Sildenafil no tratamento da hipertens?o arterial pulmonar idiopática em crian?as e adolescentes

Abstract: objective: this study aims to provide data on the use of oral sildenafil in patients in new york heart association functional class iii or iv with severe idiopathic pulmonary arterial hypertension unresponsive to conventional therapy. method: in this series, six patients with idiopathic pulmonary arterial hypertension were prospectively treated with 2 to 8 mg of oral sildenafil in four to six doses a day. all pa-tients were submitted to physical examination, electrocardiogram and echocardiogram, chest computed tomography, ventilation and pulmonary perfusion scintigraphy, coagulation studies, and tests for collagen vascular disease, acquired immune deficiency syndrome and schisto-somiasis in order to rule out secondary causes of pulmonary arterial hypertension. all patients underwent cardiac catheterization for vasoreactivity tests using nitric oxide, o2 at 100% and oral nifedipine, and a 6-minute walking test was performed in those patients who were con-sidered able to exercise. results: all patients achieved a good therapeutic response, with improvement by at least one functional class, and presented an increase in systemic arterial oxygen saturation. five patients showed a decrease in the pulmonary systolic pressure to systemic systolic pres-sure ratio and improvement in the six-minute walking test. no major side effects were ob-served at 4 to 36 months of follow-up. one patient had sudden death after sildenafil had been withdrawn by mistake. conclusions: these data suggest that sildenafil may be useful in the manage-ment of idiopathic pulmonary arterial hypertension. patients should be advised against the withdrawal of sidenafil without medical supervision.

Abstract: pulmonary arterial hypertension is classified as idiopathic or secondary (associated with collagenoses, heart disease, portal hypertension, pulmonary thromboembolism, and pulmonary vascular diseases). pulmonary vasoreactivity should be tested in order to define the best treatment option. of the many drugs that have been used to test pulmonary vasoreactivity, inhaled nitric oxide is the best choice, due its specific pulmonary effect and very short half-life (5-10 s). the results of this test identify candidates for heart surgery among patients with congenital heart disease and candidates for the use of calcium antagonists among patients with other forms of pulmonary hypertension. performing and interpreting the results of such tests are a great responsibility, since mistakes can lead to incorrect treatment decisions, resulting in the death of patients.

Abstract: the percutaneous closure of ostium secundum (os) atrial septal defect (asd) is a well-established procedure and is today considered the treatment of choice due to its good results and low morbidity and mortality. the procedure is routinely performed through the inferior vena cava (ivc). however, this route of access is not always available due to the obstruction or congenital absence of the ivc. we will present a case of asd closure by means of transhepatic puncture due to the impossibility of using the ivc.

Abstract: a two-year program of biological sampling was carried out on the area comprised by the islands of anchieta, vitória and couves on the northern coast of s?o paulo state, brazil. most of the samples were collected by otter-trawl since the demersal fish were the main interest of the study. in spite of the small size of the nets used - only 30 feet on the mouth - the results obtained as to the number and diversity of the fish found in the area seemed to be significant enough to justify the present communication. as many as 97 species of bony and cartilaginous fish were obtained from 28 controlled catches, along the years 1978-1979. the best represented family, as could be expeoted were the sciaenidae but rajidae, bothidae and cynoglossidae were also abundant. lophius gastrophysus and merluccius hubbsii. occurred only in depths above 30 m, in agreement with their known distribution in the far southern coast of brazil. a systematic list of the species and their common names, when available, is given.

Abstract: throughout 2000 and 2001 the uitoto-murui communities of the cara paraná river were mobilized by a debate on the notion of 'development.' meetings were held between the indigenous population (including both local villagers and representatives of national organizations) and the non-indigenous population (government officials, ngos and the military) to discuss the theme with the aim of producing 'diagnoses' and 'projects.' as part of this context, the san rafael village- led by cacique don ángel ortiz and the local cabildo or council- proposed a project based on the 'knowledge' acquired from nightly rounds of dialogue and ritual consumption of coca and tobacco, culminating in a corpus of symbolic, ethico-moral and material knowledge denominated 'the word of coca and tobacco' (jiibina uai diona uai). this body of knowledge (spanning from the mytho-cosmological to the medicinal-therapeutic and including forms of governance) can be traced to the sociocultural configurations lived through by indigenous peoples from the region between the caquetá and putumayo rivers (whose symbolic epicentres are coca and tobacco), especially after the 1930s with the end of the colombia-peru war and the debt-bondage system associated with the rubber trade. in the heat of the debates, the notion of 'development' therefore acquired 'indigenous' contours, becoming read and articulated on the basis of local 'uses and customs,' as indeed required by colombia's 1991 constitution. likewise, in response to the demands made from the development area, a concise outline of 'the word of coca and tobacco' was also presented.

Abstract: in this work we study the behavior of the allowed frequencies in the dual transmission line when the values of capacitances are distributed according to a disordered sequence with long-range correlation. for each frequency ω we use the lyapunov exponent λ(ω) as a tool to discriminate between a transmitting line and a filtering one. when the dual line has a periodic distribution of capacitances, a typical scheme of permitted bands and forbidden bands (gaps) appears. for the random case every signal is filtered. in this article we analyze the effect of long-range correlation, concluding that two cases can appear, depending on the kind of correlation used: a) there exist only a finite number of specific frequencies for which the signal is propagated, or b) there is a continuous band of propagating frequencies. the appearing of continuous bands is a very important result, since the existence of correlated disorder is a condition necessary but not sufficient to the appearing of propagating frequencies. understanding the mechanisms that describe the transition from a forbidden frequency to an allowed frequency opens the possibility of manufacturing electronic devices where this transition is relevant.
The shape of the human lumbar vertebral canal

Abstract: literature on the anatomy of the human vertebral column characterizes the shape of the lumbar vertebral canal as triangular. the purpose of the present study was to determine the precise shape of the lumbar vertebral canal. ten lumbar vertebral columns of adult male cadavers were dissected. two transverse sections were performed in the third lumbar vertebra. one section was performed at the level of the lower border of the ligamenta flava, and the other section was performed at the level of the pedicles. the shape of the lumbar vertebral canal at the level of the pedicles tends to be oval or circular, whereas the shape of the lumbar vertebral canal at the level of the lower border of the ligamenta flava is triangular. thus, the shape of the human lumbar vertebral canal is not exclusively triangular, as reported in the literature. it is related to the level of the transversal section performed on the lumbar vertebra. this finding should be taken into consideration among factors involved in the spread of solutions introduced into the epidural space.
