A French multicenter randomised trial comparing two dose-regimens of prothrombin complex concentrates in urgent anticoagulation reversal

Abstract: We performed a phase III, prospective, randomised, open-label study including patients with objectively diagnosed VKA-associated intracranial haemorrhage between November 2008 and April 2011 in 22 centres in France. Patients were randomised to receive 25 or 40 IU/kg of 4-factor PCC. The primary endpoint was the International Normalised Ratio (INR) 10 minutes after the end of 4-factor PCC infusion. Secondary endpoints were changes in coagulation factors, global clinical outcomes and incidence of adverse events (AEs).A total of 59 patients were randomised: 29 in the 25 IU/kg and 30 in the 40 IU/kg group. Baseline demographics and clinical characteristics were comparable between the groups. The mean INR was significantly reduced to 1.2 - and [lessthan or equal to] 1.5 in all patients of both groups - 10 min after 4-factor PCC infusion. The INR in the 40 IU/kg group was significantly lower than in the 25 IU/kg group 10 min (p=0.001), 1 hour (p=0.001) and 3 hours (p=0.02) after infusion. The 40 IU/kg dose was also effective in replacing coagulation factors such as PT (p=0.038), FII (p=0.001), FX (p<0.001), protein C (p=0.002) and protein S (0.043), 10 min after infusion. However, no differences were found in hematoma volume or global clinical outcomes between the groups. Incidence of death and thrombotic events was similar between the groups.Rapid infusion of both doses of 4-factor PCC achieved an INR of 1.5 or less in all patients with a lower INR observed in the 40 IU/kg group. No safety concerns were raised by the 40 IU/kg dose. Further trials are needed to evaluate the impact of the high dose of 4-factor PCC on functional outcomes and mortality. From America to Africa: A Parallel in the Colonial and Federal Experiences  [PDF]

Abstract: The present article aims at exhibiting similarities within the colonial experiences and the federal experiences of America and Africa, despite the disparities with the two processes, then pointing out the stepping of the African countries despite their moving from the Organization of the African Unity (OAU) to the African Union (AU). In effect, this change could just be seen as an awkward imitation of the American successful federal process, from the Articles of Confederation to the United States of America, since America had been senior in that process. From then, further research might decide to project what Africans could use as a frame of reference in their American equivalent concerning the search for an integrated society, and an accepted democracy among the separate and equal nations.
Characteristics of Patients with Upstaging by Sentinel Lymph Node Biopsy of the N0 Neck in Head and Neck Cancer  [PDF]

Abstract: Introduction: To investigate the possible role of sentinel lymph node biopsy (SLNB) to upstage the N0 neck in patients with oral and oropharyngeal squamous cell carcinoma. Methods: Patients with T1-T2 oral and oropharyngeal squamous cell carcinoma accessible to injection and staged N0 into the neck by palaption and CTscan were enrolled in the study. All patients underwent regular follow-up to identify possible recurrence. Results: A sentine lymph node biopsy was performed by 21 consecutive patients. 4 of the 21 patients were upstaged by SNLB. There was a mean follow-up of 31 months. Two patients developed subsequent disease after having been staging by SLNB, respectively negative in one case and positive in the other case. Tumor site, the staging of the primary tumor, presence of ulceration, tumor thickness were the same in the upstaged initially N0 patients. Conclusions: Sentinel lymph node biopsy can be used to upstage the N0 neck patients in perhaps well defined patients.

Abstract: When hypovolemia is strongly suspected, prediction of the fluid challenge response remains difficult for the intensivist or the anesthesiologist, because of complex interactions. In addition to the clinical art, each physician wants to use numerical parameters to make the decision to give fluid and to ensure an adequate response to the fluid given. Apart from the debate on the type of fluid, the best way to evaluate the fluid challenge response has motivated a lot of clinical research aiming to demonstrate the specificity and sensitivity of several parameters, considering the invasiveness, the accuracy, and the cost of the methods.In the past decade, many devices and parameters have been proposed to dynamically evaluate the response to fluid challenge. If fluid has to be given, the goal remains multifactorial, oscillating between hypotension correction, improvement in cardiac function, increase in cardiac output and oxygen delivery, or vascular recruitment. The effectiveness of this fluid challenge has been assessed by different methods: (1) echocardiography showing a better function of the right and left ventricles or better filled inferior or superior vena cava; (2) changes in oxygen delivery, which is simplified when arterial oxygen saturation is normal in changes in cardiac output, whatever the techniques for cardiac output measurements used; (3) changes in pulse pressure amplitude based on determinants of systolic and diastolic arterial blood pressure; or (4) cardiac filling pressure variations in association with cardiac output, according to the Frank-Starling law, but with invasive methods.The study from Trof and colleagues examines the interest in measuring cardiac filling pressures or the diastolic cardiac volume index (global end-diastolic volume index) in cardiac or vascular surgery in the post-operative period, in the presence or absence of systolic left ventricular dysfunction [1]. Dynamics of Change in Human-Driven and Natural Systems: Fast Forward, Slow Motion, Same Movie? A Case Study from Plant Protection

Abstract: Evolutionary biology and evolutionary ecology deal with change in species and ecosystems over time, and propose mechanisms to explain and predict these. In particular, they look for generic elements that will drive any organism or phylum to adaptive changes or to extinction. This paper, using examples from the field of plant protection against pests and diseases, shows that the patterns of change observed in natural and in human-driven systems are comparable, and proposes that their similarities result from the same mechanisms operating at different paces. Human-driven systems can thus be seen simply as ‘fast-forward’ versions of natural systems, making them tractable tools to test and predict elements from evolutionary theory. Conversely, the convergence between natural and human-driven systems opens opportunities for a more widespread use of evolutionary theory when analyzing and optimizing any human-driven system, or predicting its adaptability to changing conditions.

Abstract: during the recent years, social sciences have developed interest in the body, considering it as a social reality, a result of a historical construction and a product of cultural representations, but the political uses of the body have been relatively neglected. on the basis of two case-studies recently conducted in france, one on the distribution of subsidies to job-seekers, the other one on the legalization of undocumented immigrants for humanitarian reasons, the present paper analyzes the way in which the body becomes a resource for the claim of a right, justified by disease or suffering. these examples give clues for the comprehension of the moral economy of illegitimacy and the foundations for a political anthropology of the body.

Abstract: the 2005 autumn riots in france may be considered a real event: it has given shape to issues not seen before as such yielding a time boundary which changed the way society regards itself. the suggested interpretation has two levels. on the one hand, the circumstances that have triggered off the events are related, as succeeds in any riots of the last 25 years, to police violence and the resulting unsettling in the precarious moral economy of the popular classes. on the other hand, the underlying phenomena are related to a particular form of inequality not acknowledge by french society: racial discriminations.
