Abstract: The coexistence of acquired hemophilia A with a secondary retropharyngeal hematoma is an extremely unusual condition with important clinical implications. The purpose of this paper is to present a case involving a patient whose first clinical manifestation, namely dysphagia, along with specific clinical examination and imaging findings, led to an incorrect initial diagnosis of a retropharyngeal abscess. However, performance of a more thorough clinical examination led to the correct diagnosis of a hematoma secondary to acquired hemophilia A. This allowed surgery to be avoided in a patient at a high risk of bleeding. Conclusions: Acute neck masses require meticulous differential diagnosis assessing the possible presence of various causative systemic diseases before the most appropriate therapy can be determined.
Efficacy and safety of ERCP in a low-volume hospital Eficacia y seguridad de la CPRE en un hospital con bajo volumen

Abstract: Background and aims: there is little scientific evidence on the outcomes of endoscopic retrograde cholangiopancreatography (ERCP) performed in low-volume hospitals; however, in our country, it is growing up its implementation. The objectives of our study were to evaluate the efficacy and safety of this technique performed by two endoscopists with basic training in a center of this nature and analyze the learning curve in the first procedures. Patients and methods: single-center retrospective study of the first 200 ERCP performed in our hospital (analyzing the evolution between the first 100 and 100 following procedures), comparing them with the quality standards proposed in the literature. Results: from February 2009 to April 2011, we performed 200 ERCP in 169 patients, and the most common indications were: Choledocholithiasis (77 %), tumors (14.5 %) and other conditions (8.5 %). The cannulation rate rose from 85 % in the first 100 ERCP to 89 % in the next 100 procedures, clinical success from 81 % to 87 %, decreasing the post-ERCP acute pancreatitis rate from 11 % to 4 %, upper gastrointestinal bleeding (UGIB) from 3 % to 2 % and acute cholangitis from 4 % to 1 %. There was a death from a massive UGIB in a cirrhotic patient in the first group of patients and a case of biliary perforation resolved by surgery in the second one. Conclusions: the results obtained after performing 200 procedures support the ability to practice ERCP in low-volume hospitals obtaining levels of efficacy and safety in accordance with published quality standards. Introducción y objetivos: existe poca evidencia científica sobre los resultados de la CPRE realizada en hospitales con bajo volumen, sin embargo su puesta en marcha en nuestro medio es creciente. Los objetivos de nuestro estudio son evaluar la eficacia y seguridad de dicha técnica realizada por dos endoscopistas biliares noveles en un centro de estas características y analizar la curva de aprendizaje en los primeros procedimientos. Pacientes y métodos: estudio retrospectivo de las primeras 200 CPRE practicadas en nuestro hospital, analizando la progresión entre los 100 primeros procedimientos y los 100 segundos, comparándolos con los estándares de calidad propuestos en la literatura. Abstract: There is a controversy as to whether the diagnostic umbrella of post-traumatic stress reactions is directly applicable to serious health conditions like myocardial infarction (MI). The principal objective of this study was to examine longitudinally the prevalence of posttraumatic stress-related symptoms, throughout three measurement periods, for patients who had suffered a first MI. In addition to the analysis of symptoms related to stress and general distress, the presence of and temporary evolution of positive emotions and optimism in these patients was also evaluated. A longitudinal study with three periods of evaluation after the MI (Time 1 (T1): 48–72 hours, Time 2 (T2): 5 months, and Time 3 (T3): 13 months). In T1 few symptoms related to the stressful event were found. The probable prevalence of PTSD was 8–11% at 5 months after the MI and 0–3% 13 months after the event. With regard to subjective severity of the infarction, although in the first instance patients did not regard the event as excessively traumatic, in the periods T2 and T3 this perception increased significantly [F (2, 32)=20.00; p=0.0001]. At all times during the measurement period the mean positive affect was significantly greater than the negative affect. As the results show, the probable prevalence of PTSD, as well as the severity of different symptom clusters, is low at all times of the evaluation. The diagnostic implications of these findings are discussed as well as the uses and abuses of diagnostic labels to characterize the psychological experiences lived through after a potentially life-threatening health problem.For the abstract or full text in other languages, please see Supplementary files under Reading Tools online.

Abstract: This paper presents a teaching sequence explicitly designed to improve the understanding of Nature of Science and support Secondary School students to acquire adequate understanding about scientific hypothesis, theories and laws. The instructional intervention engages students in an inquiry process where they have to formulate hypothesis, analyse data and draw conclusions based on evidence. It is a student-centred methodology where teachers act as facilitators and guides, promoting the development of scientific competences and the meaningful understanding of the terms hypothesis, law and theory.

Abstract: This short communication presents a novel approach to determining the soil sustainability of productive practices in an Argentinean arid region, using the resilience degree of soil organic matter components. The study was conducted in four sites of the Arid Chaco region of the Cordoba province: one undisturbed site, two sites with livestock (with total and with selective clearing) and one site with agriculture. In each site three soil samples were taken and total soil organic matter, fulvic and humic acids, and non-humic substances were analyzed. Variations of each component (%) between each productive practice and the undisturbed site were calculated in order to establish the resilience degree. The livestock soils showed: a) moderate resilience for non-humic substances, b) low resilience for organic matter and humic acids, and c) no resilience for fulvic acids. The agricultural soils showed: a) low resilience for total organic matter and non-humic substances, and b) no resilience for fulvic and humic acids. We conclude that this approach is a powerful tool for establishing management practices according to each particular situation, allowing improved productivity in arid regions. Se analizó el grado de resiliencia de los componentes de la materia orgánica del suelo para establecer la sustentabilidad de las prácticas productivas en una región árida de Argentina. Se trabajó en cuatro sitios en el Chaco árido de la provincia de Córdoba: un sitio testigo, 2 sitios con ganadería (con desmonte total y selectivo) y un sitio con agricultura. En cada sitio se tomaron tres muestras de suelo y se analizó en contenido de materia orgánica total, ácido fúlvicos, ácidos húmicos y sustancias no humificadas. Se calculó la variación (%) de cada componente entre los sitios productivos y el sitio testigo, para establecer el grado de resiliencia. En los sitios con ganadería las sustancias no humificadas fueron moderadamente resilientes, la materia orgánica total y los ácidos húmicos escasamente resilientes y los ácidos fúlvicos no resilientes. En el sitio con agricultura la materia orgánica total y las sustancias no humificadas fueron escasamente resilientes, mientras que los ácidos fúlvicos y húmicos fueron no resilientes. Se concluye que este método de evaluación de la sustentabilidad es una excelente herramienta para establecer las prácticas de manejo de acuerdo a cada situación en particular, permitiendo mejorar la productividad de las regiones áridas. Abstract: Objective: the causal relation between rosacea and Helicobacter pylori infection is discussed. We evaluated the clinical evolution of rosacea after infection eradication. Patients and methods: we have prospectively studied 44 patients diagnosed with rosacea. Helicobacter pylori infection was determined, and infected patients were treated with eradication therapy. The evolution of dermatological symptoms in a subgroup of 29 infected patients in whom eradication had been achieved was followed during 16.8 (± 17.8) months. Median age was 50.6 (± 14.1) years for 22 women (75.9%) and 7 men (24.1%). Clinical response according to gender and clinical subtype of rosacea was evaluated. Results: complete improvement was observed in 10 patients (34.5%; 95% CI: 18.6-54.3%), relevant improvement in 9 (31.1%; 95% CI: 16-51%), poor improvement in 5 (17.2%; 95% CI: 6.5-36.4%), and absence of improvement in 5 cases (17.2%; 95% CI: 6.5-36.4%). No significant differences in dermatological evolution according to sex were observed. Regarding subtype of rosacea there was a relevant improvement in 83.3% (95% CI: 64.1-93.8%) of cases with papulopustular type as opposed to 36.5% (95% CI: 20-56.1%) of cases with erythematous predominance, p = 0.02. Conclusions: based on these results, the relation between Helicobacter pylori and rosacea is supported, and infection should be investigated in these patients because an appreciable percentage of patients diagnosed with rosacea and Helicobacter pylori infection can benefit from eradication therapy, mainly in the papulopustular subtype.

Abstract: biodiversity prospecting is the exploration of an ecosystem for information regarding possible innovative uses in different human production processes. examples of the kind of information sought are the scientific data contained in the genetic material of living organisms, the chemical compounds they produce and their interactions or in the knowledge of people that have interacted with them in various ways. one of the tools of biodiversity prospecting is searching for chemical information. this information is contained in the compounds produced by many living beings to communicate, to defend themselves and exchange information, and they are known as secondary metabolites. in this project, we propose the botanical, chemical and biological biodiversity prospecting of the gallery forest of the uruguay river basin near the guaviyú stream. this particular ecosystem presents a substantial biodiversity of approximately 200 species with an unusual subtropical profile.
