Evaluation of penicillin therapy in patients with leptospirosis and acute renal failure

Abstract: the effectiveness of specific antibiotic treatment in severe leptospirosis is still under debate. as part of a prospective study designed to evaluate renal function recovery after leptospirosis acute renal failure (arf) (arf was defined as pcr 31.5 mg/dl), the clinical evolutions of 16 treated patients (t) were compared to those of 18 untreated patients (nt). treatment or non-treatment was the option of each patient's attending infectologist. the penicillin treatment was always with 6 million iu/day for 8 days. no difference was found between the two groups in terms of age, gender, number of days from onset of symptoms to hospital admission, or results of laboratory tests performed upon admission and during hospitalization, but proteinuria was higher in the treated group. there were no significant difference in the other parameters employed to evaluate patients' clinical evolution as: length of hospital stay, days of fever, days to normalization of renal function, days to total bilirubins normalized or reached 1/3 of maximum value and days to normalization of platelet counts. dialytic treatment indication and mortality were similar between group t and nt. in conclusion, penicillin therapy did not provide better clinical outcome in patients with leptospirosis and arf.

Abstract: chronic kidney disease (ckd) is one of the most important problems of public health, worldwide. anemia is frequent in ckd, affecting approximately 90% of patients. the relationship between anemia and cardiovascular disease is well established and is due to alterations in left ventricular structure and function. anemia per se can lead to significant cardiac morbidity in the absence of renal disease and is an independent risk factor for cardiac complication in the patient with renal insufficiency. the present review evaluates the inter-relations between anemia, heart failure and renal disease, which were recently included in the so-called cardio-renal anemia syndrome.

Abstract: background: the elderly population is increasing all over the world. the need of intensive care by the elderly is also increasing. there is a lack of studies investigating the risk factors for death among critically ill elderly patients. this study aims to investigate the factors associated with death in a population of critically ill elderly patients admitted to an intensive care unit in brazil. methods: this is a retrospective cohort study including all elderly patients (>60 years) admitted to an intensive care unit in fortaleza, brazil, from january to december 2007. a comparison between survivors and nonsurvivors was done and the risk factors for death were investigated through univariate and multivariate analysis. results: a total of 84 patients were included, with an average age of 73 ± 7.6 years; 59% were female. mortality was 62.8%. the main cause of death was multiple organ dysfunction (42.3%), followed by septic shock (36.5%) and cardiogenic shock (9.7%). complications during intensive care unit icu stay associated with death were respiratory failure (or=61, p<0.001), acute kidney injury (or=23, p<0.001), sepsis (or=12, p<0.001), metabolic acidosis (or=17, p<0.001), anemia (or=8.6, p<0.005), coagulation disturbance (or=5.9, p<0.001) and atrial fibrillation (or=4.8, p<0.041). independent risk factors for death were age (or=1.15, p<0.005), coma (or=7.51, p<0.003), hypotension (or=21.75, p=0.003), respiratory failure (or=9.93, p<0.0001) and acute kidney injury (or=16.28, p<0.014). conclusion: mortality is high among critically ill elderly patients. factors associated with death were age, coma, hypotension, respiratory failure and acute kidney injury.

Abstract: the course of nursing fundamentals introduces the students to supervised training and can result in anxiety and tension. therefore, the purpose of this study was to identify positive/negative aspects related to the care provided during the supervised training, based on the critical incidents technique(1). the subjects were students of the 2°, 3° and 4° years of the nursing undergraduate program offered by the state university of maringá-pr. authors identified 95 reports: 48(50.5%) positive and 47(49.5) negative. the positive aspects were related to the faculty's presence mediating the teaching-learning process; and the negative ones were related to the professionals insensibility regarding pain and death.

Abstract: A disciplina de Fundamentos de Enfermagem introduz o aluno em estágio, podendo gerar ansiedade, tens o. O objetivo deste trabalho foi identificar aspectos positivos/negativos relacionados à presta o de assistência em estágio supervisionado, a partir da técnica dos incidentes críticos (1). Foram sujeitos alunos do 2degrees, 3degrees e 4degrees anos do Curso de Enfermagem da Universidade Estadual de Maringá-PR. Identificamos 95 relatos: 48(50,5%) positivos e 47(49,5%) negativos. A positividade foi atribuída à presen a solidária do professor, mediando o processo ensino-aprendizagem; a negatividade, à insensibilidade dos profissionais frente à dor e à morte.
Renal involvement in leprosy: retrospective analysis of 461 cases in Brazil

Abstract: background: renal lesions in leprosy have been extensively described, including amyloidosis, glomerulonephritis, nephrosclerosis, tubulointerstitial nephritis and granulomas. material and methods: a retrospective study was designed to detect renal function abnormalities in 461 leprosy patients, without any co-morbidity, seen in a university hospital in northeast brazil. the laboratory test results concerning renal function were examined in the patients' medical records. results: the mean age was 39 ± 18 years and 217 (47%) were male. the mean duration of disease was 21 ± 38 months. levels of creatinine above 1.4mg/dl had been detected in 40 patients (8.6%). the levels of creatinine and urea were higher in lepromatous leprosy patients. proteinuria, hematuria and hemoglobinuria were significantly more frequent in this presentation of leprosy. nephrotic levels of proteinuria (>3.5mg/dl) were found in three (2.1%) of 138 lepromatous leprosy patients. a positive association was found between duration of disease and high levels of creatinine in the general sample and in the lepromatous leprosy patients. there was no association between time of treatment and high levels of creatinine. conclusion: renal involvement in leprosy seems to be related to the quantity of bacilli present in the body. it is important to evaluate the renal function in all leprosy patients in order to detect any abnormality and prevent renal failure, which is still a potential cause of death in this disease.
Estudo de caso de oferta induzindo a demanda: o caso da oferta de exames de imagem (tomografia axial computadorizada e ressonancia magnética) na Unimed-Manaus

Abstract: objective: to present the experience of a health plan operator (unimed-manaus) in manaus, amazonas, brazil, with the accreditation of imaging services and the demand induced by the supply of new services (roemer's law). methods: this is a retrospective work studying a time series covering the period from january 1998 to june 2004, in which the computed tomography and the magnetic resonance imaging services were implemented as part of the services offered by that health plan operator. statistical analysis consisted of a descriptive and an inferential part, with the latter using a mean parametric test (student t-test and anova) and the pearson correlation test. a 5% alpha and a 95% confidence interval were adopted. results: at unimed-manaus, the supply of new imaging services, by itself, was identified as capable of generating an increased service demand, thus characterizing the phenomenon described by roemer. conclusion: the results underscore the need to be aware of the fact that the supply of new health services could bring about their increased use without a real demand.
Cisto periodontal lateral: relato de caso clínico

Abstract: O objetivo deste estudo foi descrever um caso clínico de Cisto Periodontal Lateral encontrado em uma paciente de 47 anos, que foi confirmado por exames radiográfico e histopatológico. O Cisto Periodontal Lateral é um tipo raro de cisto odontogênico de desenvolvimento, n o ceratinizado, que ocorre adjacente ou lateral a uma raiz dentária de dente vital e corresponde a 1,5% dos cistos dos maxilares. é encontrado principalmente em adultos entre a 5a e a 7a décadas de vida, sem distin o quanto ao sexo. Sem sinais dolorosos ou sintomas clínicos, s o normalmente diagnosticados durante exames radiográficos de rotina, porém, para um diagnóstico correto é preciso também avaliar as características histopatológicas. O tratamento se faz por enuclea o cirúrgica. Apesar de n o tender à recorrência, recomenda-se a proserva o por alguns anos.
Effect of plant growth regulators in the rooting of Pinus cuttings

Abstract: this work evaluated the rooting of pinus caribaea var. hondurensis morelet cuttings under the action of different levels of plant growth regulators. the cuttings consisted of 4-6 cm long shoots of p. caribaea var. hondurensis morelet with their basal needles removed. the basal part of the cuttings were treated for 2 seconds with the following treatments: 1- naa 2000mg l-1; 2- naa 4000mg l-1; 3- naa 6000mg l-1; 4- naa 2000mg l-1 + pbz 100mg l-1; 5- naa 4000mg l-1 + pbz; 6- naa 6000mg l-1 + pbz; 7- iba 2000mg l-1; 8- iba 4000mg l-1; 9- iba 6000mg l-1; 10-iba 2000mg l-1 + pbz; 11- iba 4000mg l-1 + pbz; 12- iba 6000mg l-1 + pbz; and a control. after receiving the treatment, the cuttings were planted in tubes containing 50% carbonized rice hulls and 50% vermiculite. the evaluations, performed 60 days after planting, showed that p. caribaea var. hondurensis cuttings treated with iba produced a higher percentage of rooted cuttings than those treated with naa; the most effective treatment was iba 4000mg l-1 plus 100mg l-1 paclobutrazol.
