Statistical voxel-wise analysis of ictal SPECT reveals pattern of abnormal perfusion in patients with temporal lobe epilepsy

Abstract: objective: to investigate the pattern of perfusion abnormalities in ictal and interictal brain perfusion spect images (bsi) from patients with temporal lobe epilepsy (tle). method: it was acquired interictal and ictal bsi from 24 patients with refractory tle. bsis were analyzed by visual inspection and statistical parametric mapping (spm2). statistical analysis compared the patients group to a control group of 50 volunteers. the images from patients with left-tle were left-right flipped. results: it was not observed significant perfusional differences in interictal scans with spm. ictal bsi in spm analysis revealed hyperperfusion within ipsilateral temporal lobe (epileptogenic focus) and also contralateral parieto-occipital region, ipsilateral posterior cingulate gyrus, occipital lobes and ipsilateral basal ganglia. ictal bsi also showed areas of hypoperfusion. conclusion: in a group analysis of ictal bsi of patients with tle, voxel-wise analysis detects a network of distant regions of perfusional alteration which may play active role in seizure genesis and propagation.

Abstract: background and objectives: this study had the purpose of studying gastric emptying in patients with chronic renal failure. material and method: thirty patients with chronic renal failure were studied, 16 in conservative clinical treatment and 14 in hemodialysis for over six months. the control group (ctl) was composed of 18 asymptomatic volunteers. the method of gastric emptying study was scintigraphy. the standardized test meal was an omelet of three chicken eggs prepared with colloidal sulfur marked with 185 mbq of 99 m technetium. gastric retention curves were studied and t? of gastric emptying was obtained from them. a t? value corresponding to the average of t? values of control group plus twice standard deviation was considered nornmal. statistical tests used were χ2 and kruskal-wallis. results: there was no statistically significant difference with regard to total gastric retention curves and t? of gastric emptying, which was similar in three studied groups. nine patients had high t? of gastric emptying, above 125 minutes. these patients were equally distributed among both genders and conservative clinical treatment and hemodialysis groups. conclusions: we concluded that gastric retention rate and t? of gastric emptying in patients with chronic renal failure in conservative clinical treatment and hemodialysis does not differ from the healthy patients group. hemodialysis does not seem to reduce the risk of gastric retention in patients with chronic renal failure

Abstract: Resumo: Os manuais de linguística ou filologia romanica costumam incluir o reto-romanico como uma das principais línguas neolatinas, ao lado do português, espanhol, francês, italiano e romeno. No entanto, tal denomina o refere-se na verdade a um conjunto extremamente heterogêneo de línguas, com uma profunda fragmenta o dialetal. Acresce-se a isso o recente problema da padroniza o da norma escrita. Este artigo pretende especificamente apresentar a situa o linguística atual dos Gris es e o problema da convivência do romanche com os dialetos alem es e o italianos. Palavras-chave: Reto-romanico; dialetologia; contato linguístico. Resumen: Los manuales de linguística o filología románica costumbran incluir el reto-románico como una de las principales lenguas neolatinas, al lado del portugués, espa ol, francés, italiano y rumano. Pero tal denominación se refere en la realidad a un conjunto extremamente heterogéneo de lenguas, con una profunda fragmentación dialectal. Se a ade a eso el reciente problema de la estandarización de la norma escrita. Este artículo pretende especificamente presentar la situación linguística actual de los Grisones y el problema de la convivencia del romanche con los dialectos alemanes e italianos. Abstract: Raeto-Romance is usually included, as well as Portuguese, Spanish, French, Italian and Rumanian, among the main Neolatin languages in the handbooks about Romance Philology or Linguistics. However, an extremely heterogeneous set of very different languages, with a huge dialect splitting is meant by such denomination. The problem is also increased with the recent standardization of the written language. The current linguistic situation of the Grisons is specifically dealt in this paper, as well as the coexistence of the Rumantsch language together with the German and the Italian dialects. Esvaziamento gástrico nos pacientes com insuficiência renal cr?nica submetidos à hemodiálise

Abstract: background: dyspeptic symptoms are frequent in renal patients. they are anorexia, nausea, vomit, pain and epigastric distension. these symptoms can be related to the delay in gastric emptying. aim: to investigate gastric emptying in patients with chronic renal failure on hemodialysis treatment. method: thirty one subjects (18-60 y) were studied, 14 on hemodialysis for at least 6 months and 17 healthy subjects). patients with diabetes, amyloidosis, collagenous disease, pregnant, dyspeptic complaints , gastric surgery and patients taking procinetics drugs were excluded. gastric emptying was investigated with radionuclide scintigraphy, using a dual-head scintillation camera. the standard test meal consisted of 185 mbq of technetium-99m sulfur colloid mixed with three scrambled eggs. for each study group a time-activity curve of gastric retention was generated and from that the t? of gastric emptying was obtained. results: the groups were similar both in gender and age. there was no significant statistical difference in total retention curves between the two studied groups. also the t? of gastric emptying was similar in the two groups. conclusion: gastric emptying of patients on hemodialysis treatment for at least 6 months was not different from the health subjects.
Brain hypoperfusion in adolescents dependent of multiple drugs

Abstract: brain spect imaging (bsi) with statistical parametric mapping (spm) offers a clear and accurate vision of perfusion changes. objective: to study brain perfusion abnormalities in adolescents' dependent of multiple drugs. method: sixteen male patients (15.1±2.1 years) were submitted to 99mtc-hmpao bsi with spm. results: cortical hypoperfusion occurred in 7/16 patients (44%). there was a significant inverse correlation between the number of hypoperfused regions and the patient's age (p= -0.6737; p=0.004) and with the age when the drug dependence began (p= -0.5616; p=0.023). there was also a tendency towards an inverse correlation between regions of hypoperfusion and the duration of the drug dependence. conclusion: bsi with spm can help detect hypoperfusion in adolescents dependent on multiple drugs. the younger the patients, the more regions of hypoperfusion are noted. probably, the neuronal plasticity has an important role in this phenomenon because the highest neural activity occurs in childhood.

Abstract: gender-related differences in gastric emptying are still controversial. the aims of this study were: to confirm the sex-related difference in gastric emptying of a solid meal and to investigate its association with different patterns of meal distribution between the proximal and distal gastric compartments. eighteen healthy volunteers (nine males, mean age 35 ± 9 years; nine females, mean age: 41 ± 11 years) were studied in the morning, after ingestion of the solid test-meal (an omelette labeled with 185mbq of 99mtc-sulfur colloid). simultaneous anterior and posterior images of the stomach were acquired immediately after ingestion of the meal and every 10 minutes for 120 minutes. time versus activity curves were obtained for the whole, proximal and distal stomach. gastric t? was longer in women (96.1 ± 17.2 min) than in men (79.9 ± 17.8 min; p = 0.02). the analysis of the meal distribution inside the stomach showed no differences between males and females in proximal gastric emptying, but the meal retention in the distal compartment was significantly increased among women (p = 0.04). in conclusion, gastric emptying of a solid meal is slower in pre-menopausal women than in age-matched men, probably due to an increased retention of the meal in the distal compartment. this should be taken into consideration to avoid misleading diagnosis of gastroparesis for female patients.

Abstract: in an article of 1920 entitled "die erscheinungsformen des irreseins," published in kraepelin's phase of theoretical maturity, this great german scholar presented a re-reading of his thinking on psychopathology, considering that clinical manifestations of mental diseases are never direct expressions of underlying pathogenic processes. manifestations depend on a complex interaction with the subject in his or her history during which changes have been installed and inscribed.

Abstract: the term "neurosis" in its technical, or medical, sense, was coined in 1769 with the publication of the first edition of the famous synopsis nosologiae methodicae, by the scottish physician william cullen (1710-1790). according to a classification based on linnaeus's taxonomy, the concept refers to a class of general affections of the nervous system that are not accompanied by fever and that especially affect sensitivity and movement. the category consists of four specific "orders" of phenomena: comas, adynamias, spasmodic affections without fever, and vesanias, such as manias (madness), and melancholia.
