Abstract: the atypical and more severe form of mayer-rokitansky-kuster-hauser syndrome (mrkh) or mrkh type ii is also known as murcs association, an acronym meaning aplasia/hypoplasia of müllerian ducts (mu), congenital renal dysplasia (r) and cervico-thoracic dysplasia (cs). it affects female patients with normal karyotype and ovarian function, evolving to primary amenorrhea. it has an incidence of 1:50,000, but it is underestimated due to late diagnosis and undefined etiology. we describe the cases of a child and an adolescent in order to predict the diagnosis even in childhood, before the onset of amenorrhea. patients had in common renal malformation, agenesis or hypoplasia of müllerian derivatives and vertebral anomalies, establishing the diagnosis of murcs. the relevance of this paper is to show the importance of further investigation when some of pathologic signs are present, researching correlated abnormalities in order to establish an early diagnosis and consequently to provide guidance to the patients and their families about the best way to conduct the case, including genetic counseling.

Objective: To evaluate the profile of the Brazilianvaginal ring user, reasons for choosing it, satisfaction, menstrual pattern, tolerability, efficacy, side effects and reasons for discontinuation. Methods: We reviewed the files of 148 users who attended the Hospital de Jacarepaguáand private clinics, between 2005 and 2011, in a total of 3107 cycles. Kaplan-Meier survival curves and regression models were used in statistical analysis. Results: The profile was: single, nulligravida, 30 years (mean) and with university education. The previous contraception used was mostly the oral hormonal contraception (75.7%). Women chose the ring for: practicality (32.4%), forgetting the pill (32.4%) and irregular vaginal bleeding (13.5%). The satisfaction degree was 80.5% (119). There were good cycle control and no pregnancies. Thirty-eight patients discontinued the use (25.7%). The main reasons were wish for pregnancy (6%), spotting (3.4%) and increased vaginal discharge (2.7%). At the end of the first year, use of the greatest reduction of continuity probability was observed (19.3%). Women with side effects were 68% less likely to be satisfied with the method than those without (p-value <0.0001). Conclusion: Nuvaring® is an excellent contraceptiveoption due to its sefficacy, cycle control, tolerability, practicality and safety.

17DD and 17D-213/77 Yellow Fever Substrains Trigger a Balanced Cytokine Profile in Primary Vaccinated Children

Abstract: Background This study aimed to compare the cytokine-mediated immune response in children submitted to primary vaccination with the YF-17D-213/77 or YF-17DD yellow fever (YF) substrains. Methods A non-probabilistic sample of eighty healthy primary vaccinated (PV) children was selected on the basis of their previously known humoral immune response to the YF vaccines. The selected children were categorized according to their YF-neutralizing antibody titers (PRNT) and referred to as seroconverters (PV-PRNT+) or nonseroconverters (PV-PRNT?). Following revaccination with the YF-17DD, the PV-PRNT? children (YF-17D-213/77 and YF-17DD groups) seroconverted and were referred as RV-PRNT+. The cytokine-mediated immune response was investigated after short-term in vitro cultures of whole blood samples. The results are expressed as frequency of high cytokine producers, taking the global median of the cytokine index (YF-Ag/control) as the cut-off. Results The YF-17D-213/77 and the YF-17DD substrains triggered a balanced overall inflammatory/regulatory cytokine pattern in PV-PRNT+, with a slight predominance of IL-12 in YF-17DD vaccinees and a modest prevalence of IL-10 in YF-17D-213/77. Prominent frequency of neutrophil-derived TNF-α and neutrophils and monocyte-producing IL-12 were the major features of PV-PRNT+ in the YF-17DD, whereas relevant inflammatory response, mediated by IL-12+CD8+ T cells, was the hallmark of the YF-17D-213/77 vaccinees. Both substrains were able to elicit particular but relevant inflammatory events, regardless of the anti-YF PRNT antibody levels. PV-PRNT? children belonging to the YF-17DD arm presented gaps in the inflammatory cytokine signature, especially in terms of the innate immunity, whereas in the YF-17D-213/77 arm the most relevant gap was the deficiency of IL-12-producing CD8+T cells. Revaccination with YF-17DD prompted a balanced cytokine profile in YF-17DD nonresponders and a robust inflammatory profile in YF-17D-213/77 nonresponders. Conclusion Our findings demonstrated that, just like the YF-17DD reference vaccine, the YF-17D-213/77 seed lot induced a mixed pattern of inflammatory and regulatory cytokines, supporting its universal use for immunization.
Incidence of cervical intraepithelial lesions in a population of adolescents treated in public health services in Rio de Janeiro, Brazil

Abstract: this study aimed to estimate the incidence and types of cervical cytopathological findings in adolescents who were treated in public health services between 1993 and 2006. this is a cohort study, with the following inclusion criteria: < 20 years of age, sexually-active, without cervical lesions upon entry into the study or sexually active < 1 year. the data were collected from 403 adolescents' medical records. incidence density of cytopathological alterations was estimated and the actuarial method was used to calculate the 5-year incidence during follow-up after sexual initiation. in the first year of sexual activity, the incidence of cervical lesions was 24.1%. the incidence decreased to 3-8% over the following 4 years. the incidence density was 4.7 cases per 100 persons per year. the first abnormal cytological diagnosis showed atypical squamous cells of undetermined significance (ascus) in 5.5% (22) of patients, low-grade squamous intra-epithelial lesions (lsil) in 28% (113), and high-grade squamous intraepithelial lesions (hsil) in 3% (12). eight (67%) cases of hsil occurred during the first year of sexual activity. the incidence of cytopathological findings at beginning of sexual life is high, suggesting the importance of including sexually-active adolescents in cervical cancer prevention programs.
Pre-invasive cervical disease and uterine cervical cancer in Brazilian adolescents: prevalence and related factors

Abstract: the objective was to describe the prevalence and factors associated with uterine cervical cancer (ca) and high-grade squamous intraepithelial lesions (hsil) in adolescents. a cross-sectional study was carried out with 702 sexually active adolescents treated at a general hospital in rio de janeiro, brazil, from 1993 to 2002. screening was performed by cytopathology and colposcopy and confirmation by biopsy. exposure variables were socio-demographic characteristics and those related to reproductive health, habits, and sexual behavior. adjusted odds ratios were estimated using multivariate logistic regression analysis. based on histopathology, the prevalence of hsil/ca was 3% (95%ci: 1.8-4.6). there was one case of invasive cancer. with each additional pregnancy, the odds of hsil/ca increased by 2.2 (95%ci: 1.1-4.4). age was also associated with this outcome, doubling the odds of acquiring this degree of disease with each year of age (or = 2.0; 95%ci: 1.2-3.4). the prevalence of lesions suggests the importance of including sexually active adolescent females in cervical cancer screening programs aimed at early detection and treatment of these lesions.
Selectivity of insecticides used in corn crops to adult Trichogrammaatopovirilia (Hymenoptera: Trichogrammatidae) Selectividad de insecticidas usados en maíz a los adultos Trichogrammaatopovirilia (Hymenoptera: Trichogrammatidae)

Abstract: The aim of this work was to evaluate the toxicity of the insecticides imidacloprid/β-cyfluthrin (Connect 100/12.5 SC), chlorfenapyr (Pirate 240 SC), chlorpyrifos (Astro 450 EW), novaluron (Rimon 100 CE), spinosad (Tracer 480 SC) and triflumuron (Certero 480 SC), as used in corn crops ( Zea mays ), to parent generation females and to F1 and F2 generation specimens of Trichogrammaatopovirilia. Eggs of Anagasta kuehniella were glued to cardstock strips and placed under a germicidal lamp to kill embryos. These were then sprayed with the chemical products using a Potter tower and exposed to parasitism 24, 48, and 96 hours after application of the compounds for a span of 24 hours. We evaluated the number of dead specimens and the number of eggs parasitized by parent generation females, as well as the percent emergence and parasitic capacity of the F1 and F2 generations. Chlorfenapyr, spinosad, chlorpyrifos, and imidacloprid/β-cyfluthrin were moderately harmful to adult T. atopovirilia, while novaluron was slightly harmful. Triflumuron was harmless and could be used in integrated pest management programs intended to preserve adult T. atopovirilia in corn crops. Se evaluó la toxicidad de los insecticidas imidacloprid/ β-ciflutrina (Connect 100/12,5 SC), clorfenapir (Pirate 240 SC), clorpirifos (Astro 450 EW), novalurom (Rimon 100 CE), spinosade (Tracer 480 SC) y triflumurom (Certero 480 SC), utilizados en el cultivo de maíz (Zea mays ), para adultos de la generación maternal de Trichogramma atopovirila y para especímenes de las generaciones F1 y F2. Huevos de Anagasta kuehniella fueron adheridos a cartulinas e irradiados con una lámpara antigermicida para matar a los embriones. Luego, se les aplicó mediante una torre de Potter los productos químicos y se expusieron durante 24 h a parasitismo a las 24, 48 y 96 h después de la aplicación de los compuestos. Se evaluó el número de especímenes muertos y el número de huevos parasitados por hembra de la generación maternal, también el porcentaje de emergencia y la capacidad de parasitismo de las generaciones F1 y F2. Clorfenapis, espinosade, clorpirifós e imidacloprid/β-ciflutrina fueron moderadamente perjudiciales para los adultos de T. atopovirila, mientras novalurom fue levemente perjudicial. Triflumurom fue inocuo y puede ser utilizado en programas de manejo integrado de plagas donde se busque preservar adultos de T. atopovirila en el cultivo de maíz.

Abstract: the head-up tilt test (hutt) is widely used for investigation of syncope and presyncope, since it allows diagnosing different types of dysautonomia. the main cause of syncope is the vasovagal syndrome, the most common diagnosis among patients with hutt indication. the test has been used for nearly 20 years, but many doctors are unaware of the methodology. after the cardiac causes of syncope are ruled out, the appropriate indication of the test and instructions to patients are important to ensure that the test will be carried out in a safe and relaxed manner. there are controversies in the literature over the diagnostic capacity and reliability of results. studies with various protocols may explain the variability of results. this review describes the guidelines-recommended methodology and indications, complications, limitations and perspectives of this test.

Abstract: Ultrasound is a diagnostic tool widely used in veterinary medicine in various specialties. In obstetrics, represents a fundamental tool for diagnosing and monitoring pregnancy. Uses physical principles of propagation and reflection of sound to generate images that enable visualization of internal organs in real time,allowing the completion of accurate diagnosis, it is possible to identify pregnancies only 20 days old in addition to enabling the assessment of fetal viability. The development of technologies such as three-dimensionaultrasonography expand the diagnostic possibilities in ultrasound pregnancy in bitches. This review discusses the technology and its use in veterinary obstetrics.A ultrassonografia é uma ferramenta diagnóstica amplamente utilizada em Medicina Veterinária nas diversas especialidades. Em obstetrícia, representa ferramenta fundamental para diagnóstico e acompanhamento gestacional. Utiliza princípios físicos de propaga o e reflex o do som para gerar imagens que permitem visibilizar órg os internos em tempo real, permitindo a conclus o de diagnósticos precisos, sendo possível identificar gesta es de apenas 20 dias além de possibilitar a avalia o da viabilidade fetal.
