Effectiveness of participatory-action-research to put in practice evidence at a nursing onco-hematology unit

Abstract: the aim was to evaluate the effectiveness of evidence implementation through participatory-action research (par). a prospective quasi-experimental design with two non-equivalent and non-concurrent groups (2006 and 2008) was adopted. the research was conducted at the bone marrow transplant unit of a tertiary-level spanish hospital. to put the evidence in practice, par was adopted as an "intervention studied". the dependent variables were: professional performance and patient outcomes (psycho-emotional area and adverse effects). in total, 125 patients were recruited (1st period=56; 2nd period=69). the results in the second period show significant improvements in professional performance in terms of the quality of the registers of signs and symptoms. in the psycho-emotional area, the psycho-social adjustment improved significantly; without caregiver burden or satisfaction showing any clear tendencies. among the adverse effects, catheter-related thrombosis and catheter-related infection improved significantly; there were no significant differences in the level of pain or mucositis. through the par, evidence could be put in practice and the outcomes under analysis could be improved.

Abstract: objective: to characterize the distinct questionnaires and scales used to measure physical activity, their conceptual frameworks, psychometric properties and application norms. method: the review included original articles that used questionnaires or scales to assess physical activity in older adults or the elderly. the cinahl and medline databases were consulted for the years 1993 to 2007. the studies selected had to provide information on the use, development and psychometric properties of the instruments. instruments used in the population aged more than 45 years old were included. articles that assessed physical activity by direct estimation, complex methods, or physical performance were excluded. results: the search produced 166 references and 36 instruments were identified. most of the studies quantified physical activity, and a minority assessed self-efficacy in physical activity performance and the stage of change. half of the instruments were self-administered. the most frequently studied reliability was test-retest. criterion validity was studied in 14 instruments, and 11 of these used at least two alternative methods of measurement. construct validity was assessed in 26 instruments. responsiveness was evaluated in only three instruments (ypas, champs and exercise stage of change) in addition to reliability and validity. conclusions: the exercise stage of change questionnaire showed responsiveness and sufficient reliability to allow individual use. the 7day par questionnaire and the modified baecke questionnaire (spanish version), which evaluate physical activity, can be used on an individual basis, although their responsiveness has not been studied. in general, the instruments analyzed do not assess mild intensity activities.
Validation of a mobility item bank for older patients in primary care

Abstract: Background To develop and validate an item bank to measure mobility in older people in primary care and to analyse differential item functioning (DIF) and differential bundle functioning (DBF) by sex. Methods A pool of 48 mobility items was administered by interview to 593 older people attending primary health care practices. The pool contained four domains based on the International Classification of Functioning: changing and maintaining body position, carrying, lifting and pushing, walking and going up and down stairs. Results The Late Life Mobility item bank consisted of 35 items, and measured with a reliability of 0.90 or more across the full spectrum of mobility, except at the higher end of better functioning. No evidence was found of non-uniform DIF but uniform DIF was observed, mainly for items in the changing and maintaining body position and carrying, lifting and pushing domains. The walking domain did not display DBF, but the other three domains did, principally the carrying, lifting and pushing items. Conclusions During the design and validation of an item bank to measure mobility in older people, we found that strength (carrying, lifting and pushing) items formed a secondary dimension that produced DBF. More research is needed to determine how best to include strength items in a mobility measure, or whether it would be more appropriate to design separate measures for each construct.
Evaluation of walking speed tests as a measurement of functional limitations in elderly people: A structured review

Abstract: El objetivo de este estudio teórico fue valorar el modelo conceptual, la carga, la interpretabilidad, los efectos suelo y techo, la fiabilidad, la validez y la sensibilidad al cambio de los tests de velocidad de andar como medida de limitaciones funcionales en personas mayores. Se realizaron búsquedas electrónicas en Medline, AgeLine, Embase, CINAHL e IME, búsquedas manuales y por referencias. La calidad de las propiedades de medición fue evaluada mediante criterios estandarizados. Se analizaron 102 estudios que identificaron 18 tests. Los tests más utilizados fueron: 2.44 metros, 4 metros y 6 metros, realizados a velocidad del paso habitual. Las evidencias se centraron en la validez predictiva y en la fiabilidad test-retest; en este caso, los valores de los coeficientes fueron superiores a los estándares de calidad recomendados. La interpretabilidad, los efectos suelo y techo y la sensibilidad al cambio son los atributos con menos evidencias. En estudios epidemiológicos, la evidencia disponible apoya el uso de los tests de andar como predictores de resultados adversos relacionados con la salud en personas mayores. Se precisa de estudios que apoyen su viabilidad y aplicabilidad en la práctica clínica, ya sea con la finalidad de screening como para la monitorización y evaluación del cambio.

Abstract: Dentro del paradigma cualitativo de la investigación, la Investigación- Acción-Participativa (IAP) integra el conocimiento y la acción; no hay que esperar a que tras producir el conocimiento se produzca la traslación de este a la práctica. Es un método que se utiliza desde hace varias décadas en disciplinas como la educación o la sociología, sin embargo es emergente en el ámbito de la salud. Existen una gran variedad de definiciones, clasificaciones y modelos de IAP y con este artículo pretendemos arrojar luz sobre este método de investigación, su historia, su filosofía y su utilización, dados su potencial y novedad en el ámbito de las ciencias de la salud. Abstract: background: the measurement and meaning of disability depend on the indicators used. in this study the indicators of dependency and severe residual disability are examined on two levels: in each of the 14 selected activities of daily living (adl) and also as a summary score of the activities count. methods: the data are extracted of the disability, impairment and health survey (ine, 1999). the sample is composed of 8.779 entries corresponding to people over 65 years old with disability in some of the 14 selected adl. tables of proportions and contingency have been elaborated, statistics of association and agreement has been calculated and the dimensionality of the aggregated measures had been analyzed through the full information factor analysis. results: the main discrepancies between the indicators are concentrated on the mobility activities (kappa coefficients ranged between 0,50 to 0,75 for mobility activities and between 0,73 to 0,87 for domestic activities and self-care). severe residual disability (indicator selected by the national institute of statistics) compared to dependency more inability includes more mobility disabled people and excludes more disabled people in domestic activities and self-care. dependency more inability is the indicator with the highest validity as a summary measure of disability (two eigenvalues: 8,37 and 1,91, the first factor accounted for 56,3% of the variance, the saturations of the first factor ranged between 0,58 to 0,87). conclusions: dependency more inability is more valid than severe residual disability as main indicator of disability.

La construcción de conocimiento en foros virtuales de discusión entre pares

Abstract: the current study analyzes the interaction among peers in two virtual discussion forums (structured and unstructured). the analysis of data considered: 1) the interactive and discursive strategies employed by participants, and 2) the knowledge produced (number of ideas and use of concepts). the results show that differences exist in the number of interactive and discursive strategies employed by the students in both forums (structured discussion: discursive, 122, and interactive, 141; unstructured discussion: discursive, 51, and interactive, 48). more ideas (121) and concepts (21) were produced in the structured discussion forum than in the unstructured forum (number of ideas: 63, concepts: 13). the obtained data suggest that the instructional design used to organize discussion in forums has a considerable effect on the knowledge constructed by the participants.
