Levosimendan no tratamento da contus o miocárdica grave pós-trauma torácico fechado: relato de caso = Levosimendan treatment for severe myocardial contusion after blunt chest trauma: case report

Abstract: Introdu o: a contus o miocárdica é causada usualmente por trauma torácico fechado, principalmente em pacientes com história de acidente de carro ou moto. Os pacientes com manifesta es clínicas graves devem manejados com intuba o, reposi o volêmica, vasopressor e inotrópicos. O levosimendan é uma nova droga com a o inodilatadora, que age sensibilizando os canais de cálcio. O objetivo deste relato é documentar o tratamento de um caso de choque cardiogênico secundário a contus o miocárdica, com resposta favorável ao uso deste fármaco. Descri o do caso: paciente feminina, 28 anos, vítima de acidente de carro, interna com quadro de politraumatismo. Após corre o cirúrgica de fratura de fêmur e tíbia apresentou dois episódios de parada cardiorrespiratória, evoluindo com deteriora o hemodinamica progressiva manifestada por quadro de choque cardiogênico, necessitando de monitoriza o invasiva, suporte ventilatório e hemodinamico. Após 24 horas foi iniciada infus o de levosimendan, devido à resposta inadequada aos inotrópicos tradicionais. A paciente evoluiu com melhora clínica, recebendo alta da unidade de terapia intensiva. Conclus o: na literatura existem dados que indicam a utiliza o de levosimendan na insuficiência cardíaca descompensada. Estudos experimentais e pequenos ensaios clínicos tem despertado o interesse na utiliza o de levosimendan para melhora da fun o miocárdica em pacientes com cardiopatia isquêmica, choque cardiogênico e choque séptico. Porém, ainda n o há relatos sobre a sua utiliza o em contus o miocárdica. Introduction: Cardiac contusion is usually caused by blunt chest trauma and therefore is frequently suspected in patients involved in car or motorcycle accidents. In patients with severe clinical manifestations, management with intubation, fluids, vasopressors, and inotropes is the treatment of choice. Levosimendan is a new inodilator agent that is reported to increase calcium sensitivity in myocardium. The purpose of this report is to describe a successful treatment with levosimendan in a case of myocardial contusion associated with cardiogenic shock. Case description: A 28-years-old woman victim of a motor vehicle accident, was admitted to the hospital with diagnosis of polytrauma. After she was submitted to surgical treatment fracture of tIbia and femur, she developed cardiac arrest twice with progressive hemodynamic deterioration with cardiogenic shock. She was managed with invasive monitoring, hemodynamic and ventilatory support. After twenty four hours, levosimendan infusion was initiated because there was not improvment

Abstract: beginning with a characterisation of the school - a recently modified scenario - we analyse the teaching of science, generalising the observations for the different levels of formal schooling. we present science as a language for the description and understanding of the natural world which a rereading of barthes supports. we assert that scientific literacy - a scientific illiterate is one who does not know how to read the language in which nature is written - could be responsible not only for facilitating our understanding of the world, but also for helping to transform it for the better. scientific literacy is also seen as a possibility for social inclusion, being a handicap for it the presentism {exclusive link with the present, without a rooting in the past, and without perspectives for the future}, as well as scientificism {an exaggerated belief in the power of science, and/or the attribution to it of only beneficial results}. dogmatism, marked by positivism, is presented as one of the marks of a non-scientific literacy.

Abstract: Herpes simplex virus (HSV) is the most common cause of acute sporadic viral encephalitides. The mortality rate is over 70% without treatment. Demonstration of HSV-DNA by polymerase chain reaction (PCR) is the most sensitive microbiologic method for the diagnosis. In this report, we present a case of herpes simplex encephalitis, who admitted to our department with fever and seizure. We demonstrated HSV-DNA by PCR and BBT findings supported the diagnosis of HSV encephalitis.

Abstract: O objetivo deste trabalho foi analisar o sistema fonológico de cinco crian as com preferência sistemática por um som de acordo com os estudos de Weiner (1981) e Yavas e Hernandorena (1989, 1991). A amostra está composta por cinco sujeitos (quatro meninos e uma menina), com idade de 6:4 (S1), 4:11 (S2), 4:4 (S3), 5:5 (S4) e 5:1 (S5). Os dados de fala foram snalisados através da avalia o fonológica. Para análise da fala utilizaram-se análises contrastiva e de tra os. Os sesultados obtidos na análise dos dados de fala dos cinco sujeitos s o descritos e comparados aos padr es identificados pelos autores acima citados quanto à preferência sistemática por um som. O S1 apresenta preferência por /s/e/z/, afetando a classe das fricativas; S2, por /f/ e /v/, substituindo fricativas e plosivas; S3, por /t/ e /d/, afetando as fricativas; S4 por /k/ e /g/ afetando plosivas e fricativas; S5, pela global /?/, substituindo fricativas e plosivas. Conclui-se que os resultados confirmam que o nível do desvio é variável, com o sistema fonológico ininteligível devido á perda de contraste. Porém, é necessário estudar um número maior de sujeitos e saber o tipo de desordem para atentar para o diagnóstico e a terapia. Além disso, é importante que os terapeutas da fala, ao detectarem e tratarem o desvio fonológico, conhe am as características dos sistemas com preferência sistemática por um som.
