Microalbuminuria in White and Black Hypertensive Nondiabetic Brazilian Patients  [PDF]

Abstract: Microalbuminuria (MAU) is a predictor of cardiovascular mortality in patients with diabetes mellitus (DM) and hypertension (HTN) and also in an unselected population. The American Diabetes Association (ADA) and the National Kidney Foundation (NKF) define MAU as an albumin/creatinine ratio (ACR) between 30 and 300 μg/mg in both men and women. Aim: To evaluate the possible relationship among MAU, HTN and gender and ethnicity in Brazilian nondiabetic primary hypertensive patients. Design: Population-based study. Participants: Ninety-eight men and women, seventy-two black and twenty-six white nondiabetic primary hypertensive patients aged 20 years or older were selected. Forty healthy individuals, paired according to age, gender, and ethnics were used as controls. Methods: Early-morning midstream urine was used. Urinary albumin was spectrophotometrically measured with Coomassie Brillant Blue G-250. Creatinine was determined by a method based on Jaffe’s reaction. ACR (μg albumin/mg creatinine) was calculated. Data are expressed as medians. Results: ACR level was significantly higher in 98 hypertensive patients (38.00) than in 40 control individuals (23.00) (P < 0.001). ACR level was significantly higher in 48 hypertensive male (46.00) than in 50 hypertensive female (34.00) (P = 0.008). No significant effect of ethnicity on ACR levels between 26 hypertensive Whites (35.50) and 72 hypertensive Blacks (38.00) was observed (P = 0.978). Conclusions: The ACR level, significantly higher in hypertensive patients than in control individuals, supports data from the literature. To our knowledge, this is the first study demonstrating that the ACR level is significantly higher in men than in women. The lack of an ethnicity effect supports what was already asserted, namely, that in Brazil, at an individual level, color, as determined by physical evaluation, is a poor predictor of genomic African ancestry, as estimated by molecular markers.

Abstract: digitalis, particularly digoxin, have played an important role in the treatment of heart failure (hf) for over 200 years. in the past three decades, with the greatest knowledge of the pathophysiology of the disease, neurohumoral blockers were more effective in reducing morbidity and mortality and gained greater prominence. in addition, studies have suggested that digoxin does not reduce the risk of death and, in some cases, may even increase it. thus, its use has been questioned and discouraged. this review aims to look at the pharmacology of digoxin, studies on its benefits and risks in patients with moderate to severe systolic hf and diastolic hf also. this review looks forward to answering the following question: is there still room for the use of digitalis in hf?
Retinal Detachment in Preeclampsia

Abstract: preeclampsia is an obstetric disease of unknown cause that affects approximately 5% of pregnant women. the visual system may be affected with variable intensity, being the retinal detachment a rare complication. the retinal detachment in preeclampsia is usually bilateral and serous, and its pathogenesis is related to the choroidal ischemia secondary to an intense arteriolar vasospasm. the majority of patients have complete recovery of vision with clinical management, and surgery is unnecessary. this is a case report of a 27 year old patient who developed the severe form of preeclampsia on her first pregnancy. she had progressive blurred vision, until she could see only shadows. ophthalmic examination diagnosed spread and bilateral retinal detachment. with blood pressure control at postpartum, the patient had her retina reattached, and recovery of vision.

Abstract: in this paper i intend to place the issue of rights and justice in another way of understanding the democracy that is more deliberative and communicative. in this sense, i propose the understanding of democracy as human rights and i wonder the specificity and aim of politicity and dialogicity vectors that should characterize this type of democracy. i conclude this article by explaining the implications of this conception of democracy in education, stressing in particular the requirements of a democratic education, open to the challenges of participation and of cosmopoliticity, to the requirements of different scales of justice.

Abstract: This paper starts with a reflection about the State role and the centrality of education for social and educational change in the revolutionary times that follow the 25th April 1974; in a second moment, and from the perspective of a left movement (MES), expressed in its newspaper, the ambiguities of the democratic management of Portuguese schools are made visible. Next, the analysis focuses the autonomy concept and its centrality in this Movement discourse as well as the critiques and proposals for the organizational configuration of the democratic management model. Abstract: after a succinct characterization of the current postmodern condition, some questions might be developed, because of its current relevance in the education field, they must to be reviewed in a critical way, namely, and firstly, the school, the conflict and the conviviality. secondly, this article underlines some school's characteristics that turns it into an organization with a little perversity, hypocrisy and irrationality. the last aspect to be treated is the understanding of the school as a communicative or convivial organization, in which the concepts of discipline, violence, conflict and conviviality assume a more democratic orientation.
