Cirurgia cardíaca videoassistida: 6 anos de experiência

Abstract: introduction: minimally invasive and video-assisted cardiac surgery (vacs) has increased in popularity over the past 15 years. the small incisions have been associated with a good aesthetic effect and less surgical trauma, therefore less postoperative pain and rapid recovery. objectives: to present our series with vacs, after 6 years of use of the method. methods: 136 patients underwent vacs, after written consent, between september 2005 and october 2011, 50% for men and age of 47.8 ± 15, 4anos, divided into two groups: with cardiopulmonary (cec) (gccec=105 patients): mitral valve disease (47/105), aortic disease (39/105), congenital heart disease (19/105) and without extracorporeal circulation (cec) (gscec=31 patients): cardiac resynchronization (18/ 31), cardiac tumor (4/31) and minimally invasive coronary artery bypass grafting (6/31). gccec was held in right minithoracotomy (3 to 5 cm) and femoral access to perform cannulation. results: in gccec, mean length of icu stay and hospital stay were respectively 2.4 ± 4.5 days and 5.0 ± 6.8 days. twelve patients presented complications in post-operative and five (4.8%) death. ninety-three (88.6%) patients evolved uneventful, were extubated in operating room, and remained a mean of 1.8 ± 0.9 days in icu and 3.6±1.3 days in the hospital. in gscec, were mean 1.3 ± 0.7 days in icu and 2.9 ± 1.4 days in hospital and without complications or deaths. conclusion: the results found in this series are comparable to those of world literature and confirm the method as an option the conventional technique.
Hipoglicemia por nesidioblastose: uma complica??o rara da cirurgia bariátrica?

Abstract: background: bariatric surgery has shown to be the most effective method in diabetes mellitus (dm) type ii resolution, reaching up to 80% of cure. however, some studies have shown the development of a hyperinsulinemic hypoglycemia state in certain morbidly obese patients after bariatric surgery, so called nesidioblastosis corresponding to pathological disseminated proliferation of pancreatic beta-cells. the purpose of this study is to review bibliographical data on nesidioblastosis after bariatric surgery, researching scientific articles published since 2000 over the subject. methods: a literature review was made over scientific articles published since 2000. researching data based on pubmed (www.pubmed.gov), crossing the following headings: nesidioblastosis, hyperinsulinism, hypoglycemia and bariatric surgery complications. literature review: fourteen publications related to the subject were reviewed. among them, cummings defends alterations in the functional activity of the intestinal tract after bariatric surgery promoting stimulation and excessive growth of pancreatic beta-cells and greater activity of pancreatic hormones, suggesting a possible cause for nesidioblastosis. corroborating in order to elucidate the cause for this disease, authors suggest that the rapid intestinal transit after gastric bypass surgery elevates the serum level of glucagon-like peptide 1, thereby stimulating hyperinsulinemic hypoglycemia in these patients. similarly, other authors defend the idea that the weight lost after bariatric surgery decrease intensively insulin-resistance due to hypertrophy and hyperfunctional state of pancreatic beta-cells, which is very common in obesity, promoting the development of nesidioblastosis. conclusion: nesidioblastosis after bariatric surgery may represent the pathological extreme of a beneficial phenomenon for the great majority of morbidly obese patients presenting with diabetes mellitus type ii, however, in some rare situations, this benefic pheno

Abstract: The idea is to realize a great container where you can feel the friction between close but distant worlds, where the dramatic shrill sounds, that characterize the city, are perceptible but where, at the same time, great harmony and peace can be found. A world that sums up history, the great protagonist of Jerusalem, in an overlaying of strata of golden ages with dark and cruel ages; a continuous mixing of contradictory situations and contrasting events that create a huge hole, at the bottom of which there is a common, unitary world. The huge void of No Man’s Land is an underground square, the FORUM, a room that paradoxically could be the first link in a cultural, social and finally, urban connections: the space that gathers, keeping intact the identity of populations. The lotus blossom of Jerusalem. Subtracting room to contradictions and fights, we create a neutral space. The void is conceived as a neutral space, no?contact space, but where different cultures and traditions can meet, where it is possible to build up a dialog. The void itself facilitates the growth of peace. The city lives in the underground space a new dimension, with its contradictions, but with a contamination of cultures through several human activities (study center, library, restaurant, cinema, theatre, shopping mall, art galleries, offices…). Focusing the FORUM, but always with a glance at the upper concrete world, people is called quietly but firmly to work and live for peace and multiratial dialog. The gamble is to make the individual reflect, by the instruments of architecture, through evocations of the deep gorges of the desert of Judea and Negev, of the karstic phenomena of the slopes of the Dead Sea, so dramatic but in front of which the individual finds himself pervaded with an overwhelming sense of peace and unity in front of the big enigma of the Nature. An architecture which evokes the subterranean nature of Jerusalem, a city that seems condemned by its history to roll up on itself as if an incurable fault from the time of all times were swallowing it cyclically with continuous seismic phenomena. Abstract: aim: the goal of this study was to make the laryngological, auditory perceptual and acoustic computer analyses of young adults' (smokers and non-smokers) voices, without vocal complaint, compare them and verify the incidence of vocal alterations. study design: clinical comparative. material and method: the voices of 80 individuals with age range from 20 to 40 years were analyzed. these individuals were divided in four groups: 20 male smokers, 20 male non-smokers, 20 female smokers and 20 female non-smokers. this analysis involved laryngoscopy, which was performed and interpreted by an otolaryngologist, and cassette tape recordings of the sustained vowels /a/, /m/, /i/ e /u/, number counting from 1 to 20, speech of the days of the week, months of the year and singing of "happy birthday". the cassette tape recordings were edited for further spectrographic analysis and auditory perceptual evaluation made by four speech therapists experienced in the major of voice. results: after the analysis, a slight reduction of the fundamental frequency of the voice of smokers of both sexes was noticed, as well as higher incidence of hoarseness and laryngeal alterations among the smokers.

Abstract: OBJETIVO: Realizar a avalia o laringológica, análise perceptivo-auditiva e acústica computadorizada das vozes de adultos jovens fumantes e n o-fumantes, sem queixa vocal, compará-las e verificar a incidência de altera es laríngeas. FORMA DE ESTUDO: Caso-controle. MATERIAL E MéTODO: Foram analisadas as vozes de 80 indivíduos com idades compreendidas entre 20 e 40 anos. Estes foram divididos em quatro grupos: 20 homens fumantes, 20 homens n o-fumantes, 20 mulheres fumantes e 20 mulheres n o-fumantes. Este estudo envolveu laringoscopia, realizada e interpretada por uma médica otorrinolaringologista, e grava o em fita cassete das vogais sustentadas /a/, /m/, /i/ e /u/, contagem dos números de 1 a 20, emiss o dos dias da semana, dos meses do ano e da can o "Parabéns a você". A grava o em fita cassete foi editada para posterior análise espectrográfica e avalia o perceptiva auditiva por quatro avaliadores com experiência na área de voz. RESULTADOS: Após a análise, foi constatada uma discreta diminui o da freqüência fundamental da voz dos indivíduos fumantes de ambos os sexos, bem como maior incidência de rouquid o e de altera es laríngeas entre os tabagistas.

Abstract: We did an art state of research on training teachers/educators, in Environmental Education (EE). Into abstracts analysis on dissertations and theses, from the thesis bank in the CAPES, Brazil. In this paper we present the main themes of 130 research produced in post-graduate courses, about Education field, and, teaching Science and Mathematics, (2003-2007). We identified three themes: (1) identity and professionalization in EE, whose theme includes analyses about conceptions , representations, perceptions, or world’s views, and its relationship to educational practice, and specific studies about construction of Teacher identity (53.49%), (2) analysis of the nature and/or impact of a training intervention (44.18%), and (3) theoretical studies on teacher training in EE, that contribute on teacher training in EE (2.32%).
The association between hypertension and diabetes in a Family Health Center

Abstract: Objective: To describe the clinical characteristics of patients with hypertension treated in a health unit and its association with diabetes, relating to sex and age. Methods: We conducted this quantitative and documentary research with 297 registered users in the Plan of Reorganization of Care for Hypertension and Diabetes Mellitus (HIPERDIA / MS) of a Family Health Center of the Regional Executive Office VI (SER VI) at Fortaleza -Ceará, in May 2009. The sample was divided according to gender, age and relationship betweenhypertension and diabetes. Results: We found a predominance of women, over 40 years, with hypertension and without diabetes. Fifty-eight (19.5%) users had hypertension and diabetes associated, suggesting an additional risk factor, relevant to this clientele. One hundredand fifty-five (52.2%) of hypertensive patients, with and without diabetes, controlled blood pressure and among those who were not in control, 85 (28.6%) were elderly. It was identified that 46 (15.5%) users were affected by some complication, especially the stroke as the most prevalent among female 13 (52%) and acute myocardial infarction in male 11 (38%). The antihypertensive drugs and hypoglycemic most used were 170 (71.1%) hydrochlorothiazide,44 (75.9%) glibenclamide and 04 (6.9%) insulin. Conclusion: High blood pressure affects men and women, and its prevalence increases with age. When associated with diabetes, increases the risk of developing cardiovascular disease. It is believed that the education of individuals with chronic illness is the best way to reduce these harms.
Evaluation of the diffusion capacity of calcium hydroxide pastes through the dentinal tubules

Abstract: this study aimed to evaluate the diffusion capacity of calcium hydroxide pastes with different vehicles through dentinal tubules. the study was conducted on 60 extracted single-rooted human teeth whose crowns had been removed. the root canals were instrumented and divided into 4 groups according to the vehicle of the calcium hydroxide paste: group i - distilled water; group ii - propylene glycol; group iii - 0.2% chlorhexidine; group iv - 2% chlorhexidine. after placement of the root canal dressings, the teeth were sealed and placed in flasks containing deionized water. after 1, 2, 7, 15, 30, 45 and 60 days, the ph of the water was measured to determine the diffusion of calcium hydroxide through the dentinal tubules. the data were recorded and statistically compared by the tukey test. the results showed that all pastes presented a similar diffusion capacity through dentin. group iv did not present difference compared to group i. group ii presented difference compared to the other groups, as did group iii. in conclusion, groups i and iv presented a better diffusion capacity through dentin than groups ii and iii; 2% chlorhexidine can be used as a vehicle in calcium hydroxide pastes.

Grande parte da literatura trata do processo, das atividades e dos instrumentos especialmente desenvolvidos no tratamento de documentos para armazenamento, dissemina o, recupera o e uso em sistemas e servi os de informa o. O presente artigo apresenta uma análise bibliométrica sobre a atividade científica no campo da Organiza o do Conhecimento, utilizando-se publica es da base de dados ISI Web of Science – WoS. A importancia do estudo bibliométrico é sustentada pela necessidade de se conhecer e avaliar a produtividade e mensurar pesquisas e autores, permitindo a detec o de modelos e padr es sobre a Organiza o do Conhecimento. Os procedimentos metodológicos s o do tipo exploratório-descritivo e algumas etapas foram estabelecidas para melhor desdobramento: revis o bibliográfica para fundamentar a pesquisa, identifica o da base de dados, elabora o da express o de busca, coleta de dados, organiza o e tratamento bibliométrico com o software Vantage Point e por fim, representa es gráficas com o auxílio do Microsoft Excel. The Organization of Knowledge is an area of multiple applications and very traditional in research / teaching in Information Science. Most of the literature deals with the process, activities and tools specially developed for the treatment of documents for storage, dissemination, retrieval and use of information systems and services. This paper presents a bibliometric analysis of scientific activity in the field of Knowledge Organization, using publications database ISI Web of Science - WoS. The importance of bibliometric study is underpinned by the need to understand and evaluate and measure research productivity and authors, allowing the detection of designs and patterns on the Organization of Knowledge. The methodological procedures are exploratory-descriptive and some steps have been established to better deployment: a literature review to support the research, identification database, preparation of expression search, data collection, organization and bibliometric treatment with Vantage Point software and finally graphical representations with the aid of Microsoft Excel. Abstract: native pastures in southern brazil are used for cattle and sheep grazing. the forage offer in the spring-summer season is greater than the animal requirement, so the forage left over is usually burned. pasture burning mineralizes nutrients and leaves the soil bare, triggering water erosion. this study aimed to quantify total soil and water losses and p, k and nh4+ contents in the runoff water from an oxisol with native pasture. in the treatments, the native pastures, were i) unburned and unfertilized, ii) unburned and p-fertilized, iii) burned and unfertilized, and iv) burned and p-fertilized. rain was simulated at an intensity of 75 mm h-1 and duration of 3 h on the plots (11 m long along the slope and 3.5 m wide). burning of the native vegetation decreased the times until runoff began and peaked and reduced soil water infiltration, but increased the maximum runoff rate, soil and water losses by water erosion and the p, k and nh4+ concentration and total losses in the runoff water, compared to no burning. the runoff sediment concentration was higher in the treatment with vegetation burning than without. the highest and lowest values of sediment concentration were detected after a shorter runoff period in the treatment with vegetation burning than in that without, which is related to the 8.9 times higher total soil loss from burnt than from unburnt grassland. the p concentration and total loss in the runoff water were higher in the treatment with than without phosphate fertilizer, in the treatment with burning as well as that without. for k and the nh4+ , the concentration and total losses were higher in the treatment with burning than in that without. phosphorus, k and nh4+ concentration in the runoff water fit the exponential model y = ae-bx in relation to runoff duration (p < 0.01).
