Delayed presentation of blunt traumatic diaphragmatic hernia: A case report

Abstract: Blunt traumatic diaphragmatic rupture is an uncommon but severe problem that is usually seen in poly-traumatized patients. Diagnosis is often difficult resulting in delayed presentation and increased morbidity. We report a case of blunt traumatic diaphragmatic hernia in a 39-year-old man presenting 10 years after the initial abdomino-thoracic injury sustained in a road traffic accident. He had herniation of the spleen and stomach. Through a left thoracotomy, the herniated organs were reduced and diaphragmatic defect closed with interrupted nylon sutures. A high index of suspicion would minimize the morbidity and mortality associated with delayed diagnosis.
Ultrasound Biometry of Nigerian Fetuses: 2. Femur Length

Abstract: Femur length studies are few in Nigeria and the population used for the studies have been too small to provide a meaningful statistically significant data for the relationship between it and gestational age. Femur length is a very useful parameter for gestational age assessment in the third trimester and therefore requires to be properly studied. Charts of fetal size are widely used in the follow-up of pregnant women, yet no charts have been constructed for the Nigerian population. This study was designed to construct a size chart for femur length in Nigerian fetuses using a large sample size which is evenly distributed from 12 to 42 weeks of pregnancy. A total of 13,740 pregnant women were scanned in a cross-sectional study at the Centre for Reproductive Health Research, Jos over a period of five years. The mean fetal femur length measurements of 13,740 fetuses from 12 to 42 weeks are presented here in a tabulated form together with the regression equation. Mathematical modeling of data demonstrated that the best-fitted regression model to describe the relationship between femur length and gestational age was the second order polynomial regression equation y = – 0.017x2 + 3.2794x – 25.282 with a correlation of determination R2 = 0.999 (p < 0.001) where y is the femur length in millimeters and x is the gestational age in weeks. The mean weekly increase in the femur length in the 4th month of life was 2.6 mm/week, in the 6th month; it was 2.48 mm/week and 2.1 mm/week in the 9th week. A comparison of our chart with others showed significant difference. This chart can be used to provide assessment of gestational age in this country without reliance on Caucasian data as we had done in the past.

Abstract: Fetal weight studies are few in Nigeria and the population used for these studies are too small to provide a meaningful statistical significant data for the relationship between it and age. In the developed countries, fetal weight reference values have been produced but there are no such in our environment. This study was designed to establish chart of fetal weight in Jos. A total of 12,080 pregnant women were scanned in a cross-sectional study at the Centre for Reproductive Health Research, Jos over a period of five years. The mean estimated weights and percentiles of 12,080 fetuses from 17-42 weeks are presented in a tabular form. Mathematical modeling of data demonstrated that the best-fitted regression model to describe the relationship between estimated fetal weight and gestational age was the power regression equation y = 0.038x3.1347 where y is the fetal weight in grams and x is the fetal age in weeks with a correlation of determination R2 = 0.9951 (p< 0.001). When fetal weight was plotted against symphysio-fundal height, it was found out that there is a positive correlation between fetal weight and symphysio-fundal height with a correlation of determination R2 = 0.9951 (p< 0.001). The relationship is best described by the power regression equation y = 0.0409x3.1217 where y is the fetal weight in grams and x is the symphysio-fundal height in centimeters. It is concluded that the estimated weight of fetuses in Jos correlated well with gestational age and symphysio-fundal height.
Familial polycystic kidney disease in Nigeria: A report of two cases

Abstract: A case of familial polycystic kidney disease is reported. Although isolated cases of adult polycystic kidney disease have been reported in our environment, no case to our knowledge has been reported with a familial link. Polycystic kidney disease is said to be rare in Africans. Although it commonly terminates in chronic renal failure, it hardly features in the aetiopathogenesis of end stage renal disease requiring some form of renal replacement therapy in African series. This, some workers believe may be due to misdiagnosis and under reporting. This report is to show that it may not be as rare as suspected, and that the familial link shown in the advanced countries is also applicable here. Case 1 was diagnosed in the course of evaluation of her clinical disease. Case 2, an aunt of Case 1, was diagnosed following investigation of a casual complaint of a painless abdominal mass in the wake of her senior brother's death from haemorrhagic stroke. II s'agit d'un cas de la maladie polykystique des reins bien connu. Quoique des cas isolés des adultes atteints de la maladie polykystique des reins ont été rapportés dans notre milieu, aucun cas, autant que nous sachons, n'a été rapporté avec un lien familier. On dirait que la maladie polyskystique est rare chez des Africains. Quoiqu' elle, le plus souvent, provoque l'insuffisance renale chronique, c' est à peine si elle figure parmi l'aétiopathogène de la maladie des reins étape finale qui demande une forme de la thérapie de remplacement rénal dans les séries africaines. D'après quelques chercheurs, ceci pourrait être provoqué par erreur diagnostic' et manque des informations. L'objet de ce rapport est de montrer qu'elle n'est pas aussi rare comme on doit y attendre, et que ce lien familier montré dans les pays industrialisés est également applicable. Le 1re cas était diagnostiqué au cours d'évaluation de sa maladie clinique. Le deuzième cas, une tante du premier cas, était diagnostiqué à la suite d'une enquète effectuée sur une pleinte insouciante d'abdominal de masse sans douleur à la suite de la mort de son frère ainé provoquée par une attaque hémorrhagique
Paediatric Echocardiography in Jos University Teaching Hospital: Problems, Prospects and Preliminary Audit

Abstract: Background and Objectives: The pattern of paediatric heart diseases at the Jos University Teaching Hospital has not been described previously. The objective of this study was to document the pattern of heart diseases in children undergoing echocardiography at Jos University Teaching Hospital during an eight-month period and to identify the problems encountered. Patients and Methods: The indications and findings in 48 children undergoing routine trans-thoracic echocardiography from December 1999 to July 2000 were retrospectively analyzed. The problems encountered were also noted. Results: The main indications for echocardiography were rheumatic heart disease (RHD) in 15 (31.5 percent), acyanotic congenital heart disease (CHD) in 10 (20.8 percent) and cyanotic CHD in six (12.5 percent) children. Others were evaluation of cardiac murmurs in six (12.5 percent), unexplained heart failure in three (6.3 percent), pericardial effusion in two (4.2 percent) and cardiac arrhythmia in two (4.2 percent). Significant abnormalities were found in 38 (79.2 percent) of the patients; these were congenital heart diseases in 16 (33.3 percent) and acquired heart diseases in 22 (45.8 percent). Ventricular septal defect (VSD) was an isolated lesion in six (37.5 percent) but was associated with other congenital lesions in nine others. Fallot tetralogy was the commonest cyanotic heart condition encountered. Doubling of cost during the second half of the period of review reduced the access of our patients to the investigation by one third. Excessive strain on the machine from overuse, multiple users and power fluctuations were identified as some of the factors that led to the breakdown of the equipment after only eight months. Conclusions: The pattern of paediatric heart disease in the hospital is similar to those reported from other parts of the country. The cost of paediatric echocardiography should be kept to a minimum, while the lifespan of the machine could be prolonged by restricting its use to echocardiography and vascular procedures and by the provision of stable power supply. Cluster Model of Formation of Subnuclear and Subatomic Objects  [PDF]

The paper describes the development results on one-dimensional (1D) asymptotic model of the formation kinetics for the objects (clusters) of subnuclear (quark) and subatomic (nuclear) matters. A concept of the objects distribution density wave φ(a, t) in space of sizes a lies in the basis for analytical description of the processes under consideration. The proposed formalism makes it possible to describe in an adequate way the final outcomes of the well-known catastrophic phenomena in the world of elementary particles. Mass characteristics of different processes of approach to equilibrium in nuclear reactions are calculated.

Abstract: As an application of the grand unified theory (GUT) to medicine, this paper introduces two natural nanomaterials—the gene and electromagnetic wave. They are involved in the proposed treatment of genetic and infectious diseases. It introduces the superstring as the fundamental building block of matter and the basic constituent of dark matter, one of the two fundamental states of matter. It also introduces the primum, unit of visible or ordinary matter, the other fundamental state of matter. They are convertible to each other by the action of basic cosmic or electromagnetic waves. Eleven biological laws define biology as a biological and physical theory. The gene is the primitive ingredient of living things and, therefore, the beginning of biological life and the virus the simplest living thing. Treatment ranges from genetic modification and alteration to remove undesirable symptom through genetic sterilization to stop the spread of the disease and outright destruction of the disease. Genetic sterilization and outright destruction apply also to infectious diseases. The paper concludes with a blueprint for research and development of appropriate technologies for the treatment.

Abstract: The paper reviews the most consequential defects and rectification of traditional mathematics and its foundations. While this work is only the tip of the iceberg, so to speak, it gives us a totally different picture of mathematics from what we have known for a long time. This journey started with two teasers posted in SciMath in 1997: 1) The equation 1 = 0.99… does not make sense. 2) The concept \"\"?does not exist. The first statement sparked a debate that raged over a decade. Both statements generated a series of publications that continues to grow to this day. Among the new findings are: 3) There does not exist nondenumerable set. 4) There does not exist non-measurable set. 5) Cantor’s diagonal method is flawed. 6) The real numbers are discrete and countable. 7) Formal logic does not apply to mathematics. The unfinished debate between logicism, intuitionism-constructivism and formalism is resolved. The resolution is the constructivist foundations of mathematics with a summary of all the rectification undertaken in 2015, 2016 and in this paper. The extensions of the constructivist real number system include the complex vector plane and transcendental functions. Two important results in the 2015 are noted: The solution and resolution of Hilbert’s 23 problems that includes the resolution of Fermat’s last theorem and proof Goldbach’s conjecture.

Проанал зовано вплив дом шок он в зал за кобальту на х м чний склад отриманих зразк в Manganese dioxide samples have been prepared by the anode deposition from fluoride containing electrolytes based on manganese sulphate at the presence of additives of iron(II), cobalt(II) ions. Obtained materials were characterised with AAS and DTA. The influence of doping by ions of iron(II) and cobalt(II) on the chemical composition of the samples obtained has been analysed. Образцы диоксида марганца получены методом анодного осаждения из фторсодержащих электролитов на основе сульфата марганца в присутствии примесей ионов двухвалентных железа или кобальта. Полученные продукты характеризовались методами ААС и ДТА. Проанализировано влияние примесей ионов железа и кобальта на химический состав полученных образцов.
Numerical Investigations of a New N-body Simulation Method  [PDF]

Abstract: Numerical investigation of a new similarity method (the Aldar-Kose method) for N-body simulations is described. Using this method we have carried out numerical simulations for two tasks: 1) calculation of the temporal behavior of different physical parameters of active galactic nuclei (AGN) containing a super massive black hole (SMBH), an accretion disk, and a compact stellar cluster; 2) calculation of the stellar capture rate to the central SMBH without accretion disk. The calculations show good perspectives for applications of the similarity method to optimize the evolution model calculations of large stellar systems and of AGN.
