Deriva??o gástrica em Y-de-Roux e a atividade inflamatória do tecido adiposo

Abstract: objective: to evaluate the oxidative stress and inflammatory markers in obese patients before and after roux-en-y vertical banded gastroplasty. methods: we studied 20 class iii obese patients undergoing roux-en-y vertical banded gastroplasty, seven men and 13 women, mean age 39 years, and 20 non-obese subjects, nine males and 11 females, mean age 38 years. we determined the plasma levels of malondialdehyde, stress an index, total antioxidant capacity, catalase, reduced and oxidized glutathione and inflammatory markers (c reactive protein and á-1 acid glycoprotein). in the obese group, these parameters were determined before and 2, 6 and 12 months after gastroplasty. results: preoperatively, the obese group showed elevated levels of inflammatory markers of oxidative stress (malondialdehyde levels and stress index) and lower levels of indicators of antioxidant defense compared to the control group. weight loss was accompanied by gradual reduction in the levels of malondialdehyde and stress index. we found an increased concentration of reduced glutathione and total antioxidant status and reduced levels of inflammatory markers. conclusion: weight loss improves the inflammatory state and oxidative stress levels.

Abstract: the benefits of physical activity to the health and well-being of the general population and the risks of inactivity are now well documented. however, evidence concerning physical activity levels has focused largely on adults. relatively little is known about the physical activity patterns of adolescents. the purpose of this study was to analyze levels of regular physical activity in representative samples of adolescents selected from a secondary school in the city of londrina, paraná, in the southern region of brazil. a total of 281 adolescents (157 girls and 124 boys) ages ranging from 15 to 18 years were included in the study. the levels of regular physical activity were determined by means of a self-report instrument which provided an estimate of energy expenditure and of the time spent in different activities. results showed that boys were consistently more active than girls. a percentage of 54% of the boys was considered active or moderately active based on their estimated daily average energy expenditure (kcal/kg/day), while approximately 65% of the girls were found to be inactive or very inactive. boys spent a significantly longer time in physical exercise and in sports than girls (3:20 vs 0:48 hours/week). levels of regular physical activity decreased significantly with age, and the decline was greater in girls. girls spent less time watching tv than boys (3.3 vs 4.0 hours/days). boys, but not girls, from lower socioeconomic families performed less physical activity than boys from higher socioeconomic families. most adolescents (girls 97%; boys 74%) did not practice sufficient physical exercise so as to achieve the health benefits suggested by current guidelines and recommendations for young people. in conclusion, the levels of physical inactivity reported constitute an important issue for social agencies and organizations involved in the promotion of health-enhancing physical activity.
Fatores de risco cardiovasculares em adolescentes: indicadores biológicos e comportamentais

Abstract: objective: to provide descriptive information and investigate to what extent behavioral risk factors for cardiovascular diseases are associated with biological risk factors in the young population. methods: the sample included 452 adolescents (246 girls and 206 boys) between 15 and 18 years of age selected in a high school in the city of londrina, paraná. risk factors of a behavioral nature were analyzed as regards insufficient physical activity, excessive intake of fat and cholesterol and smoking. as biological risk factor indicators we used overweight, high arterial pressure levels, and adverse concentration of serum lipids and lipoproteins. results: approximately 20% of the girls and 16% of the boys presented at least one biological risk factor for development of cardiovascular diseases. overweight was significantly associated with excessive intake of fats, while elevated arterial pressure was related to sedentary lifestyle and smoking. excessive intake of fat and cholesterol indicated increased risk of undesirable concentrations of serum lipids and lipoproteins. the risk associated with altered blood pressure and serum lipids and lipoprotein was increased two-fold in smokers when compared with nonsmokers. conclusion: the results reinforce the need for interventions targeted at the adoption of a healthy lifestyle, including the regular practice of physical exercise, appropriate dietary patterns, and abstention from smoking as of an early age.

Macroeconomic trends and reforms in Germany

Abstract: This paper traces the main macroeconomic developments in the German economy from national unification. Its performance is compared with that of the rest of the euro area and its largest economies. The study documents as Germany s modest growth in the later 1990s was due to the restrictive impact on domestic demand coming from the deep restructuring and modernization of the production system, followed by sweeping reforms after the turn of the century. Rapid productivity increases and prolonged wage moderation, especially in industry, fuelled a large and mounting current account surplus in Germany, that compares with the deficits registered in most European countries. The study retraces the recent debate on how to correct those imbalances, recalling the arguments for and against the thesis that the countries with a current payments surplus, above all Germany, must also play an active role in fostering the adjustment of the deficit countries. A possible synthesis is proposed, based on an analysis of the formation of national income and the use of resources according to the national accounts system. The implication is that Germany may contribute to the correction of imbalances within the euro area not so much by altering the wage formation mechanism as by creating incentives for domestic investment, hence fostering employment creation, in the service sectors that are currently lagging behind the extraordinary perfomance of a number of core activities in the industry.

Abstract: Business to Business (B2B) Internet commerce causes a significant contraction of transaction costs. According to the Coase paradigm, we would thus expect a deverticalization of the industry and broader scope for anonymous market mechanisms. In reality, such expectations are not fully borne out by the facts. When the industrial structure is concentrated the B2Bgenerally loses its independence, and is owned by the firms which most contribute to its development, e.g. the ones able to bring the liquidity to it. The B2B governance mechanism established by these firms gives hierarchical mechanisms a role which they do not usually play in extensive, anonymous markets.

Abstract: Aim: To determine whether bladder functions deteriorate with age. Methods: Data contained in electronic medical record (INFOMED?) were used in this institutional retrospective review. Analysis was done on the urodynamic studies in women over 18 years old conducted between May 2011 and November 2015. Patients with previous history of pelvic surgery or radiotherapy, neurological disease, vaginal prolapse greater than grade I, congenital urogenital malformations, urinary obstructive disease, diabetes, or the use of any medication that could interfere with bladder function were excluded from the analysis. The urodynamic parameters analyzed were the Maximum Cystometric Capacity (MCC), Voiding Volume (VV), Maximum Flow (Qmax), Bladder Compliance (BC), Detrusor Pressure at Maximum Flow (PdetQmax), Bladder Contractility Index (BCI), Bladder Voiding Efficiency (BVE) and Post-Void Residual Urine Volume (PVR). Patients were further stratified in five groups according to age (A—18 to 40; B—41 to 50; C—51 to 60; D—61 to 70; E—over 70 years old). Results: Out of 3103 urodynamic studies analyzed, 719 were eligible for the study. The average age of patients was 49.3 (+13.2) years old and in all evaluated parameters, statistically significant correlation between age and decline of bladder function was obtained (p < 0.05). Moreover, mathematical equations were presented to estimate the parameters values in relation to age. Conclusions: This study showed a decline in bladder storage function (reduction in MCC and BC) and in bladder emptying function (reduction in Qmax, PdetQmax, VV, BCI and BVE with an increase in PVR) with age.

Abstract: objective: the use of co2 to promote pneumoperitoneun in laparoscopic surgeries causes hipercarbia. expired air co2 partial pressure may estimate this change.the purpose of this study is to determine whether there is a correlation between arterial co2 partial pressure and expired air co2 partial pressure during laparoscopic surgeries. methods: twenty female pigs were assigned to two groups: one without pneumoperitoneun and another with a 12 mmhg intra abdominal pressure. endotracheal pressure, arterial co2 saturation, expired air co2 partial pressure, ph and arterial co2 partial pressure were determined. results: after performing pneumoperitoneun there was a marked increase in endotracheal pressure, expired air co2 partial pressure and arterial co2 partial pressure. conclusions: in spite of the increase in both co2 arterial partial pressure and expired air partial pressure, there was no linear correlation between them, so that it is not possible to estimate arterial co2 partial pressure by the expired air co2 partial pressure during laparoscopic surgeries.
Gastroduodenopancreatectomia ampliada para tumores n?o periampulares

Abstract: background: to evaluate the surgical morbimortality and the outcome of patients submitted to extended duodenopancreatectomy (dp) for nonperiampulary tumors. methods: a retrospective evaluation of 15 patients submitted to dp for nonperiampulary tumors was performed in the abdominal-pelvic surgery service of national cancer institute- brazil, from 1990 to 2005. results: the histological types of primary tumors included adenocarcinoma of colon (n=9), gastric adenocarcinoma (n=3), gastrointestinal stromal tumors (n=2) and renal carcinoma (n=1). the average hospital length of stay was 13.5 days (6 to 36), the average operation time was 360 minutes, the hemotransfusion average volume was 307 ml, the ressected lymph nodes average number was 19, where 3 patients had positive lymph nodes. the average size of the tumor was 7.8cm (2.5 to 24), the surgical morbidity was 53%, the operative mortality was 6.6% (one patient) and the overall survival average was 38 months. conclusion: the duodenopancreatectomy for nonperiampulary tumors is an procedure of exception and it must be considered only for selected patients.

Abstract: background: pancreaticoduodenectomy has been used as a safe and appropriate surgical option in selected patients with malignant and benign disorders of the pancreas and periampullary region. among the complications of the pancreatoduodenectomy, pancreatic anastomosis failure is the most important and sometimes fatal complication. according to recent reports, the incidence of leakage of pancreatic anastomosis after pd is high (between 5% and 25%). our objectives were to analize the morbidity and mortality rates related to pancreatic anastomosis technique (duct-to-mucosa vs. telescopic) and to compare the results. methods: from january 1987 to december 2002, 64 patients underwent pancreaticoduodenal resection at brazilian national cancer institute. data were recorded retrospectively on all patients. forty-two patients underwent pancreatojejunostomy duct-to-mucosa and the telescopic technique was performed in 22 patients. statistical analyses were performed using the fischer's test. results: the pancreatic fistula rate after pancreatojejunostomy duct-to-mucosa and telescopic technique was 12% and 36%, respectively (p = 0.02). the operative mortality rate after duct-to-mucosa technique was 12% and 36% after pancreatojejunostomy telescopic (p = 0.4). conclusions: the leakage rate was significantly lower in patients underwent to pancreatojejunostomy duct-to-mucosa, however an operative mortality rate was not significantly different between the two groups.
