Testosterone and Its Bioactive Components Are Associated with Libido and the Metabolic Syndrome in Men  [PDF]

Abstract: Background: Some evidence has shown that the prevalence of hypoactive sexual desire and erectile dysfunction (ED) is associated with testosterone levels whilst higher levels of testosterone have been reported to increase the frequency of intercourse. Available evidence points towards an etiologic role for hypogonadism in the causation of diabetes, metabolic syndrome (Mets) and sexual dysfunction (SD) even though the exact pathophysiological linkage is yet to be fully elucidated. This study therefore sought to evaluate the impact of testosterone as well as its bioactive components on both the MetS and SD. Method: Diabetic men engaged in a stable heterosexual relationship for at least 2 years were recruited for this study. Participants were at least 18 years and provided an informed and signed consent to partake in this study. Fasting blood samples were taken from the participants for biochemical and hormonal assay. The participants were then evaluated using the Golombok Rust Inventory of Sexual Satisfaction for males (GRISS-M). Metabolic syndrome was assessed using the NCEP-ATP III, IDF and WHO criteria. All data analyses were performed using the SPSS software, version 11.0 systat, Inc. Germany and GraphPad Prism, version 5.0, San Diego California, USA. Results: The mean total, free and bioavailable testosterone as well as SHBG recorded among the participants was 7.10 ± 1.23 ng/ml, 0.10 ± 0.01 ng/ml, 7.01 ± 1.41 ng/ml and 4.33 ± 1.12 nmol/l respectively. Subjects with the MetS showed significantly lower SHBG levels in comparison with subjects without the MetS. Participants with raised triglyceride levels showed significantly lower levels of total, free and bioavailable testosterone when compared to participant without raised triglycerides. Participants with problems of avoidance and infrequency of sexual activity showed significantly lower levels of total, free and bioavailable testosterone when compared to participants without avoidance or infrequency problems. The +SD/+MetS group recorded the lowest SHBG levels. Conclusion: Testosterone and its bioactive components are associated with male libido as well as the metabolic syndrome. Low levels of free and bioavailable testosterone are true determinants of the MetS in men.
Alcohol Consumption Is Associated with Hypogonadism and Decreased Sexual Function in Ghanaian Diabetics  [PDF]

Abstract: Introduction: Alcohol usage has largely been seen as a risk factor for the development of sexual dysfunction as well as erectile dysfunction. Others have reported that prolonged alcohol usage and abuse is compatible with normal sexual function in the absence of endocrinological problems as well as hepatic dysfunction. About seventy five (75) percent of alcoholics have various sexual difficulties with improvements in sexual functions occurring after treatment of alcoholism and psychosexual therapy. It is evident from the various reports over the years that mild and occasional alcohol usage is not as much implicated in the causation of SD and its other forms as heavy, addictive or dependent alcohol usage. Alcohol usage has also long been linked to hypogonadism, testicular atrophy as well as leydig cell toxicity. Alcohol induced hypogonadism has been reported to resolve after withdrawal of alcohol use. Since both diabetes and alcohol usage have been strongly associated with both hypogonadism and sexual dysfunction, it is logical to expect that diabetics who frequently consume alcohol will have a worsened hypogonadal state and sexual function. This research therefore seeks to provide evidence of an association between alcohol consumption in diabetics and a worsened sexual dysfunction in comparison to diabetics who did not consume alcohol. Methods: Type II diabetic patients attending the Diabetic Clinic at the Maamobi General Hospital between the periods of January 2010 and March 2011 were consecutively recruited for this study. Diabetics with other known endocrinological diseases and physical disabilities were excluded from the study. Sexual function was assessed using the GRISS-M. Early morning fasting samples were used in lipid and testosterone profile assays. Results: Study participants who consumed alcohol recorded higher levels of triglycerides and LDL-Cholesterol. They also recorded significantly lower levels of bioavailable testosterone. Furthermore they also recorded higher scores for impotence, premature ejaculation, non-sensuality and infrequency but lower scores for avoidance and were about six times more likely to be infrequent in their sexual activity in comparison with those who did not consume alcohol. Conclusion: Alcohol consumption among diabetic males is associated with hypogonadism and has an impact on several domains of male sexual function. Modified Heyman packing performed with small number of Simon-Norman catheters

Abstract: Purpose: Modified Heyman packing method, performed usually with 6-11 flexible catheters is one out of many brachytherapy treatments. We described the insertion and dose planning method, with the use of 4-6 Simon-Norman catheters positioned to the fundus uteri, which is the most frequent source of the endometrial cancer.Material and methods: We investigated the dose distribution of the main classes of regular catheter insertions with Simon-Norman applicators using dose profiles, obtained in lateral and axial direction at the fundus, and compared with the dose distribution of the Y-shaped catheter arrangements.Results: The insertions with five and six Simon-Norman catheters produced higher dose at the fundus uteri, compared with the Y-shaped applicator.Conclusions: Insertions performed with small number of catheters permits to perform dosimetry optimisation foreach treatment fraction.

Abstract: The geospatial analyst is required to apply art, science, and technology to measure relative positions of natural and man-made features above or beneath the earths surface, and to present this information either graphically or numerically. The reference positions for these measurements need to be well archived and managed to effectively sustain the activities in the spatial industry. The research herein described highlights the need for an information system for the Land Surveyors Equipment Store. Such a system is a database management system with a user friendly graphical interface. This paper describes one such system that has been developed for the Equipment Store of the Department of Geomatic Engineering, Kwame Nkrumah University of Science and Technology, Ghana. The system facilitates efficient management and location of instruments, as well as easy location of beacons together with their attribute information, it provides multimedia information about instruments in an Equipment Store. Digital camera was used capture the pictorial descriptions of the beacons. Geographic Information System software was employed to visualize the spatial location of beacons and to publish the various layers for the Graphical User Interface. The aesthetics of the interface was developed with user interface design tools and coded by programming. The developed Suite, powered by a reliable and fully scalable database, provides an efficient way of booking and analyzing transactions in an Equipment Store.
A Single Centre Experience with Phenotype Dependent Perioperative Chemotherapy for Primary Breast Cancer  [PDF]

Abstract: Background: Standard adjuvant or neo-adjuvant chemotherapy of primary breast cancers consists of docetaxel and the combination of anthracyclin-cyclophosphamide in two steps. According to international literature, the neo-adjuvant treatment of triple negative tumours by the combination of docetaxel-carboplatin allows a high rate of pathological complete remission and the adjuvant treatment of small, node negative HER2+ tumours by the combination of paclitaxel-trastuzumab allows a negligible recurrence rate together with a very good tolerance. Methods: Our aim was the retrospective analysis of the alternative chemotherapy regimens in our patient population. Results: The neo-adjuvant docetaxel-carboplatin allowed a 55% pathological complete remission rate among 20 patients. With the adjuvant paclitaxel-trastuzumab, we did not notice any recurrence among 5 patients at a median follow-up of 18 months. Conclusion: Our results correspond to the literature data. The spread of the protocols in the clinical practice is proposed.
Differences in Body Fat Distribution Play a Role in the Lower Levels of Elevated Fasting Glucose amongst Ghanaian Migrant Women Compared to Men

Abstract: Background Despite higher levels of obesity, West African migrant women appear to have lower rates of type 2 diabetes than their male counterparts. We investigated the role of body fat distribution in these differences. Methods Cross-sectional study of Ghanaian migrants (97 men, 115 women) aged 18–60 years in Amsterdam, the Netherlands. Weight, height, waist and hip circumferences were measured. Logistic regression was used to explore the association of BMI, waist and hip measurements with elevated fasting glucose (glucose≥5.6 mmol/L). Linear regression was used to study the association of the same parameters with fasting glucose. Results Mean BMI, waist and hip circumferences were higher in women than men while the prevalence of elevated fasting glucose was higher in men than in women, 33% versus 19%. With adjustment for age only, men were non-significantly more likely than women to have an elevated fasting glucose, odds ratio (OR) 1.81, 95% CI: 0.95, 3.46. With correction for BMI, the higher odds among men increased and were statistically significant (OR 2.84, 95% CI: 1.32, 6.10), but with consideration of body fat distribution (by adding both hip and waist in the analysis) differences were no longer significant (OR 1.56 95% CI: 0.66, 3.68). Analysis with fasting glucose as continuous outcome measure showed somewhat similar results. Conclusion Compared to men, the lower rates of elevated fasting glucose observed among Ghanaian women may be partly due to a more favorable body fat distribution, characterized by both hip and waist measurements.

Abstract: 为了实现烟气CO2的低成本、高效捕集,研究负载1,8-二氮杂二环[5,4,0]十一碳-7-烯(1,8-diazabicyclo[5,4,0]undec-7-ene,DBU)的颗粒活性炭(granularactivatedcarbon,GAC)吸附剂对CO2捕集性能。DBU负载率越高,吸附剂比表面积、总孔容积和微孔容积越小,平均孔径与负载率呈明显的负相关性;Langmuir模型是拟合DBU在GAC上吸附行为拟合优度最佳的模型,DBU在GAC上单分子层吸附要多于多分子层吸附;DBU-GAC吸附剂在低于140℃时具有良好的热稳定性,DBU能抑制吸附剂水分蒸发,有利于DBU与CO2耗水反应;DBU溶液对CO2具有很高的捕集效率,是常用一乙醇胺–甲基二乙醇胺(monoethanolamine-methyl-diethanolamine,MEA-MDEA)复合胺溶液的2.6~3.3倍;GAC负载适量DBU对模拟烟气CO2具有良好的捕集效率,消除膜封效应能提高DBU-GAC吸附剂对CO2的吸附容量。
Soil Fertility Analysis in Two Cocoa Farming Towns in the Central Region of Ghana

Abstract: Our soil resource can be compared to a bank where continued withdrawal without repayment cannot continue indefinitely. As nutrients are removed by one crop and not replaced for subsequent crop production, yields will decrease accordingly. Accurate accounting of nutrient removal and replacement, crop production statistics, and soil analysis results will help the producer manage fertilizer applications. Macronutrients, micronutrients, pH and salinity were determined in soil samples from two cocoa farming towns in Assin North District in the Central Region of Ghana. Neutron Activation method was used for the elemental analysis. The pHs from both towns were within the acidic range and from both towns also salinity increased with depth. For the soil samples from Assin Akonfudi potassium concentration was the highest and molybdenum was the least in the soil. For the soil samples from Assin Bereku potassium recorded the highest concentration and molybdenum recorded the least concentration in the soil.
Stress among Nursing and Midwifery Students Offering a Top-Up Module in the University of Health and Allied Health Sciences, Ho  [PDF]

Previous studies have posited that students studying in medical-related fields are particularly susceptible to high levels of academic stress with implication for both psychological and physical health. This study set in the environment of a newly established public university in Ghana, seeks to evaluate the levels and component stress among students offering nursing and midwifery in the top-up module in the University of Health and Allied Science, Ho. In descriptive study using a modified version of the pre-validated College Undergraduate Stress Scale, 315 second and third year Midwifery 82 (26.03%), Nursing 155 (49.21%) and Public Health Nursing 78 (24.76%) students obtaining bachelors degree in top-up module were surveyed for this study. Among the total study population, 109 (34.60%) experienced high stress level, 106 (33.65%) experienced moderate stress level and 100 (31.75%) experienced less stress level. Stress levels were higher among the female participants. Participants with children under 13 years and female participants with breastfeeding babies showed higher levels of stress. A majority of students in the sandwich top-up bachelor degree programme experience moderate to high stress mostly due to curriculum overload. Therefore, innovative means should be sought to lessen the level of stress among this category of students. For example, pre-recorded video lectures could be incorporated into the module to lessen the overload in school work.
Inhibition of vascular response in inflammation by crude aqueous extract of the root bark of zanthoxylum xanthoxyloides

Abstract: Background: The root bark extract of Zanthoxylum xanthozyloides is used in folklore medicine in Ghana and Nigeria to treat inflammation. A previous pharmacological study confirmed the anti-inflammatory activity of the extract. Objective: To study the effect of the extract on vascular response in inflammation. Method: The extract was obtained by Soxhlet extraction and rotatory evaporation, followed by freezedrying. Groups of rats (with carrageenin-induced paw inflammation) and mice (with xylene-induced pinna inflammation) were, respectively, assigned randomly to treatment groups. The animals were given three different treatments orally: 0.9% saline (control), the extract (400mg/kg and 800mg/kg for mice; 1000mg/kg, 2000 mg/kg, and 4000mg/kg for rats), and indomethacin (5mg/kg and 10mg/kg for mice; 10mg/kg, 20mg/kg, and 40mg/kg for rats). In another set of experiment, each treatment group received phenylephrine subcutaneously (30μg/kg for rats and 20μg/kg for mice) in addition to the specified treatment aforementioned. Inboth sets of experiments, each group of rats was rotated through the entire treatment groups such that each animal served as control as well as received all the treatments. Analysis of variance was used as the statistical test. Results: The extract and indomethacin both caused dose-dependent reduction in the carrageenin-induced increase in paw volume in rats and also reduced xylene- induced increase in blood flow in mice pinna arteries. Phenylephrine enhanced the decrease in capillary permeability and vasodilatation caused by low dose extract but not that caused by high dose extract or both low and high dose indomethacin. Conclusion: The extract reduced vasodilatation and decreased capillary permeability in inflammation.
