Chylous ascites as the main manifestation of left ventricular dysfunction: a case report

Abstract: A 70 year old man was admitted with more than one year history of abdominal distension and a weight gain of 10 kilograms. He is asymptomatic and walks 2000–3000 meters a day without angor or dyspnea. The physical examination shows moderate abdominal distension, with no hepatosplenomegaly or edema, and there is mild jugular vein distension. The studies performed (complete laboratory work up, paracentesis, liver biopsy, echocardiogram, intrahepatic pressure measurements, etc.) showed a chylous ascites related to portal hypertension, and left ventricular dysfunction was the only probable cause found.Asymptomatic heart dysfunction can mimic liver disease and should be kept in mind as a cause of chylous ascites.Ascites is one of the most common complications of liver diseases, even though in 15% of the cases it is related to extrahepatic diseases; 3% are of cardiac nature and they appear associated with signs and symptoms of heart failure. It is also associated with certain blood laboratory abnormalities. In this cases, the ascitic fluid shows a high albumin gradient (>1.1 g/dl) and high concentrations of proteins [1-4].We report an asymptomatic patient with chylous ascites as the only symptom of heart failureA 70 year old man was admitted with more than one year history of abdominal distension and a weight gain of 10 kilograms. He was evaluated in another center and was given the diagnosis of ascites associated with cirrhosis.His past medical history was significant due to acute myocardial infarction in 1974, mild asthma and paroxysmal atrial fibrillation. Since his last evaluation, he has been receiving diltiazem 180 mg/day, aspirin 100 mg/day, digoxin 0,750 mg/week and spironalactone 50 mg/day. Abstract: objetive: to show the computed tomography (ct) usefulness after treatment with transcatheter arterial quimioembolization and radiofrequency ablation of hepatocellular carcinoma. material and methods: in a period between march 2006 to april 2008 a total of 90 patient presenting 148 nodular lesions with diagnosis of hepatocellular carcinoma were controlled with triphasic ct. all the lesions were treated with minimally invasive procedure. for the treatment, the patients were classified in two groups following milan criteria. the first group, constituted by 75 patients with 109 nodules, was treated with quimioembolization. the second group, of 15 patients with 25 nodules, was treated with radiofrequency ablation. in our population, a subgroup of 10 patients was treated with both methods. results: of 90 patients after ct control on a month, 3 months and for each 3 months during 2 years, on 63 cases (70%) was observed homogeneous accumulation of iodized oil, parcial defect without enhancement or absence of enhancement on treated lesions. in these patients a new treatment after initial one was not performed. the remaining 27 patients (30%) underwent new treatment because we founded partial defect or absence of iodized oil with enhancement or peripheral enhancement on arterial phase in treated lesions. in this last group, 16 treated patients (17.7%) had new nodular enhancement on the remaining hepatic parenquimal. conclusion: the ct unenhanced and the arterial phase on a month and for each 3 months, allow monitoring the effectivenes, residual disease and/or relapse of hepatocellular carcinoma after minimally invasive treatment.
HBsAg as predictor of outcome in renal transplant patients HBsAg como predictor de evolución en trasplantados renales

Abstract: Chronic liver infections related to hepatitis B and C viruses are a common problem in renal transplant patients with a prevalence of 1.5 to 50% in different countries. There is no uniform agreement regarding their influence on the incidence of acute rejection, graft outcome and survival of renal transplant patients. We retrospectively evaluated the influence of antiHBc, antiHCV and HBsAg positive status; gender; age over 50 years of age at the time of transplantation; pre and postransplantation alaninaminotransferase (ALT) elevation; acute rejection; type of graft; number of transplants; and maintenance and induction immunosuppression treatment on the incidence of acute rejection and both graft and patient survival in the population transplanted in our center between 1991 and 1998. The univariate analysis showed that antiHCV, HBsAg and antiHBc status, more than one renal transplant and one or more episodes of acute rejection were associated with diminished graft survival; and being over the age of 50 at the time of transplantation was also associated with diminished patient survival. In the multivariate analysis HBsAg positive and one or more episodes of rejection were associated with a diminished graft survival, and none of the variables studied was associated with diminished patient survival. In conclusion antiHCV and HBsAg positive status was associated with an increased risk of losing the transplanted kidney, and HBsAg positivity was associated with an increased risk of death, but this was not a statistically significant association. Las hepatitis virales crónicas causadas por los virus B y C son un problema común en los pacientes trasplantados renales. No hay un consenso en cuanto a su influencia en la evolución del injerto y la sobrevida de los pacientes trasplantados renales. Evaluamos en forma retrospectiva la influencia de la positividad de antiHBc, antiHCV y HBsAg; sexo; edad mayor de 50 a os al momento del trasplante; elevación de la alaninaminotransferasa en el período pre y postrasplante; rechazo agudo; tipo de injerto; número de trasplantes; y tratamiento inmunosupresor en la sobrevida del injerto renal y del paciente en los pacientes trasplantados en nuestro centro entre 1991 y 1998. Hepatitis C Virus Diversification in Argentina: Comparative Analysis between the Large City of Buenos Aires and the Small Rural Town of O'Brien

Abstract: Background The estimated prevalence of HCV infection in Argentina is around 2%. However, higher rates of infection have been described in population studies of small urban and rural communities. The aim of this work was to compare the origin and diversification of HCV-1b in samples from two different epidemiological scenarios: Buenos Aires, a large cosmopolitan city, and O'Brien, a small rural town with a high prevalence of HCV infection. Patients and Methods The E1/E2 and NS5B regions of the viral genome from 83 patients infected with HCV-1b were sequenced. Phylogenetic analysis and Bayesian Coalescent methods were used to study the origin and diversification of HCV-1b in both patient populations. Results Samples from Buenos Aires showed a polyphyletic behavior with a tMRCA around 1887–1900 and a time of spread of infection approximately 60 years ago. In contrast, samples from óBrien showed a monophyletic behavior with a tMRCA around 1950–1960 and a time of spread of infection more recent than in Buenos Aires, around 20–30 years ago. Conclusion Phylogenetic and coalescence analysis revealed a different behavior in the epidemiological histories of Buenos Aires and óBrien. HCV infection in Buenos Aires shows a polyphyletic behavior and an exponential growth in two phases, whereas that in O'Brien shows a monophyletic cluster and an exponential growth in one single step with a more recent tMRCA. The polyphyletic origin and the probability of encountering susceptible individuals in a large cosmopolitan city like Buenos Aires are in agreement with a longer period of expansion. In contrast, in less populated areas such as O'Brien, the chances of HCV transmission are strongly restricted. Furthermore, the monophyletic character and the most recent time of emergence suggest that different HCV-1b ancestors (variants) that were in expansion in Buenos Aires had the opportunity to colonize and expand in O’Brien.
Pericardiocentesis en una Unidad de Cuidados Intensivos

Abstract: objective: study pacients diagnosed of pericardial effusion and pericardiocentesis was made. we search the ethiology, complications and thecnic`s mortality. method: 69 patients of clinic hospital of zaragoza admited in the cricital care unit. results: we present 69 patients with 20-87 years old, 21 women and 48 men. the most frecuently ethiology was neoplasic, 19 patients (27.5%), in most cases lung and breast cancer; 17 (24.6%) yatrogenic and 14 (20.3%) idiopatic. diagnosis was made previously at icu in 63.8% (44 patients). pericardiocentesis was made in the 12 hours after admission in 40 cases (58%), and was superior than 500cc in 37 (53.6%). we registered pericardiocentesis complications in 7 (10.3%). 16 patients dead (23%) most of them with pericardial effusion by mecanic cause. conclusion: was the pericardial effusion etiology has changed last years, the diagnosis is simply and minimun complications in evacuation, because the means and actual monitorization allow to make pericardiocentesis in optimun conditions.

Abstract: in the last decades, gender archaeology has gained increasing importance in world archaeology (see gero and conkey 1991; brumfield 1996; escoriza mateu 2002; among others). nonetheless, in our country this topic has been scarcely explored. in this paper, i seek to analyze the way gender relations were represented in a particular site of the northern calchaqui valley during the late period -900/1470 dc- (tarragó 1974). through the study of the petroglyphs found in los cerrillos site, and centering in the analysis of anthropomorphic representations, i aim to explore the way gender relations were produced and reproduced through the narrative that los cerrillos rock art offered. i conclude that the male and female representations are quantitatively different, expressing an explicit discourse through which gender differences were highlighted.

Abstract: in the present paper, i examine how michael devitt's proposal as to how to understand the notion of rigidity for general terms fares as regards what i have called the 'criterion of extensional adequacy' for any such notion -namely, the condition according to which any notion of general term rigidity should make the class of rigid terms coincide with that of natural kind terms. i try to show that devitt's defense of his view from the usual objections raised in the literature fails. in the first place, the proposal seems to overgeneralize, as terms such as 'television set' or 'table', would also be rigid appliers; as i try to show here, the arguments presented by devitt in order to show that that is not the case are based on some alleged properties of those terms that are also true of some of the expressions he does consider as rigid. in the second place, the proposal also undergeneralizes; even though devitt himself accepts that this is so, i try to show that his strategy to lessen the disappointment this causes also fails.

Abstract: in this essay, i present a problem that originates in kripke's contention, in naming and necessity, that natural kind terms are rigid, namely, the problem of how to understand the notion of rigidity when it is applied to general terms. i also give an account, in a principled way, of the main theoretical options that seem to be available to solve that problem, and sketch the main features of michael devitt's proposal against that background.
El paralelo Rusia/Estados Unidos en Francia y la formación de una identidad "occidental": usos políticos tempranos, de Le Trosne a Tocqueville y Beaumont

Abstract: one of the most important aspects of the making of "western" identity is the way in which it symbolically constructed geographical space. the exclusion of russia from the symbolical space of europe, and the inclusion of the usa as part of the same "western" world, are two of the most important shifts in the ideological construction of geographical space and in the liberal narrative of "civilization". this article examines one of the images that contributed the most to the success of those shifts: the parallel russia/usa, that is, the comparison of the two nations as embodying opposing historical "principles" for the future of humankind. the parallel russia/usa was popularized by tocqueville in 1835, and thereafter became part of commonsensical geographical imagination -especially during the cold war-. however, it has an older and revealing "prehistory". the evolution in the meaning, and the remarkable success, of the parallel russia/usa constitutes an important chapter in the story of the emergence of a liberal narrative of "the west". as despotism and social egalitarianism (and therefore socialism) were expelled from european identity as something alien, characteristic of 'uncivilized' countries like russia, the usa emerged as the model for the successful (liberal) transformation of turbulent europe. by finding in american society a model able to articulate wider political consensus to legitimize capitalist institutions, european (liberal) identity went "western". the parallel russia/usa was fundamental to justify this shift.
