Limitations of selective deltamethrin application for triatomine control in central coastal Ecuador

Abstract: Surveys for triatomines revealed peridomestic infestation with Rhodnius ecuadoriensis and Panstrongylus howardi, with infestation indices remaining high during the study (13%, 17%, and 10%, at initial, 6-month, and 12-month visits, respectively), which indicates a limitation of this strategy for triatomine population control. Infestation was found 6 and 12 months after spraying with deltamethrin. In addition, a large number of previously vector-free domestic units also were found infested at the 6- and 12-month surveys, which indicates new infestations by sylvatic triatomines. The predominance of young nymphs and adults suggests new infestation events, likely from sylvatic foci. In addition, infection with Trypanosoma cruzi was found in 65%, 21% and 29% at initial, 6-month and 12-month visits, respectively. All parasites isolated (n = 20) were identified as TcI.New vector control strategies need to be devised and evaluated for reduction of T. cruzi transmission in this region.Chagas disease in Ecuador affects approximately 230,000 people, and 6.2 million people are at risk of infection by Trypanosoma cruzi, its causative agent [1]. In the absence of satisfactory therapy or vaccines against Chagas disease, control of the disease relies primarily on interrupting transmission by eliminating domestic populations of triatomines. Triatomines Rhodnius ecuadoriensis and Panstrongylus howardi are important vectors of Chagas disease in the Manabí province, located on the central coast of Ecuador, and there is evidence of host (vectors and reservoirs) infected with T. cruzi, circulating in the area [2-6].R. ecuadoriensis is widely distributed in Ecuador's central and southern coast [7], its southern Andean region, and in northern Perú [3]. Abstract: in the coming years the worldwide energetic resources based on hydrocarbons will diminish, and methane hydrates can become an alternative source, given its huge deposits. chile does not have great amounts of energetic resources; however, during the cruise c-2901 on board the r/v conrad in 1998, seismic profiles that took place in the chilean continental margin between 35o and 45os show the presence of methane hydrates through bsr analysis. the following parameters for the hydrate layer can be assumed: thickness = 100 m, longitude e-w = 20 km, latitude n-s = 1000 km, hydrate concentration in the sediments = 10%, and 160 m3 of gas per m3 of hydrate. with these figures, the volume of estimated gas is 3.2 x 1013 m3.

Abstract: it has been noticed that the variable of anxiety has a relevant impact on the work of the physicians, showing the seventy percent a medium level of anxiety, and the eighteen percent a high level of anxiety. in the same way we detect an important depression in a 5%. the lack of satisfaction in the relationship and the state of suffering of the patient are the variables that create the most anxiety. in terms of the medical organization, the lack of public support, education and time are the most stressing aspects. in terms of dealing with illness, the physician"s fears are giving a wrong diagnosis and treatment. the work satisfaction is directly related to the medical organization they belong to, and of how this institution functions, as weil as the relationship they has with their boss and peers when doing team work. team work can make physicians keep their anxiety internally.

Abstract: Public policy makers and administrators around the world recurrently face the question of whether to grant tax privileges to businesses in order to promote investment, jobs, or economic development in general. This article analyzes a very popular form of local incentive, the property tax abatement, and its ability to reduce capital cost. The research question is: By how much do property tax abatements reduce the capital cost of business and homes? Results show that abatements can account for quite a large range of possible percentage reductions in the price of investment for firms. The findings provide public administrators and policy makers with data and a tool to assess the benefits that firms derive from abatements. This in turn can contribute to more informed abatement decisions and to an overall assessment concerning the suitability of this tool to promote economic development.

Abstract: Romania and other Eastern European countries have undergone dramatic reforms as they have sought to democratize political institutions, develop their economies, rely on private markets for the provision of goods and services, and pursue a course of economic integration with Western European nations (Lazar, 2005). Of course, these reforms have included the complete overhaul of tax and revenue systems (Lazar, 2005). As these tax reforms mature and are adapted to the differing realities of each country, it might be useful to reflect on the experiences and mistakes of countries whose tax systems they have used as blueprint for their own reforms. This is the spirit in which this analysis is written. The article presents a synthesis of the American experience with tax-exempt municipal bonds, and the advantages and disadvantages associated with this tax exemption. The exemption represents a subsidy from the federal government to states and local governments, and as such, it has powerful incentives with implications from the economic and redistributive standpoints. This article explains these implications and how they have been addressed in the U.S.

Abstract: La relación médico-paciente es uno de los aspectos más importantes dentro del ejercicio de la medicina y es un instrumento terapéutico fundamental. Diversas circunstancias histórico-sociales han determinado que la atención del médico se centre sobre la enfermedad y se descentre del paciente, objetivo que debería ser prioritario en su trabajo. Describe brevemente los múltiples determinantes socioeconómicos, culturales, políticos, institucionales, grupales e individuales. Se ala la importancia de que el médico haga un análisis de la demanda del paciente, la trascendencia de la palabra y de la escucha, el planteamiento de un diagnóstico situacional, la necesidad de la formación psicológica del médico y la importancia de la entrevista como medio de observación, investigación y diagnóstico.

Abstract: Artículo hecho a partir de una experiencia intensiva grupal con profesionales de atención primaria, en su mayoría médicos. La tarea del grupo era elaborar la información recibida previamente y las dificultades en el trato cotidiano con los pacientes y en su trabajo en general. Trata de mostrar las múltiples determinaciones que influyen en la relación médico-paciente, las diversas circunstancias que crean ansiedad al médico, la importancia que tiene que éste aprenda a ESCUCHAR, la necesidad de trabajar en equipo y la prioridad de la coordinación entre los grupos de atención primaria y salud mental de cara a la consecución de programas conjuntos. El uso de la metodología grupal por parte de los equipos de salud mental es un instrumento de gran utilidad en el ámbito comunitario y en la coordinación con las instituciones del mismo

Abstract: Se ha detectado que la variable ansiedad tiene un gran impacto en la práctica del médico, el setenta por ciento presenta una ansiedad media y el dieciocho por ciento, un nivel de ansiedad alto. Asimismo se detecta depresión, importante en un 5%0. Respecto al paciente, la falta de satisfacción en la relación y el sufrimiento de éste son las variables que causan mayor ansiedad. A nivel institucional las variables más estresantes son el estrés laboral, la falta de apoyo social, de formación y de tiempo. La variable más ansiógena referente a la enfermedad es el error en el tratamiento o en el diagnóstico. La satisfacción laboral se correlaciona directamente con la organización, el buen funcionamiento del Centro, el trabajo en equipo y las relaciones interpersonales con los compa eros y jefes. Se constata que el trabajo en equipo es un factor de contención de la ansiedad de los médicos.

Abstract: Se plantean los cambios en relación a la muerte a lo largo de la historia. Se recoge el discurso más significativo del proceso de un grupo de médicos cos cuya tarea era trabajar sobre la relación médico-paciente, analizando los emergentes surgidos en relación a la problemática de la muerte. Se evidencian en los médicos mecanismos defensivos de negación muy intensos frente a la muerte de los pacientes, ma-yores cuanto más compleja es su propia vinculación con la muerte. Se trabajan estas ansiedades y meca-nismos defensivos en grupo operativo observando que es un instrumento de gran utilidad para la elaboración de estas dificultades de los médicos frente a la problemática de la muerte de los pacientes.
