Pré-eclampsia precoce e tardia: uma classifica??o mais adequada para o prognóstico materno e perinatal?

Abstract: purpose: to evaluate the differences between the maternal and perinatal outcomes of pregnancies complicated by preeclampsia, according to the classification as the severe/mild form, and the early/late onset form. methods: a retrospective study with 211 pregnancies complicated by preeclampsia, assessed at a university reference center from 2000 to 2010. the diagnosis and disease severity were based on the values of blood pressure, proteinuria, and clinical and laboratory findings. the pregnant's age, skin color, parity, blood pressure, urine protein semiquantitative values, presence of bilateral notch in the uterine artery dopplervelocimetry and birth conditions were compared between patients with mild and severe disease, as well as between those of early/late onset. the disease was considered to be of early onset when diagnosed at less than 34 weeks of gestational age. results: most patients had the severe form of preeclampsia (82.8%), and the onset of the condition was early in 50.7%. blood pressure values (133.6±14.8 versus 115.4 mmhg, p=0.0004 and 132.2±16.5 versus 125.7 mmhg, p=0.0004) and semiquantitative proteinuria (p=0.0003 and p=0.0005) were higher in the early and severe forms compared to mild and late forms. infant birth weight (1,435.4±521.6 versus 2,710±605.0 g, 1,923.7±807.9 versus 2,415.0±925.0 g, p<0.0001 for both) and apgar score (p=0.01 for both) were smaller for severe and early preeclampsia compared to mild and late preeclampsia. on the other hand, the presence of a bilateral notch in the uterine arteries was linked to the forms of early onset (69.2 versus 47.9%, p=0.02), whereas fetal growth restriction was more frequent in the severe forms of preeclampsia (30 versus 4.4%, p=0.008). conclusion: the preeclampsia classification based on maternal clinical parameters better reflected the conditions of fetal nutrition, while the early onset of the condition was associated with placental vasculopathy detected by dopplervelocimetry.

Abstract: objective: ductus venosus and inferior vena cava flow velocity was assessed in fetuses in isoimmunized pregnancies. methods: examination of 61 fetuses aged 27 to 35 weeks from rh-erythrocyte antigen isoimmunized women was carried out from june 1999 to june 2004. all fetuses were submitted to the examination of ductus venosus and inferior vena cava flow velocity. blood samples were collected to determine hemoglobin values and hemoglobin concentration deficits. accordingly, fetuses were grouped as follows: non-anemic; mildly anemic; moderately anemic and severely anemic fetuses. comparison of the variation of average flow velocity in the inferior vena cava and ductus venosus across the four groups was carried out using the chi-square test. results: inferior vena cava flow velocity was found to be altered in 3.8% of non-anemic fetuses; in 3.1% of the mildly anemic, in 40.0% of those moderately anemic; and in 76.0% of the severely anemic ones. alteration in ductus venosus flow velocity, in turn, was identified in 7.7% of non-anemic fetuses; 3.1% of mildly anemic; 32.5% of moderately anemic and 68.0% of those severely anemic. results were statistically significant with p < 0.001. conclusion: the study shows that alteration of flow velocity in the inferior vena cava and ductus venosus increased with the severity of anemia.

Abstract: vague in the outlines and abounding in figures of real or fabulous creatures, the iconography of medieval maps played a significant role in helping users to recognize lands that were virtually unknown to christendom. in a world with no reliable latitudes and longitudes, the land contours, inhabitants and even certain elements of fauna and flora could become variables of utmost importance for geographic orientation. notwithstanding the higher levels of precision achieved in their efforts to represent geographic space, the considerable advancements made in latitude determination, and their continuous pursuit of a practical method for the establishment of longitudes, the cartographers of the age of discoveries still went to the trouble of depicting notable elements - whether real or imaginary - to help travelers get their bearings around different regions of the globe. as the presence of europeans continued to expand and an authentic scientific revolution took place in the 17th century, the lengthy notes and eye-catching representations of nature and local inhabitants soon lost their utility as points of reference on 17th-century maps to become mere accessories of aesthetic and commercial value. the advent of the 18th century consolidated, once and for all, the transformation of such illustrations into essentially decorative elements with no other relevant role to play in mapmaking. in addition to decreasing in number, the figures became more stylized and moved to the borders of the maps as ornamental motifs. although they often maintained some sort of relation with the geographic space depicted on the map, the motifs chosen could also constitute a rather independent element. in fact, allegories, compositions with a variety of mythological figures, and historical representations were often used to underscore the power of certain political agents.

Abstract: this paper uses tools in group theory and symbolic computing to classify the representations of finite groups with order lower than, or equal to 9 that can be derived from the study of local reversible-equivariant vector fields in 4 . the results are obtained by solving matricial equations. in particular, we exhibit the involutions used in a local study of reversible-equivariant vector fields. based on such approach we present, for each element in this class, a simplified belitskii normal form.

Abstract: bancroftian filariasis has been an endemic parasite in pernambuco since 1918 expanding into the metropolitan region of recife, and becoming a permanent challenge to be faced by the public health authorities as well as by the population and schools in the risk areas. as support in creating pedagogic strategies that stimulate pupils to put into practice preventative measures against the disease, we identified the mental models of 50 pupils aged from 9 to 10, living at endemic areas of the recife metropolitan region. the results indicated that the children: (a) believe that the filariasis can be avoided and treated, that it is caused by external factors and that the observance of medical recommendations is fundamental to the success of the treatment; (b) have difficulties in identifying the symptoms and phases of the disease; (c) they based their models on features they can see. this makes it possible to build pedagogical activities that simulate, in school, real situations lived by the children in its community.

Abstract: the acquisition of potassium nitrate for any purpose, including preparation of nutritive media for micropropagation, depends on authorization from the ministry of defense, what turns out in difficulties in its acquisition. with the purpose of studying the possibility of replacing this component of the culture for another product found freely in the market, an experiment was carried out, where several concentrations of potassic saltpetre were tried, in comparison to the chemically pure potassium nitrate, as a component of the ms salts formulation. the commercial granulated product murer? was used in the following (g l-1): a) 7,0; b) 7,4; c) 7,8; d) 8,2; and e) chemically pure potassium nitrate (control-treatment) as it appears in the ms salts, or 1,9 g l-1fáfia was tested, starting from nodal segments. the basal nutrient medium comprised the ms salts, white vitamins, 30 g l-1 sucrose, 2.0 mg l-1 indol butyric acid, 2.0 mg l-1 naftaleneacetic acid, 2.0 mg l-1 benzilaminopurine, ph 6.0 + 0.1 and 1.5 g l-1 phytagel? as the gelling agent. the number of branches formed in all concentrations of potassic saltpetre was lower than those ones in the control treatment; however, their lengths were higher. the mean length of branches in the treatment with the smallest potassic saltpetre concentration, reached 17.0 cm in length, while in the control-treatment it was 12,6 cm. the dry biomass weight reached 68.3 mg for the smallest potassic saltpetre concentration, but only 50,0 mg for the control treatment. the results indicated to be advantageous the substitution of the chemically pure potassium nitrate for the potassic saltpeter, when either shoot elongation or biomass production is desired, being the nutrient medium sterilized with sodium hipoclorite. however, when a faster multiplication rate is the main objective, the nutrient medium should be prepared with chemically pure potassium nitrate.
