Abstract: objective: the strategic démarche approach questions the dissociation between the strategic direction and the operational level of an institution, in search of a continuous and flexible planning of projects. the objective of the study is to describe the application of this approach in the evaluation of a hospital unit. methodological procedures: the approach was adapted to and applied at a mother and child unit of a public state hospital in the municipality of serra, northeastern brazil. hospital management analysis was conducted with the indirect involvement of institutional agents, who acted as key informants from april to july, 2006. besides the meetings with key informants, data was also collected from the following sources for the year 2005: inpatient records from the obstetric center, maternity and nursery wards, statistical report of the perinatal outcomes from the mother and child unit; records from the ambulatory care unit for breastfeeding. results: the most valued segments were: the neonatal intensive care unit, the obstetric surgery center, and the obstetric center. the program of humanized care for the mother and newborn was also highly valued. the main weaknesses pointed out were the low level of articulation with the network (partnership with municipal network); the lack of hospital beds and technological investment necessary for responding adequately to the demand; excessive professional rotation due to temporary assignments and the deficient implementation of the humanization policies, compromising the strategic success factors of the segments. conclusions: the results indicate the method's potential for discussing the hospital mission and performing analysis of hospital management, pointing out strategies for improving the quality and competitiveness of the segments and for greater integration and insertion in the service network.

Abstract: this study mapped the strategies of resistance constructed by professors front to a harmful work, objectifying to to fortify these forms of confrontations and to collaborate for the transformation of the factors that intervene with the health of these educators. we mapped these movements that are put in functioning in school by means of: institutional experience, interviews, a construction of a diary, questionnaires, photography and confrontation of the results in the collective of workers. we perceive that although all the adversities, the teachers are not left to paralyze and they create proper strategies, new forms to make their work, searching less harmful conditions for their life. by making public these forms of confrontation, this research fortify the creative process of these workers and, at the same time, makes the results allow to the educators the accomplishment of quarrels on the bred strategies front to the adversities, making visible these forms of fight and fortifying the transformation of this reality.

Abstract: this article aims at the particularities concerning the tutorial council and children that were sent by their schools from the city of cariacica es to this institution. from this qualitative method, the study with the tutorial counselors was prioritized in an attempt to learn, through their experiences how to describe the assistance given to the children brought to them. the theoretical reference works with authors that share the idea of a story that can be approached in a genealogical perspective, together with foucault, that allows a vision of a wide variety of knowledge existing in a determined space and social moment. interviews were made with four counselors. through the analysis, it comes a relation marked by psychology, competent speech, proximity, familiarization and the formation of a strength field among the schools and the tutorial council, on which most of the times, the child's behavior is considered unacceptable.
Avalia??o da dor em pós-operatório de cirurgia cardíaca

Abstract: objectives: to evaluate the intensity of pain in patients after cardiac surgery; the study aimed to: 1- identify possible associations between physiological alterations (tachycardia, tachypnea, increased blood pressure, sweating, paleness, nausea and vomiting) and postoperative pain, and 2- describe the used analgesia. methods: this prospective study evaluated 30 patients in a public teaching hospital, from the immediately postoperative period till the 4th postoperative day. to analyze the data was used the software statistical package for social sciences. results: most (26/86.7%) patients had pain, being 19 (63.3%) in the 1st postoperative day. it was found a predominance of mild pain during all occasions of evaluation. also was found a correlation (p<0.001) between pain and the presence of physiological alterations, most frequently, tachypnea and increased blood pressure. the analgesia was treated with drugs, being the opiates and simple analgesics the most prescribed. conclusion: a mild pain was present during all periods which influenced mainly the occurrence of tachypnea and high blood pressure. only drugs were used for analgesia.

Abstract: this article presents the results of a study of the type before and after training in shelter with risk classification compared to the humanization of social representations of health among 111 health professionals in an emergency hospital. data collection was performed using the technique of free evocation and analysis was done using the evoc software. the results showed changes in the symbolic meaning attributed to terms inducers towards incorporation of the right perspective in the representation of the unified health system, the translation of humanization in health as shelter, and the progression of understanding the shelter, the humanistic focus to the qualification of processes of care of the users demand at emergencies. the results indicate that the differences found in the core before and after training were due to an apprenticeship, which based in peripheral elements, was able to question the core elements and interchange between the central and peripheral system, recognizing the functional complementarity between these two systems and the relationships between representations and practices. however, the method does not assert the persistence of such changes in the social representations of the objects studied in depth or measure the changes in daily practices.

Abstract: De acordo com a Lei Federal 8. 069, que institui o Estatuto da Crian a e do Adolescente, prevê-se a cria o dos Conselhos Municipais de Direitos, dos Conselhos Tutelares e do Fundo Municipal para a Infancia e Adolescência como instancias responsáveis pela implementa o da Política de Assistência à Infancia e a Juventude brasileira. O objetivo desse artigo é discutir como os Conselheiros Tutelares têm se constituído como protagonistas desta política, ressaltando aspectos de suas vidas e a cren a que possuem quanto à necessidade de criarem novos espa os de participa o social. O estudo que serviu de base para tal discuss o baseou-se no método qualitativo de investiga o, tendo como parametro o aporte da história oral, mais especificamente a perspectiva da história de vida. Apresenta-se como a categoria analítica sofrimento ético-político se entrecruza na vida desses entrevistados com a prática religiosa, um cotidiano de aprendizado, compartilhamento, luta permanente por sobrevivência e transforma o social. Finalmente sinaliza-se para os diferentes movimentos que est o sendo forjados por esses atores sociais em nome de uma prática cidad que afirma a urgência da democracia participativa.
Assistência e mortalidade neonatal no setor público do Município do Rio de Janeiro, Brasil: uma análise do período 1994/2000

Abstract: this article analyzes an intervention by the rio de janeiro municipal health department (sms-rj), brazil, to reduce the neonatal mortality rate (strategies for organizing and upgrading neonatal care in the municipal system, including an increase in the number of neonatal high-risk beds). we studied the trends in neonatal mortality rate (1995/2000), neonatal care provided in different public hospitals (1994/2000), and admissions profile and mortality in four neonatal intensive care units (nicus) under the sms-rj (2000). there was a concentration of high-risk neonatal care in the municipal hospitals (an increase from 28.0% of the care provided for live premature neonates in 1994 to 67.0% in 2000) and a reduction in the neonatal mortality rate in units under the unified national health system (from 19.9 deaths per thousand live births in 1996 to 15.5 in 2000). there was no reduction in the prematurity and low birth weight rates among mothers residing in the municipality of rio de janeiro. analysis of admissions to the nicus showed a high proportion of neonates born to mothers from municipalities outside rio de janeiro, while 14.0% of the mothers had not received prenatal care, and the mortality rate among newborns with birth weight < 1.500g was 32.0%.

Abstract: Este artigo apresenta uma avalia o da interven o realizada pela Secretaria Municipal de Saúde do Rio de Janeiro(SMS-RJ) (estratégias de organiza o e qualifica o da assistência neonatal na rede municipal, incluindo a amplia o dos leitos neonatais de risco), com o objetivo de reduzir a mortalidade neonatal. Analisamos as mudan as ocorridas no atendimento dos diferentes prestadores do setor público (período 1994/2000), na taxa de mortalidade neonatal dos nascimentos ocorridos nas institui es do Sistema único de Saúde (1995/2000) e o perfil das interna es em quatro Unidades de Terapia Intensiva Neonatal (UTIN) da SMS-RJ (2000). Encontramos a concentra o do atendimento neonatal de risco nas unidades municipais (de 28,0% do atendimento de nascidos vivos prematuros, em 1994, para 67,0% em 2000), redu o na mortalidade neonatal dos nascimentos ocorridos no SUS (de 19,9 óbitos por mil nascidos vivos em 1996 para 15,5 em 2000). N o houve redu o nas taxas de prematuridade e baixo peso ao nascer entre as m es residentes no Município do Rio de Janeiro. Na análise das interna es nas UTIN encontramos elevada propor o de neonatos de m es moradoras de outros municípios, 14,0% de m es que n o realizaram pré-natal e 32,0% de mortalidade entre neonatos com peso ao nascer < 1.500g.
Intra- and inter-examiner reproducibility of manual probing depth

Abstract: the periodontal probe remains the best clinical diagnostic tool for the collection of information regarding the health status and the attachment level of periodontal tissues. the aim of this study was to evaluate intra- and inter-examiner reproducibility of probing depth (pd) measurements made with a manual probe. with the approval of an ethics committee, 20 individuals without periodontal disease were selected if they presented at least 6 teeth per quadrant. using a williams periodontal probe, three calibrated thesis-level students (k > 0.6) assessed pd at 6 sites per tooth, from the gingival margin to the bottom of the periodontal sulcus (rounded to the next 0.5 mm). initial and repeated measurements were performed by the same three examiners. the intra-examiner agreement (± 1 mm > 90%) was 99.85%, 100%, and 100% for the three examiners, respectively. when the variables vestibular/lingual surfaces, mesial/distal surfaces, or superior/inferior jaws were evaluated, no significant differences in reproducibility were detected at the inter-examiner level (p < 0.05). at this level, the only significant differences observed were in the three examiners' measurements of the anterior and posterior sites. while high intra-examiner reproducibility was detected, inter-examiner level proved to be low. we can conclude that measurement of pd with a manual periodontal probe produced high reproducibility in healthy individuals. the operator's position can affect the reproducibility of repeated measures of pd. calibration and operator training, rather than operator experience, were fundamental for reproducibility. other factors, such as individual technique and probing depth force, can affect inter-examiner reproducibility.
