap of the parallel swathing and optimizing the efficiency of the agricultural application. the objective of this work was to determine the accuracy of a light bar compared to a foam marker, and to verify the adaptation of the operators to this new technology. experiments were conducted at two different places with several operators. the overlap errors and missed applications in the parallel swathing system were compared for both systems. there was a high adaptability of the operators to the light bar since this system was easier to operate than the foam marker. it was concluded that the light bar for the application of agricultural chemicals could replace the foam marker.
Morphological changes in the vaginal epithelium during the oestrous cycle of Calomys callosus (Rodentia, Cricetidae)

Abstract: this study describes changes in the pattern of microridges, keratinization, desquamation, secretion, leukocyte infiltration as well as the increasing number of mitotic cells in the vaginal epithelium of calomys callosus during the oestrous cycle. in proestrus, the epithelium is squamous and stratified with a fine layer of keratin and it is overlain by secretory prismatic cells. in oestrous, the epithelium is squamous, stratified and keratinized. in metoestrus, the epithelium is squamous and stratified with loss of the keratin layer. a leukocyte infiltration, extending from the base to the surface of the epithelial layer is also present. at the end of this phase, the surface cells start to become pas-positive. in dioestrus, the epithelium is stratified. the superficial cells are prismatic, exhibiting the structural and ultrastructural characteristics of glycoprotein secreting cells supported by a layer of squamous cells. at the end of this phase, kerato-hyaline granules appear in the granular layer of the epithelium, indicating the beginning of the keratinization process, present in the next proestrus.
