Abstract: the authors review the use of staplers in general surgery, mainly in gastroenterological surgery highlighting safety and effectiveness. they emphasize that the mechanic anastomosis are a viable technique and in some conditions, the best way to perform the anastomosis. the use of staplers makes surgery faster and easier and complications are, generally related to surgeon's experience in using them. although its costs are higher compared to handsewn suture, it can make the patient total cost lower.

Abstract: most patients with partiai gastrectomy have no postoperative problems. some of them, however, develop severe nutritional disturbance, particularly those with bilroth ii reconstruction, which can be treated clinically in the majority of the cases. those with important mal absorptive problems require surgery. in this article we present a technical alternative for those patients who need reintervention over a bii operation. our technique has the advantage of not touching the duodenum previously sutured.

Abstract: background: the association of abdominoplasty with other surgeries of the abdominal wall and cavity is an attractive approach although it has been a matter of controversies.our objective is to evaluate the morbidity and mortality of these procedures. methods: we studied retrospectively 75 patients with indication for abdominoplasty. thirty nine of them were submitted to abdominoplasty alone (group 1) and 36 were associated with other abdominal surgeries (group2), as hernioplasties, hysterectomies or cholecystectomies. results: the complications were seroma (group 1: 2.56%, group 2: 25%), epidermolisis (group 1: 12.82%, group 2: 5.55%), suture dehiscence (group 1: 5.12%, group 2: 5.55%), wound infection (group 1: 0%, group 2: 8.33%) and haematoma (group 1: 0%, group 2: 5.55%). there was statistical difference only in the presence of seroma (p=0.009). conclusion: there was no significant increase in morbidity and mortality when abdominoplasty was combined to other abdominal surgeries compared to isolated abdominoplasty.

Abstract: seeds of schizolobium amazonicum present physical dormancy which can be broken through different scarification methods. in this work, the effects of sandpaper and hot water seed scarification on physiological aspects related to seed germination and seedling quality were evaluated. seeds of s. amazonicum were scarified by (i) immersion in water at 100°c/2 min and (ii) by scrubbing seeds on sandpaper. non-scarified seeds were used as control. the following variables were evaluated: tissue viability (by using tetrazolium test), germination rate, speed germination index (sgi), seed imbibition rate, endosperm depletion, and seedling emergence rate, uniformity and biomass accumulation. seeds from both treatments were equally dyed by tetrazolium. seeds scarified on sandpaper showed higher germination rate, higher sgi, faster seed imbibition and endosperm depletion than those scarified in water at 100°c/2 min. seeds scarified on sandpaper presented higher seedling emergence rate and emergence index, and their seedlings were more uniform and with greater biomass than those obtained after scarification in water at 100°c/2 min. therefore, scarification of paricá seeds on sandpaper is more efficient in promoting germination and production of uniform seedlings.

Abstract: Este artigo tem duas dimens es de análise: uma busca compreender o sentido e a finalidade da política concebida e executada pelo Estado capitalista;a outra investiga a sua natureza, se pública ou estatal. Com isso, tem-se como propósito construir bases conceituais e metodológicas que ajudem a entender um pouco melhor as raz es dos sucessos ou fracassos das políticas ditas públicas, particularmente em contextos de países periféricos. Essa constru o para o entendimento passa por compreender que o resultado está associado muito mais à forma de gest o da demanda social - administra o política - do que à precis o do cálculo previamente estabelecido. This article has two analytical dimensions: the first deals with the meaning and purpose of the policies designed and implemented by the capitalist state; the second investigates their nature, whether public or state. Therefore, the article intends to build a conceptual and methodological framework that will help to better understand about the reasons of the success or failure of the so-called public policies, especially in the context of the peripheral countries. This involves understanding that the outcome is associated much more to the way social demands are managed - political administration - that to the precision of previously established calculations.
Studies on the feeding habits of Lutzomyia (N.) intermedia (Diptera, Psychodidae), vector of cutaneous leishmaniasis in Brazil

Abstract: the precipitin test was applied to identify the blood meal sources of lutzomyia intermedia, collected in two distinct habitats: inside houses and in the peridomicile, in mesquita, rio de janeiro state, brazil, a transmission area of leishmania (v.) braziliensis. the following antisera were tested: human, avian, equine, rodent, and opossum. from a total of 370 females analyzed, 128 specimens from the domicile and 59 from the peridomicile reacted with specific antisera. the anthropophily of l. intermedia was confirmed in both habitats; likewise, the feeding of this sand fly species on domestic animals, observed in previous entomological surveys, was confirmed by the strong reactivity with avian, canine, and equine antisera. however, feeding on rodents, mammals frequently found inside and around houses, represents further evidence related to the vector competence of l. intermedia, since synanthropic and sylvatic rodents have been considered a putative reservoir of l. (v.) braziliensis.

Abstract: this article reports on the opinions of applicants admitted to the medical residency programs at the federal university in rio de janeiro (ufrj) (based on those that applied for specialties without prerequisites) in relation to the test model, according to the changes proposed by the 2005 admissions committee. the admissions tests were elaborated on the basis of the national curriculum guidelines for undergraduate studies in medicine and a specific ruling by the national commission on medical residency. a structured questionnaire with closed questions was distributed randomly to the applicants that passed the admissions tests. the majority of the respondents (84.16%) rated each stage of the admissions process as "very good" or "good", with the highest rating going to the practical examination with patients. the authors discuss the necessary healthcare skills that served as the basis for the principal changes in the admissions process. another relevant point was the importance of interdisciplinary integration in the admissions committee. the high approval rate shows that the changes were pertinent, and that the scores attributed to the various stages can help improve the admissions process and serve as an evaluation tool for recent medical school graduates.

Abstract: objective: to study the sentinel lymph node in early gastric cancer as a diagnostic method of unsuspected lymph node metastasis, which may allow the performance, in those with negative lymph nodes, of smaller gastric resections with limited lymphadenectomy. methods: we studied seven patients with early gastric cancer treated at the hospital universitário clementino fraga filho, federal university of rio de janeiro, from september 2008 to may 2011, who underwent sentinel lymph node exams, performed by intraoperative peritumoral endoscopic injection of patent blue dye. results: we found an average of three sentinel nodes per patient. the frozen biopsy of lymph nodes was negative for metastases, which allowed the realization of atypical gastric resection in three cases and antrectomy with bi reconstruction in four. the performed lymphadenectomy was modified d1. there was no operative mortality. the duration of postoperative follow-up ranged from five to 37 months, without evidence of recurrence. one patient developed a second early tumor 13 months after the initial surgery and underwent total gastrectomy. conclusion: the sentinel lymph node in early gastric cancer proved to be an effective method for the evaluation of nodal metastases in seven patients and allowed for smaller gastric resections and limited lymphadenectomies. these minor procedures reduce the risk of postoperative complications, maintaining, on the other hand, the oncological radicality that is required in the treatment of gastric cancer.
