Klebsiella pneumoniae outbreak in a cancer unit of a general hospital: predisposing factors and evaluation of the impact of intervention measures

Abstract: we made a retrospective cohort study of a primary bloodstream infection outbreak in patients of a cancer unit in a general hospital, evaluated the impact of intervention measures and investigated the predisposing factors. the targeted predisposing factors were selected based on the medical literature. the data were treated with univariate analysis to calculate the relative risk, and statistical significance was set at p<0.05. the presence of a long-term totally-implanted central venous catheter appears to be a predisposing factor, while a peripheral venous catheter appears to have offered protection from infection. this is reinforced by fact that intervention measures controlled the outbreak, suggesting cross-contamination from a common source. these findings corroborate the fundamental role of the hospital infection control service in early intervention and reinforce the need for continued training of health professionals that perform this type of care.

Abstract: objective: to validate the construct and discriminant properties of a hospital infection prevention and control program. methods: the program consisted of four indicators: technical-operational structure; operational prevention and control guidelines; epidemiological surveillance system; and prevention and control activities. these indicators, with previously validated content, were applied to 50 healthcare institutions in the city of s?o paulo, southeastern brazil, in 2009. descriptive statistics were used to characterize the hospitals and indicator scores, and cronbach's α coefficient was used to evaluate the internal consistency. the discriminant validity was analyzed by comparing indicator scores between groups of hospitals: with versus without quality certification. the construct validity analysis was based on exploratory factor analysis with a tetrachoric correlation matrix. results: the indicators for the technical-operational structure and epidemiological surveillance presented almost 100% conformity in the whole sample. the indicators for the operational prevention and control guidelines and the prevention and control activities presented internal consistency ranging from 0.67 to 0.80. the discriminant validity of these indicators indicated higher and statistically significant mean conformity scores among the group of institutions with healthcare certification or accreditation processes. in the construct validation, two dimensions were identified for the operational prevention and control guidelines: recommendations for preventing hospital infection and recommendations for standardizing prophylaxis procedures, with good correlation between the analysis units that formed the guidelines. the same was found for the prevention and control activities: interfaces with treatment units and support units were identified. conclusions: validation of the measurement properties of the hospital infection prevention and control program indicators made it possible to devel

Abstract: a case study aimed at knowing the perception of a teenager suffering from recurrent acute lymphocytic leukemia and of her mother regarding the visit of her pet dog during hospitalization, as well as at describing the experience as a nursing intervention. data was obtained and organized through reports on the experience. beneficial effects of such therapeutic action were observed, thus demonstrating that animal assisted therapy may be extended to other clinical situations and should be the object of new investigations.

Abstract: methodological study to elaborate and validate measures of evaluation in health contributing to an evaluation system of practices related to the control and prevention of hospital infection. it was elaborated eight dental-medical-hospital supply reprocessing conformity indicators. indicators are described using items of the structure, process and outcomes that permitted the elaboration of conformity indicators related to the best practices (gold pattern). it was performed the content validity of the attributes of applicability and representativeness by a consensus expert judgment process using a psychometric scale. all the indicators showed to have content validity. expert judgments, including comments and suggestions, evidenced the importance of perform validity of the evaluation processes, independently of the existence of golden pattern criteria and/or relevancies to the nursing practice.

Abstract: the objective of this research was to evaluate the effects of treatment with 1-methylcyclopropene (1-mcp) and different ethylene concentrations in the storage room on f 'laetitia' plums quality stored under controlled atmosphere. the treatments evaluated were 1-mcp application (1.0μl l-1) and ethylene (c2h4) in the concentrations <0.04μl l-1, 1.0μl l-1, and 10μl l-1 inside the ca storage (1.0kpa of o2+3.0kpa of co2 / 0.5°c±0.1°c and 95±2% rh) of. respiratory rates were lower in fruits treated with 1-mcp and stored at 10μl l-1 of c2h4. fruits treated with 1-mcp showed the lowest ethylene production rates. at removal from ca storage, fruits treated with 1-mcp and stored at c2h4 concentration <0.04μl l-1 showed higher flesh firmness and fruit compression resistance and lower flesh browning incidence. after shelf life, fruits treated with 1-mcp had the highest flesh firmness. titratable acidity was higher in fruit treated with 1-mcp and left in ca storage with <0.04μl l-1 c2h4 concentration. 1-mcp and ethylene absorption delay ripening and reduce flesh browning incidence in 'laetitia' plums. the effects of 1-mcp persist during shelf life. however, even with the use of these technologies, flesh browning occurrence does not allow fruit storage for 60 days under de ca storage condition used.

On the Subsystem Formulation of Linear-Response Time-Dependent DFT

Abstract: A new and thorough derivation of linear-response subsystem time-dependent density functional theory (TD-DFT) is presented and analyzed in detail. Two equivalent derivations are presented and naturally yield self-consistent subsystem TD-DFT equations. One reduces to the subsystem TD-DFT formalism of Neugebauer [J. Chem. Phys. 126, 134116 (2007)10.1063/1.2713754]. The other yields Dyson type equations involving three types of subsystem response functions: coupled, uncoupled, and Kohn-Sham. The Dyson type equations for subsystem TD-DFT are derived here for the first time. The response function formalism reveals previously hidden qualities and complications of subsystem TD-DFT compared with the regular TD-DFT of the supersystem. For example, analysis of the pole structure of the subsystem response functions shows that each function contains information about the electronic spectrum of the entire supersystem. In addition, comparison of the subsystem and supersystem response functions shows that, while the correlated response is subsystem additive, the Kohn-Sham response is not. Comparison with the non-subjective partition DFT theory shows that this non-additivity is largely an artifact introduced by the subjective nature of the density partitioning in subsystem DFT.

Abstract: aim: the purpose of this study was to evaluate the shear bond strength of orthodontic bonding with the use of a hydrophilic primer (transbond moisture-insensitive primer, 3m unitek, monrovia, calif.), a self-etching primer (transbond plus self-etching primer, 3m unitek, monrovia, calif.) and without primer application, in dry and saliva contaminated enamel surfaces. methods: bovine incisors (60) were divided into 6 groups: (1) uncontaminated control (no primer), (2) control with saliva contamination (no primer), (3) uncontaminated self-etching primer, (4) saliva contamination before self-etching primer, (5) uncontaminated hydrophilic primer and (6) saliva contamination before hydrophilic primer. stainless steel brackets were bonded with composite resin (transbond xt, 3m unitek, monrovia, calif.). after bonding, all samples were stored at 37±1°c in a humidity environment until the shear bond strength test. results: under dry uncontaminated enamel conditions comparable results were found between the control (1) and the self-etching primer (3), while the hydrophilic primer (5) presented higher bond strength. under saliva contamination, a significant difference was found between the control (2) and the self-etching primer (4), however, there was no difference between the self-etching primer (4) and the hydrophilic primer group (6). conclusion: therefore, with moisture control, the use of a primer in order to achieve acceptable bond strength is not required. in clinical situations where there is a risk of saliva contamination either primer could be used.
