Adolescent Alexithymia Research: Indigenous Sample Compared to Hispanic Sample in Southern Chile  [PDF]

Abstract: Toronto Alexithymia Scale (TAS-20) adolescent research became increasingly popular. We found no data examining two different ethnical adolescent groups sharing comparable environment. Furthermore, there are no indications that TAS-20 has ever been used in Chile. We conducted a transcultural comparison investigating the influence of ethnicity, gender and age on a low socioeconomic teenage population. Additionally, Confirmatory Factor Analysis (CFA) was performed. In this cohort study (n = 225), 95 indigenous students were compared to the Hispanic control group of 130 participants. We found proper replicability and internal reliability of TAS-20 and the three-factor solution for our sample. We measured high alexithymia rates and significant differences between the ethnicities and genders but there was no influence of age. Although factor 3 (EOT) was inconsistent to some degree, TAS-20 resulted to be an appropriate measure for Chilean adolescents. Indigenous ethnicity, gender, low socioeconomic status, and power distance in a rural environment contribute to high alexithymia.

Evaluation of the Marginal Adaptation in Metal Crowns Using CAD/CAM and Manual Wax Patterns  [PDF]

Abstract: The aim of this study is to evaluate the maximum and minimum distances between the model and the cast crown of three techniques using Scanning Electron Microscopy (SEM). Three technique groups were used for this study: group A (control), traditional manual wax patterns; group B, dipping wax patterns; group C, resin patterns made with CAD/CAM. For each group, 10 samples were made using the same model, and then metal cast. Marginal accuracies of the samples were evaluated by performing gap measurements using SEM with a magnification of 1200× (minimum distance). The data were statistically analyzed using the one-way analysis of variance (ANOVA) at the 0.05 significance level. The average (standard deviation) of the minimum distance [μm] was 22.5 (12.1), 9.9 (4.3), and 14.7 (6.6), in groups A, B, and C, respectively. The average standard deviation of gap area [μm2] was 21667.2 (3476.4), 9906.4 (1512.1), and 16048.8 (8123). In the minimum distance comparison, groups A and B (p = 0.006) showed statistically significant results. In the gap area comparison, there was no statistical significance among groups A, B, and C (p = 0.174). The marginal adaptations of all three techniques were within a reported clinically acceptable range of margin.
A Note on Time Discretion and the Welfare Cost of Lump-Sum Taxation  [PDF]

Abstract: The lump-sum tax is broadly regarded by standard optimal tax theory as the only non-distortionary tax instrument; any other tax instrument distorts relative prices and thus creates a deadweight loss. This paper discusses an unintended effect of lump-sum taxation that can be considered a distortion of the time endowment. Whenever this tax exceeds the amount of non-labor income, it reduces the taxpayer’s ability to freely allocate her time endowment. As long as the taxpayer assigns a positive value to time discretion, the lump-sum tax creates a welfare cost that has not been identified in the relevant literature. The welfare cost of the lump-sum tax could plausibly be greater than the traditional measure of deadweight loss of an equal yield labor income tax, which does not affect time discretion. Since the lump-sum tax does not unambiguously lead to a greater welfare level, we can conclude that it is not a proper efficiency standard at low levels of non-labor income. The same argument can be used to call for caution in the use of taxes based on the value of assets that are not the source of income flows, like owner-occupied property taxes and some types of wealth taxes. At low levels of non-labor income, these tax instruments will also have a negative effect on time discretion.

Abstract: Normalmente las empresas, poseen información sobre datos de inspección y monitoreo, que no son correlacionados debido a falta de conocimiento especializado de sus jefes de mantenimiento. Con la implementación de una metodología de aseguramiento de riesgo, pretendemos presentar una estrategia de análisis que correlacione datos de inspección y monitoreo; para pronosticar, inspeccionar y evaluar la ocurrencia de posibles fallas, y así, minimizar el da o ambiental y pérdidas económicas por producción. Estos pronósticos serán realizados por medio del análisis de la información utilizando sistemas de información geográfica, generando patrones de comportamiento estadístico para conocer la probabilidad de ocurrencia de da os futuros.
Adherence to the practice guideline of community acquired pneumonia in the Hospital Universitario San Jorge, Pereira, Colombia

Abstract: Objetive: To determine the variables associated with nonadherence to practice guideline of Community Acquired Pneumonia CAP in the Hospital Universitario San Jorge of Pereira (HUSJ). Methods: Cross-sectional study of all patients over 18 years of either sex with a diagnosis of NAC between January 1, 2010 and March 30, 2012. The information was taken from the medical records considering socio-demographic, clinical (diagnosis, symptoms, comorbidities), paraclinical (leukogram, X-ray, blood cultures) and therapeutic variables (time of hospitalization and medication costs). Adherence was determined according to the fulfillment of this guide. 2 tests and logistic regression were applied. Results: We found 297 cases of NAC, with slight male predominance (159 men vs 138 women) and mean age 56.3±22.4 years. Adherence to the practice guideline was 14.1% of cases. The antibiotics used were Ampicillin + Sulbactam in 180 patients (60.6% of cases), Ceftriaxone in 53 (17.8%) and Clarithromycin in 31 (10.4%). Blood cultures were 92 and 8.6% reported gram-positive cocci. The low cost of hospitalization (p=0,03, OR:2,07; 95%CI: 1,02-4,14) and chest X-ray report (p<0,001, OR:16,7; 95%CI:2,26-124,11)was statistically significantly associated with adherence to the guide. Non-adherence was associated with lack of guidance from physicians. Discussion: The HUSJ should update your practice guideline and socialize NAC care among physicians and follow up the implementation to reduce hospital stay, complications and costs.
