Abstract: the first description of severe pain in the distribution of the glossopharyngeal nerve is credited to weisenberg, in 19101, in a patient with cerebellopontine angle tumor. however, it was harris, in 19262, who coined the term glossopharyngeal neuralgia to describe this rare condition characterized by paroxysms of excruciating pain located laterally at the back of the tongue, soft palate, throat, and lateral and posterior pharynx, radiating to the ear. swallowing, coughing, yawning or chewing may trigger pain, which usually lasts from seconds to minutes. the association between glossopharyngeal neuralgia and syncope is very rare, being identified by brief episodes of bradycardia, asystole, and hypotension. such an association, with this same pathophysiology, was first described by riley et al3 in 1942.
Evolu??o clínica de pacientes com síncope neurocardiogênica (SNC) após suspens?o da terapia específica

Abstract: objective: to evaluate the outcome of patients with ncs after interruption of pharmacological therapy and to investigate the possible clinical variables predicting recurrence. methods: thirty-seven patients (age 31±16 years) with refractory recurrent ncs being 19 females where prospectively studied. all patients became asymptomatic and had a negative tilt table test (tt) after pharmacological therapy. the treatment was interrupted and one month later, a new tt with no medication was carried out. the probability free of symptoms recurrence was analyzed according to sex, age, number of syncope episodes previously to the treatment, clinical history time, treatment time, drug free from treatment time and tt result. results: twenty-two patients (59%) presented recurrence during a mean follow-up of 21±19.7 months. the variables related to greater recurrence were number of previous syncope (p=0.0248), positive tt after interruption of the therapy (p=0.0002) and female gender (p= 0.0131). conclusions: most of the very symptomatic patients with ncs present recurrence after the suppression of a specific therapy. a tt carried out after treatment discontinuation can identify patients with higher risk of recurrence, specially in the first year of follow-up.
Síncope em pacientes com extra-sístoles de via de saída de ventrículo direito e sem cardiopatia estrutural aparente

Abstract: objective: study the prevalence of neurocardiogenic etiology in patients with unexplained syncope and ventricular premature beats, with right ventricle outflow tract morphology (rvot) and no apparent structural cardiopathy. methods: ninety patients (66 women, mean age 40.2±16.95 years of age) with monomorphic premature beats originated at rvot were evaluated prospectively. fifty-four patients reported syncopes or near-syncopes associated to palpitations or not; twenty-seven presented palpitations with no pre-syncope or syncope, and nine were asymptomatic. all patients were submitted to echocardiogram, high resolution ecg and cardiac mri to rule out structural cardiopathy, to exertion test to rule out adrenergic dependent ventricular tachycardia, and ecg prolonged outpatient monitoring (holter and symptomatic events monitor) to correlate symptoms and ventricular arrhythmias. investigation on the susceptibility to neurocardiogenic syncope was evaluated by tilt table test (ttt). groups were compared regarding gender, age, premature beats frequency and complexity, at exertion or not, ttt results and clinical course. results: in the syncope and pre-syncope groups, ttt was positive for 38% of cases, and in groups with palpitations and assymptomatics, it was positive for 11% (p = 0.0257). after recommendations and treatment of neurocardiogenic syncope, 85% of syncope and pre-syncope patients and positive ttt was asymptomatic along the 40-month follow-up. two patients with syncope and negative ttt presented spontaneous, sustained ventricular tachycardia during clinic course. conclusion: the prevalence of neurocardiogenic syncope in patients with idiopathic rvot premature beats is high. patients with recurrent, unexplained syncope and idiopathic ve must be kept under investigation.
Efeito de anestésicos locais com e sem vasoconstritor em pacientes com arritmias ventriculares

Abstract: background: the routine use of local anesthetics associated to vasoconstrictors for the dental treatment of patients with cardiopathies is still controversial, due to the risk of adverse cardiovascular effects. objective: to evaluate and compare the hemodynamic effects of the use of local anesthetics with a non-adrenergic vasoconstrictor in patients with ventricular arrhythmia, when compared to the use of anesthetics without vasoconstrictor. methods: a prospective randomized study evaluated 33 patients with positive serology for chagas' disease and 32 patients with coronary artery disease that presented complex ventricular arrhythmia at holter monitoring (>10 ev/h and nsvt), of which 21 were females, aged 54.73 + 7.94 years, submitted to routine dental treatment with pterygomandibular anesthesia. these patients were divided in two groups: group i received prilocaine 3% associated with felypressin 0.03 iu/ml and group ii received lidocaine 2% without vasoconstrictor. the number and complexity of extrasystoles were analyzed, as well as the heart rate and systemic arterial pressure of the patients on the day before, one hour before, during the procedure and one hour after the dental procedure. results: no hemodynamic alterations or increase in the number and complexity of the ventricular arrhythmia related to the anesthetic used in the dental procedure were observed in either group. conclusion: the results suggest that prilocaine 3% associated to a felypressin 0.03 iu/ml can be safely used in patients with chagas' disease or coronariopathy with complex ventricular arrhythmia.
Integrative review of the clean and sterile technique: agreement and disagreement in the execution of dressing

Abstract: objective: this study aimed to describe, analyze, and categorize the evidence associated to the use of the clean and sterile technique in wounds. methods: it is an integrative literature review using lilacs, medline and cinahl databases. results: seven publications were found,, of which four compare the clean and sterile technique, regarding the occurrence of infection; two evaluate the microbiological safety of gloves, and one analyzed the cleansing solution. conclusion: there is consensus that the clean technique reduces costs. considering the scarcity of studies, we stress the need to perform more level i and ii research, according to the evidence hierarchy.
Micobactérias n?o tuberculosas em cirurgias: desafio passível de enfrentamento no Brasil?

Abstract: infections caused by nontuberculous mycobacteria (mnt) represent an epidemiological and health emergency, especially in patients undergoing invasive procedures. based on these, we aimed to analyze the scientific evidence, the scientific literature, on the occurrence in brazil of mnt infections in surgical patients. we used as a research method integrative review of the literature using the databases lilacs, medline/pubmed, isi web of science and the cochrane library. we selected 15 publications on this theme from the last 30 years that were directed at methods of prevention and control, with a focus on post-discharge surveillance, the use of antibiotics and glutaraldehyde. eye surgery, cosmetic, heart, laparoscopic and arthroscopic procedures were the most commonly investigated. the national situation of mnts is concerning, especially when one recognizes the possibility of underreporting.

Abstract: ventilator-associated pneumonia (vap) is a common infection in intensive care units (icus), and oral antiseptic is used as a preventive measure. we reviewed meta-analyses and randomized clinical trials indexed in the medical literature analysis and retrieval system and cumulative index to nursing and allied health literature databases regarding the topical use of chlorhexidine in the prevention of vap. eight publications were analyzed. in seven (87.5%) chlorhexidine diminished the colonization of the oropharynx, and in four (50%) there was a reduction of vap. chlorhexidine seems to reduce colonization, thus reducing the incidence of vap.
La familia y los factores de riesgo relacionados con el consumo de alcohol y tabaco en los ni?os y adolescentes (Guayaquil-Ecuador)

Abstract: the present investigation had as objective identifying in a family the possible factors of risk related to the use of alcohol and tobacco in the children and adolescents. it is important to emphasize that study of this nature within a social and culture perspective expresses the attempt to include/understand the factors of risk for the use of tobacco and to drink alcoholic the environmental influences in the familiar surroundings views to prevent futures cases with dependency. for the study used a sample of one hundred families, to that applied to an instrument pre to them established with the people in charge of the respective families. as result were obtained 51% of the schooling level are low, 54% has inferior wage to the basic one, 61% to drink alcoholic. to emphasize that unquestionable the reduction of the casuistry of alcoholism and/or tabaquismo to influence significantly in the quality of the individuals life.

Abstract: O ensino de ciências n o deve mais ser visto como transmiss o de conceitos, mas sim como constru o de conhecimentos para que o processo ensino-aprendizagem tenha sentido e contextualidade. Este artigo tem por objetivo mostrar como aplicar conceitos da disciplina de Física de maneira contextualizada empregando conceitos de educa o ambiental. Para isso, trabalhou-se por quatro semanas em seis turmas do primeiro ano do ensino médio de uma escola pública da cidade de Resende-RJ. O tema abodado foi “as formas de transmiss o de calor e o meio ambiente”, com ênfase no efeito estufa e no aquecimento global. Como resultados, verificou-se que os alunos tiveram bom desenpenho em rela o aos conceitos desenvolvidos. Chegou-se ao consenso geral de que algo deve ser feito por cada um de nós para cuidar do nosso ambiente e melhorar as condi es de vida da sociedade de forma geral.
