Study of the therapeutic effects of an advanced hippotherapy simulator in children with cerebral palsy: a randomised controlled trial

Abstract: The study is a randomised controlled trial. It will be carried out with a sample of 37 children with cerebral palsy divided into two treatment groups. Eligible participants will be randomly allocated to receive either (a) Treatment Group with hippotherapy simulator, maintaining sitting posture, with legs in abduction and rhythmic movement of the simulator or (b) Treatment Group maintaining sitting posture, with legs in abduction and without rhythmic movement of the simulator. Data collection and analysis: all measurements will be carried out by a specially trained blind assessor. To ensure standardization quality of the assessors, an inter-examiner agreement will be worked out at the start of the study. The trial is funded by the Department of Research, Innovation and Development of the Regional Government of Aragon (Official Bulletin of Aragon 23 July 2007), project number PM059/2007.Interest in this project is due to the following factors: Clinical originality (there are no previous studies analysing the effect of simulators on the population group of children with CP, nor any studies using as many variables as this project); Clinical impact (infantile cerebral palsy is a chronic multisystemic condition that affects not only the patient but also the patient's family and their close circle of friends); Practical benefits (the development of an effective treatment is very important for introducing this element into the rehabilitation of these children).Current Controlled Trials ISRCTN03663478.Hippotherapy is a physical treatment strategy in which the movement of a horse is used to improve posture, balance and general development of people with or without motor difficulties. Abstract: Case-control study in 7,052 high cardiovascular risk subjects (3,430 type 2 diabetes cases and 3,622 non-diabetic subjects) with no differences in BMI. Diet was assessed by validated questionnaires. FTO-rs9939609 and MC4R-rs17782313 were determined. An aggregate genetic score was calculated to test additive effects. Gene-diet interactions were analyzed.Neither of the polymorphisms was associated with type 2 diabetes in the whole population. However, we found consistent gene-diet interactions with adherence to the MedDiet both for the FTO-rs9939609 (P-interaction=0.039), the MC4R-rs17782313 (P-interaction=0.009) and for their aggregate score (P-interaction=0.006). When adherence to the MedDiet was low, carriers of the variant alleles had higher type 2 diabetes risk (OR=1.21, 95%CI: 1.03-1.40; P=0.019 for FTO-rs9939609 and OR=1.17, 95%CI:1.01-1.36; P=0.035 for MC4R-rs17782313) than wild-type subjects. However, when adherence to the MedDiet was high, these associations disappeared (OR=0.97, 95%CI: 0.85-1.16; P=0.673 for FTO-rs9939609 and OR=0.89, 95%CI:0.78-1.02; P=0.097 for MC4R-rs17782313). These gene-diet interactions remained significant even after adjustment for BMI. As MedDiet is rich in folate, we also specifically examined folate intake and detected statistically significant interaction effects on fasting plasma glucose concentrations in non-diabetic subjects. However these findings should be interpreted with caution because folate intake may simply reflect a healthy dietary pattern.These novel results suggest that the association of the FTO-rs9939609 and the MC4R-rs17782313 polymorphisms with type 2 diabetes depends on diet and that a high adherence to the MedDiet counteracts the genetic predisposition.The Fat Mass and Obesity (FTO) and Melanocortin-4 Receptor (MC4R) genes are considered leading obesity-associated loci [1-6]. Competencias para la tutoría: Experiencia de formación con profesores universitarios

Abstract: La acción tutorial constituye uno de los pilares de la docencia universitaria. La tutoría ha supuesto un importante acercamiento a las necesidades del estudiante y al seguimiento de su proceso de aprendizaje, que implican cambios en la metodología de la docencia. El seguimiento académico, la planificación de objetivos de aprendizaje y el apoyo en la adaptación del alumno al entorno académico, resultan requisitos indispensables en el nuevo modelo educativo. Este cambio de paradigma confiere al tutor un nuevo rol de asesor universitario en el desarrollo de competencias académicas, personales y profesionales, que favorecen la autonomía personal y la toma de decisiones. Para poder realizar esta función el profesor universitario necesita el adquirir y desarrollar unas nuevas competencias propias de la tutoría, como: Comunicación eficaz, Escucha activa, Manejo de conflictos, Planificación y Trabajo en equipo. Todas ellas planteadas desde el prisma de la acción tutorial, que podemos entenderlas desde una doble vertiente, por un lado las competencias que debe tener el profesor para desarrollar su función como tutor y por otro aquellas que el tutor deberá intentar desarrollar en sus pupilos. Los planes de acción tutorial están encontrandose con una serie de dificultades de puesta en marcha debido a la inercia con la que se mueve la universidad espa ola, ello implica la necesidad de apoyarlo con la aportación de recursos, tanto institucionales como personales, así como con la organización de cursos de formación específicos que permitan desarrollar nuevas estategias y conocer herramientas para el trabajo de la tutoria desde las competencias. Tutorial has meant an important approach to the student needs and to the monitoring of their learning process, involving changes in the methods of teaching. The academic monitoring, the learning objectives planning and the support to the adaptation of the students to the academic environment are essential requirements in the new educational model.This paradigm shifts and gives the tutor a new University advisor role in the development of academic, personal and professional skills, which promote personal autonomy and decision-making. Abstract: The steel reinforcement by means of the addition of primary carbides via liquid metallurgy needs to fulfill two essential requirements to be technically viable: the development of a solid reinforcing product which is appropriate to be mixed with the steel and the optimization of the metallurgic addition of these reinforcing particles into the liquid steel. Besides, the improvement of the reinforced alloy characteristics is directly related to the particles' nature of reinforcing particles, their homogeneous and uniform distribution in the matrix and their size and morphology. The manufacture of these carbides by SHS ('Self propagating High temperature Synthesis') and their addition before steel is poured into moulds allows achieving the above-mentioned conditions. Para que el reforzamiento de los aceros con carburos primarios vía metalurgia líquida sea técnicamente viable se requiere del desarrollo de un tipo de producto reforzante sólido, adecuado para su incorporación al acero, así como de la optimización de dicha vía metalúrgica de adición del producto reforzante al material base en estado líquido. Además, la mejora de características de la aleación reforzada es función directa de la naturaleza de las partículas, de su homogénea y uniforme distribución en la matriz y del tama o y morfología regular de las mismas. Estas condiciones se consiguen cuando las partículas de carburo se elaboran previamente mediante la técnica SHS (selfpropagating high temperature synthesis) y se a aden al ba o de acero líquido previo a su colada en moldes o lingoteras.
