POPQ system and dynamic MRI in assessment of female genital prolapse  [PDF]

Abstract:

Aim: To assess the role of Magnetic resonance imaging (MRI) and pelvic organ prolapse quantification (POPQ) system in the evaluation of female pelvic organ prolapse. Design: Prospective study. Setting: TantaUniversityHospital. Patients: The study was carried out on 60 patients having clinical manifestations suggesting pelvic floor weakness. Intervention: All the patients were subjected to history taking, physical examination including POPQ, and pelvic MRI (static and dynamic) examination. Outcome measures: Quantitative measurements of genital prolapse. Results: All patients showed loss of the normal position of the perineal plate indicating generalized weakness of the whole pelvic floor muscles. Dynamic MRI exhibited pelvic floor abnormalities in 46 patients who did not show any abnormalities on the static images. Conclusion: Dynamic MRI is a helpful tool in defining the nature and quantitative measurements of pelvic organ prolapse. Also, MRI has and advantage over POPQ system in diagnosing lateral prolapse.

Abstract: El objetivo de este trabajo es describir la técnica imaginológica denominada defecografía, para el estudio de las enfermedades del suelo pélvico y promover su utilización. Se requiere de un equipo de fluoroscopia con una silla especial de defecografía anexada, nuevo implemento para realizar dicho estudio, con la característica especial de no ser opaca a los rayos X, tener las dimensiones y altura adecuadas al equipo de fluoroscopia digital, además de la posibilidad de grabar las imágenes durante el acto de la defecación. Es necesario administrar medio de contraste (pasta de sulfato de bario) con una consistencia similar a la materia fecal, que se inserta en el recto del paciente. En conclusión, el método resulta eficaz para detectar numerosas alteraciones funcionales y morfológicas anorectales y del suelo pélvico, además es bien tolerado por los pacientes. Está adaptado y aplicado para las investigaciones, resulta adecuado y aporta los datos requeridos para un diagnóstico correcto. The purpose of the present paper is to describe the imaging technique known as defecography and foster its use in the study of pelvic floor disorders. To conduct the examination, a fluroscope is required with a special defecography chair attached to it. The chair should not be opaque to x-rays, its size and height should be appropriate for the digital fluoroscopy unit, and it should offer the possibility of recording images during the act of defecation. A contrast medium (barium sulfate paste) with a consistency similar to that of fecal matter will be inserted into the patient's rectum. It is concluded that the method is effective to reveal a large number of functional and morphological anorectal and pelvic floor alterations, and is well tolerated by patients. It has been adapted for and applied to research, and has proven to be suitable and to provide the data required for an accurate diagnosis.
